1.Research Progress in Mechanism of Prevention and Treatment of Diabetic Myocardial Fibrosis with TCM

Diabetic myocardial fibrosis mainly occurrs at late stage of diabetic cardiomyopathy, which is closely associated with the high mortality of this disease. Studies have shown that this disease is lack of specific treatment medicine, while the advantages of TCM in prevention and treatment of diabetic myocardial fibrosis are increasingly significant. The intervention mechanism mainly involves renin-angiotensin-aldosterone system, oxidative stress, endothelial activity, inflammation response, the dynamic balance of extracellular matrix, cell apoptosis, regulation of cytokines and other aspects. This article reviewed the research progress in mechanism of prevention and treatment of TCM for diabetic myocardial fibrosis, with a purpose to provide scientific basis for prevention and treatment of diabetic myocardial fibrosis with TCM in clinic.
2.Quality Index Evaluation Method for Quality Analysis of Flupentixol and Melitracen Tablets

OBJECTIVE:To explore a comprehensive drug quality evaluation method so as to provide constructive suggestions for the production and quality inspection of drugs.METHODS:The quality inspection data of 198 batches of flupentixol and melitracen tablets collected automatically by Laboratory Information Management System(LIMS) in Guangdong institute for drug control in 2008 were analyzed statistically,and the drug quality indexes were computed and the drug quality was analyzed comprehensively.RESULTS:Of all the quality items,the content uniformity showed the maximum impact on the quality of the tablets,followed by the content determination item,with disintegration time item showing the least impact on the quality of the tablets.CONCLUSIONS:LIMS is a powerful tool in collecting large quantities of drug data,and the quality index serves as an effective means for the evaluation of drug quality.
3.Research Progress in Pharmacological Effects of Salidroside on Cardiovascular System

Salidroside is the most important bioactive component of Rhodiolae Crenulatae Radix et Rhizoma. Many recent studies have shown that salidroside plays an protective role in cardiovascular system by inhibiting myocardial cell death, promoting angiogenesis, improving heart function, inhibiting platelet aggregation, promoting body fat mobilization and cholesterol metabolism and other pharmacological effects. This article reviewed recent studies on pharmacological effects of salidroside on the cardiovascular system, providing references for the research and application of salidroside.
4.Study on serum levels of vitamin D and calcium and expression of tissue calcium-sensing receptor in the breast cancers

ObjectiveTo explore the relationship among 25-hydroxyvitamin D,serum calcium,calcium-sensing receptor,and breast cancer. Methods The expressions of calcium-sensing receptor (CaSR) in primary breast cancer,breast benign tumors,and normal breast tissue beside tumors were determined by immunohistochemistry S-P method as well as the concentration of serum 25 (OH) D and serum calcium in breast cancer and breast benign tumors by Enzyme-linked immunosorbent assay (ELISA),Tribromoarsenazo Ⅲ method.ResultsSerum 25 (OH) D level of breast cancer was significantly lower than the breast benign tumors [ (34.13 ± 14.14) nmol/L vs (50.29 ± 25.65 ) nmol/L,t =2.870,P =0.001 ].Serum level of 25 ( OH ) D in lymph node metastasis positive patient was lower than that in negative group [ (30.8 ± 9.71 ) nmol/L vs (43.7 ± 23.59) nmol/L,t =2.467,P =0.021 ].The positive expression of CaSR in breast cancer(88.9% )was higher than breast benign tumors(60%,x2 =6.717,P < 0.01 ) and normal breast tissue beside tumors (60%,x2 =5.628,P < 0.05 ).ConclusionsConcentration of serum 25 (OH)D and expression of calcium-sensing receptor in the tissues may be associated with occurrence,development and prognosis of breast cancer.
5.Pathogenic spectrum and bacterial resistance of severe community-acquired pneumonia

Objective To study the distribution of pathogenic spectrum in children with severe community-acquired pneumonia(CAP) and bacteria antibiotic resistance.Methods One hundred and ninety-three children with severe CAP were enrolled from Mar 2011 to Feb 2012.Sputum specimens were collected for bacterial culture and drug sensitive test.Meanwhile mycoplasma pneumonia and chlamydia trachomatis were detected by fluorescent quantitative polymerase enzyme technology.Antigen of virus were detected by immunofluorescence assay.Results A total of 96 cases (49.7%) were bacteria positive in 193 children with severe CAP.The top four bacteria strains were klebsiella pneumoniae,staphylococcus aureus,escherichia coli and streptococcus pneumoniae.Most of gram-negative bacteria were resistant to ampicillin,cefazolin,ceftriaxone,ceftazidime,and compound sulfamethoxazole,but were sensitive to piperacillin/tazobactam,imipenem,ciprofloxacin,levofloxacin,amikacin.Gram-positive bacteria were resistant to penicillin and erythromycin,but sensitive to vancomycin.Fifty-three cases (27.5 %,53/193) were virus Positive,81.1% of which were less than 1 year old.Respiratory syncytial virus accounted for the most prevalent pathogen,followed by adenovirus,influenza virus A.Mycoplasma pneumoniae were positive in 4 patients (2.1%,4/193),chlamydia trachomatis were positive in 3 patients (1.6%,3/193).Mixed infection was found in 23 cases (11.9%,23/193).There were 14 cases (7.2%,14/193) with undetected pathogens.Conclusion Bacterium is the major pathogen in children with severe CAP and the virus is the second.The initial antibiotics administration of piperacillin/tazobactam or carbapenem and vancomycin should be chosen for severe bacteria pneumonia.
6.The combination of deoxynivalenol and zearalenone at permitted feed concentrations causes serious physiological effects in young pigs.

This study was to investigate the effects of the combination of deoxynivalenol (DON) and zearalenone (ZON) on pigs. Twenty-four weaning piglets were divided into a control group fed a diet free of mycotoxins and a toxin group fed a diet containing 1 mg/kg DON and 250 microgram/kg ZON. The results showed that supplementation of DON and ZON in diets had extensive effects on pigs. More specifically, DON and ZON caused levels of total protein, albumin, and globulin in sera to decrease (p < 0.05) by 14.5%, 6.5% and 11.3%, respectively, and at the same time increased (p < 0.05) the serum enzyme activities of gamma-glutamyltransferase, aspartate aminotransferase and alanine aminotransferase by 72.0%, 32.6% and 36.6%, respectively. In addition, DON and ZON decreased (p < 0.05) the level of anticlassical swine fever antibody titers by 14.8%. Real-time PCR showed that DON and ZON caused the mRNA expression levels of IFN-gamma, TNF-alpha, IL-2, to decrease (p < 0.05) by 36.0%, 29.0% and 35.4%, respectively. Histopathological studies demonstrated that DON and ZON caused abnormalities in the liver, spleen, lymph nodes, uterus, and kidney. The concentrations of DON and ZON used in this study are in line with the published critical values permitted by BML. Our study clearly put the standard and adequacy of safety measures for these toxins into question. The authors suggest that with the increasing availability of cellular and molecular technologies, it is time to revisit the safety standards for toxins in feeds so as to make feeds safer, providing consumers with safer products.

7.Using ultrasound for hemodynamic therapy among infants with septic shock

Objective:To investigate the effects of critical care chest ultrasonic examination(CCUE)on different fluid management phases among septic shock infants in pediatric intensive care unit(PICU).Methods:Twenty-two infants who were hospitalized in PICU during January 2017 to December 2018 and diagnosed as septic shock were included in this study.These infants received shock and infection management as well as mechanical ventilation according to the septic shock management guidelines.CCUE was applied as needed to monitor the hemodynamic status for titrated adjustment in fluid and vasoactive drug management and its impacts were evaluated.Results:The change frequencies of treatment regimen according to CCUE evaluation were different among different phases( P<0.001). Compared with the other 3 phases, the number of adjustment made to fluid management scheme caused by CCUE during the first phase was the largest(75.0%, P<0.001), and that during the fourth phase was the smallest(2.3%, P<0.01). The frequency of change during the second phase(30.5%) and the third phase(23.5%) showed no difference( P=0.210). During the first phase, compared with the group with intravenous infusion speed<10 mL/(kg·h), the group with faster intravenous infusion speed had lower LUS score and more proportion of LVEF and RVEF above 50%( P<0.05). During the second phase and the third phase, compared with group receiving slower intravenous infusion, group with faster intravenous infusion had more LVEF>50%( P<0.05). Conclusion:Application of CCUE to monitor dynamic hemodynamic of infants with septic shock in PICU has different effects on fluid management scheme adjustment at different phase.CCUE evaluation during the early 3 phases, especially during the first phase has greater influence on fluid management strategy.Rapid infusion under CCUE monitoring is often limited by cardiac ejection fraction, LUS, and mainly LVEF, especially during the first phase.Multiple ultrasonic indicators should be combined with clinical data for full evaluation.
8.The level and clinical significance of Rab11 in children with sepsis

Objective To investigate the expression of Rab11 in children with sepsis at different sta-ges and severe sepsis and its relationship with the occurrence and development of sepsis in children. Methods A prospective control study was performed. All cases were enrolled from Bao′an Maternal and Children Health Care Hospital, and they were divided into sepsis group(40 cases) who were diagnosed as sepsis, severe sepsis group(20 cases) with diagnosis of severe sepsis,and healthy control group(40 healthy chil-dren) . Venous blood samples were collected at admission,and the expression level of blood leukocyte Rab11 was determined by Western blot. In the sepsis group,the expression levels of Rab11 were evaluated at the initial,the extreme and the recovery stages of sepsis,and were compared with those in severe sepsis group and healthy control group, respectively. Spearman correlation analysis was used to evaluate the relationship between the expressions of Rab11 and the levels of some parameters in blood,including white blood cell,neu-trophils,lymphocytes,monocytes,eosinophilic granulocyte,C-reactive protein and procalcitonin in blood,at the extreme stage. Meanwhile,the levels of Rab11 at extreme stage of sepsis,caused by different diseases, such as severe pneumonia,bronchiectasis complicated pulmonary infection,biliary tract infection,urinary tract infection, necrotizing enterocolitis and severe enteric viruses infection, were compared with each other. Results At the initial and the extreme stages of sepsis, as well as in severe sepsis group,the levels of Rab11 were significantly lower than that in the healthy control group(0. 54 times,0. 23 times and 0. 07 times, P<0. 05,respectively). There were no significant differences in the expression levels of Rab11 at the recovery stages of sepsis compared with that in the healthy control group(P>0. 05). There was no relation-ship between the level of Rab11 and the number of white blood cell, neutrophils, lymphocytes, monocytes, eosinophilic granulocyte, while the level of Rab11 was negatively correlated with C-reactive protein ( r =-0. 58,P=0. 014) and procalcitonin(r= -0. 63,P=0. 003) at the extreme stage of sepsis. There was no significant difference in the expression level of Rab11,at the extreme stage of sepsis,among those patients with severe pneumonia,bronchiectasis and pulmonary infection,biliary tract infection,urinary tract infection, necrotizing enterocolitis and severe enterovirus infection(P>0. 05). Conclusion The level of Rab11 is differently expressed at different stages of sepsis,and could be used as a predictor of the severity of sepsis in children.
9.Analysis on the causes and countermeasures of the dilemma in medical patents transfer

Objective To investigate the causes of the dilemma in the medical patents transfer,analyze the corresponding countermeasures.Methods The research clarify the causes of the dilemma in the medical patents transfer and put forward countermeasures to promote the medical patents trans fer according to depth interviews to relevant personnel involved in medical patents transfer and literature analysis.Results A lot of factors,including the professionalism and technical of the medical patents,the special status of the inventor,the imperfect institutions,attitude of hospital administration to the medical patents transfer and environment which ignore the medical patents transfer,lead to the dilemma of the medical patents transfer.Countermeasures like the establishment of a special transfor mation entity,the incentives for transformation personnel,improve the transformation system and processes can promote the medical patents transfer.Conclusions Comprehensive consideration of the objective environment,the effective using of available resources,encouraging collaborations between hospitals and enterprises to making good use of their own advantages,as well as ask for related regulatory support,can help the smooth transfer of medical patents.
10. Clinical study of fluid management in children with septic shock guided by ultrasound

Objective:
To investigate the effects of critical care chest ultrasonic examination (CCUE) by intensive care physician on fluid management among septic shock patients in pediatric intensive care unit (PICU).
Methods:
Forty children from PICU who were diagnosed as septic shock in Shenzhen Bao′an Maternal and Child Health Hospital were included in this study.Twenty-two of them who were hospitalized in PICU during January 2017 to December 2018, under the care of 4 PICU physicians who had certificates of the Chinese Critical Ultrasound Study Group(CCUSG) were defined as CCUE group.Eighteen PICU patients from January 2014 to December 2015 having no access to CCUE were recruited as control group.Both groups were treated according to the septic shock management guidelines with routine anti-shock and anti-infection therapy, as well as mechanical ventilation.Fluid management following conventional protocol was performed in the control group.While in the CCUE group, CCUE was applied to monitor the hemodynamic status for adjustment in fluid management.
Results:
Compared with the control group, the CCUE group had shorter mechanical ventilation time as well as less fluid intake and output within 48 hours after admission[(4.68±2.06)d vs.(7.33±0.49)d, (6.34±1.85)ml/(kg·h) vs.(8.55±0.39) ml/(kg·h), (2.47±1.22)ml/(kg·h) vs.(6.18±1.72)ml/(kg·h)] (
