1.Clinical application value of recombinant human growth hormone in the treatment of severe burns

Objective To investigate the application value of recombinant human growth hormone ( rhGH) in the treatment of severe burns .Methods 36 patients with severe burns were selected and randomly divided into the observation group and control group ,18 cases in each group .Both two groups used the same anti-infection therapy , nutritional support and surgical methods .The observation group received rhGH subcutaneous injection therapy , the control group used the same amount of daily injections of 0.9%sodium chloride.The levels of tumor necrosis factor-α( TNF-α) and interleukin 6 ( IL-6 ) , wound healing time , hospitalization time and other indicators were compared between the two groups.Results 7d,14d after treatment,the TNF-αand IL-6 levels of the two groups were (18.4 ± 3.3)ng/L vs (24.9 ±7.3)ng/L,(133.2 ±66.9)ng/L vs (271.4 ±88.9)ng/L,(15.4 ±3.2)ng/L vs (25.4 ± 4.9)ng/L,(90.9 ±20.8) ng/L vs (200.7 ±62.2) ng/L,the differences were significant between the two groups (t=3.74,4.75,4.02,4.69,all P<0.05).The healing time and hospital stay in the observation group were signifi-cantly shorter than those of the control group (t=3.75,3.86,4.75,4.64,all P<0.05).Conclusion rhGH therapy can significantly improve the wound healing of severe burn patients ,reduce hospitalization time,it has good clinical results.
2.Shenshao decoction improves myocardial inflammatory injury in diabetic rats by regulation of TLR4/MyD88 pathway

Objective To investigate the improving effects of Shenshao decoction on myocardial structure and function in diabetic cardiomyopathy,and its effect on expression of TLR4/MyD88-dependent pathway signal in diabetic cardiomyopathy.Methods Diabetes mellitus was induced in 8-week-old male Wistar rats by a single intraperitoneal injection of streptozotocin.The changes of plasma myocardial enzyme (CK, LDH) and high sensitive C reactive protein (hsCRP) were measured.Cardiac function was measured by left ventricular intubation.Hematoxylin and eosin staining and electron microscopy were used to observe the changes of myocardial morphology and ultrastructure in rats.Expression of Toll-like receptor 4(TLR4), myeloid differentiation protein 88(MyD88), and nuclear factor-κB P65(NF-κB P65) were tested by immunohistochemistry.Results After 6 weeks of treatment, the left ventricular diastolic and systolic functions were obviously improved;the degrees of myocardial fibers and mitochondrial damage were obviously relieved;the content of CK, LDH and hsCRP decreased (P< 0.05), and hsCPR was positively correlated with CK and LDH (r=0.823,r=0.819,P < 0.05).The expressions of TLR4, MyD88, NF-κB P65 were significantly decreased (P< 0.05).There was no significant difference in the above mentioned indicators between the control group and control+Shenshao decoction group (P> 0.05).Conclusions Shenshao decoction can reduce the myocardial injury in diabetic cardiomyopathy and improve cardiac diastolic and systolic functions.The mechanism may be related to attenuated inflammation by TLR4/MyD88 dependent signaling pathway.
3.Clinical characteristic and perioperative management of colonic obstruction caused by colonic carcinoma in elderly

ObjectiveTo discuss the clinical characteristic and perioperative management of colonic obstruction caused by colonic carcinoma in elderly.MethodsThe clinical characteristic and perioperative management of 30 patients with colonic obstruction caused by colonic carcinoma from January 2000 to November 2010 were collected and analyzed retrospectively.Among 30 cases,11 cases were accepted emergency operation,19 cases were accepted limited operation.Resection and anastomosis at stage Ⅰ was in 26 cases (86.7%),shortcut surgery and simple fistulation was in 4 cases ( 13.5% ).ResultsThe length of stay was 22-56 days.Postoperative complication including wound infection in 5 cases,wound liquescence in 3 cases,wound dehisce in 2 cases,anastomotic fistula in 2 cases,pulmonary infection in 3 cases.Two cases died during perioperative period.Conclusions Besides diagnosis as early as possible,appropriate perioperative management should be taken to the elderly patients,including reasonable operative time and surgical approaches and these are the keys to improve clinical curative effect.
4.Clinical study of laparoscopic cholecystectomy combined with endoscopic retrograde cholangiopancreatography/endoscopic Oddi sphincterotomy on the treatment of cholecystolithiasis complicated by choledocholithiasis

Objective To discuss the clinical value of laparoscopic cholecystectomy (LC) combined with endoscopic retrograde cholangiopancreatography (ERCP)/endoscopic Oddi sphincterotomy (EST) on treating choledocholithiasis with cholecystolithiasis. Method The clinical data of 48 patients with choledocholithiasis complicated by cholecystolithiasis treated with LC combined with ERCP/EST from January 2005 to August 2010 was collected and analyzed retrospectively. Results Forty-six patients achieyed success by ERCP,and 45 patients finished LC,all patients underwent LC combined with ERCP/EST were recovered more rapidly,with shorter hospital stays. No severe complications or residual stone and refluent cholangitis in the follow-up of 3-12 months of 46 patients. Conclusions Combining the advantages of LC with ERCP/EST treating patients with choledocholithiasis complicated by cholecystolithiasis,according to the theoretics of minimally invasive surgery, with less invasive and the advantages of shorter hospital stays and rapid recovery. It is the comparatively ideal choice for the treatment of choledocholithiasis complicated by cholecystolithiasis at present.
5.Clinical use of fast-track surgery in colorectal surgery

Objective To investigate the feasibility of fast-track surgery in colorectal surgery.Methods Eighty consecutive patients with colorectal cancer admitted for colorectal surgery were divided into two groups by random digits table with 40 cases each. Group A was treated with the new concept of fasttrack surgery and group B was treated with the traditional methods of operation. The time of postoperative bowel venting and defecation,hospital stay time, the rate of complication, the rate of readmission and the total cost during hospitalization were compared. Results The time of postoperative bowel venting and defecation,hospital stay time were shorter in group A [(2.7 ± 0.9), (2.9 ± 0.1 ), (5.8 ± 1.0) d,respectively]than those in group B [( 3.9 ± 0.5 ), (4.2 ± 0.3 ), ( 8.3 ± 1.2) d, respectively] and the rate of complication and the total cost during hospitalization in group A [7.5%(3/40), ( 1.83 ± 0.22) ten thousand yuan] were lower than those in group B[27.5%(11/40), (2.35 ± 0.36) ten thousand yuan](P< 0.05). Conclusion The new concept of fast-track surgery can accelerate recovery after colorectal resection,reduce the rate of overall complications and total cost during hospitalization.
6.Tripterine inhibits all-trans retinoic acid-caused adhesion between leukemia cells and endothelial cells

OBJECTIVE: Increasing of adhesion between leukemia cells and endothelial cells during all-trans retinoic acid (ATRA) treatment plays an important role in retinoic acid syndrome. This work observed the effects of tripterine on this ATRA-caused increasing in adhesion. METHODS: The effects of tripterine on ATRA-induced expressions of adhesive molecules in acute promyelocytic leukemia cell line NB4 and human umbilical vascular endothelial cells (HUVEC) were detected by flow cytometry. The effects of tripterine on adhesion between ATRA-treated NB4 and HUVEC were determined by adhesive assays. RESULTS: ATRA caused remarkable elevation of intercellular adhesion molecule-1 (ICAM-1) in NB4 cells, which could be significantly reduced by tripterine (P<0.01). The expressions of E-selectin, vascular cell adhesion molecule-1 (VCAM-1) and ICAM-1 in HUVEC were elevated by conditioned medium from ATRA-induced NB4 (ATRA-NB4-CM) (P<0.01), and inhibited by tripterine with inhibition rates being 25.3%, 42.4% and 61.0% respectively. ATRA increased the adhesion between NB4 and HUVEC, which was reversed completely by tripterine. CONCLUSION: Tripterine can inhibit ATRA-caused adhesion between leukemia cells and endothelial cells, and it might be a potential agent for treating retinoic acid syndrome.
7.Covered Cheatham-Platinum stent implantation for the treatment of coarctation of the aorta:preliminary results in 11 patients

Objective To discuss the efficacy and safety of covered Cheatham-Platinum stent (CCPS) implantation in treating coarctation of the aorta (CoA).Methods The clinical data of 11 patients with CoA who had received CCPS implantation were retrospectively analyzed.Results Successful implantation of single CCPS was accomplished in all 11 patients.After the implantation,the diameter of coarctation site was immediately dilated from preoperative (4.76+0.89) mm to postoperative (12.86+0.90) mm (t=24.86,P<0.001),and the average systolic blood pressure difference across the coarctation was immediately reduced from preoperative (38.55+10.02) mmHg to postoperative (9.82+6.60) mmHg,the difference was statistically significant (t=10.8,P<0.001).The patients were followed up for 3-79 months,with a mean of (31.91±27.58) months.The clinical symptoms were relieved in all patients and the activity tolerance was obviously improved.No complications such as endoleak,acute injury of aortic wall,re-coarctation or re-stenosis,vascular injury of puncture site,or death occurred in all patients.Conclusion For the treatment of CoA,implantation of CCPS has excellent short-term and mid-term effect,meanwhile,this technique can effectively avoid complications such as aortic wall iniurv.(J Intervent Radiol.2017.26:211-214)
8.Study on Range of Motion of Manual Performance for Ergonomics Evaluation

Objective To search for perfect and logical evaluation indexes of range of motion of manual performance and maximal voluntary range of motion. Methods From mechanism of hand movement and characteristics of manual action,some relative comprehensive evaluation indexes on range of motion of manual performance were provided.Twenty six undergraduates (14 male and 12 female)participated in the test for 16 indexes. Results The maximal voluntary range of motion of thumb,four fingers and wrist were obtained. Conclusion The results can provide foundation for ergonomics design of spacesuit gloves.
9.Extended Spectrum ?-Lactamases Genes in Multidrug-resistant Pseudomonas aeruginosa

OBJECTIVE To investigate the situation of prevalence for extended spectrum ?-lactamases(ESBLs)genes in multidrug-resistant Pseudomonas aeruginosa(PA). METHODS Antimicrobial susceptibility test was performed by agar dilution method and 7 kinds of ESBLs genes were detected by PCR methods in 35 strains of PA. RESULTS In 35 strains of PA, the positive rates of genes of TEM, OXA, PER and GES were 51.4%, 42.8%, 31.4%, and 22.9%, respectively, and genes of SHV, VEB, and CTX-M-1 were all negative. CONCLUSIONS This study shows that there are high positive rates of TEM, OXA, PER and GES genes in PA in our hospital. The GES genes in PA are first reported in China.
10.Effects of triptolide-medicated serum on secretion function of adrenocortical cells isolated from rats.

Objective: To study the effects of triptolide-medicated serum on secretory function of adrenocortical cells isolated from rats. Methods: Thirty SD rats were randomly divided into control group, prednisone group, and low-, medium- and high-dose triptolide groups. Rats were administered with normal saline, prednisone and low-, medium- and high-dose triptolide respectively by gastrogavage to prepare sera containing drugs. Primary adrenocortical cells were isolated from normal male rats and cultured with sera containing drug for 48 hours. Expression of proliferating cell nuclear antigen (PCNA) was observed by immunohistochemical method and number of PCNA-positive cells was counted. Ultrastructure of adrenocortical cells was observed under a transmission electron microscope. Content of corticosterone in supernatant of adrenocortical cell culture was detected by enzyme-linked immunosorbent assay, and real-time fluorescence quantitative polymerase chain reaction (PCR) was employed to investigate the expression of 3beta-hydroxysteroid dehydrogenase (3beta-HSD) mRNA. Results: As compared with the control group, content of corticosterone in supernatant of adrenocortical cell culture and expression of 3beta-HSD mRNA were significantly increased in the triptolide-treated groups, and the numbers of PCNA-positive cells were increased in the medium- and high-dose triptolide groups, however, they were decreased in the prednisone group. Conclusion: Triptolide-medicated serum can increase the secretion of corticosterone in rat adrenocortical cells in vitro.
