Chinese Journal of General Practitioners 2010;09(7):491-492
A survey with self-administered questionnaire conducted in 357 families with children suffering from with hand-foot-mouth disease showed that 74. 5 percent of patients' home was at a distance of less than one kilometer to medical-care institutions, and 41.7 % of patients went to village clinics or community health-care service stations as their first visit. Diagnosis could be established for 31. 5% of them in their first visit, and 78. 4% of them underwent referral taking public vehicles.

Objective To study the pathway of miR-221/222 in enhancing radiation resistance of glioblastoma. Methods After 2 Gy of X-ray irradiation, the expressions of miR-221/222 in U251,U87 and LN229 glioblastma cells were detected with real-time PCR.Clonogenic assay was used to measure the radiosensitivity of glioblastoma after knocking down miR-221/222.ChIP assay was used to identify the combination situation of c-jun and miR-221/222.Luciferase assay was applied to check whether PTEN was a target of miR-221/222.Western blot was used to detect the expression of relevant proteins in the glioblastoma cells after knocking down miR-221/222.The effect of miR-221/222 and irradiation on growth of glioblastoma in nude mice was also observed. Results The expression of miR-221/222 was increased by irradiation(t =5.48 ~29.21,P < 0.05) and the radiosensitivity of anti-miR-221/222-transfected cells was alsoincreased(F=1 202.22,1 789.12,1 012.32,P<0.05).MiR-221/ 222 was transcriptionally regulated by c-jun with a target of PTEN (t =13.16,P < 0.05).When miR-221/222 was knocked-down,the expression of pAkt and DNA-PKcs were down-regulated while PTEN and GSK-3β were up-regulated,and the expression of Akt were not changed. Moreover, the growth of xenograft tumor was significantly inhibited by the combination treatment of anti-miR-221/222 and irradiation(F =56.36,P < 0.05).Conclusions The expression of miR-221/222 in glioblastoma cells can be increased by irradiation, and the activation of Akt pathway downstream miR-221/222 could enhance the radiation resistance of glioblastoma.

Objective To raise the awareness of congenital ornithine transcarbamylase deficiency (OTCD) by analyzing the clinical features and the family pedigree in a child with OTCD.Methods Clinical characteristics were analyzed in a child with OTCD.The genetic analysis was performed in ten exons and nine intron-exon boundaries in the ornithine transcarbamylase (OTC) gene of the case and his family by direct sequencing of PCR-DNA from peripheral blood.Family history including his grandparents and uncle was registered. Results The onset age of the case was 6 months old,the clinical picture was characterized by recurrent episodes of vomiting, low spirits, hyperammonemia, and abnormal liver function. His brother died of the similar performance at 6 months old.His sister was healthy.IVS1-2A>G intron mutation of OTC gene was found in the case, while his mother and sister were the carriers and his father revealed no mutation. His maternal grandparents and his uncle's family were healthy. Conclusion OTCD is a kind of congenital genetic disease, and is one of the main causes of hyperammonemia.Misdiagnosis is very common because clinical symptoms of this disease are variable.IVS1-2A>G mutation in OTC gene is the pathogeny of this case. The family genealogy indicates that this case is a kind of X-linked recessive hereditary disease.

Objective To observe the efficacy of electroacupuncture(EA) on the acupoint combination of He-(sea) points, front-mu points, and shu-points(abbrev. He-Mu-Shu) for the treatment of functional diarrhea, and to explore the effect of He-Mu-Shu point combination EA on the anxiety of patients. Methods A randomized controlled clinical trial was performed in 62 functional diarrhea patients. All of the patients were randomly divided into He-Mu-Shu group(32 cases) and medicine group(30 cases). He-Mu-Shu group was treated with EA on unilateral Quchi, Shangjuxu, Tianshu, Dachangshu point alternatively, and medicine group was treated with oral use of Loperamide hydrochloride capsules. Both groups were treated for 4 continuous weeks. Defecation frequency, stool property and anxiety scores were observed before treatment and 2, 4 weeks after treatment. Results After treatment, weekly defecation frequency, stool property and anxiety scores were improved in both groups, and the differences were statistically significant compared with those before treatment(P < 0.05). He-Mu-Shu point combination EA had better effect on improving weekly defecation frequency, stool property and anxiety scores of functional diarrhea patients than Loperamide hydrochloride capsules, and the differences were statistically significant (P < 0.05). Conclusion The clinical efficacy and anxiety-relief action of He-Mu-Shu point combination EA on functional diarrhea are superior to oral use of Loperamide hydrochloride capsules.

OBJECTIVE:To explore the differences of plasma concentration of norvancomycin by HPLC and microbiological method. METHODS:Microbiological method and HPLC were used to detect the plasma concentration of norvancomycin, and clinical test result of both techniques was retrospectively analyzed. RESULTS:There were no significant differences in the plasma concentra-tion of norvancomycin by microbiological method and HPLC(y=0.992 7x+0.155 8,r=0.997 6)(P>0.05). CONCLUSIONS:Both microbiological method and HPLC are more effective and reliable for the plasma concentration detection of norvancomycin. The hospi-tals can choose corresponding method according to their condition when determining plasma concentration of norvancomycin.

Objective T o investigate the m ain point of long backbone fracture caused by blunt force in forensic clinical identification and to provide a reference for the inspection and appraisal practices of such injury. Methods N inety-nine cases of adult long backbone fractures were collected from January 2006 to D ecem ber 2013 in G utian C ounty of Fujian Province. A ccording to the term s of fracture loca-tion, m ode of injury, type, the data were sum m arized. Results In the 99 cases, there were 36 cases caused by hitting, kicking, and falling and 63 cases caused by vehicle collision. T he m ajority of the for-m er was ulna, and those of the latter were tibia and fibula. T he types of fracture were transverse one, short oblique one, long oblique one, and spiral one. Conclusion D ifferent types of long backbone frac-ture, not only causing stress load of fractures as well as structural differences related to each segm ent.

Objective To study the signs and the value of CT enhanced scanning in diagnosing the liver cancer combined arteriovenous fistulae(AVF).Methods 50 cases of the liver cancer combined AVF had plain and enhanced scanning by spiral CT,then proved by hepatic arteriography. The signs of CT and hepatic arteriography were contrast analyzed. Results CT scan showed 17 cases of artery-portal vein fistulae (centre),10 cases of artery-the small branch of portal vein fistulae(periphery),3 cases of artery-liver vein fistulae,5 cases combined the signs aforementioned,2 cases combined the artery-portal vein fistulae and the artery-liver vein fistulae, and the 13 cases no the characteristic signs of CT,37 cases of the signs of AVF were found by CT,sensitivity 74%.26 cases were diagnosed as AVF,postitive rate 52%.Conclusion The development of portal vein and/or the inferior caval vein ahead in artery phase, especially the parenchyma of the lobes, segments and subsegments of liver, may be a foundation of the diagnosis of AVF.And we discussed the limitation of the CT examination in diagnosing this complication.
