1.The evaluation of biochemical parameters in diagnosis of liver disease

ObjectiveTo investigate the changes of biochemical parameters in liver disease. MethodsSernm contents of NAG, AFU, LAP, ASTm, GLDH,ADA, and CHE were determined with auto- biochemical analyzer and analyzed in 300 hepatitis patients and 30 healthy subjects. ResultsThe mean values of LAP, ASTm, GLDH,ADA and AFU in acute hepatitis patients were significantly higher than control, positive rate were 93.0% 、81.0%、76. 0% 、82. 0% and 83.2% respectively;The mean values of ADA、AFU and NAG in liver cirrhosis patients were higher than control significantly, positive rate of were 88.5% 、38.2% and 94. 2% respectively;The mean values of ADA and NAG in severe hepatitis patients were higher than control significantly,positive rate are 64. 9% and 95.7% respectively; The mean values of LAP and NAG in liver cancer patients were higher than control significantly, positive rate were75.0% and 88.0% respectively. ConclusionThe index of LAP and ASTm are valuable markers for diagnosing of acute hepatitis;The index of ADA, ASTm and CHE are valuable marker for diagnosing of liver cirrhosis;The index of ASTm, CHE, AFU and GLDH are valuable marker for diagnosing of severe hepatitis, and AFU and LAP are valuable markers for diagnosing of hepatocellular carcinoma.
2.Abnormal Autophagy in Pathogenesis of Inflammatory Bowel Disease

Inflammatory bowel disease( IBD)is a chronic and nonspecific intestinal inflammatory disease caused by multiple factors such as environment,genetic susceptibility and immune disturbance. Autophagy plays an important role in cellular homeostasis and immune. Autophagy abnormalities may be involved in the pathogenesis of IBD. Genome-wide association studies(GWAS)have provided compelling evidence that autophagy related genes,including ATG16L1,IRGM and NOD2,are significantly associated with IBD. This article reviewed the abnormal autophagy in pathogenesis of IBD.
3.The value of combined determination of serum hepatic fibrosis index for evaluation on the degree of liver fibrosis in patients with chronic hepatitis B

Objective To explore the value of combined determination of serum hepatic fibrosis index for evaluation on the degree of liver fibrosis in patients with chronic hepatitis B(HBV).Methods 275 patients with HBV were selected as chronic hepatitis group,which were divided into the mild chronic hepatitis group(75 cases),chronic moderate hepatitis group (72 cases),chronic severe hepatitis group (65 cases),liver cirrhosis group (63 cases) according to the severity of disease.In addition,50 healthy population were selected as the healthy control group.The four liver fibrosis indicators included serum hyaluronic acid(HA),laminin (LN),collagen type Ⅲ (PC Ⅲ),type IV collagen(IV-C) were determined by chemiluminescence immunoassay and compared among the groups,and the diagnostic results of combined different indicators were compared.Results The indexes of HA,PC Ⅲ,IV-C,LN were significantly higher than those of the healthy control group (t =2.567,2.687,2.811,2.741,all P < 0.01).The hepatic fibrosis indexes were increased by with the severity and duration increased gradually (t =2.665,2.787,2.755,2.811,all P < 0.01).The concentration-time curves area (AUC) of PC Ⅲ,Ⅳ-C,LN,HA were 0.680,0.825,0.716,0.826,respectively.Diagnosis value of three combination indicators in HA + PC Ⅲl + Ⅳ-C was highest.Conclusion The chemiluminescence dynamic detection of four liver fibrosis indicators is a good non-invasive detection method for guiding clinical diagnosis on the degree of liver fibrosis.
5.Determination of N,N-Dimethylaniline in Quetiapine Fumarate by LC-MS

Objective To establish a LC-MS analytical method for determination of N,N-dimethylaniline which is genotoxic impurity in quetiapine fumarate.Methods The method was achieved by an Waters ACQUITY UPLC CSHTM PhenylHexyl(2.1 mm× 100 mm,1.7 μm) utilizing a mobile phase of buffer-methanol(900∶ 100) (A)-acetonitrile(B) with gradient elution at the flow rate of 0.4 mL·min-1.The temperature of column was set at 50 ℃;The DIONEX Ultimate 3000 HPLC-AB Science 4000 QTrap Tripling Four bar LC-MS to detect N,N-Dimethylaniline (ESI source,in MRM positive mode).Results Standard curve was linear in the range of 0.4-8.0 ng(r =0.999 3);The limit of detection was 0.2 ng;The limit of quantification of N,N-dimethylaniline was 0.4 ng,respectively.The average recovery of N,N-dimethylaniline was 103.3 %;RSD was 4.3% (n =9),respectively.Conclusion The method is convenient and sensitive for the determination of N,N-dimethylaniline in quetiapine fumarate.
6.Clinical efficacy of montelukast on bronchial asthma and influence on the immune state of patients

Objective To observe the effect of montelukast on bronchial asthma and influence on the immune state , in order to provide support for the treatment of montelukast.Methods 92 patients with bronchial asthma in our hospital from December 2014 to January 2016 were selected in the study and randomLy divided into two group.The control group was treated with the conventional therapy of bronchial dilation and glucocorticoid inhalation , and on this basis, the treatment group was loaded with montelukast, 10 mg/times, one times of one day, and the course of treatment was one months.Then the frequency of acute attack of asthma and the number of beta receptor agonist were before and after treatment were recorded , and the lung function the level of T lymphocyte subsets and the levels of inflammatory cytokines before treatment and after treatment were detected , and the difference between the two groups was compared.Results After treatment, t the frequency of acute attack of asthma of day and night and the number of beta receptor agonist of the treatment group were significantly lower than the control group, the FEV1%, FVC% and PEF% of the treatment group were significantly higher than the control group, the level of CD8 + was significantly lower than the control group, the ratio of CD4 +/CD8 + was higher than the control group; the levels of IL-2, INF-γwere higher than the control group, while the levels of IL-4, IL-8 were lower than the control group (P<0.05).Conclusion The montelukast has certain regulation effect on imbalance of CD4 +/CD8 +and Th1/Th2 in children with bronchial asthma, and have a good effect on the prevention of asthma.
7.Effect of miR-200b on intestinal epithelial tight junction via MLCK/P-MLC signaling pathway

Objective To explore the impact and mechanism of miR-200 b on intestinal epithelial tight junction. Methods The negative-lentivirus and human-miR-200 b-lentivirus were employed to infect the Caco-2 cell thus establishing two stable cell lines which were then stimulated by 10 ng/mL human tumor necrosis factor-α(TNF-α) to establish the model of the intestinal epithelial injury. Those Caco-2 cells were divided into NC, NC+TNF-α, 200b, and 200b+TNF-αgroups.The tight junction permeability was detected by transepithelial electrical resistance (TEER) and Fluorescein isothiocyanate-labeled dextran (FITC-dextran). The protein alterations myosin light chain kinase (MLCK)/phosphorylated myosin light chain (P-MLC) pathways were measured by Western blot analysis. Results Compared to NC group, NC+TNF-αgroup had lower TEER, higher FITC-dextran, and up-regulated expressions of MLCK and P-MLC proteins (P0 . 05 ). Compared to NC+TNF-αgroup, 200 b+TNF-αgroup had higher TEER, lower FITC-dextran and down-regulated expressions of MLCK and P-MLC proteins (P<0 . 05 ). Conclusion miR-200 b ameliorated TNF-α-induced intestinal epithelial tight junction disruption via regulation MLCK/P-MLC pathway.
8.Relationship Between Free Radical Content and Tar Content in Cigarettes

Objective To study the methods of preparing samples, analyse free radical in smoke of cigarette and the relationship between free radical and the other harmful components in the cigarette smoke. Methods The contents of two types of free radical in smoke of 13 kinds of cigarettes were determined with electron spim resonance(ESR) and the self-made equipment. Results The analytic results showed that the main component was the free radical in gas state in all cigarette samples and it was about 10-30 times higher than the content of free radical in solid state. The content of free radical in gas state of cigarette made in China was lower than that made in foreign country. Conclusion The analysis method using ESR and the self-made equipment will be easily used in the related experiment; The level of free radical in gas and solid state is positively related to the level of tar and nicotine in the cigarette sample of same trademark but it is not the same in the samples of different trademark. Therefore, we can not to infer the content of free radial in cigarette smoke by the content of tar and nicotine in it,especially for the content of free radial in gas state. Using charcoal filter is benefit to reduce the level of free radical in cigarette smoke.
9.Application of Tentacles Medical Records in Teaching

Objective]Help students to construct the system of TCM clinical thinking and to cultivate the ability of differential diagnosis and prescription. [Methods]In the late clinical teaching, make junior doctor with a high grade of students, graduate students, or some clinical experience as a research object, designing as a medical record has gradually evolved into a number of different medical records dynamic group, let students practice the tentacles medical records. [Results]Gradually,students learned to distinguish the different parties in the different clinical symptoms, and enhance the understanding of drugs and prescriptions. [Conclusion] The tentacles medical records practicing can cultivate the students' ability of dialectical thinking integration.
10.The treatment of liver trauma in 106 cases

Objective To improve the diagnosis and treatment of liver trauma. Methods We retrospectively analyzed different therapeutic means on liver trauma including 79 cases with operative treatment and 27 cases with nonoperative treatment. Results In nonoperative treatment group, 25 cases were cured, 2 cases died. In operative treatment group, 73 cases were cured, 6 cases died. Cure rate was 92.5%(98/106), mortality was 7.5%(8/106). 5 cases died of major blood vessels rupture, 3 cases died on multiple organ function failure. Postoperative complications included 3 cases of subphrenic infection, 10 cases of hepatic abscess, 8 cases of pleural hydrops, 7 cases of incisional infection. Conclutions Type I of liver trauma can be treated by nonoperation, type Ⅱ~Ⅳ of blunt liver trauma can be treated by nonoperation on the condition of intensive monitoring. Type Ⅱ~ Ⅵ of liver trauma should be operated on emergently in case of massive intraabdominal bleeding and combined organ injury.
