[Objective]To evaluate Buzhong Yiqi Decoction’s functions of reducing toxic and side effects of chemo-therapy,as well as improving life quality,by observing its being administered to advanced lung cancer.[Method] Randomly divide 46 cases into treatment group and control one with 23 cases respectively.Both take NP or EP plan according to their pathological types separately,28 days as a course,for 3 courses in succession;the treatment group takes Buzhong Yiqi Decoction 3 days before chemo-therapy,14 days as a course,for 3 courses.The control one only takes chemotherapy to the whole body.Compare their toxic and side effects,also the Karnofsky score of the their strength after 3 chemo-therapy courses,make statistic analysis.[Result] On the rate of toxic and side effects,the treatment group was lower than the control one,with statistical meaning (P
2. Value of impulse oscillometry technique in assessment of silicosis

OBJECTIVE: To investigate the value of impulse oscillometry(IOS) technique in the assessment of silicosis. METHODS: A judgment sampling method was used to select 90 patients with silicosis as the silicosis group, which includes 30 cases in each stage of silicosis(stage 1-3). Thirty workers with no exposure history to free silica dust were selected as the control group. All subjects underwent routine pulmonary function and IOS test. RESULTS: The total respiratory impedanceat at 5 Hz(Z5), reactance at 5 Hz(X5), airway resistance at 5 Hz(R5), airway resistance at 20 Hz(R20), difference of R5 and R20(R5-R20), low frequency reactance area(AX) and resonance frequency(Fres) were higher in silicosis group than those in control group(P<0.05). The above IOS indicators showed an upward trend with the higher stages of silicosis(P<0.05). X5 was positively correlated with forced vital capacity(FVC), one-second expiratory capacity(FEV_1), FEV_1/FVC(P<0.05). Z5, R5, R50-R20 and Fres were negatively correlated with FVC, FEV_1 and FEV_1/FVC(P<0.05). CONCLUSION: IOS detection is highly sensitive for early diagnosis of silicosis. The parameters of airway resistance in IOS is correlated with the parameters of ventilatory dysfunction in routine pulmonary function test, which is of great value for the evaluation of silicosis.

The mistakes in clinical diagnosis and treatment due to dealing with symptoms, signs and imaging data in isolation, and unclear differentiation of relative diseases are found by reviewing definition and cause of disease and pathogenesis of prolapse of lumbar intervertebral disc, so as to probe into mistakes and countermeasures in diagnosis and treatment of prolapse of lumbar intervertebral disc. Only combined analysis of clinical symptoms, signs and imaging data, can correct diagnosis and treatment be made.

4.Impact of sleeve gastrectomy with ileal interposition duodenojejunal bypass operation on lipid metabolism in non-obese type 2 diabetes mellitus patients.

OBJECTIVETo evaluate the effect of sleeve gastrectomy with ileal interposition duodenojejunal bypass operation on lipid metabolism in non-obese type 2 diabetes mellitus patients.
METHODSTwenty-nine non-obese patients with type 2 diabetes mellitus underwent sleeve gastrectomy with ileal interposition duodenojejunal bypass operation. All the patients were subjected to the measurement of total cholesterol (TC), triglycerides (TG), high density lipoprotein (HDL), low density lipoprotein (LDL), homeostatic model assessment for insulin resistance (Homa-IR), glycosylated hemoglobin (HbA1c) at postoperative 12th month.
RESULTSTwelve months after ileal interposition duodenojejunal bypass operation, the blood glucose was controlled without taking hypoglycemic drugs in 28 patients (96.5%) and HbA1c decreased from (8.4±1.3)% to (6.5±1.6)% (P<0.01). Dyslipidemia were corrected in 25 cases (86.2%). TC became normal in 84.2% (15/19), and TG became normal in 82.3% (14/17). HDL became normal in 66.6% (8/12). LDL became normal in 31.2% (5/16). TC/HDL ratio decreased from 5.6±1.2 to 2.8±1.0 (P<0.01). TG/HDL ratio decreased from 3.2±1.3 to 1.5±0.8 (all P<0.01).
CONCLUSIONSleeve gastrectomy with ileal interposition duodenojejunal bypass is an effective operation for the correction of dyslipidemia in non-obese patients with type 2 diabetes mellitus.

5.Improvement of islet β-cell function after sleeve gastrectomy with ileal interposition duodenojejunal bypass in non-obese type 2 diabetes mellitus.

OBJECTIVETo investigate the improvement of islet β-cell function after sleeve gastrectomy with ileal interposition duodenojejunal bypass operation in non-obese type 2 diabetes mellitus.
METHODSClinical data of 54 non-obese type 2 diabetes mellitus cases undergoing sleeve gastrectomy with ileal interposition duodenojejunal bypass operation in our hospital from March 2009 to October 2011 were retrospectively analyzed. Fasting glucose, glycosylated hemoglobin(HbA1c), fasting insulin, body mass index(BMI), insulin resistance index(HOMA-IR), homeostasis model β-cell function(HOMA-β), early phase insulin secretion index (DelteI30/DelteG30) and area under curve of insulin(AUCINS) were measured before operation, and 1, 3, 6, 12, 24 months after operation with standard oral glucose tolerance test(OGTT).
RESULTSAt 24 months after operation, HbA1c decreased from preoperative (8.2±0.8)% to postoperative (6.3±0.1)%(P<0.01), as did the fasting glucose [(9.2±0.6) mmol/L vs. (5.9±0.5) mmol/L, P<0.01] and HOMA-IR (2.1±0.6 vs. 0.8±0.3, P<0.01). The postoperative BMI was not significantly different from the preoperative level. HOMA-β increased (28.4±9.2 vs. 56.3±12.8, P<0.05). DelteI30/DelteG30 increased after surgery (0.8±0.2 vs. 1.8±0.7, P<0.01). AUCINS was (42.6±17.1) mIU/L, (31.5±18.6) mIU/L, (34.71±12.9) mIU/L, (49.2±16.3) mIU/L, (78.3±21.7) mIU/L, (74.8±15.2) mIU/L before operation and at postoperative 1 month, 3 months, 6 months, 12 months, 24 months, respectively, indicating an increase in AUCINS 6 months later. Linear correlation analysis showed that HbA1c was negatively correlated with HOMA-β, DelteI30/DelteG30 and AUCINS (r=-0.628, P<0.01; r=-0.571, P<0.01; r=-0.606, P<0.01), and positively correlated with HOMA-IR (r=0.784, P<0.01).
CONCLUSIONSSleeve gastrectomy with ileal interposition duodenojejunal bypass can improve islet β cells function. It plays an important role in the surgical treatment of diabetes.

6.Clinical study on application of Chinese herbs during the perioperative period of laparoscopic cholecystectomy.

OBJECTIVETo explore the effect of Chinese herbs during the perioperative period of laparoscopic cholecystectomy (LC).
METHODSThree hundred and sixty patients of chronic lithic cholecystitis (LCCT) were randomly assigned to two groups by lottery, 180 patients in each group. During the peri-operative period, the control group was treated with conventional Western medicine and placebo. The treated group was given the same conventional Western medicine and Chinese herbal decoctions, with Shitong mixture No. 1 added before LC, and Liujunzi decoction added after LC for three days. The operation time, body temperature after LC, white blood cell count, wind-breaking time after operation, as well as the changes of tongue coating in the first three post-operative days were recorded.
RESULTSThere was no significant difference between the two groups in operation time (P>0.05), while the improvement in body temperature recovery, wind-breaking time and changes of tongue coating in the treated group were better than those in the control group (P<0.01).
CONCLUSIONApplying Chinese herbs during perioperative period of LC could effectively benefit early recovery in such patients.

7.Correlation between intracranial carotid artery stenosis and ocular arterial blood flow and retinal vascular morphology

AIM: To investigate the correlation between intracranial internal carotid artery stenosis and ocular arterial blood flow and retinal vascular morphology.
METHODS: Totally, 251 patients admitted to our hospital due to suspected cerebral ischemia from January 2017 to June 2018 were selected. According to the degree of internal carotid artery stenosis, the patients were divided into non-stenosis group(39 cases), mild stenosis group(80 cases), moderate stenosis group(83 cases), and severe stenosis group(49 cases). The ocular hemodynamic indexes and retinal vascular diameters of the four groups were compared. The correlation and diagnostic value were analyzed between eye blood flow parameters and severe intracranial internal carotid artery stenosis.
RESULTS: In the severe stenosis group, the peak systolic velocity(PSV)and diastolic peak velocity(EDV)of ophthalmic artery(OA), central retinal artery(CRA)and posterior ciliary artery(PCA)were significantly lower than those of the other three groups. In the blood flow parameters of OA, CRA and PCA, PSV and EDV were significantly negatively correlated with severe intracranial internal carotid artery stenosis. The optimal diagnostic thresholds of PCA PSV and PCA EDV for severe intracranial internal carotid artery stenosis were 11.26cm/s and 5.21cm/s, respectively.
CONCLUSION: Intracranial internal carotid artery stenosis was significantly negatively correlated with PSV and EDV in the ocular arteries. PCA PSV and PCA EDV were most sensitive to intracranial internal carotid artery stenosis under the same index.
8. Mechanism of chemotherapy⁃induced tumor metastasis

Chemotherapy is one of the main treatments for malignant tumors, aiming to kill tumor cells and improve patient survival. However, the new study have found that chemotherapy also promotes distant metastasis of tumors while treating tumor, and the article will give further new meaning to "chemotherapy side effects." On the other hand, tumor metastasis is the main cause of short-term survival of patients. Inhibition of tumor metastasis caused by chemotherapy is a hot topic that should be widely concerned. Modern research suggests that chemotherapeutic drugs promote tumor metastasis by inducing changes in the biological characteristics of tumors and causing a series of "host reactions". The article summarizes the mechanisms of chemotherapy-induced tumor metastasis in combination with the seed and soil theory related to tumor metastasis.
