1.A technical study of radiotherapy after surgery for gastric carcinoma

Purpose:To evaluate the safety and efficacy of the box-shape field technique of radiotherapy for postoperative gastric carcinoma.Methods:Thirty-two patients of gastric carcinoma after subtotal or total gastrectomy with an extensive (D2) lymph-node dissection were irradiated with a total dose of 46~50Gy Gy of external box-shape field technical in 23-25 fractions by linac with 6-MV photons. All patients received concomitant chemotherapy which two cycles of paclitaxel (135mg/m~2 ) and cisplatin (70mg/m~2) in d1-3 and d29-31.Results:The overall survival,the relapse free survival of two years of all patients was 87.50% and 16.67%, respectively. 32 patients with treatment failure had recurrent disease, 4 (6.3%)in the liver,2(6.3%) in the peritoneum, 2(12.5%) in the anastomosis ,respectively. the plan of box-shape fields were evaluated with dose-volume histogram of three-dimensional therapy plan system, The 90% isodose line included the whole clinical target volume,and the dose of organat at rist was under the tolerance dose, except the dose of left kidney.Conclusions:The box-shape field technique of radiotherapy for postoperative gastric carcinoma were a technique which the dose distribution of clinical target volume was satisfaction ,and well tolerated with no severe side effects.[
2.Concurrent chemoradiation therapy with cisplatin and paclitaxel for locally advanced non-small cell lung cancer

Objective To evaluate the efficacy of concurrent systemic routine dose of paclitaxel/ cisplatin combined with conventional thoracie irradiation in locally advanced non-small cell lung cancer(NSCLC).Methods Forty-two unresectable stage ⅢA and ⅢB NSCLC patients were entered into this study.All patients received conventional thoracic irradiation to a total dose of 60Gy within 6 weeks,with concurrent paclitaxel 135mg/m~2,d1,and cisplatin 75mg/m~2 in the first and fourth week of radiotherapy.Results The complete response(CR) and partial response(PR) was 2/42,and 30/42 patients,with an overall response rate of 76.2% and a median survival time of 18 months.The 1-,2-,and 3-year survival rate was 64.3%,30.2%,12.0%,respectively.The 1-,2-,and 3-year progression-free survival rates was 48.1%,21.4%,5.7%,with a median progression-free survival of 12 months.Fourteen patients failed only locoregionally,10 in distant metastastasis only and 5 in both.The locoregional/distant failure rate was higher in stageⅢB than in stage ⅢA(P
3.Intensify modulated radiotherapy (IMRT) for nasopharyngeal carcinoma

Objective To evaluate the feasibility, toxicity and clinical efficacy of simultaneous modulated accelerated radiation therapy(SMART)for nasopharyngeal carcinoma. Methods Thirty eight patients with nasopharyngeal carcinoma were treated by SMART with 2.5?Gy/fraction at gross tumor volume(GTV)to a total dose of 70?Gy and 2.0?Gy/fraction at the clinical treatment volume(CTV)to a total dose of 56?Gy in 38 days. Quantitative 99m Tc pertechnetate salivary scintigraphy was used to assess the uptake and excretion index (EI、UI) of parotid gland in order to validate the value of IMRT in parotid gland sparing. Results The mean doses delivered to the GTV and CTV were 67.2?Gy and 57.0?Gy, respectively. An average of 1% of GTV and 2% of CTV received less than 90% or 95% of the prescribed dose. The mean dose to the contralateral parotid were 23?Gy and no significant decline in EI and UI as compared with significant decline in the ipsilateral parotid by 43.6% and 26.3%(P
4.Structural equation model of marital quality factors of urban and rural residents

Objective To explore the difference of social support,self-esteem and marital quality between urban and rural areas and discuss the impact on each other.Methods Chinese marital quality questionnaire,selfesteem scale (SES) and social support rating scale were used to test 386 urban and rural married residents.Results ①The results of descriptive statistics showed that marriage quality,social support and self-esteem of urban and rural residents were (318.46±37.41),(42.44±5.84) and (30.65 ± 3.95) respectively.②There was no significant difference among social support,self-esteem and marital quality in different gender(t =0.41,-0.16,0.03,P> 0.05),but had significant difference between urban and rural areas(t=2.83,3.91,5.24,P<0.01).There were significant differences between social support and marital quality in different age structures(t=5.22,3.85,P<0.05).There were significant differences between social support and self-esteem in different educational level (t =3.48,4.16,P<0.01).③Social support,subjective support,objective support and marital quality had positive correlation (r=0.114~0.394,P<0.05).The degree and character compatibility,communication between couples,resolve conflicts are positively related to support (r=0.308~ 0.476,P<0.01).④Stepwise multiple regression analysis and structural equation modeling analysis showed that self-esteem played partial intermediary role between social support and marital quality(37.7%),which could explain 26.8% of marital quality.The social suptort and self-esteem could better predict marital quality.Conclusion The higher social support and self-esteem are,the higher marital quality is.Social support can directly affect the marital quality and indirectly affect marital quality through self-esteem.
5.Diagnostic utility of immunohistochemical markers SALL4, D2-40 and Glypican-3 in primary testicular germ cell tumors

Purpose To investigate the diagnostic utility of the immunohistochemical markers SALL4, D2-40 and Glypican-3 in prima-ry testicular germ cell tumors (TGCTs). Methods The expression of SALL4, D2-40 and Glypican-3 protein was detected by EnVi-sion immunohistochemical method in 56 cases of primary testicular germ cell tumors, including 5 intratubular germ cell neoplasms ( IT-GCNs) , 10 seminomas, 14 embryonal carcinomas ( ECs) , 14 yolk sac tumors ( YSTs) , 1 choriocarcinoma, 5 immature teratomas and 12 mature teratomas. 10 normal testicular tissues and 5 lymphomas were selected as control. Results All of ITGCNs, seminomas, YSTs and ECs were diffusely strongly positive for SALL4. Focal SALL4 staining was seen in choriocarcinoma, 3 of 5 immature terato-mas and 3 of 12 mature teratomas. All of ITGCNs, seminomas showed diffusely strong D2-40 staining. ECs (4/14) were focally posi-tive for D2-40, while choriocarcinoma, YSTs and teratomas were negative for D2-40. Glypican-3 was diffusely positive in YSTs (13/14), and focally weakly positive in ECs (2/14), respectively. ITGCNs, seminomas, choriocarcinoma and teratoma were negative for Glypican-3. In contrast, 10 normal testicular tissues and 5 lymphomas showed no SALL4, D2-40 and Glypican-3 staining. Conclu-sions SALL4 is a useful diagnostic marker with high sensitivity and specificity for TGCTs. Combination of SALL4, D2-40 and Glypi-can-3 is helpful to the diagnosis and differential diagnosis for TGCTs.
6.Research on Quality Standards of Granati Pericarpium and Granati Semen

This study was aimed to establish quality standards of Granati pericarpium and Granati semen. The microscopy and thin layer chromatography (TLC) were used in the identification of Granati semen. According to the 2010 Chinese Pharmacopoeia, the content of moisture, total ash, acid-insoluble ash and alcohol-soluble extract were determined. And the HPLC was used in the content determination of ellagic acid in Granati semen and Granati pericarpium. The results showed that the microscopic characteristics of Granati semen were identified. And TLC identification of Granati semen was established. The content determination method of ellagic acid in Granati pericarpium and Granati semen were established. It was concluded that the established qualitative and quantitative method can be used for quality control of Granati pericarpium and Granati semen.
7.Effect of OB-cadherin gene transfection on osteogeule differentiation of human mesenchymal stem cells in vitro

Objective To study the effects of Cad-Ⅱgene on osteogenic differentiation of human mesenchymal stem cells in vitro. Methods The human marrow mesenchymal stem cells (hBMSCs) were isolated and cultured in vitro before they were divided into 2 groups. In the transfeetion group, the hBMSCs were transfeeted with Cad- Ⅱgene;in the simple osteogenic inducement group, they were cultured in the condition medium. Then the expressions of Cad- Ⅱ protein were determined and the alkaline phosphatase (ALP) activities and the expressions of ostcocalein were measured at 3, 7, 14 and 21 days for comparison between the 2 groups. Results The ALP activity and positive expression of osteoealcin were regulated significantly higher in the transfeetion group than in the simple osteogenic inducement group at different times (P <0.05). The mineralized nodes began to appear at 14 days in the 2 groups and increased with time. Conclusion Cad-Ⅱgene transfeetion can promote differentiation of hBMSCs into the osteoblasts.
8.Experimental studies of the correlation between vitronectin and collagen expressions post-irradiation

Objective To analyze radiation induced alterations of vitronectin and collagen expressions in fibroblasts at different times post-irradiation,so as to evaluate the potential to apply vitronectin as a biomarker of radiation-induced lung fibrosis.Methods The human fibroblast cells WI-38 and IMR-90 were irradiated with 137Cs γ-rays at doses of 0 (control),4,6,8,10 and 12 Gy,respectively.The cells and its supernatant were collected at 6,12,24,36,48 and 60 h post-irradiation.The expressions of vitronectin and collagen Ⅰ and Ⅲ were analyzed by Western blot,PCR and ELISA.Results After irradiation,the expressions of vitronectin and collagen Ⅰ and Ⅲ were positively correlated (r=0.40-0.79,P<0.05) and were all significantly higher than that in control group (t =3.04-25.45,P <0.05) and reached the highest expression levels at 48 h after 8-10 Gy of irradiation (t =2.92-18.86,P < 0.05).Analyses of Real-time PCR and ELISA assay showed that expressions of vitronectin mRNA and its protein level in the cell lysis were significantly increased by radiation (F =27.09-42.62,P < 0.05).Conclusions The expressions of vitronectin in cellular supernatant and its mRNA may be a potential biomarker of radiation-induced fibrosis,and 48 h after 8 Gy irradiation may be an optimum condition of measurement.
9.Effects of occupational therapy on the cognitive function and event-related potentials of patients with acute cerebral infraction

Objective To explore the effect of occupational therapy on the cognitive function and P300 event-related potentials (ERPs) of patients with acute cerebral infraction.Methods Ninety-eight acute cerebral infarction patients with cognitive impairment were selected using the mini-mental state examination (MMSE).They were randomly divided into an observation group (n =49) who received occupational therapy plus routine rehabilitation treatment and a control group (n =49) who received only the routine rehabilitation treatment.Before and after 8 weeks of treatment,the patients' cognitive function and their ability in the activities of daily living(ADL) were evaluated using the Loewenstein Occupational Therapy Cognitive Assessment (LOTCA) and the modified Barthel index (MBI),respectively.P300 ERPs were also detected.Results There was no significant difference between the two groups with regard to various scores before the treatment (P > 0.05).After 8 weeks of treatment,the LOTCA and MBI scores and the latency and amplitude of ERPs were significantly better than those before treatment in both groups (P < 0.05).In the observation group,the scores of LOTCA (99.4 ± 8.4),MBI (80.7 ± 5.9) and the latency and amplitude of ERPs [(373.45 ± 52.13) ms and (5.87 ± 2.63) μV,respectively] were significantly better than those in the control group(P < 0.05).Conclusion Occupational therapy program based on conventional rehabilitation training can promote the ADL ability and cognitive function of patients with acute cerebral infraction.It is worth applying in clinical practice.
10.Postoperative radiotherapy concurrent with paclitaxel and cisplatin in the adjuvant treatment of gastric cancer:An analysis of 64 cases

Objective To evaluate the efficacy and toxicity of postoperative chemoradiation for D2 dissection gastric cancer,and to compare the difference of toxicity between confromal and traditional radiotherapy.Methods Sixty four patients with T3-4,N + or R1 were enrolled.Radioation was given to a total dose of 46Gy delivered in 23fractions by use of 3D-CRT or 4 fields traditional radiotherapy.Chemothrepy was administered with paclitaxel 135 mg/m2 day 1 and 29,cisplantin 20 mg/m2 day 1 ~3 and day 29 ~31 during radiotherapy.Results The median follow-up time was 40 months.The 3-year overall and relapse-free survival rates were 78.2% and 70.9%,respectively.Eighteen patients had tumor relapse.Fifty-three patients completed chemoradiotheray.Toxicities on grade 3 or above included gastrointestinal toxicity (28.1% ),eutropenia (21.8 % ) and alopecia ( 18.7% ).One patient died of hemorrhage of upper digestion tract.Conclusion Adjuvant radiotherapy with paclitaxol and cisplatin yielded satisfactory overall survival and disease-free survival in gastric cancer patients.The toxicity was manageable.Conformal radiotherapy seems to decrease the gastrointestinal toxicities compared to that occurred in the traditional radiotherapy.
