1.Clinical Significance of Determining TXB_2、PAgT、6-Keto-PGF_(1?) in Plasma of Lung Carcinoma Patients

Objective To explore the clinical significance of determining the levels of plasma TXB 2,PAgT,6-Keto-PGF 1? in the lung carcinoma patients.Methods The levels of plasma TXB 2,PAgT and 6-Keto-PGF 1? were determined in 43 patients with lung carcinoma and 45 healthy controls,and 36 cases of 43 patients with lung carcinoma were randomly divided into two groups:(A) conventional treatment plus warfarin sodium,(B) conventional treatment without anticoagulant.We also determined the level of plasma Fib in both group A and group B,besides examined the levels of plasma TXB 2,PAgT and 6-Keto-PGF 1? before and after treatment.Results Statistical analysis showed that the levels of TXB 2 and PAgT were significantly higher,but the levels of 6-Keto-PGF 1? was significantly lower in the patients with lung carcinoma than those in healthy controls(P0 05).Conclusion The determination of plasma TXB 2,PAgT and 6-Keto-PGF 1? for lung carcinoma patients are helpful in diagnosis and treatment of lung carcinoma.

3.Analysis of Influencing Factors for Attitude Shifting to Mental Health Work among Non-psychiatric Doctors in General Hospitals

Objective:To investigate the attitude towards task -shifting of mental health among non -psychiat-ric physicians of general hospitals in Guangzhou and to provide evidence for continuing education model and health policy making .Methods:A cross-sectional study was implemented among 1363 participants of 17 general hospi-tals from December 2012 to April 2013 in Guangzhou.Results:Among 1363 participants, there were 941 (69.6%) approved the concept of mental health task shift .Factors of attitude to the concept of mental health task shift include work 8-10 h/day (β=0.404, P=0.006), practice qualification (β=-1.345, P=0.016), less than 10%physical diseases combined with psychological or mental problems in clinical work (β=-0 .452 , P=0.014), demand of training cost/time(β=-0.596, P =0.021), get ready (β=1.436, P <0.001) and have little preparation (β=0 .694 ,P<0 .001 ) preparation for treating mental disorders at present work ( recognition and treatment).The possibility of participants′attitude toward task -shifting mental health is positive the highest (71.0%), followed by neutral (25.5%) and negative (3.5%).Conclusions:Strengthening non -psychiatric physicians′support for mental health task shift with respect to policy and legal levels in general hospitals , improving their recognition and preparation of mental disorders , as well as developing targeted training and courses are the highest priority .
4.Transurethral resection of prostate in the treatment of high risky and senile patients with large volume benign prostatic hyperplasia

Objective To improve the skills and level of TURP and peri-operative period managements to the high risky senile patients with large volume benign prostatic hyperplasia.Methods The clinical data of 318 high risky and senile patients whose ages above 80ys,ASA score > 2 and prostate volume > 60g were analyzed retrospectively.They underwent the treatment of TURP.ResultsTotal 318 patients underwent TURP were safe.The operating time ranged from 40 to 85 minutes,averaged 58.2minutes;the volume of blood transfusion ranged from 200ml to 600ml;No serious complications happened during and after the operation.With follow up of 1 ~ 12 months,the International Prostate Symptom Scores(I-PSS) decreased 14.7 averagely,Quality of Life(QOL) decreased 3.3 averagely,Maximal flow rate( Qmax ) increased 6.4ml/s averagely,and Post-voided Residual(PVR) decreased 85.3ml averagely.Conclusion The actions including sufficient preparations and evaluations pre-operatively,the maintenance to the stability of circulatory system during the operation,guaranteeing the demands of blood exchange in the vital organs such as heart,lungs and brain,are the key points to the success.
5.Childhood trauma experience and its relation to the eye emotion recognition ability in patients with schizophrenia

ObjectiveTo explore the childhood trauma experience and its relation to the eye emotion recognition ability in patients with schizophrenia.MethodsSeventy-one schizophrenic patients and fifty-two healthy subjects were assessed with the Childhood Trauma Questionnaire (CTQ) and the Eye Basic Emotion Discrimination Task (EBEDT).ResultsThe CTQ total score in the patients group was significantly higher than those in the control group (47.86 ± 13.26 vs 34.27 ± 7.27,P < 0.01 ).The subscores in emotional abuse,physical abuse,sexual abuse,emotional neglect and physical neglect of CTQ in the patients group were also significantly higher than those in the control group.(all P< 0.01 ).Compared with controls,the patients had significantly lower total accuracy( (86.74 ± 6.16vs73.42 ± 11.86) %,P < 0.01 ) and longer total reaction time ( (2776.75 ± 703.42 vs 4321.40 ± 1956.83 )ms,P < 0.01 )of EBEDT.The significant difference were also found on the accuracy and reaction time of the EBEDT six basic emotion between the two groups ( all P < 0.01 ).Correlation analysis indicated that CTQ total score were negatively correlated with the EBEDT total accuracy ( r=-0.390,P<0.01 ),and its sub-accuracy of happiness,surprise,sadness,and anger ( respectively r =- 0.419,r =- 0.451,r =- 0.391,r =- 0.344,P < 0.01 ) in the patients group.The correlation between CTQ total score and EBEDT total accuracy was also found in the control group.ConclusionChildhood trauma is related to eye basic emotion recognition in schizophrenia patients,suggesting that the trauma during childhood can affect the social recognition ability.
6.A cross-sectional study on the mental and behaviour education among non-psychiatric medical workers of the general hospitals in Guangzhou

Objective To explore the mental education and continuing education among non-psychiatric medical workers of the general hospitals in Guangzhou city.Methods A cross-sectional study was carried out in 17 general hospitals of Guangzhou city including 2617 participants from December 2012 to April 2013.Results Among 2617 participants,41.6% of them hadn't taken part in psychiatric course study,66.9% hadn't received psychiatric rotation,and 66.7% hadn't received psychiatry and related training;93.8% of the medical staff had interest in mental specialized training courses,of which 72.0% medical staff stood for training classes/participation,72.3% medical staff most satisfied with 4-5 days of a training course,strengthen training every 6 months (2 days) and a psychiatrist to connect.Conclusion Non-psychiatric medical workers of the general hospitals in Guangzhou city lack the knowledge of mental health,and mental health care training are urgently needed.
7.Application of scientific literature in the teaching of nutrition and food hygiene

Applying scientific literature in the teaching of nutrition and food hygiene is neces-sary. It can not only deepen students' understanding of the course and improve students' English level and scientific research ability, but also can improve teachers' professional skill. To improve the effi-ciency of teaching, teachers should choose literature close to the course and pay attention to selecting review literature and English literature. Textbook should be principal line of study during the teach-ing. Teachers should control the number and timing of explaining literature and grasp the complexity of literature. In order to facilitate students' understanding, teachers should make full use of related figures and tables.
8.The study on the application of wire-guided supporting catheterization in the clinical urology

ObjectiveTo improve the treatment of urethral stricture and evaluate the applicative effects of wire-guided supporting catheterization in clinical urology. MethodsDuring January 2004 and December 2005, the patients with urethral stricture were dwelled with the catheters using the traditional guideless catheterization (group A). If the dwelling failed,it will be replaced by the wire-guided supporting catheterization using the improved catheters. During January 2006 and December 2009, the patients with urethral stricture were dwelled with the improved catheters using wire-guided supporting catheterization ( group B) straightly. Compared the first-time dwelling success incidence,the incidence of catheter associated urinary tract infection and side-effect events. Then after 1 year, compared the urethral stricture recrudescence and the course of treatment. ResultsThe success incidence of first-time catheterization in group A was 48.98% (24/49) ,and in group B was 97.94% (95/97) ,there was significant difference between 2 groups;The incidence of catheter associated urinary tract infection in group A was 12.24%, and 8. 25% in group B, there was significant difference between 2 groups; As for the course of treatment, group A was 46. 2w,group B was 32.7w;The urethral stricture recrudescence in group A and B were 16.33% and 9.28% respectively,and there was significant difference between 2 groups. ConclusionThe wire-guided supporting catheterization, which minimize the injury and simplify the operation of internal urethrotomy,could makes the improved catheter easy to induct and replace, improve the success rate of first time-catheterization and prevent the false tunnel damage and new scar expansion. It could make benefit to reduce of incidence of catheter associated urinary tract infection and urethral stricture recrudescence,but also could shorten the course of treatment significantly.
9.Effect of dexmedetomidine on HMGB1/TLRs signaling pathway during lung injury in septic rats

Objective To evaluate the effect of dexmedetomidine on high-mobility group box 1 protein (HMGB1)/Toll-like receptors (TLRs) signaling pathway during lung injury in septic rats.Methods Twenty-four SPF healthy adult male Wistar rats,aged 15-18 weeks,weighing 200-250 g,were divided into 3 groups (n =8 each) using a random number table:sham operation group (group S),sepsis group (group Sep) and dexmedetomidine group (group D).Dexmedetomidine 25 μg/kg was intraperitoneally injected in D group,while the equal volume of normal saline was given instead in S and Sep groups.Sepsis was produced by cecal ligation and puncture in Sep and D groups.The rats were sacrificed at 24 h after operation,and the right lung was removed for examination of the pathological changes which were scored and for determination of myeloperoxidase (MPO) activity,content of interleukin-1β (IL-1β),IL-6 and tumor necrosis factor-α (TNF-α) in lung tissues (by enzyme-linked immunosorbent assay),wet to dry weight ratio (W/D ratio) and expression of HMGB1,TLR2 and TLR4 in lung tissues (by Western blot).Results Compared with group S,the MPO activity,lung injury score,W/D ratio,content of IL-1β,IL-6,TNF-α and expression of HMGB1,TLR2 and TLR4 were significantly increased in Sep and D groups (P<0.05).Compared with group Sep,the MPO activity,lung injury score,W/D ratio,content of IL-1β,IL-6,TNF-α and expression of HMGB1,TLR2 and TLR4 were significantly decreased in group D (P<0.05).Conclusion Dexmedetomidine reduces lung injury through inhibiting HMGB1/TLRs signaling pathway in septic rats.
10.Diagnosis effect of thrombelastography in chronic kidney disease

Objective To discuss the diagnosis effect and clinical significance of thrombelastography in chronic kidney disease. Methods From June 2016 to February 2017, two hundred and seventy non-dialysis patients with chronic kidney disease ( CKD) treated in the Fourth Hospital of Hebei Medical University were divided into non-hypercoagulable group and hypercoagulable group according to TEG comprehensive coagulation index. The changes of related clinical indexes between the two groups were analyzed and the related factors affecting the differences between the two groups were studied. Results The correlation between the two groups showed that the coagulation reaction time ( R ) , coagulation formation time ( K ) and albumin in the hypercoagulable group were significantly lower than those in the non-hypercoagulable group ((4. 69±0. 94) min vs. (6. 29±1. 63) min,(0. 93±0. 13) min vs. (1. 51±0. 58) min,(27. 54±7. 81) g/L vs. (34. 26±8. 39) g/L, P= 0. 000 ) Angle angle, maximum thrombus strength ( MA ) , fibrinogen, D-dimer, platelet count, protein/creatinine and protein content in hypercoagulable group were significantly higher than those in non-hypercoagulable group((76. 76±2. 23)°vs. (68. 19±7. 65)°;(75. 13±3. 81)mm vs. (66. 35±7. 81)mm;(4. 28 ±0. 93) g/L vs. (3. 56±1. 10) g/L ;0. 4(0. 15,0. 91) mg/L vs. 0. 22(0. 12,0. 52) mg/L;(276. 03±127. 15) ×109/L vs. (198. 18±78. 46)×109/L;5430(2579,9634) mg vs. 2620(692,5286) mg;4864(2341,7712) mg/g vs. 2557(840,5805) mg/g,P<0. 05). The differences were statistically significant. There was no significant difference in prothrombin, thromboplastin time, thrombin time, total cholesterol, triglyceride, low density lipoprotein,high density lipoprotein,creatinine between the two groups ( P>0. 05) . Correlation analysis of common clinical indicators showed that the comprehensive coagulation index ( CI) was positively correlated with Angle angle,maximum thrombus strength,fibrinogen,platelet count,protein/creatinine and protein quantification (r=0. 532,0. 522,0. 307,0. 354,0. 293,0. 216,P<0. 05),was negatively correlated with coagulation reaction time,coagulation formation time and albumin (r=- 0. 462,- 0. 496,- 0. 360,P<0. 05). Logistic regression analysis showed that platelet count, albumin and fibrinogen were the influencing factors for the grouping of comprehensive coagulation index ( OR ( 95%CI ) :1. 007 ( 1. 002-1. 013 ) , 0. 868 ( 0. 827-0. 912 ) , 1. 510 (1. 042-2. 187),P<0. 05). Conclusion TEG is a more sensitive indicator to reflect the coagulation status of patients with CKD, and has a certain guiding significance for anticoagulation treatment of patients with CKD;platelet count,albumin,fibrinogen are the factors affecting coagulation function of patients.
