Objective To investigate the curative effect of noninvasive positive pressure ventilation combined with aerosol inhalation of budesonide on patients with severe asthma. Methods One hundred and twenty-one patients with severe asthma were randomly assigned to two groups. Both groups were treated with noninvasive positive pressure ventilation based on routine treatment. In addition, patients in the treatment group were given aerosol inhalation of 2 ml budesonide and 2 ml NS,two times each day,14 days for a course.Clinical symptoms improvement and arterial blood pressure changes of two groups before and after treatment were studied to compare the curative effect of the treatments in the two groups. Results There were no significant differences in pH,PaO2 and PaCO2 between the treatment and the control groups(P > 0.05).After one course of treatment, the treatment group had significantly higher levels of pH(7.43 ± 0.04 vs.7.34 ± 0.02,t =3.423,P < 0.05) and PaO2 ( [77.4 ± 16.7 ] mm Hg vs.[ 62.4 ± 16.5 ] mm Hg,t =3.414,P < 0.05 ) and lower levels of PaCO2 ( [43.4 ±8.9] mm Hg vs.[50.9 ± 9.1 ] mm Hg,t =5.317,P < 0.05 ) than the control group. There were significant differences in the total effective rate between the Budesonide group and the control group (96.7% vs.88.3%,x2 =4.218,P < 0.05 ).Conclusion Noninvasive positive pressure ventilation combined with aerosol inhalation of budesonide is a simple and effective strategy for patients with severe asthma. Thus it may be worthy to be widely used clinically.

Objective To study the clinical significance of plasma N-teminal pro brain natriuretic peptide (NT-pro-BNP) level in diagnosis and evaluation prognosis of patients with different heart failure. Methods Plasma NT-pro-BNP level was detected and compared in all patients at the first day and a week after the patients' condition turn better. Results The level of plasma NT-pro-BNP for the high blood pressure's heart disease and the chronic heart failure of coronary heart disease and the chronic heart failure of cor pulmonale were significantly higher than normal (t =2.98,t =2.98,t =2.98,P < 0.01).The level of plasma NT-pro-BNP was reduced after treatment (t =2.56,t =2.75,t =2.88,P < 0.05,P < 0.01,P < 0.01).The level of plasma NT-pro-BNP was gradually increased by the increased degree of the heart failure(t =2.78,P < 0.05).Conclusion The levels of plasma NT-pro-BNP at acute stage for the high blood pressure's heart disease and the chronic heart failure of coronary heart disease and the chronic heart failure of cor pulmonale were increased obviously and descend by the patients' condition return. The level of plasma NT-pro-BNP can monitor patients' condition and estimate the heart failure patients' prognosis.

Objective To establish the method for the determination of the content of total alkaloids extracted from seeds of Strychnos nux-vomica L. Methods The content of total alkaloids in the samples was determined by the acid dye colorimetry, which was performed with brucine and strychnine as the reference substances, dibromothymolsulfonphthalein as the acidic dye under the condition of a buffer solution (pH=7.2), and chloroform was used as the extractant. Results Both the concentration of brucine being 0.041 2~ 0.206 mg/mL and that of strychnine being 0.042 8~ 0.214 mg/mL had a good linear relationship with the absorbance. The average recoveries for brucine and strychnine were (101.8? 4.6) % and (103.3? 2.7) % , respectively. Conclusion The established method is simple, accurate and stable, and can be used for the determination of the content of the total alkaloids extracted from the seeds of Strychnos nux-vomica L.

Objective To investigate the genetic basis of patients with intellectual disability, and to assess the application of single nucleotide polymorphisms (SNP)-array in the molecular diagnosis of intellectual disability. Methods Sixty-four patients with intellectual disability who were identified in Maternal and Child Health Hospital of Guangxi Zhuang Autonomous Region from January 2013 to June of 2015 were enrolled.Genomic DNA was extracted from peripheral blood and was analyzed with Illumina Humancyto SNP-12 300K gene array chip. All identified copy number variants (CNVs) were analyzed with references from databases such as ClinVar,DECIPHER,OMIM and DGV(Database of Genomic Variants),as well as comprehensive literature review from PubMed database to determine the pathogenicity of CNVs.Results Sixteen cases of the above 64 patients were found to have CNVs with genomic alterations, including 6 cases microdeletions/microduplications associated with known syndromes,3 cases microdeletions and microduplications with clear clinical relevance (non-syndrome),1 case numerical chromosome aberration,1 case unbalanced translocation and 5 cases CNVs of unknown clinical significance. The detection rate was 25% (16/64 cases).Among these 16 abnormalities,6 cases of them could not be detected by using karyotyping analysis because their sizes were less than 5 Mb,and the smallest detected missing fragment was 0.53 Mb.Conclusion SNP-array gene chip technique with the advantages of higher efficiency,high-resolution and good accuracy, which can be applied to the genetic diagnosis of intellectual disability.

OBJECTIVETo explore the interaction between herbal medicines and western drugs based on CYP3A4 enzyme metabolism by using testotesrone as a probe in liver microsome metabolism system in vitro.
METHODThe mixed liver microsome enzymatic system consisting of rat liver microsomes by ultra-high-speed centrifuge was established. The substrate testosterone was added into the system and enzyme CYP3A4 metabolic activity was expressed by the output of 6beta-hydroxy-testosterone which was measured by HPLC method. The proper conditions for testotesrone metabolism in liver microsome system included substrate concentration, incubation time, pH and incubation temperature. When the conditions in vitro were determined, three kinds of Chinese herbal medicinal ingredients (Tetrahydropalmatine, neferine, panax notoginseng saponins) were diluted into different concentrations and incubated with testotesrone in the liver microsomes incubation system, respectively. The results were measured through metabolite production with or without the presence of Chinese medicines. We assessed the Chinese herbal medicinal ingredients effect on the metabolism of CYP3A4 enzyme through 6beta-hydroxy metabolite of testosterone production.
RESULTLiver microsomes were incubated in the system, the testosterone metabolited into 6beta-hydroxy testosterone. The metabolism conditions were proper at the concentration of testosterone 200 micromol x L(-1) which was incubated for 3.5 hours at 37 degrees C in pH 7.0, PBS 0.1 mol x L(-1). The inhibition of tetrahydropalmatine and panax notoginseng saponins on testotesrone were weak with IC50 > 100 micromol x L(-1). The neferine had a little inhibition on testotesrone metabolism, IC50 < 100 micromol L(-1).
CONCLUSIONTetrahydropalmatine and panax notoginseng saponins had no obvious effect on testotesrone metabolism. Neferine had a little effect on testotesrone metabolism. It prompted that drug-interaction could not be apparent between two kinds of Chinese medicines and the CYP3A4 enzyme substrate, Neferine could bring about drug-interaction.
