BACKGROUND:Knee osteoarthritis and rheumatoid arthritis are common arthritis in the knee.In late stage of arthritis, severe deformation and activity obstacle are found, which require total knee replacement. The total knee replacement not only can significantly relieve symptom, but also correct deformation and improve joint function.OBJECTIVE:To summarize classification of knee joint, techniques of knee replacement surgery and complications following replacement.METHODS:We retrieved Vip Database (http://www.cqvip.com) for titles and abstracts with the key words of "knee replacement, prosthesis, complication".Articles concerning clinical application of knee prosthesis were included.Articles in the same field that published recently or in authorized journals were included.A total of 20 literatures were included for review according to inclusion criteria.RESULTS AND CONCLUSION:Various artificial total knee prostheses were used in different patients in accordance with patients' condition, joint limitation degree, fixation method and meniscus function.Indication of different knee prostheses depends on bone and soft tissue of the knee, and quality and status of knee ligament, joint deformation and destruction degree of articular cartilage are responsible for the choice of prosthesis.Thrombosis of deep veins of the lower limb, pulmonary embolism, infection surrounding the prosthesis and prosthesis loose are common complications in joint replacement.

Objective To construct and express human ScFv against keratin in E.coli, and identify its binding activity with antigens and antigenic specificity. Methods By genetic engineering technology, human anti-keratin ScFv was constructed from Fab fragment selected from established semi-synthetic phage antibody library. The binding activity with antigens and antigenic specificity of expressed products were i-dentified by ELISA.Results Results of DNA sequence analysis showed that nucleotide sequence of V?and VH of ScFv gene was similar to that of V?and VH of Fab gene, suggesting that there was no mutation in the construction of ScFv.The soluble anti-keratin ScFv was found to have a good antigenic specificity as well as excellent binding activity with target antigens by ELISA.However, the binding activity of ScFv with keratin was lower than that of Fab.Conclusions The successful expression and identification of human an-ti-keratin ScFv might lay a solid foundation for further observation of biological activities of anti-keratin au-toantibody,manufacture of genetic-engineering anti-keratin products, and promotion of clinical application of genetic-engineering anti-keratin antibodies.

Objective To construct the permanent colostomy patients with rectal cancer in archives information module content. Methods Using expert meeting,semi- structured interview, literature review, Delphi expert consultation method, establish the system of permanent rectal cancer patients file information specific module. Results In two- round consultation authority coefficient was 0.847. Kendall coordination coefficient (Kendall′s W) was 0.195-0.331, the difference had statistical significance (P<0.01); through the expert consultation method to construct the file information module for permanent colostomy patients with rectal cancer, which contained 4 first-level indicators (colostomy patient information module, stoma nursing modules, stoma patients transitional care modules, colostomy patients with network platform module), 11 secondary indicators, 36 third grade indicators. Conclusions This study established file information module content for permanent stoma patients with rectal cancer. It has good scientificity and reliability, and can provide a theoretical basis for the clinical development of a file information system network for transitional care of permanent stoma patients with rectal cancer.
