1.Observation on Azygos System in 78 Chinese Adults

The azygos system was studied on 78 Chinese adult cadavers. The occurrence of the type of coexisted azygos, hemiazygos and accessory hemiazygos veins is 64.10%. The components of hemiazygos and accessory hemiazygos veins are sub- ject to such a large variation that the division of the azygos system into three major trunks is not always practicable since no generally-agreed criterion can be observed. The authors, according to Seib's suggestion! classify the azygos system into three types, that is, the double trunk, the intermediate, and the single trunk types, based on the condition of the interruptions of the hemiazygos. system Intermediate type occurs most frequently and its occurrence is 52.56% in this series.The interruptions of the hemiazygos system and the communications between azygos vein and hemiazygos system are far more numerous in Chinese than in Caucasians. The racial difference of the azygos system between Chinese and Caucasian subjects suggests that the azygos system of the Caucasians more likely belongs to the embryonic type. The middle segment of the azygos vein runs to the left of the midline in adults, and the origin of the azygos system usually connects with the inferior vena cava or the renal vein.The findings mentioned above might arouse the interest or the attention of the clinicians.
2.Three routes for interventional chemotherapy in the treatment of NSCLC later lung cancer

Objective To analyze the treating of 443 NSCLC cases in the middle and late stages. Three ways of drug administration had been reported as follows: intravenous drop (IVD) 301 cases, bronchial artery infusion (BAI) 64 cases, bronchial and pulmonary arteres for dual infusin (DAI) 78 cases. Methods From 1980 on 97th Hospital of PLA had already treated 443 cases of NSCLE of advance lung cancer. Three ways of drug administration had been analyzed. Results The recent effective rates attained as 53.0%, 73.4% and 98.7% with mean survival rates of 7.3、10.8 and 12.4 months respectively for the 3 groups. Conclusion The authors consider that the combination of MFP or EAP and CAMB is a better plan to treat NSCLC. The high concentration of chemical drugs directly act on the local tumor by applying BAI with high shrinking rate of tumor and increased resection rate. Because of double blood supply of lung by bronchial and pulmonary arteries, DAI will correct certain defects of BAI to increase therapeutic effect as well as reduce and avoid certain side effects of BAI.
3.Over-expression of GSTP1 increases oxaliplatin-induced apoptosis in hu-man hepatoma HepG2 cells

AIM: To investigate the effect of GSTP1 over-expression on the sensitivity of human hepatoma HepG2 cells to oxaliplatin (OXA).METHODS: Adenovirus carrying GSTP1 (Ad-GSTP1) was used to infect HepG2 cells for establishing the cell line over-expressing GSTP1.The cells were randomly divided into 6 groups: control, vehicle, Ad-GSTP1, OXA, OXA +vehicle and OXA +Ad-GSTP1.The cell survival rates were examined by CCK-8 assay, and the apoptotic rates were analyzed by flow cytometry.The protein levels of GSTP1, Akt, mTOR, p-Akt and p-mTOR were deter-mined by Western blot.RESULTS: OXA decreased the cell survival rate in a dose-dependent manner (P <0.05).The protein expression of GSTP1 increased after transfection with adenovirus.At basal level, up-regulation of GSTP1 signifi-cantly decreased the cell survival rate, increased the cell apoptosis, and inhibited the phosphorylation levels of Akt and mTOR (P <0.05).Moreover, GSTP1 over-expression enhanced the effect of OXA on the cell viability, cell apoptosis, and further inhibited the phosphorylation levels of Akt and mTOR ( P <0.05).CONCLUSION: Over-expression of GSTP1 augments the enhanced effect of OXA on HepG2 cell apoptosis, which may be related to the inactivation of Akt/mTOR signaling pathway.
4.Cytokine-induced killer cell for interventional chemotherapy of hepatocellular carcinoma

Objective To evaluate the clinical effect of hepatocellular carcinoma treatment with a combination therapy of transcather arterial super liquefied lipiodol embolization and cytokine-induced killer cell(CIK) infusion.Methods There were 3 groups in this study,group 1:38 cases of HCC patients treated with a combination therapy of transcather arterial super liquified lipiodol embolization and CIK infusion;group 2:80 cases of HCC patients treated with a combination therapy of transcather arterial super liquefied lipiodol embolization and percutaneous intratumoral ethanol injection;group 3:134 cases of HCC patient treated with transcather arterial super liquefied embolization.Finally,the outcomes of the 3 groups were compared.Results The short term effective rates of group 1,2 and 3 were 76.1%,41.3% and 14.9% respectively,simultaneously with significant difference of changes concerning AFP value among the three groups especially in group 1 the AFP decrease to normal level while those of the other two groups still remain in higher levels.Conclusions The living quality and survival rate of HCC patients could be improved by a combination therapy of transcather arterial super liquefied lipiodal embolization and CIK infusion.(J Intervent Radiol,2007,16:235-239)
5.Anatomy and preliminary clinical applications of endoscopic endonasal approach to optic canal and orbit

Objective To provide the endoscopic anatomic basis and anatomic parameters for endoscopic surgical therapy on orbital lesions , and to analyze the advantages and key points of this surgical approach .Methods Five fresh adult heads were used in this experiment .Endonasal thanslamina approach and endoscopic technique were applied to observe important anatomic marks while intraoperative medicalization of the medical rectus muscle was applied to observe the exposure and positioning of important structures and trends of the optic canal and intra orbit .Results Uncinate process was at the lower front corner of middle nasal concha; ethmoidalis bulla was behind the uncinate process , and ethmoidei sinus was reachable after an incision was made on ethmoidalis bulla;anterior ethmoidal artery and posterior ethmoidal artery were the important anatomic landmarks of the inner ethmoidei sinus;optic canal prominence , carotid artery prominence and OCR were the important anatomic landmarks of the inner sphenoid sinus; lamina papyracea was at the lateral wall of ethmoidei sinus , and orbital contents were approacchable after lamina papyracea was cut off ;inside orbit , the optic nerve was approachable through the gap between the medial rectus muscle and inferior rectus muscle .The ophthalmic artery of 9 out of 10 sides of the specimens was originated from the supraclinoidal segment of the internal carotid artery while the remaining one was originated from the cavernous segment of internal carotid artery .There were 7 sides in which ophthalmic artery was located at the inferior lateral of the optic nerve;there were 2 sides in which ophthalmic artery was located at the inferior of the optic nerve; the remaining one was located at the inferior medial of the optic nerve .Conclusion The endoscopic endonasal thanslamina approach can sufficiently expose the optic nerve and the structures in the medical space of the orbit.Uncinate process, ethmoid bulla, anterior ethmoidal artery, posterior ethmoidal artery and posterior ethmoid sinus are the important landmarks of the endoscopic endonasal thanslamina approach .Optic canal prominence , internal carotid artery prominence and OCR are the important landmarks for optic canal decompression .Ophthalmic artery , orbital branches , anterior ethmoidal artery , posterior ethmoidal artery , internal carotid artery are the important vessels . Medialization of the medial rectus muscle is effective to approach the orbital anatomical structures .
6.Correlation between hypoglycemia and increased mortality of patients with acute decompensated liver cirrhosis

Objective To explore the correlation between hypoglycemia and the increased mortality of patients with acute decompensated liver cirrhosis.Methods A retrospective study was conducted on the clinical data of 120 patients with acute decompensated liver cirrhosis admitted to the Department of Hepatobiliary Surgery of the Second Hospital of Hebei Medical University from December 2011 to December 2014. The patients were divided into three groups: hypoglycemia group (glucose < 5.0 mmol/L, 21 cases), normoglycemia group (glucose 5.1 - 10.0 mmol/L, 84 cases), and hyperglycemia group (glucose > 10.0 mmol/L, 15 cases). The differences in hepatic carcinoma, decompensation symptoms, the incidence of known glycometabolic disorder, hospitalization situation, indicators of liver function and indexes of blood gas analysis were compared among three groups. The patients' age, hepatic carcinoma, ascites, hepatorenal syndrome, encephalopathy, bleeding, jaundice and glycometabolic disorder, etc were analyzed by the univariate analysis. The resulting risk factors with statistically significant differences were analyzed by multivariate logistic regression method in order to screen out the risk factors of increased mortality.Results The incidences of hepatorenal syndrome [42.9% (9/21) vs. 22.6% (19/84), 33.3% (5/15)] and jaundice [38.1% (7/21) vs. 20.2% (17/84), 13.3% (2/15)], rate of admission into intensive care unit (ICU) [14.3% (3/21) vs. 10.7% (9/84), 13.3% (2/15)] and in-hospital mortality [23.8% (5/21) vs. 10.7% (9/84), 20.0% (3/15)] in the hypoglycemia group were significantly higher than those in the normoglycemia group and hyperglycemia group (P < 0.05 orP < 0.01). The levels of aspartate-aminotransferase (AST), total bilirubin (TBil), serum creatinine (SCr) and international normalized ratio (INR) in hypoglycemia group were obviously higher than those in normoglycemia group and hyperglycemia group [AST (U/L): 628.412±78.625 vs. 170.167±87.035, 156.716±98.047; TBil (μmol/L): 154.122±34.201 vs. 86.712±48.905, 74.313±39.883; SCr (μmol/L): 160.243±56.341 vs. 107.211±59.692, 121.342±84.059; INR: 1.951±0.987 vs. 1.439±0.919, 1.423±0.653,P < 0.05 orP < 0.01]. The levels of HCO3- and base excess (BE) in hypoglycemia group were signicantly lower than those of normoglycemia group and hyperglycemia group [HCO3- (mmol/L): 18.154±10.937 vs. 23.135±11.119, 19.081±12.022; BE (mmol/L): -7.578±2.042 vs. -1.648±0.887, -5.402±2.005, allP < 0.01]. The pH value among three groups showed significant difference (7.352±2.878, 7.461±2.036, 7.219±2.017,P < 0.01). There were no statistically significant differences in alanine transaminase (ALT), blood ammonium, arterial partial pressure of oxygen (PaO2) and arterial partial pressure of carbon dioxide (PaCO2) and lactate among the three groups (all P > 0.05). Univariate analysis showed that advanced age, hepatic carcinoma, hepatorenal syndrome, bleeding, jaundice and glycometabolic disorder hypoglycemia were the risk factors of the death in patients with acute decompensated liver cirrhosis (P < 0.05 orP < 0.01). Multivariate logistic regression analysis showed that advanced age [odds ratio (OR) = 2.101, 95% confidence interval (95%CI) = 1.297 - 3.403,P = 0.000], hepatorenal syndrome (OR = 3.032, 95%CI = 1.462 - 6.286,P = 0.000) and hypoglycemia (OR = 3.267, 95%CI = 2.135 - 4.999,P = 0.031) were the independent risk factors of the patients' death.Conclusion Hypoglycemia has certain correlation to the increase of mortality in patients with acute decompensated liver cirrhosis.
7.Study on protective effect and mechanism of Arisaema Cum Bile on acute liver injury induced by CCl4 in mice

OBJECTIVE To investigate the protective effect and potential mechanism of Arisaema Cum Bile on acute liver injury induced by carbon tetrachloride (CCl4) in mice. METHODS Fifty mice were randomly divided into normal group, model group, positive control group (Biphenyl diester dropping pills, 150 mg/kg), Arisaema Cum Bile low-dose and high-dose groups (0.78, 2.34 g/kg), with 10 mice in each group. The mice in each group were given relevant medicine intragastrically, once a day, for 7 consecutive days. Two hours after the last administration, those groups were given intraperitoneal injection of 0.2% CCl4-olive oil solution to induce acute liver injury model except for normal group. Seventeen hours after intraperitoneal injection, the serum levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), interleukin-6 (IL-6), tumor necrosis factor-α(TNF- α), and malondialdehyde (MDA), superoxide dismutase (SOD) in liver tissue were measured with kit method. The hepatic index was detected. The pathological changes of liver tissue were observed by HE staining, and the degree of liver injury was scored quantitatively. The mRNA expressions of TNF-α and IL-6 in liver tissue were detected by real-time fluorescence quantitative PCR; the protein expressions of Janus kinase 2 (JAK2), signal transducer and activator of transcription protein 3 (STAT3) and nuclear factor-κB p65 (NF-κB p65) in liver tissue were detected by Western blot assay. RESULTS Compared with normal group,the levels of ALT, AST, IL-6, TNF-α and MDA, the hepatic index were increased significantly (P<0.05), while the level of SOD was decreased significantly (P<0.05); the mRNA E-mail:qhwang668@sina.com expressions of IL-6 and TNF-α, and the protein expressions of JAK2, STAT3 and NF-κB p65 were up-regulated significantly (P<0.05); the pathological observation of liver tissue showed that the structure of hepatic cord was seriously disordered, there were many inflammatory cells infiltration of liver cells, and the liver injury score was significantly increased (P<0.05). Compared with model group, pathological changes and above indexes in mice were improved significantly in Arisaema Cum Bile low-dose and high-dose groups (P<0.05). CONCLUSIONS Arisaema Cum Bile has a protective effect on CCl4-induced acute liver injury in mice, which may be related to the inhibition of inflammatory response mediated by JAK2/STAT3/NF-κB signal pathway and antioxidant stress.
