Objective To investigate the role of double stapling device in anus-preserving operation of low rectal cancer. Methods Retrospective study was used to analyze the experience of double stapling device in anus-preserving operation of low rectal cancer for 79 patients between March 1999 to March 2003. Results 79 cases in the group all had the radical resection, no patient died. Complications included anastomosis fistula in 2 patients(2.5%),anastomosis stenosis in 3 patients(3.8%). The defecation function after surgery,the patients who received colonic Jpouch or transverse coloplasty pouch were much better than those not did. Five years survival rate 63.3 %, anastomosis recurrence rate 10.1% and recurrence rate in the pelvic soft tissue was 5.1%. Conclusion Anus-preserving operation of low rectal cancer can accomplished with double stapling device,and five years survival rate and local recurrence rate did not effect. Colonic J-pouch or transverse coloplasty pouch is good for improving the defecation function after anus-preserving operation.

Objective To evaluate the spiral CT finding and diagnostic value of gastrointestinal stromal tumor.Methods The CT signs of gastrointestinal stromal tumor proved by pathologically and surgically in 25 cases were retrospectively analyzed .Results There were 9 cases of benign stromal tumor and 16 cases of malignant stromal tumor,including stromal tumor of stomach(18 cases),stromal tumor of esophagus(1 cases),stromal tumor of jejunum(4 cases) ,ileac stromal tumor(2 cases).All of them were single lesions,CT manifestations :(1) Plain scans demonstrated roundness or analogy-roundness mass,the edges of 8 cases with benign stromal tumor displayed clearly.The edges of 11 cases displayed unclearly in 16 cases of malignant stromal tumor and 7 cases of malignant tumors were lobulated,2 cases appeared small calcifications;(2 ) After contrast medium injection,there was enhancement in all cases.Enhancement was more obviously in arterial phase,however,the enhancement in venous phase was more extensive than that in arterial phase.Conclusion helical CT is a meaningful way to check patients with gastrointestinal stromal tumor.Helical CT dual-phase enhancement plays a significant role in judging benign or malignant stromal tumor,and has great value in clinic applications.

Objective To study the relationship between magnetic resonance imaging (MRI)of rheumatoid arthritis (RA)and traditional Chinese medicine (TCM)syndrome types.Methods 90 patients with RA were select-ed as the research subjects.Through 2 attended doctors more strict according to the specific diagnostic criteria of the syndrome,the implementation of the diagnosis of TCM syndromes.The conventional treatment of the patients were recorded,and the X line of the two hands joint was obtained by double blind reading.At the same time,the relevant symptoms and symptoms of the laboratory and other related information were analyzed.Results Of 90 RA patients, 22.22% patients were cold dampness stasis type,27.78% patients were damp heat blocking collaterals,26.67%patients were cold and heat syndromes,23.33% patients were deficiency of liver and kidney and phlegm stasis stasis type.Patients after the initial diagnosis,hands joint implementation hands joint side position was detected by MRI,the MRI results showed thickened synovium,bone erosion,bone marrow edema and joint effusion,and the constituent ratio of MRI images had significant difference (χ2 =13.372,P <0.05).Four TCM syndrome types of RA patients,after treatment,again implement hands joint anteroposterior and lateral radiographs of the MRI detection,after detection of radiology hands joint MRI detection results showed that implementation stage and detailed records,and at the time of initial diagnosis.The patients'hands joint MRI examination test showed that the control,any bone damage intensifies, the MRI signs of more serious progress.However,impaired bone level unchanged,MRI without aggravate was not changed.Conclusion For RA patients,MRI manifestations and TCM syndrome type has relationship.At early stage of the disease,active and reasonable use of anti -rheumatic drug treatment to improve the condition of RA can reduce the morbidity.

OBJECTIVE:To evaluate the status quo and trend of the use of anticancer drugs in our hospital.METHODS:The anticancer drugs used in our hospital from July 2004 to June 2007 were analyzed in respect to the consumption sum and DDDs etc.RESULTS:The consumption sum of anticancer drugs increased year by year,with anticancer assistant drugs showing high proportion,and some new anticancer drugs derived from natural plants and antimetabolites have been increasingly used in the clinic.CONCLUSION:The variety of the anticancer drugs in our hospital is stable,and the anticancer drugs derived from natural plants,the monoclonal antibody medicine and the genetically engineered drug are expected to become the predominant therapy for cancer treatment.

OBJECTIVE:To evaluate the status quo and the tendency of three-stage analgestics for patients with cancer in our hospitals.METHODS:The use of three-stage analgestics for patients with cancer during the period2002～2005in our hospital were analyzed statistically in respect to consumption sum,DDDs and average daily costs,etc.RESULTS:The con-sumption sum and DDDs of three-stage analgesics increased year on year,of which,morphine and related drugs had gradually dominated the first places.The prices of sustained and controlled release preparations were high.CONCLUSION:The use of three-stage analgestics for patients with cancer in our hospital was rational on the whole,yet the application of three-stage analgestic therapy for pain relief in cancer patients should be further publicized.

This article has reviewed the invasion,prevention and cure situation and the latest research progresses of "bird flu"(flu carried by birds: found in Hong Kong in late 1997)with high pathogenicity on human beings in Hong Kong since 1997.By analyzing the invasive regularity of "bird flu"on human beings with the theory of traditional Chinese medicine,it was considered that H5N1 virus was attributed to the "fire" "smelly"poison,the epidemic poison got into the human body from the mouth,noses,and eyes.It attacked the lung first,and then got down to the stomach and intestine,direct attack of pericardium,consumption of body-fluid with wind symptom.It mainly attacked the children,youths and adults in crowd.Clinically,state of evil domination considered as sthenia-syndrome was the common disease type.The author studied out the modified Puerariae and Scutellariae and Coptidis Decoction as the main reference formula in treating "bird flu"with high pathogenicity on human beings and avians.

Objective To study the effect of off-pump coronary artery bypass on heart and lung function of patients.Methods 206 patients were randomly divided into the observation group (n=103 cases)and the control group(n=103 cases).The patients in the observation group were operated through off-pump coronary artery bypass, while the patients in the control group were operated through on-pump coronary artery bypass .Left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD),plasma N-terminal B-type natriuretic peptide(NT-proBNP) and pulmonary function were measured in two groups .Results The postoperative LVEDD and NT-proBNP of the two groups significantly increased ,and the control group increased more significantly (t=3.226,2.726,all P<0.05).Postoperative lung function were significantly lower in both groups ,and compared with the observation group , postoperative lung function in control group decreased more significantly (t=2.620,2.094,3.016,all P<0.05). Conclusion Off-pump coronary artery bypass not only was less trauma ,but also affect small for the heart ,lungs.

Objective To evaluate the effectiveness and safety of Xuebijing (XBJ) injection in therapy of sepsis.Methods It is a prospective,randomized controlled trial conducted in multicenter.Totally 731 patients with sepsis were enrolled from January to December 2011 at 37 centers and randomly (random number) separated into the XBJ group (n =392) and the control group (n =339).Basic therapy was adoptd in the both groups,but XBJ injection was additionally used in the XBJ group.The effectiveness of XBJ injection was evaluated by comparing the differences of MEDS score,APACHE Ⅱ score,DIC score,SOFA score,coagulation parameters,duration of mechanical ventilation and antibiotic therapy between the two groups.The influence on the outcome of XBJ injection was assessed by mortalities of 28-day and 90-day.The side-effects of XBJ injection were recorded.Results The score of each system and the abnormal rates of coagulation parameters were significantly higher in the XBJ group than in the control group at admission (P ＜ 0.05).After combined treatment of XBJ injection and basic therapy,the score values of the XBJ group decreased distinctly,and the differences of pre-therapy and post-therapy in the two groups had statistical significance (P＜0.01).The abnormal rates of coagulation parameters in the XBJ group decreased after therapy.The 90-day mortality of the XBJ group was lower than the control group (P ＜ 0.05).No severe side-effects were found during study.Conclusions This study proved that the severity of sepsis can be alleviated by XBJ injection.Meanwhile,the organ function and the coagulation status can be protected,and then the prognosis can be improved.The safety of XBJ injection was good.

Objective To compare the point-of-care testing (POCT) and laboratory testing of myocardial damage markers in the diagnosis and prognosis of acute coronary syndrome (ACS).Methods A total of 3467 patients with ACS who were treated in the Emergency Department Beijing Chaoyang Hospital Affiliated to Capital Medical University between January 1,2006 and June 30,2010 were retrospectively reviewed.The patient demographics (age,sex,past medical history and smoking history) and laboratory testing results (myocardial damage markers,D-dimer,NTproBNP,and ejection fraction [EF]) were analyzed.The patients who received POCT or laboratory testing of myocardial damage markers were compared with regard to emergency department stay (i.e.,the time from the emergency visit to interventional or conservative treatment),cardiovascular events during hospitalization (congestive heart failure,ventricular fibrillation,and cerebrovascular disease),and 28-day mortality rate.Results The emergency department stay,incidence of a cardiovascular event,and 28-day mortality in the POCT group were all lower than that in the laboratory testing group (P =0.000).A prolonged emergency department stay result in an increased incidence of 28-day mortality.The higher level of D-dimer and decreased EF prompted an increased incidence of 28-day mortality.Conclusions Compared with conventional laboratory testing,POCT can significantly shorten the length of an emergency department stay for an ACS patient,decrease the incidence of a cardiovascular event,and improve the prognosis.
