Objective To discuss the clinical effect of combination therapy under the theory of "treating soft tissue for bone disease" on knee osteoarthritis.Methods The 160 cases were randomly divided into control group(80 cases) and experimental group(80 cases).The control group was treated by diclofenac sodium sustained release capsules(per os) and sodium hyaluronate(intra-joint injection).The experimental group was treated by traction,steaming and washing,massage,needle-knife therapy,manipulation on soft tissues for loosening the joint,and orthepedic insoles.The course of treatment was 3 weeks.The effect was evaluated according to Lysholm Score,knee movement and subjective satisfaction after the follow-up for 6 months.Results Of the 154 cases completed the study(control group 75 cases and experimental group 79 cases),the Lysholm Score and knee movement had a very significant improvement(P

In order to provide insights into sampling in suspected cases of morphine poisoning,the present study was conducted to investigate the distribution patterns of morphine in organs in rats with acute morphine poisoning at different post mortem intervals.Localization and semi quantitation of morphine in the brain,kidney,heart and liver were determined in rats at 15min to 5h after intravenous administration of morphine and at different intervals from 0 to 48h after death using the immunohistochemical SP method and image analysis.The results showed that the morphine could be detected in cytoplasms of certain parenchymal cells in all organs examined short time after delivery of morphine.With the extension of post delivery intervals,the level of morphine inereased,peaked,followed by decrease thereafter.The amount and the distribution patterns of morphine varied greatly with different organs.In the brain,the morphine could be detected earlier than in the heart,kidney and liver.It could detected 15min after the drug administration,with a peak,and disppeared at 5h after drug delivery.It decayed slowly as compared with the heart and lung.The kidney was the second organ in which the morphine was detected early with a high level and decayed slowly.In conclusion,immunohistochemical SP method can be used as a specific technique to detect morphine in organs in suspected cases of morphine poisoning.Both brain and kidney were the candidates for sampling in such case.

Objective To observe the curative effect of Xianlinggubao capsules combined with mecobalamin tablets,manipulation and cervical traction in treatment of cervicalspondylotic radiculopathy(CSR).Methods Forty-eight cases CSR patients who were treated in Affiliated Hospital of North China University of Science and Technology from February 2013 to May 2015 were randomly divided into treatment group and control group,each group with 24 cases.All patients were treated with manipulation,cervical traction and mecobalamin tablets(0.5 mg,3 times/d),treatment group received Xianlinggubao capsule(1.5 g,2 times/d)for 21 d.Nervous pain was valued with short-form of McGill questionnaire(SF-MPQ),the difference between the two groups before and after treatment was compared with that before treatment,the pain rating index(PRI),the fuzzy pain rating scale(VAS)and the current degree of pain(PPI).Table 20 quantitative analysis of symptoms of cervical spondylosis of nerve root type was applied to assess the comprehensive evaluation of symptoms and signs,the curative effect was calculate(before and after treatment,and the ratio of the integral before treatment)of the omnibus symptoms and signs.The curative ratio and the healing time were calculated.Results The treatment group McGill in PRI,VAS and PPI score difference were significantly higher than the control group((6.16±1.94)points vs(4.87±2.12)points,(5.23±2.22)points vs.(3.63±1.74)points,(2.76±0.87)points vs.(1.26±1.01)points),the differences were statistically significant(P=0.02,0.03,0.04).Table 20 quantitative analysis index of treatment was significant higher than control group(0.72±0.15 vs.0.37±0.07),the differences were statistically significant(t=1.31,P＜0.05).The curative ratio of treatment group was much higher in control group(70.83％(17/24)vs.45.83％(11/24)),the differences were statistically significant(x2=8.15,P=0.02).The treatment group gained shorter healing time((8.31±3.11)d vs.(11.24±4.52)d,t=3.36,P=0.01).Conclusion Assured curative effect was found in Xianlinggubao capsules combined with mecobalamin tablets,manipulation and cervical traction for cervicalspondylotic radiculopathy.

Objective To evaluate clinical features, diagnosis and treatment of anorectal malignant melanoma (ARMM).Methods The clinical data of 15 patients of ARMM in our hospital and 18 patients in the First Affiliated Hospital of China Medical University from 1990 to 2010 were reviewed.Twenty-five patients underwent curative surgical resection, 14 patients underwent abdominoperineal excision of the rectum (APR), and 11 patients underwent local excision (LE).Survival analysis was carried out.Fisher's exact test and Log-rank test was used to compare the effects of these two different surgical procedures.Results ARMM had a female predominance, the mean age was 22 -77(54.5 ± 7.6) years.The major clinical signs included hematochezia, anus pain.The misdiagnosis rate was 67% (22/33).The average tumor size was (3.5 ±1.7) cm.Thirty-one petients(94% ,31/33) had ARMM within 5 cm from anus margin.Mean survival time was (14.0 ± 6.5)months.The overall 1-,3-,and 5-year survival rates were 48% ,22% , and 10% , respectively.Local recurrence after curative LE was higher than APR (LE,64% vs APR, 21% , P = 0.049) , The overall 3-year disease-specific survival rates after curative LE was not significantly different from that of APR (LE, 28% vs APR ,31%, x2 = 0.268, P = 0.582).Conclusions Anorectal malignant melanoma has a high rate of misdiagnosis.Radical resection could not prolong the survival time significantly in anorectal malignent melanoma patients.

BACKGROUND: Currently, the morphologic characteristics of the torn discoid lateral meniscus remain unclear, andmorphological indexes used to evaluate the discoid lateral meniscus tears still need to be explored. OBJECTIVE: To discuss the specificity and sensitivity of discoid lateral meniscal tears in children using morphological indexes. METHODS: Seventy-three patients (seventy-six knees) with torn discoid lateral meniscus underwentarthroscopy and MRI data of knee examinations were retrospectively reviewed. All MR images were double-blinded, independently, retrospectively analyzed by attending radiologist (doctor A) and sports physician (doctor B). The following morphological signs of discoid lateral meniscus, such as a internal displaced free edge sign, a hypertrophic free edge sign and a hypertrophied-horn sign, were selected to evaluate the discoid lateral meniscus tears. The likelihood of the discoid lateral meniscus tears was analyzed on basis of arthroscopic findings constituted the gold standard. Sensitivity, specificity and accuracy of MRI diagnosis of the discoid lateral meniscus tears by two physicians were calculated, and the consistency of diagnosis results by two physicians was evaluated using the Kappa statistics.RESULTS AND CONCLUSION: (1) The sensitivity, specificity and accuracy in the diagnosis of discoid lateral meniscustears by internal displaced free edge sign for doctor A were 19%, 94% and 53%, respectively, and for doctor B were 21%, 97% and 55%. (2) The sensitivity, specificity and accuracy in the diagnosis of discoid lateral meniscus tears by hypertrophic free edge sign for doctor A were 52%, 100% and 74%, respectively, and for doctor B were 57%, 97% and 75%. (3) The sensitivity, specificity and accuracy in the diagnosis of discoid lateral meniscus tears by hypertrophied-horn sign for doctor A were 26%, 97% and 58%, respectively, and for doctor B were 24%, 100% and 58%. (4) The sensitivity, specificity and accuracy in the diagnosis of discoid lateral meniscus tears by binding morphological index for doctor A were 86%, 91% and 87%, respectively, and for doctor B were 88%, 94% and 91%. (5) The Kappa statistics for discoid lateral meniscus tears respectively demonstrated a good inter-observer agreement (K > 0.75, P < 0.001). (6) These findings suggest that single morphological index used to evaluate the discoid lateral meniscus tears shows a good specificity, but which also shows a low sensitivity, while a binding morphological index can significantly improve the diagnostic sensitivity. Morphological characteristics may be reliable indexes to evaluate the discoid lateral meniscal tears in children.

Objective To explore the effects of different weeks of high intensity interval training (HIIT) and moderate intensity aerobic exercise on maximal oxygen uptake (VO2max) and the expression of the skeletal muscle oxidative capacity related factor AMP-activated protein kinase (AMPK) and peroxisome proliferators γactivated receptor coativator-1-α (PGC-1α),so as to provide the basis for the choice of effective load intensity.Methods One hundred and twenty 6-week-old SPF male Wistar rats were equally randomized into 3 groups according to their body weight:a sedentary control group (not receiving any exercise),a moderate-intensity exercise group (undergoing 50-min continuous running at an intensity of 60％-70％ VO2max) and a HIIT group (conducting 3-min running at 90％ VO2max interspersed with 3-min recovery periods at 50％ VO2max and repeating that process 6 times,with a 7-min warm-up and cool-down period at 70％ VO2max).All rats except those in the control group exercised five days a week and 50min per day.All rats were measured their weight at 8:00-9:00am every Sunday.Soleus was taken from ten randomly chosen rats of each group at 2nd,4th,6th and 10th week after the onset of the intervention.The VO2max test was done before taking muscles.Western blotting was used to detect the protein expression of AMPK and PGC-1α.Results (1) Both the exercise duration and mode had a significant impact on rats' body weight,and they had interaction with each other (P＜0.01).(2) The average PGC-1α expression in the skeletal muscle of 10-week HIIT group were significantly higher than those in the control group (P＜0.05),and the average VO2max at the same time was also significantly higher than the other two groups.(3) In the HIIT group,the average VO2max at the 10th week was significantly higher than that at the 2nd,4th and 6th week (P＜ 0.05).The average protein expression of AMPK in the skeletal muscle at the 4th week in the moderate-intensity exercise group was significantly higher than that at the 2nd and 6th week of the same group,and that at the 2nd week of the control group (P＜0.05).The average protein expression of PGC-1α in the HIIT group from the 2nd to the 10th week was significantly higher than the control group at the 2nd week (P＜0.05).(4)The protein expression of PGC-1α was significantly correlated with temporal variation of VO2max in the HIIT group (P＜0.05).Conclusion Ten-week HIIT can effectively promote the expression of AMPK and PGC-1α in skeletal muscles and the maximal oxygen uptake.It plays a more quick and effective role in improving oxidant capacity and cardiorespiratory endurance of skeletal muscles than traditional moderate-intensity exercises.

The rehabilitation and nursing for the complication and secondary disturbance are the important partition of treatment for patients with traumatic brain injury(TBI).Understanding the category,mechanism,prevention and treatment of complications can efficaciously decrease the incidence rate of them,relieve the distress of patients,increase the ablity of ADL and the quality of life,improve the effect of rehabilitation,and reduce the economic burden of family and society.It also can promote patients back to family and society as earlier as possible.

BACKGROUND:Currently, the morphologic changes of the discoid lateral meniscus (DLM) remain unclear, and morphological indexes used to evaluate the injuries of DLM stil need to be explored. OBJECTIVE:To compare the MR imaging difference between symptomatic and asymptomatic DLM in adults, and to select reliablemorphologicalparametersfor assessing DLM. METHODS:A total of 36 patients (36 knees) with a symptomatic DLM (study group)and 34 patients (34 knees) with an asymptomatic DLM (control group) were included in this study. GE Healthcare Centricity RIS/PACKS System was used to measure the primary morphological parameters of two patients with DLM on coronal MR imaging. Intergroup comparison was performed with the Wilcoxon test. RESULTS AND CONCLUSION:On the coronal MR imaging, the width of the body portion and height of the free edge of the DLM was significantly increased in the study group compared with the control group (P< 0.001), while the height of the peripheral portion which connects with the capsule of the DLM was significantly decreased in the study group compared with the control group (P= 0.002). The heightof the middle of the DLM and the width of the lateral femoral condyle showed no significant differences between the both groups (P> 0.05). The width ratio of the body of the DLM to the lateral femoral condyle and the height ratio of height of the free edge to the middle of the DLM were significantly increased in thestudy group compared with the control group (P< 0.001), while the height ratio of the peripheral portion to the middle for the DLM was significantly decreased compared with the control group (P< 0.001). These findings suggest that compared with asymptomatic discoid lateral meniscus, the width of the body portion and the height of the free edge for the DLM are increased;however,the height of the peripheral portion which connects with the capsule of the DLM is decreased. The width ratio of the body portion of the DLM to the lateral femoral condyle and the height ratio of the free edge to the middle for the DLM may be reliable morphological parameters for the assessment of the DLM in adults.

OBJECTIVETo compare the sensitivity of Brown Norway rats (BN) with Guinea pigs (GP) as allergen assessment animal models.

METHODBN rats and GP were randomly assigned to 1 control group, 2 Bovine serum albumin group (BSA), respectively. Animals in BSA groups of BN rats and GPs were sensitized by intraperitoneal injection of 0.6% BSA 1 ml on day 1, 3, 5, respectively, and irritated by intravenous injection of 2.4% BSA 1 ml on day 7 and day 14 after the last sensitization, while the same volume of normal saline was given to control group on each time point mentioned above. The allergic reactions were scored within 1 h after each irritation treatment, and the sera of both BN rats and GPs were collected to detect IgE concentration by using ELISA. The sera were also applied for passive cutaneous anaphylaxis test (PCA test) in SD rats.

RESULTNo obvious allergic reactions were observed in BSA group of GPs after each irritation treat, however, the score of allergic response in BSA group of BN rats was evidently higher than that in control group after first irritation. PCA test by using sera from BSA group of BN rats after both irritations showed the strong positive result characterized as large amount of subcutaneous effusions of Evans blue in SD rats, however, the sera from BSA group of GP were negative in PCA test. Serum IgE concentration did not increase after each irritation in BSA group of both BN rats and GP.

CONCLUSIONBN rats were more sensitive than GPs on initiative systemic anaphylaxis test and passive cutaneous anaphylaxis test. Meanwhile, BN rats has an advantage in experimental treatment compared with Guinea pigs.

OBJECTIVETo investigate the effect of parathyroid hormone (1-34) (PTH) on tumor growth in a mouse model of lung cancer with bone metastasis.

METHODSMouse models of proximal tibial bone metastasis of lung cancer were established in 30 female BALB%c mice. The mouse models were randomly divided into 3 groups and received injections with 40 mg%kg PTH (1-34), equal amount of solvent (blank control), or cyclophosphamide (positive control). Body weight of the mice was measured every 2 days and the right hind limb tumor growth was observed. The mice were sacrificed after 28 days for X-ray and CT examinations to observe the tumor shape, size, tibial bone density, and tumor volume. HE staining and immunohistochemistry were performed to observe the tumor morphology and pathological type, and serum concentration of serum alkaline phosphatase (ALP) was detected.

RESULTSThe body weight change curves did not show significant difference between PTH (1-34) group and the blank control group (P>0.05). In both PTH (1-34) group and the blank control group, X-ray and micro-CT revealed significant bone defects, and in cyclophosphamide group the bone cortex was basically intact with only mild bone destruction. The tumor volume was similar between PTH (1-34) group and the blank control group (P>0.05), but significantly smaller in cyclophosphamide group (P<0.05). The bone density in PTH (1-34) group was significantly greater than that in the blank control group, but lower than that in cyclophosphamide group (P<0.05). Pathological examination revealed mainly osteolytic lesions mixed with bone destruction, which was severer in PTH (1-34) group and blank control group with obvious tumor cell filling of the defects; immunohistochemistry identified the tumors as adenomas. ALP activity was higher in PTH (1-34) group than in the other two group and differed significantly between the 3 groups (P<0.05).

CONCLUSIONIntermittent small-dose injections of parathyroid hormone PTH (1-34) does not promote bone metastatic tumor growth in mice and increases the bone quantity around the metastatic lesions.
