This paper discusses that the focus of establishing the modern university system is to put into effect the university self-government,the academic freedom and the university administering by professor.Besides,setting up the modern university system also requires to built up the correct concept of talents,development,quality and mode,and to cultivate the university spirit and the university culture with feature.

BACKGROUND: The early symptoms of amyotrophic lateral sclerosis (ASL) are usually located at a certain part of the body, and are similar to the clinical manifestations of cervical spondylotic myelopathy (CSM). However, their treatment and prognosis differ greatly. Therefore, multiple-part electromyography and electroneurography possess an important value of reference, particularly electromyography of thoracic paraspinal muscles can be used as an objective parameter for differentiation between ASL and CSM.OBJECTIVE: To investigate the electrophysiological changes in ASL and CSM.DESIGN: A retrospective clinical analysis.SETTING: Neurological Department of Second Hospital Affiliated to Jiangxi Medical College.PARTICIPANTS: Totally 30 inpatients and outpatients with ASL and 30 patients with CSM were selected from the Neurological Department of the Second Hospital Affiliated to Jiangxi Medical College between December 2001 and November 2004.METHODS: Totally 30 inpatients and outpatients with ASL as well as 30 inpatients and outpatients with CSM were subjected to routine electromyographic and electroneurographic examination. Routine electromyography examining three limb muscles + sternocleidomastoid + thoracic paraspinal muscles intended to observe the spontaneous potential at resting stage, time limit and wave-amplitude of motor unit potential, and recruit-ment phase during energetic contraction. Electroneurography was used to detect motsr conduction velocity and sensory conduction velocity,as well as the terminal latency and wave-amplitude of motor evoked potential.limb muscles, sternocleidomastoid and thoracic paraspinal muscles in paASL and SCM.RESULTS: Data of all the 60 patients entered the final statistical analysis.in patients with ALS and CSM, and were more severe in the former. The abnormality rate of spontaneous potential of thoracic paraspinal muscles reached 93.3% (28/30) as compared to 3.3% (1/30) in patients with CSM (P ＜ 0.001). The average time limit and abnormal increment of wave amplitude of motor unit potential were significantly different in patients with conduction velocity: The decrease rate of motor evoked potential amplitude of complex muscles was obviously higher in ASL than in CSM (75.6% and 86.7%, ~=7.25, P ＜ 0.01). So was the decrease rate of motor conduction sory nerve conduction velocity: It was not affected either in ASL or CSM.ALS and CSM, but the abnormality rate of spontaneous potential of thorography indicates that nerve conduction velocity was not affected in both playing decreased conduction velocity and wave amplitude than those with CSM.

ObjectiveTo investigate the curative effect of structural bone graft in treatment of high energy injury-induced complicated distal radius fractures combined with articular surface collapse and comminuted epiphyseal bone defect.MethodsThe study involved 38 patients with distal radius fractures (AO classification, type C3) treated from January 2007 to October 2010.The patients were treated with temporary Kirschner wire fixation to recover the wrist articular surface, distal radius length and palmar tilt angle at the greatest possibility.According to the form of the bone defects, the iliac bone taken from three layers of the cortical bone was embedded in the subchondral bone to support the bone graft, and a small amount of cancellous bone graft was collected to fill the articular surface collapse as much as possible for anatomical reduction of the articular surface, and anatomical form of the distal radius was reconstructed.Modified Sarmiento Gartland and Werley evaluation systems were used to evaluate the articular function recovery.ResultsThe follow-up lasted for 1-3.2 years, which showed no complications such as plate rupture or iatrogenic nerve blood vessels injuries.The fracture healing time was 15-22 weeks (mean 18.3 weeks).The wrist articular function in some patients obtained obvious improvement through functional rehabilitation training.According to the modified Gartland and Werley function evaluation systems, the curative effects reached excellence rate of 87％.During follow-up, there were two patients with high progressive loss in the height of the distal radius and two with articular surface collapse and severe carpal joint flexion and extension dysfunction.ConclusionsThe high energy injury-induced distal radius fractures with articular surface collapse and epiphyseal bone defect need recovery of the wrist articular surface, distal radius length and palmar tilt angle.Structural bone graft can effectively support the grafted bone and maintain the flatness of the radiocarpal articular surface, restore the anatomy of the distal radius and facilitate the functional recovery of the wrist.

Objective To explore the heme oxygenase-1 (HO-1) content of carbon monoxide (CO) levels and clinical significance of Mycoplasma pneumoniae infection in children.Methods 219 cases of mycoplasma pneumonia from October 2013 to June 2015 in our hospital were divided into two groups,which were non-asthmatic group (156 cases) and wheezing group (63 cases),and the wheezing group were divided into two groups according to the presence or absence of hypoxemia and mild asthmatic group (39 cases) and severe asthmatic group (24 cases).HO-1 content in the blood was measured by enzyme-linked immunosorbent assay,using dual-wavelength spectrophotometric determination of hemoglobin oxygen percentage of broken (COHb％),analysis of the differences between the groups.Results Children with blood COHb content wheezing group were (2.59 ± 0.40)％ and HO-1 levels were (1 813.24 ± 28.34)ng/L and non-respite group were significantly increased,it had significant differences between the two groups (P＜ 0.05).Group of children with severe wheezing COHb content were (3.63 ± 0.45)％ and HO-1 levels were (2 594.34±23.94)ng/L and mild wheezing group were significantly increased,it had significant differences between the two groups (P＜0.05).Positive correlation between blood COHb content and HO-1 levels were significantly (r=0.733,P＜0.05).Conclusion Mycoplasma pneumoniae infection in children with wheezing blood COHb content and HO-1 levels were significantly increased,and with wheezing exacerbation and showed an increasing trend,suggesting that CO and HO-1 may be involved in Mycoplasma pneumoniae infection in children in the pathogenesis of wheezing,should lead to clinical attention.

Objective To evaluate the accuracy of 3D-CT navigation in guiding pedicle screw fixation in the thoracic spine.Methods Thoracic pedicle screw placement guided by 3D-CT navigation was performed on 24 cases,including 14 patients with thoracic spine fractures,6 thoracic tumors,and 4 scoliosis.CT data was input into the navigation system,and point-matching test was chosen.The optimal position,diameter,and length of the screw were set up using navigation stick after true-up.Then,the screw was placed dynamically in a stereo way according to navigation plan.After the operation,the position of the pedicle screw was assessed with CT using the Rampersaud grading system.Results Under the guidance of 3D-CT navigation,a total of 144 screws were successfully fixed into the 24 patients.According to the Rampersaud grading system,136 of the screws were at grade A(94.4%),6 at grade B,1 grade C,and 1 grade D.No postoperative injury of the nerve or spinal cord was observed.Of the patients,22 were followed up for a mean of 8 months(range,6-14 months).During the follow-up,no loose or breakage of the screw was detected by X-ray and CT.No delayed spinal cord injury was found.Conclusions The thoracic pedicle screw placement can be accurately guided by 3D-CT navigation.

The lymphatic vessels and regional nodes of the nasopharynx in 70 foetuses and infant cadavers were studied with the method of injection of the lymphatics of the organ.There is a network of lymphatic capillaries in the mucous membrane of the nasopharynx, which drains into the submucous lymphatics. The latter join together to form a number of efferent ducts.The efferents emerging from the posterior wall of the nasopharynx end in the retropharyngeal lateral and medial nodes, or pass to the posterior aspect of the internal carotid artery and internal jugular vein and end in the upper deep cervical nodes lying deep to the tip of the mastoid.The lymphatics emerging from the lateral wall drain into the nodes right under the base of the skull anterior to the internal carotid artery and internal jugular vein, or descend to the jugulodigastric node, and the upper deep cervical nodes between the beginning point of the lingual artery and the bifurcate point of the common carotid artery.

BACKGROUND: The appearances of spiral computer tomograph (CT) and three-dimensional image processing are of very importance for fully evaluating preoperative pathological changes,which represent various cervical spondylotic myelopathy (CSM) in clinic, and choosing proper clinical therapeutic occasion and methods.OBJECTIVE: To analyze the effects of imageological image and technical advantages of three-dimensional spiral CT on the preoperative evaluation and surgical plan of CSM.DESIGN: Retrospective analysis, controlled observation.SETTING: Nanjing First Hospital Affiliated to Nanjing Medical University. PARTICIPANTS:Altogether 268 patients with CSM admitted to Nanjing First Hospital from January 2002 to January 2005 were involved in this experiment. The involved patients were randomly assigned into preoperative evaluation group (n =146) and control group (n =122). The baseline materials of two groups were comparable.METHODS:Preoperative evaluation group:① CT scan and three-dimensional reconstruction:Helical scanning was conducted at transverse plane with TOSHIBA-HiSpeed/I screw machine. After scanning, reconstruction was performed finally, three-dimensional reconstruction was conducted on computer workstation (RadworkS. 1 ). Correlative data were measured and analyzed with image analysis software (ADW3.1). ② Design of three-dimensional CT-assisted operation plan: Cervical vertebral osseous anatomic landmark and the anatomic relationship of its adjacent structure were revealed stereoscopically.Individual orthopaedic location marker could be provided.Preoperative routine CT examination was performed in the control group with conventional operation method.MAIN OUTCOME MEASURES: ① The hyperostosis of vertebral body, hamular process joint and small articular process were observed. ②The degree and type of intervertebral disc protrusion were observed. ③The changes of hyperostosis before and behind the vertebral canal. Spinal decompression was observed by CT virtual endoscopy (CT-VE).RESULTS: All the 268 patients participated in the result analysis. ① In the preoperative evaluation group, 129 patients were found with intervertebral disc protrusion,109 patients with vertebral posterior marginal hyperostosis, 61 patients with hypertrophic ligamentum flavum and corrugation, 27 patients with posterior longitudinal ligament calcification,31 patients with hyperostosis at small articular process,29 patients with vertebral plate thickening and 18 patients with vertebral olisthy.The visible anatomical structures were in accord with clinical manifestations and imageological structures. Death, laryngeal nerve injury, bone graft displacement, infection and other complications were not found in all the patients. In the postoperative 6th month, X-ray image showed that all the patients had good bone graft fusion, and no titanic plate or bolt loosening or fragmentation was found. The excellent and good rate evaluated by Odom was 95.9%.② In the control group,postoperative X-ray image and CT showed that vertebral canal decompression of 24 patients was not fully, and the positions of titanic plate or bolt of 17 patients were not satisfying. The excellent and good rate evaluated by Odom (84.4%) in the control group was lower than that in the preoperative evaluation group (P＜0.05).CONCLUSION: Preoperative observation of cervical vertebral individual three-dimensional CT is helpful to exactly and generally evaluate cervical spondylotic myelopathy and assistantly make operative plan,whick makes intraoperative manipulation more exactive, safer and easier to be controlled.

Objective To analyze the misdiagnosis in extraovarian peritoneal carcinoma(EOPC). Methods The clinical manifestation, blood tumor marker, imaging examination, laparotomy, pathology, treatment and prognosis of 19 EOPC patients were retrospectively analyzed. Results The clinical symptoms were abdominal distention, abdominal pain and swelling. 17 cases were tested for blood CA-125 with 15 at abnormaly high level, and in 4 cases ascites was CA-125 positive ( ＞ 10 KU/L). Tumor cells were detected in the ascites in 12 cases. Three cases were misdiagnosed as of uterine origin, 13 cases misdiagnosed as adnexal tumor, 1 case was misdiagnosed as bladder and intestinal tumor. Only 2 cases were correctly diagnosed before the operation. All of the cases were pathologically proved as peritoneal carcinoma. The five-year survival rate was only 20 percent. Conclusions Abdominal distention and pain with unknown causes were the primary manifestations of EOPC, elaborate physical examination sometimes may provide with important information. The significant elevation of serum CA-125 and the ascites cytology may be helpul for the differential diagnosis of PPC. CT scan is valuable for the diagnosis of EOPC.

A survey with self-administered questionnaire conducted in 357 families with children suffering from with hand-foot-mouth disease showed that 74. 5 percent of patients' home was at a distance of less than one kilometer to medical-care institutions, and 41.7 % of patients went to village clinics or community health-care service stations as their first visit. Diagnosis could be established for 31. 5% of them in their first visit, and 78. 4% of them underwent referral taking public vehicles.

Objective To study the signs and the value of CT enhanced scanning in diagnosing the liver cancer combined arteriovenous fistulae(AVF).Methods 50 cases of the liver cancer combined AVF had plain and enhanced scanning by spiral CT,then proved by hepatic arteriography.The signs of CT and hepatic arteriography were contrast analyzed.Results CT scan showed 17 cases of artery-portal vein fistulae (centre),10 cases of artery-the small branch of portal vein fistulae(periphery),3 cases of artery-liver vein fistulae,5 cases combined the signs aforementioned,2 cases combined the artery-portal vein fistulae and the artery-liver vein fistulae,and the 13 cases no the characteristic signs of CT,37 cases of the signs of AVF were found by CT,sensitivity 74%.26 cases were diagnosed as AVF,postitive rate 52%.Conclusion The development of portal vein and/or the inferior caval vein ahead in artery phase,especially the parenchyma of the lobes,segments and subsegments of liver,may be a foundation of the diagnosis of AVF.And we discussed the limitation of the CT examination in diagnosing this complication.
