2.Ocular bacterial flora and antibiotic sensitivity among Filipino patients undergoing routine cataract surgery

Objective:
This study determined the most common normal lid-margin and
conjunctival bacterial flora, antibiotic sensitivity, and resistance patterns in
eyes of Filipinos scheduled for routine cataract surgery at a tertiary government
hospital.
Methods:
Lid-margin and conjunctival swab specimens were collected from patients
scheduled to undergo routine cataract-extraction surgery at the Department
of Ophthalmology and Visual Sciences of the University of the Philippines–
Philippine General Hospital. The specimens were cultured using standard
technique and those with bacterial growth were tested for antibiotic sensitivity
and resistance using the disc-diffusion method.
Results:
Sixty eyes of 30 patients were sampled, 17 females (57%) and 13 males
(43%), with a mean age of 61.4 12.5 years (range, 37 to 84). Of the 30
patients swabbed, 27 (90%) were culture positive. The most common organism
identified was Staphylococcus epidermidis (24/27, 89%). Bacillus sp. was identified
in 12 (44%) patients. Staphylococcus aureus was identified in 3 (12%) patients.
Results of antibiotic sensitivity testing revealed that 85% or more of the
coagulase-negative staphylococcus isolates were sensitive to moxifloxacin,
gatifloxacin, ciprofloxacin, vancomycin, gentamycin, tobramycin, and
chloramphenicol.
Conclusions
Staphylococcus epidermidis was the most frequently isolated organism in the
eyelid margin and conjunctiva of patients for routine cataract surgery. The
isolates showed high sensitivity to commonly prescribed topical antibiotic
preparations.

3.Cyst Between the Levator Aponeurosis and the Palpebral Conjunctiva.

Cysts of accessory lacrimal gland are rare. They are generally retension cysts that result from obstruction of the excretory duct and are found between the palpebral conjunctiva and the levator aponeurosis. We experienced three cases of subconjunctival cyst in the upper fornix. Histopathological examination showed the cysts lined by two layers and occasionally one layer of epithelium. These cysts seem to be originated from Krause's gland, considering their anatomical location and histopathological findings.

5.Recurrence and Complication Rates among Current Pterygium Treatment Techniques: Pre-operative Subpterygeal Mitomycin-C Injection, Intraoperative Mitomycin C Application and Pterygium Excision with Conjunctival Autograft

Objective:
This study aims to determine recurrence and complication rates among patients who underwent three current pterygium treatment techniques: preoperative subpterygeal injection of mitomycin C, intraoperative application of mitomycin with pterygium excision and pterygium excision with conjunctival autograft.
Methods:
This is a randomized controlled clinical trial in a tertiary hospital. We included patients with diagnosed primary pterygium and who underwent either: A = pre-operative injection of 0.02% mitomycin C one month prior to pterygium excision; B = pterygium excision with intraoperative mitomycin C application; or C = pterygium excision with conjunctival autograft.
Results:
We included 111 patients: a total of 120 eyes randomized to 3 groups (A, B, C) at 40 eyes per group. After 24 months of follow-up, there was no significant difference in the recurrence rates among the groups (6/40 [15%] in groups A and B and 2/40 cases [5%] in group C; P=0.29). No complications were noted in groups B and C, while 1 case of scleral thinning was noted in group A. There was no significant difference in the complication rates among the three procedures (P=1.00).
Conclusion
There were no significant differences in the recurrence and complication rates among the three techniques. Careful patient selection and follow-up are recommended to prevent complications such as scleral thinning.

6.Epithelial Cyst of the Lower Conjunctival Fornix.

Epithelial cyst formed from downgrowth of the epithelium into the submucous tissue in chronic inflammatory condition is the commest type of the conjunctival cysts. The cyst frequently occur in the upper lid and fornix coniunctiva, but they also have been noted in the lower fornix or in the bulbar conjunctiva. A 11 years old Korean boy was found to have small, soft, slightly bluish mass in the lower fornix (OD) which showed an epithelial cyst of the conjunctiva histopathologically.

7.Clinical Features of Pinguecula in North-Western Gyeonggi Province.

PURPOSE: To investigate the clinical features of pingueculae in North-western Gyeonggi Province. METHODS: In this prospective study, we determined the clinical features of pingueculae for 120 patients who visited our hospital due to pinguecula-related symptoms between October 2011 and May 2012. RESULTS: Among the 374 pingueculae of 120 patients, 208 (56%) pingueculae were found on the nasal side of the conjunctiva and 166 (44%) on the temporal side. The grade of the pingueculae on the temporal conjunctiva was 0.81 +/- 0.626 and was 1.18 +/- 0.648 on the nasal conjunctiva, which was higher (p < 0.001). Pingueculae on the nasal conjunctiva showed greater elevation than on the temporal side (p < 0.001). The horizontal and average diameters of the pingueculae on the nasal and temporal conjunctiva showed no significant difference, but the vertical diameters were 2.40 +/- 0.88 mm and 2.14 +/- 0.91 mm, respectively, showing the vertical diameter on the nasal conjunctiva was significantly greater (p < 0.044). Moreover, the horizontal diameter of each pinguecula was greater than the vertical diameter (p < 0.003). CONCLUSIONS: The average grade of pingueculae in North-western Gyeonggi Province increased with age. Compared to the temporal pingueculae, the nasal pingueculae were more protrudent in shape and had higher grades. Furthermore, each pinguecula was oval-shaped with a larger horizontal than vertical diameter. The pingueculae on the temporal side were flatter and more oval-shaped.

8.A Case of Conjunctival and Lid Lymphedema Confirmed with Lymphoscintigraphy.

Lymphedema is due to a defect in lymphatic drainage which may be classified as two types; primary and secondary lymphedema. The authors experienced a case of lymphedema of lower lid and conjunctiva secondary to periorbital trauma and scar revision was done to reduce lymphedema. Lymphoscintigraphy was used to evaluate lymphatic obstruction and result was satisfactory for confirming the disease.

9.The Histopathologic Findings after Insertion of Biodegradible Polymer Sheet Made of PGA and PLGA/5-FU with Filtering Surgery in the Rabbit.

PURPOSE: To evaluate the histopathologic changes after insertion of polymer made of poly- glycolic acid(PGA) and poly(lactic acid-co-glycolic acid)(PLGA)/5-FU following filtering surgery in the rabbit. METHOD: A polymer with 0.3 mm thickness made of PGA and PLGA/5-Fluorouracil(5-FU, 10% by weight) composite were made in a size of 3 x 7 mm. After full-thickness filtration surgery performed, PGA was inserted under the conjunctiva in group I, PLGA/5-FU was inserted in group II and none in control group. Twelve rabbit eyes were enrolled in each group. RESULT: Postoperatively, no serious complication was found in anterior segment. In the control eyes inflammatory cell reaction was minimal at 2 weeks after surgery and decreased thereafter. While severe fibrovascular tissue reaction with loss of conjunctival filtration space was noted at 2 and 4 weeks after surgery in group I, the filtration space was well maintained without fibrovascular tissue reaction until 4 weeks after surgery in group II. At 8 weeks after surgery, fibrous tissue reaction was observed in group I, but not in group II and contol. The polymer was not found and the filtration space was obliterated at 8 weeks after surgery in group I and II. CONCLUSION: PLGA/5-FU may be used as an effective adjunct to improve the success rate of the filtration surgery, without inducing serious fibrous tissue reaction. PGA was not suitable.

10.Clinical Observation on Reconstruction of the Contracted Anophthalmic Socket by the Dermis-fat Graft.

The various method have been used in reconstruction of the anophthalmic socket. Dermis-fat graft as an orbital implant is a relatively new approach. Dermis-fat graft restores the volume lost by enucleation, gives additional conjunctival lining and is permanent, with a minimal chance of absorption. Dermis-fat graft was used in reconstruction of an ophthalmic socket in our hospital. We found it successful procedure in case of primary enucleation, because it is permanent and it preserves the conjunctiva.
