1.Defunctioning stoma in low anterior resection for rectal carcinoma:a clinical analysis of 113 cases

Objective:To discuss the relationship between defunctionign stoma in low anterior resection for rectal carcinoma and the incidence of anastomotic leakage.Methods:Totally 270 patients who received low anterior resection for rectal carcinoma(with the distance between anastomosis and anal edge being 6 cm or less)from Jan.2005 to Oct.2006 were included in the present study.Defunctioning stoma was performed in 113(41.9%)patients considered with high risk of anastomotic leakage.The clinicopathological data were used to construct database.SPSS 10.0 software was used to analyze the incidences of anastomosis leakage in patients with and without receiving defunctioning stoma.Results:There were 20(7.4%)leakages in the 270 cases.The symptoms included abdominal pain(37%),increase of pulse(53%),fever(47%),leukocytosis(53%),pelvic fluid properties change(68%),anal discharge of bloody fluid(26%)and others(10%).The average age of patients with leakage was 57.8 years.No death was caused by anastomotic leakage.Four(3.5%)leakages happened in defunctioning stoma group,and 16(10.2%)leakages happened in non-stoma group(P=0.04).Eighteen(75%)of the 24 patients with preoperative radiotherapy history received defunctioning stoma and none of them had leakage.Two of the 6 cases who had preoperative radiotherapy history receiving no defunctioning stoma had anastomotic leakages;the fisher exact probability test showed P=0.054.The leakage rate was 4.9%(7 cases)in 142 cases who had received protective measures during low anterior resection and the rate was 10.2%(13 cases)in the rest 128 cases who had not received any protective measures(P=0.10).Leakages subsided with conservative treatment in the 4 patients with stoma,but 8 patients without stoma had severe symptoms and required surgical intervention;one developed peritonitis.The average fasting periods were significantly different between the 2 groups(P
2.Value of HBP in diagnosis of catheter-related infections in maintenance hemodialysis patients

Objective To investigate the diagnostic value of Heparin-binding protein (HBP) in maintenance hemodialysis patients in catheter-related infections.Methods 75 patients with maintenance hemodialysis were enrolled in the study and divided into observation group(n=45,with catheter-related infections) and control group(n=45,without catheter-related infections).The serum HBP,procalcitonin (PCT) and C-reactive protein (CRP) levels were detected.The values in diagnostic of catheter-related infections and Gram-negative bacterial infections were analyzed by receiver operating characteristic(ROC) curve.Results There were 34 cases of patients infected by Gram-positive bacterial with the rate of 75.6% and 11 cases of patients infected by Gram-negative bacterial with rate of 24.4%.The serum HBP,PCT and CRP levels in observation group were significantly higher than those in control group and the serum HBP,PCT levels of patients infected by Gram-negative bacterial were higher than those of patients infected by Gram-positive bacterial (P<0.05).The area under curves of HBP,PCT,CRP in diagnosis of catheter-related infections were 0.955,0.890,0.871 with sensitivity of 86.7%,88.9%,62.2% and specificity of 93.3%,73.3%,93.3%.The value of HBP in diagnosis of Gram-negative bacteria infections was better than PCT and CRP.The area under ROC was 0.818 and the cut off value was HBP>27.52 μg/L with sensitivity of 100.0% and specificity of 61.8%.Conclusion Serum HBP,PCT and CRP may be the effective indicators for diagnosis of catheter-related infections,and the HBP has important reference value for diagnosis of Gram-negative bacterial infections.
3.Determination of benzyl glucosinolate in Lepidium meyenii from different regions by HPLC.

The content of benzyl isothiocyanate (BITC) which as the enzymatic hydrolysis product of benzyl glucosinolate through thioglucosidase was determined by HPLC. The content of benzyl isothiocyanate (BITC) which as the enzymatic hydrolysis product of benzyl glucosinolate through thioglucosidase was determined by HPLC. The chromatography condition was as follows: Kaseisorb LC ODS 2000 (4.6 mm x 150 mm, 5 min) column with the mobile phase of acetonitrile(A)-water( B) under gradient elution (0-5 min, 3%-8% A; 5-9 min, 8%-48% A; 9-23 min, 48%-62% A; 23-28 min, 62%-99% A); the flow rate was 1.0 mL x min(-1) with 10 microL injection volume; detection wavelength was 246 nm and temperature of column was 40 degrees C. The content of benzyl glucosinolate was in the range of 10.76-17.91 g x L(-1). The method is simple, accurate and good reproducibility which can be used for the determination of benzyl glucosinolate in Lepidium meyenii, effectively.

4.Effect of lightweight polypropylene meshversus standard polypropylene mesh on tension-freerepair ofinguinal hernia

BACKGROUND:Lightweight polypropylene mesh with lower content of polypropylene is added with some absorbable ingredients, and wil these ingredients cause adverse reactionsin vivo?
OBJECTIVE:To compare the effect of lightweight polypropylene mesh and standard polypropylene mesh on tension-freerepair ofinguinal hernia.
METHODS:Forty patients with inguinal hernia were randomly divided into two groups (n=20 per group):the patients were treated by lightweight polypropylene mesh as experimental group andthose treated by standard polypropylene mesh ascontrol group. At 2 daysafter surgery, white blood cel counts, neutrophils percentage and level of C-reactive protein were detected. Furthermore, pain scores as wel as discomfort incidence and complications were assessed and recorded at 2 days, 1, 3 and 6 months after surgery, respectively.
RESULTS AND CONCLUSION:There were no significant differences in postoperative complications between two groups. White blood cel counts and neutrophils percentage in the experimental group were significantly lower than those in the control group (P< 0.05), butthelevel of C-reactive protein had no significant difference. Besides, pain scores had no significant differencebetween the two groups, but the postoperative discomfort incidence in the experimental group was significantly decreasedcompared with the control group (P< 0.05). In conclusion, lightweight polypropylene mesh can reduce postoperative inflammation and discomfort incidence.
5.Establishment of in vitro evaluation model for CYP2B6 induction and its application to screen inducers among TCMs.

This paper is to report the development of a high-throughput in vitro system to screen hPXR/CAR mediated CYP2B6 drug inducers, and the application of it into the quick determination of induction activity toward CYP2B6 by various commonly used traditional Chinese medicines (TCMs) extract. Dual reporter gene assays were performed. The hPXR/CAR expression vectors and the reporter vector pGL3-CYP2B6-Luc involved in the distal and proximal promoters of CYP2B6 were co-transfected into HepG2 cells. Relative luciferase activities in cell lysate were analyzed after 48 h treatment of blank vehicle or drugs to determine the induction activity toward CYP2B6 by various commonly used TCMs extract. The positive hPXR/hCAR activators rifampicin and CITCO were applied to make sure that the reporter gene model was successfully established. Then 5 kinds of commonly used TCM extracts and 1 herbal compound were successfully investigated, some were found to activate hPXR or hCAR and therefore have the potential to induce CYP2B6 enzyme. This is the first domestic article to report the hCAR3-mediated CYP2B6 induction model and the establishment of a reporter gene system for hPXR/CAR-mediated CYP2B6 induction can be an effective and systemic in vitro method to investigate the drug inducers of CYP2B6 and to explain the mechanism involved.
6.Reconstruction of engineered bacteria in preparing recombinant human peptide antibiotic ?

Objective To reconstruct the new engineered bacteria expressing hPAB-? triploids so as to improve the outputs of recombinant human peptide antibiotic ?. Methods The recombinant plasmid pQE31-hPAB-?(3) was transformed into E. coli. M15 to screen the new engineered bacteria expressing hPAB-? triploids. The stabilities of phPAB-?(3)/M15 were observed in continuous cultures. The expression levels of the fusion peptides of interest and the bacterial yields of the new engineered bacteria phPAB-?(3)/M15 were compared with that of phPAB-?(3)/JM109 in different fermentation scales. Results Genetic stability of the recombinant plasmid and phPAB-?(3)/M15 was 100 after 10 passages. 7.Simultaneous measurement of magnesium isoglycyrrhizinate and glycyrrhetic acid in plasma of dog by HPLC

AIM: To quantify magnesium isoglycyrrhizinate(MGL) and glycyrrhetic acid in the plasma of dog by develop a simple, rapid, sensitive high-performance liquid chromatography (HPLC-UV) method. METHODS: HPLC-UV methods with wavelength 252 nm were used for the quantitation of MGL and GA in plasma. The concentration of MGL and GA were assayed on a Kromasil ODS-1 C18 column with the column temperature 25 ℃. The mobile phase was a gradient system with 0.1 % diethylamine in water (pH 4.60 ) and acetonitrile at a flow rate of 1.0 ml?min~ -1 . RESULTS: There was a good linear response range of 0.2 -2.5 mg?L~ -1 and 2.5 -100 mg?L~ -1 , while the limit of quantification was 0.2 mg?ml~ -1 . The relative recovery rate of MGL was 94.3 %-101.9 %,GA 96.4 %-101.9 % (n=5);the absolute recovery rate was MGL 78.7 %-87.0 %, GA 77.5 %-87.7 % (n=5). The intra-day and inter-day variations were all less than 15% (n=5). The plasma samples preserved in refrigerator is stable at -20 ℃ for 14 days, freeze thawing 3 times and at room temperature for 10 h without degradation. CONCLUSION: This method is shown to be specific, sensitive, reliable and suitable for the quantitative determination of magnesium isoglycyrrhizinate following oral administration in biological sample.
8.Analysis of survey results of iodine deficiency disorders in Huai'an City Jiangsu Province in 2011

Objective To evaluate the status of iodine deficiency disorders(IDD) in Huai'an City in 2011 and to provide a scientific basis for adopting target prevention strategies timely and adjusting the intervention policy scientifically.Methods According to The National Project of Surveillance on IDD,probability sampling method (PPS) was used to select 30 counties in Huai'an City,and one primary school was selected randomly from each county,then 40 children aged 8 to 10 were randomly selected in each school.Children's thyroids were examined by palpation and the iodine in salt samples collected in their home was tested by direct titration.At the same time,urine samples from 12 children of the 40 selected children were collected and urinary iodine was tested by As(Ⅲ)-Ce4+ catalytic spectrophotometry.Questionnaire survey of IDD was conducted among 30 students of grade 5 and 5 housewives around that school.Results A total of 1200 children aged 8 to 10 were examined,and the goiter rate was 1.3%(15/1200).All the goiters were level Ⅰ.The goiters of each age group was 0.8%(3/390),1.8%(7/390) and 1.3%(5/420),respectively.The 360 urine samples were collected from children aged 8-10,and the median of urinary iodine was 171.5 μg/L with the proportions of < 20 μg/L,< 50 μg/L and < 100 μg/L were 0(0/360),1.1% (4/360) and 5.6% (20/360),respectively.A total of 1200 salt samples were tested in children's family.The coverage rate of iodized salt was 99.4% (1193/1200) and the qualified rate of iodized salt was 97.7% (1165/1193).The consuming rate of qualified iodized salt was 97.1%(1165/1200),and non iodized salt rate was 0.6%(7/1200).The knowledge rates of IDD were 82.7% (2234/2700) and 69.8% (314/450) in 900 students and 150 housewives,respectively.Conclusions In Huai'an City,except the knowledge rate of IDD,the goiter rate and urinary iodine of children aged 8-10,iodized salt consumption maintain at the stage of the target level.In addition to strengthen the monitoring of iodized salt and to supply qualified iodized salt,it should also increase the focus on health education propaganda in the future.

10.The influence of CsA in CD40/CD40 ligand expression of T lymphocytes in patients with coronary artery disease

Objective To investigate the effect of CD40/CD40 ligand on the genesis and development of coronary artery disease (CAD),and the inhibitory effect of cyclosporine A (CsA) on CD40/CD40 ligand. Methods A total of 71 patients were divided into four groups:acute myocardial infarction group (AMI, n=19), unstable angina pectoris group (UAP, n=18), stable angina pectoris group (SAP, n=17) and normal control group (N, n=17). Flow cytometry was used to detect the expres?sion of CD40 and CD40L in peripheral blood mononuclear cells (PBMCs) of four groups. The group in which CD40 and CD40L were produced at the highest level was chosen, and a series concentrations of CsA(H1:0 mg/L, H2:0.01 mg/L, H3:0.1 mg/L, H4:1 mg/L)were used to treat the cells. Then the expressions of CD40 and CD40L were measured by flow cytome?try. Results Compared with N group,the expression of CD40 was significant higher in other groups (P<0.05). The expres?sion of CD40 was significant higher in UAP group and AMI group than that of SAP group. There was no significant difference in the expression of CD40 between UAP group and AMI group (P>0.05). The expression of CD40L was elevated and fol?lowed by different severity of CAD. There was significant difference in the expression of CD40L between groups (P<0.05) . AMI group showed the highest expression of CD40 and CD40L. After being treated with CsA, the expression of CD40 was higher in H1 group than that of H3 group and H4 group (P<0.05). The expression of CD40L was significantly higher in H1 group than that of other three groups (P < 0.05). Conclusion CD40 and CD40L may be involved in the development of CAD. Moreover, it might be restrained by CsA via regulation of CD40/CD40L.
