Grey system analysis method was used to study the correlation between water pollution in D Lake area and death rate of malignancy with death rate of malignancy as effect sequence and a variety of water pollution index as factor sequence. 2.Clinical study on relationship between Helicobacter pylori infection and type 2 diabetes complications

Objective To investigate the relationship between Helicobacter prlori (Hp) infection and type 2 diabetes related complications.Methods Eighty-five patients with type 2 diabetes had been tested and divided into Hp negative group (42 cases) and Hp positive group (43 cases),the triglyceride (TG),low density lipoprotein cholesterol (LDL-C),fasting plasma glucose (FPG),glycosylated hemoglobin (HbA1c),tumor necrosis factor-α (TNF-α) and complications occurred were compared between two groups.Results The TG,LDL-C,FPG,HbA1c and TNF-α in Hp positive group was significantly higher than that in Hp negative group [(4.83 ± 0.95) mmol/L vs.(3.03 ± 0.75) mmol/L,(4.99 ± 0.96) mmol/L vs.(3.91 ±0.87)mmol/L,(10.85 ±2.97) mmol/L vs.(8.11 ±2.69) mmol/L,(9.35 ± 1.96)% vs.(7.08 ± 1.53)%,(7.33 ± 0.78) ng/L vs.(3.65 ± 0.98) ng/L] (P< 0.01).The cardiovascular disease,cerebrovascular disease,kidney disease,retinal changes and podiatric complication rate in Hp positive group was 51.16% (22/43),37.21%(16/43),27.91%(12/43),34.88%(15/43),23.26%(10/43),significantly higher than that in Hp negative group [28.57% (12/42),16.67% (7/42),9.52% (4/42),14.29% (6/42),7.14% (3/42)],there was significant difference between two groups (P < 0.05).Conclusion Type 2 diabetes Hp infection can make the higher TG,LDL-C,FPG,HbA1c,TNF-α and can increase the risk of diabetes complications.

4.Effects of Tougu Xiaotong Capsules on the expression of Rac1 and Cdc42 in chondrocytes

BACKGROUND:Tougu Xiaotong Capsule has pretty good clinical therapeutic effect on osteoarthritis of early and middle periods. However, the mechanism of Tougu Xiaotong Capsule is not ful y clarified. The RhoA GTPases can regulate chondrocyte apoptosis and hypertrophy.
OBJECTIVE:To observe the Tougu Xiaotong Capsule on the expression of Rac1and Cdc42 in tumor necrosis factor-α-induced in vitro cultured rat articular chondrocytes, and to explore its mechanism of action for combating osteoarthritis.
METHODS:Knee cartilage of the 4-week-old Sprague-Dawley rats was used to stably establish in vitro culture system of chondrocytes. Passage 3 chondrocytes were identified by toluidine blue staining. Chondrocyte apoptosis was successful y induced by 20μg/L tumor necrosis factor-αand then Tougu Xiaotong Capsule at different dosage (500, 100, 20 mg/L) was given after 24-hour incubation. MTT assay was used to detect cellsurvival, flow cytrometry to measure mitochondrial membrane potential, and western blot assay to determine the protein expression of Rac1, Cdc42, Bax and Bcl-2.
RESULTS AND CONCLUSION:Tougu Xiaotong Capsule could reduce tumor necrosis factor-α-induced apoptosis of chondrocytes to improve the survival rate of the cells, and at the same time, could down-regulate the protein expression of Rac1, Cdc42 and Bax and increase the protein expression of Bcl-2 significantly (P<0.05). Tougu Xiaotong Capsule possibly plays a therapeutic efficacy on osteoarthritis by reducing promote apoptosis Rac1, Cdc42 and Bax expression and increasing apoptosis inhibiting gene Bcl-2 expression, thereby to inhibit apoptosis of chondrocytes.
5.Effects of Tougu Xiaotong Capsule medicated serum on the expression of uPA system and infammatory factors in osteoarthritis synovial cells

BACKGROUND:Tougu Xiaotong Capsule (TGXTC) is a clinical prescription for the treatment of osteoarthritis;however, its mechanism has not been ful y elucidated. Urokinase-type plasminogen activator (uPA) system participating in the degradation of the extracel ular matrix of articular cartilage and hyperplasia of joint synovium plays an important role in the pathological process of osteoarthritis. OBJECTIVE:To investigate the effects of TGXTC medicated serum on the expression of uPA, uPA receptor (uPAR), plasminogen activator inhibitors (PAIs), matrix metal oproteinase-3 (MMP-3), interleukin-1 beta (IL-1β) and tumor necrosis factor-α(TNF-α) in osteoarthritis synovial cel s of rats and to discuss the mechanism by TGXTC medicated serum prevents and cures osteoarthritis. METHODS:Rat models with knee osteoarthritis were established by injecting 4%papain into the knee joint cavity. Primary synoviocytes and osteoarthritis synoviocytes were cultured with col agenase digestion method. The cultured synoviocytes were divided into normal group, model group and TGXTC group. The western blot method was adopted to detect uPA, uPAR, PAI, MMP-3, IL-1βand TNF-αprotein expression of synoviocytes after acting by TGXTC medicated serum for 72 hours. RESULTS AND CONCLUSION:The expression of uPA, uPAR, MMP-3, IL-1βand TNF-αwere decreased, while PAI was increased in the TGXTC group, and there were significant differences when compared with model group. In a word, TGXTC can significantly inhibit the expression of uPA, uPAR, MMP-3, IL-1β, TNF-α, and improve PAI expression in synoviocytes, which may partly explain the mechanism of the treatment of Tougu Xiaotong Capsule on osteoarthritis.
6.Effect ofTougu Xiaotong capsule on articular cartilage changes in rat models of osteoarthritis

BACKGROUND:Tougu Xiaotong capsule is the clinical prescription for the treatment of osteoarthritis, however, its mechanism has not been fuly elucidated. Urokinase type plasminogen activator system which participated in the degradation of the extracelular matrix of articular cartilage and hyperplasia of joint synovium plays an important role in the pathological process of osteoarthritis. OBJECTIVE: To determine the effect ofTougu Xiaotong capsule on urokinase-type plasminogen activator system in knee cartilage tissues of knee osteoarthritis rats. METHODS: Of 144 Sprague-Dawley rats, 120 rats were randomly made into models of knee osteoarthritisvia intra-articular injection of papain, and randomly assigned to model group,Zhuanggu Guanjie Wan group [1.2 g/(kg?d)], low-doseTougu Xiaotong capsule group [0.092 g/(kg?d)], moderate-doseTougu Xiaotong capsule group [0.184 g/(kg?d)] and high-doseTougu Xiaotong capsule group [0.368 g/(kg?d)]. Each group contained 24 rats. Every 2 weeks was considered as a course, with a 2-day interval, totaly 4 courses. The remaining 24 normal rats were included in the blank group. After every two courses, a batch of experimental animals was sacrificed. The pathological changes were observed folowing staining with hematoxylin and eosin. The positive cels of urokinase-type plasminogen activator, urokinase-type plasminogen activator receptor and plasminogen activator inhibitor were measured by immunohistochemistry. The protein levels of urokinase-type plasminogen activator, urokinase-type plasminogen activator receptor and plasminogen activator inhibitor were measured by western blot assay. RESULTS AND CONCLUSION:Mankin’s score was significantly lower in theTougu Xiaotong capsule group and Zhuanggu Guanjie Wan group compared with the model group (P < 0.01), in a time-dependent manner. Immunohistochemical staining indicated that the positive cels of urokinase-type plasminogen activator and urokinase-type plasminogen activator receptor were significantly decreased, but plasminogen activator inhibitor was significantly increased in theTougu Xiaotong capsule group andZhuanggu Guanjie Wangroup in a time-dependent manner. Western blot assay results had an identical trend to immunohistochemistry. These indicated thatTougu Xiaotong capsule showed preventive and therapeutic effects on osteoarthritis by regulating urokinase-type plasminogen activator system.
7.Electroacupuncture delays articular cartilage degeneration in osteoarthritisvia Ras-Raf-MEK1/2-ERK1/2 signaling pathway

BACKGROUND:Previous studies have found that electroacupuncture can delay articular cartilage degeneration mediated by JAK-STAT signaling pathway through upregulating the expression level of transforming growth factor β1 as well as mRNA expression levels of STAT3, Smad3 and LepR. In the meanwhile, electroacupuncture can inhibit the mRNA expression of p38 and Fas mRNA mediated by MAPK signaling pathways, further inhibiting the apoptosis of chondrocytes. OBJECTIVE: To explore the effect of electroacupuncture on the degeneration of articular cartilage in rats with knee osteoarthritis based on Ras-Raf-MEK1/2-ERK1/2 signaling pathway. METHODS:120 male healthy Sprague-Dawley rats aged 2 months olds were selected and randomly divided into normal, model, 15-minite electroacupuncture and 30-minute electroacupuncture groups (n=30 per group). The rats in the latter three groups received the intra-articular injection of 4% papain bilaterally, and the remaining rats received no intervention. At 2 weeks after modeling, the latter two groups were respectively given 15- and 30-minute electroacupuncture, five times weekly for consecutive 12 weeks. The morphology of the cartilage was observed by hematoxylin-eosin staining, the expression level of interleukin-1β in the synovium was detected by ELISA assay, and the protein expression levels of Ras, Raf, MEK1/2, ERK1/2, C-MYC, C-FOS, and C-JUN were detected by western blot analysis. RESULTS AND CONCLUSION: Hematoxylin-eosin staining showed that: in the model group, the cartilage surface was rough, the cartilage layer became thinner, and the cartilage structure was damaged with incomplete tidal line; in the 15- and 30-minute electroacupuncture groups, the cartilage structure was complete with clear layers and complete tidal line. ELISA showed that the expression level of interleukin-1β in the model group was significantly higher than that in the normal group (P< 0.01), and the level in the 15- and 30-minute electroacupuncture groups was significantly lower than that in the model group (P < 0.05). Western blot assay found that compared with the normal group, the protein expression levels of Ras, Raf, MEK1/2, ERK1/2, C-MYC, C-FOS, and C-JUN were increased in the model group. However, all above protein levels except ERK1/2 in the 15- and 30-minute electroacupuncture groups were significantly lower than those in the model group (P < 0.01,P < 0.05). To conclude, electroacupuncture inhibits the degeneration of articular cartilage in osteoarthritisvia Ras-Raf-MEK1/2-ERK1/2 signaling pathway and downregulating the expression level of interleukin-1β.
8.The characters of lower urinary tract symptoms in patients with Parkinson's disease

Objective To explore the characters of lower urinary tract symptoms (LUTS) in patients with Parkinson's Disease (PD).Methods From Oct 2013 to Jun 2016,after evaluating of movement disorder by modified Hoehn-Yahr(H-Y) scale and LUTS by international prostate symptom score (IPSS),urodynamic study was performed in PD patients with LUTS.The incidence of every symptom of LUTS,the relationships between the IPSS categories and urodynamic study were analyzed.Results 64 patients (containing 26 male and 38 female) with 40-80 (62.7 ± 10.2) years old were included.2,4,30,19,12 and 6 patients were belonged to modified H-Y scale 1-4,respectively.Frequency (50 patients,78.1%) was the most common LUTS,while frequency all day (20 patients,31.3%) was the most common symptom for the most severe LUTS.IPSS was 1 7.5 ± 7.8 (4-35) and quality of life was 5.1 ± 0.6 (4-6) for the patients.There was no significant correlation between modified H-Y scale and IPSS (P > 0.05).According to the criteria of IPSS,28 patients (43.8%) only had irritative symptoms,3 patients (4.7%) only had obstructive symptoms,while 26 patients (40.6%) had mixed symptoms and 7 patients (10.9%) belonged to no one.Urodynamic study showed 11 patients (39.3%) with only irritative symptoms had detrusor overactivity(DO),6 patients (23.1%) with mixed symptoms had DO + bladder outlet obstruction (BOO) or DO + detrusor underactivity,however,there was no one with BOO in the three patients with only obstructive symptoms.Conclusion Frequency was the most common LUTS,while frequency all day was the most common symptom for the most severe LUTS in PD patients.Irritative and mixed symptoms were common in PD patients with LUTS,but the urodynamic results were not consistent with the category of LUTS in most of the patients.LUTS severity was not correlated with movement disorders in PD patients.
9.Cohort Study on the Effectiveness and Safety of Tiapride Combined with Clonidine versus Tiapride and Clonidine Alone for Children with Tic Disorders

OBJECTIVE:To compa re the effectiveness and safety of three regimens of tiapride ,clonidine and tiapride combined with clonidine in the treatment of tic disorder (TD)in children. METHODS :A sequential collection of 312 children with TD from the outpatient department of West China Second Hospital of Sichuan University were conducted during Jan.-Dec. 2019. They were divided into clonidine group ,tiapride group ,tiapride combined with clo nidine group ,with 104 cases in each group. Tiapride group was given Tiapride hydrochloride tablets with initial dose of 50-100 mg per day ,and the dose was gradually increased to 150-500 mg per day according to tolerance and clinical experience. Clonidine group was given Clonidine transdermal patches ,once a week ,with initial dose of 1 mg each week ,maintenance dose of 1-2 mg each week ,once a week. Tiapride combined with clonidine group was given Tiapride hydrochloride tablets (same usage and dosage as tiapride group )+ Clonidine transdermal patches (same usage and dosage as clonidine group ). The treatment course of 3 groups was 3 months. After the treatment ,they were followed up every 3 months(the following were expressed as 24,36 and 48 weeks after treatment ). Yale global tie severity scale (YGTSS)scores of 3 groups were observed before treatment ,after 4,8,12,24,36,48 weeks of treatment,and the occurrence of ADR was recorded at different follow up time points. RESULTS :Before treatment ,there was no statistical significance in YGTSS scores among 3 groups(P>0.05). After 4,8,12,24,36 and 48 weeks of treatment ,YGTSS scores of 3 groups were significantly lower than those before treatment (P<0.05). After 4,8 and 12 weeks of treatment ,YGTSS scores of tiapride combined with clonidine group were significantly lower than tiapride group and clonidine group (P<0.05),while there was no statistical significance between tiapride group and clonidine group (P>0.05). At 24 weeks of treatment ,YGTSS score of children in tiopride combined with clonidine group was significantly lower than tiopride group (P<0.05),but there were no significant differences between tiopride combined with clonidine group and tiopride group ,and between tiopride group and clonidine group (P>0.05). After 36 and 48 weeks of treatment ,there was no significant difference in YGTSS scores among 3 groups(P>0.05). After 12 weeks of treatment ,the results of P value corrected by Bonferroni method showed that YGTSS score of tiopride combined with clonidine group was significantly lower than those of tiopride group and clonidine group (P<0.016 7), while there was no statistical significance in the difference between tiopride group and clonidine group (P>0.016 7). There was no statistically significant difference in the total incidence of ADR among 3 groups(P>0.05). CONCLUSIONS :Clonidine,tiapride and tiapride combined with clonidine can significantly improve the tic symptoms of TD children with good safety .
10.Study on the Medication Compliance of Children with Tic Disorder Based on the Theory of Protective Motivation

OBJECTIVE:To study the current status and influencing factors of medication compliance in children with tic disorder(TD),and to provide reference for improving medication compliance in TD children. METHODS:The questionnaire was designed according to the protection motivation theory. The cross-sectional study was adopted to conduct questionnaire survey among TD children in West China Second Hospital of Sichuan University from Jan. 2018 to Dec. 2019. The structural equation model was established according to the theoretical assumptions,and the maximum likelihood method was used to estimate the model;multiple linear regression analysis was carried out for the factors with significant influence in the single factor analysis,and path analysis and intermediary effect test were carried out. RESULTS:A total of 317 patients with TD were included,the mean age was(8.38±2.54)years,and the mean course of disease was(3.19±2.46)years. Average medication compliance scores was (5.70±1.69),among which 15.1% was low compliance,37.5% moderate compliance,and 47.3% high compliance. Multivariate linear regression analysis showed that comorbidities(β=0.124,SE=0.167,P=0.011),education level of the main guardian(β= 0.236,SE=0.110,P<0.001),quality of life(β=0.399,SE=0.112,P<0.001)and the types of drugs taken(β=0.166,SE= 0.047,P=0.001)were the factors affecting medication compliance of children with TD. Structural equation model analysis showed that severity(β=0.295,95%CI:0.103-0.493),external return(β=0.830,95%CI:0.662-1.002),self-efficacy(β=0.200,95%CI: 0.057-0.353),susceptibility(β=0.220,95%CI:0.084-0.352)and quality of life(β=0.353,95%CI:0.211-0.500)had a direct positive impact on medication compliance. Quality of life mediated between external returns and compliance variables(intermediary effect accounted for 13.9% of the total effect value). CONCLUSIONS:Children with TD have low medication compliance. It is recommended that pediatricians in medical institutions at all levels to manage the medication compliance of patients with TD from the severity,susceptibility,external returns and self-efficacy,so as to improve patients and guardians’awareness of the severity and susceptibility of disease and medication non-adherence,weaken external returns and increase self-efficacy,and ultimately improve medication compliance of patients
