This study taking gestodene (GEST) as a model, investigated the factors affecting reservoir-type intravaginal ring (IVR)'s drug release. This paper reported a gestodene intravaginal ring of reservoir design, comprising a gestodene silicone elastomer core encased in a non-medicated silicone sheath, separately manufactured by reaction injection moulding at 80 degrees C and heating vulcanization at 130 degrees C is reported. The test investigated the factors affecting drug release through a single variable method, taking the drug release rates of 21 days as standards. When changing the thickness of the controlling sheath outside, the ratio of the first day of drug release and mean daily release (MDR), named the relatively burst effect, is closing to 1 with the thickness of controlling sheath increasing, while the 1.25 mm sheath corresponding to 1.04 controlled the burst release effectively; a positive correlation (r = 0.992 2) existed between the average drug release (Q/t) and drug loading (A) within a certain range. The C6-165 controlling sheath with high solubility of GEST is easier to achieve controlled release of the drug; GEST crystalline power is more effective to implement controlled release of drugs among difficent states of the drug. A 1/4 fractional segment core gives a relatively burst effect of 1.76, while the 1/1 and 1/2 are 1.93 and 1.87 separately, at the same drug loading, concluding that use of a fractional segment core would allow development of a suitable GEST reservoir IVR. In summary, GEST reservoir-type IVR could be adjusted by the thickness of controlling sheath, the loading of drug, the material properties of controlling sheath, the dispersion state of drug, the additive composition and structure of intravaginal ring, to control the drug release behavior and achieve the desired drug release rate.

Objective To study the role of laparoscopic ventrohysteropexy in the treatment of endometriosis with a retroverted uterus. Methods 64 patients with endometriosis diagnosed under laparoscopy were retrospectively analyzed.After performing conservative operative procedures for the treatment of endometriosis,21 patients underwent ventrohysteropexy(group Ⅰ) and 43 patients didn't(groupⅡ). Objective To investigate the clinlic therapeutic result of detenial sigmoid neobladder after radical cystectomy. Methods 50 patients with bladder cancer (41 males and 9 females;age range from 35 to 82 years) were admitted and underwent radical cystectomy and detenial sigmoid neobladder. Results All the 50 patients were followed up for a mean of 18.6 months(range from 6 to 29 months).The blood Cr and BUN levels were both in the normal range without acidosis in all the cases.Only 2 had unilateral ureteral urine reflux during cystography.Of the 50 patients 45 cases(90%) achieved continence during daytime;10 cases(20%) were incontinent at night,but 8 of the 10 cases could control urination by being woken up at night.The capacity of the neobladder was 220 to 550 ml with a mean of 350 ml and the maximal pressure of the neobladder during filling was 15 to 55 cmH 2O(1 cmH 2O=0.098 kPa) with a mean of 35 cmH 2O. Conclusions As a simple operation the detenial sigmoid neobladder can be performed easily with less complications and more reliable results.This operation may be generally applied in clinical practice.

Objective To develop a stable and quick method to determine the urea in swimming pool water.Methods On the conditions of strong acid and calefaction,urea can react on antipyrine and diacetyl monoxime and become yellow which can be determined by colorimetric analysis in 450 nm with the ALLIANCE FUTURA continuous flow injection auto analysis system.Results Determined by this method,the linear range was 0.5-4.0 mg/L,the regression equation was y=0.077 5x+0.021 9,the correlative coefficient r=0.999,the detection limit was 0.07 mg/L.With residual chlorine of 0.3 and 3.0 mg/L,the recovery rates were 98.8%-104.0% and did not disturb the determination.The results determined by the present method were the same as those by the method in GB/T 18204.29-2000(Wilcoxon Signed Ranks Test:Z=0.051,P=0.959).Conclusion Determination of the urea in swimming pool water with continuous flow injection auto analysis,the sensitivity,accuracy and limit of determination can meet to the requirement,moreover,the analysis speed is faster,less reagent will be needed,the present method can be generalized in the laboratory that has the conditions.

ObjectiveTo describe utilizations of maternal health care in migrant maternity in Putuo District and to identify the gap between primipara and multipara.In order to improve utilizations of Maternal Health Care in migrant maternity.MethodsQuestionnaire survey was carried out among lying-in migrant maternity to obtain information on utilization of maternal health care.Database was established the by Epidata 2.1 and Statistical analyzed by SPSS11.0.ResultsThere was significant difference between multipara and primipara in age,household registration and cultural degree(P＜0.01 ).Women registered at early stage of the pregnancy with 50.51％,completed full pre-natal health check-up with 82.83％,received postnatal visits with 78.45％ in migrant maternity.There was significant difference between multipara and primipara in registered at early stage of the pregnancy,full pre-natal health check-up,received the postpartuu visit,registered at first level hospital and the medical insurance coverage in migrant maternity(P＜0.01 ).ConclusionsThe levels of utilizations of maternal health care in migrant maternity in putuo district are significantly lower than local maternity,the levels of utilizations of maternal health care in multipara are lower than primipara in Putuo district.Improve utilizations of maternal health care in multipara and enhance utilizations of maternal health care in migrant maternity in Putuo district.

Objective To explore the effects of physiological ischemia training (PIT) on the function of endothelial progenitor cells.Methods Eighteen male New Zealand rabbits (2.5 ± 0.5) kg were divided into 3 groups:a sham operation (SO) group,a myocardial ischemia (MI) group and a MI ± physiological ischemia training (MI ± PIT) group.The PIT was modeled by ischemic isometric contraction of gastrocnemius muscle induced by electric stimulation (40 Hz,1 ms,40％ maximum current strength),and the MI was modeled by implantation of a water balloon to occlude the left ventrical branch of the heart.The experiment was conducted for 4 weeks.Before and after the experiments,the ability of mobilization and adhesion of endothelial progenitor cells were detected,the number and the capillary density were counted.Results Post-experiment,compared with groups SO and MI,PIT group significantly improved the ability of mobilization (151 ± 16 cells/HPF,P ＜ 0.01) and adhesion (17.8 ± 2.8 cells/ HPF,P ＜ 0.05),increased the circulating amount (P ＜ 0.05) and capillary density (824.0 ± 106.6 /mm2,P ＜ 0.05).Pearson analyze showed that there was positive correlation between circulating EPCs and capillary density.Conclusion Physiological ischemia training could improve EPCs mobilization and function,resulted to neovascularization in the ischemic heart tissues.

Objective To study the effect of Sopfrology childbirth method training,Kegel training and Lamaze respiratory training (SLK training) on depression and sexual life of perinatal pregnant woman..Methods The women in 24 weeks of pregnancy who came to the clinic to receive the pregancy test were divided into study group(n=232) and control group(n=126) according whether they voluntered to participate the study.All the women in two group were subjected to regular prenatal care and the women in study group received SLK training additionally.The depression,pelvic floor muscle tension and female sexual function index were evaluated by Self Rating Depression Scale(SDS),the Pelvic Floor Muscle Examination Grading Standards and the Female Sexual Function Inventory (FSFI) respectively.Results 6 months after delivery,the incidence of depression,SDS score,pelvic floor muscle tension score and live index standard rate in study group and the control group respectivelywere(10.3％ vs 18.3％),((43.6±5.1) vs(45.3±6.3)),((3.36±0.33) vs(2.46±0 89)),(81.5％ vs 65.9％),and the differences were statistically significant(P＜0.05).Conclusion SLK training program for perinatal women has the effects of preventing,treating prenatal depression and postpartum depression,improves pregnancy index and the quality of sexual life,worthy of further study.

Objective By investigating the departments of ECG and the staff in China,to study the current ECG status and thus provide references for training ECG workers,improving the work environment and stabilizing the ECG professional team.Methods One hundred and twenty four domestie grass-root hospitals were investigated through real-name questionnaire surveys.The data was collected and analyzed.Results Among the 124 investigated hospitals,the educational background description is:(1) The ECG workers in provincial or municipal hospitals had mainly been entitled with Bachelor's degree or above accounting for 72.7％ of whole ECG workers;the ECG workers in the county or township hospitals were entitled with college degree or below,accounting for 85.7％ of whole ECG workers.The difference between the two kind of workers was significant(P＜0.01).(2) The proportion of ECG workers owning senior professional titles in provincial or municipal hospitals and county or township hospitals were both low,which were 12.2％ and 3.6％,and the former one was significantly higher than the later(P＜0.01).(3) The proportion of hospitals which established dependent department of ECG was low;the percentage of provincial or municipal hospitals was 23.4％,which was significantly higher than that of county or township hospitals (3.3％,P＜0.05).(4) The provincial or municipal hospitals were well equipped and also conducted various of inspection projects in comparison to county or township hospitals.Conclusions The grass-root ECG staff in China has no satisfying education background and professional titles;ECG department belonging varies and the special inspection projects are insufficient and the economic benefit was low.So this study suggest that the competent departments take measures in improving ECG personnel enthusiasm,improving the working environment,and ensuring the healthy and orderly development of ECG profession.

Objective To investigate the assessment value of diffusion-weighted magnetic resonance imaging (DWI)and apparent diffusion coefficient (ADC)in the differential diagnosis of common cerebellar tumors of children.Methods 45 patients with cerebel-lar tumors were retrospectively analyzed,including 23 medulloblastomas,14 astrocytomas,and 8 ependymomas .The ADCmin of parenchyma of different tumors were measured and Receiver operating characteristic (ROC)curve were delineated.The optimum ADC value for differential diagnosis of common cerebellar tumors of children was analyzed and determined.Results The mean ADC-min of medulloblastoma (0.497 ± 0.023 )× 10 -3 mm2/s was lowest,the mean ADCmin of astrocytoma (1.572 ± 0.145 )× 10 -3 mm2/s was highest,while that of ependymoma was (0.784 ± 0.037 )× 10 -3 mm2/s.The optimum ADC to distinguish ependymomas from medulloblastomas was 0.61 1 × 10 -3 mm2/s (100% sensitivity and 95.7% specificity).The optimum ADC to distinguish low-grade astrocytomas from ependymomas was 1.064×10 -3 mm2/s (92.9% sensitivity and 100% specificity).Conclu-sion ADC is very helpful with differential diagnosis of common cerebellar tumors of children.
