1.Diagnostic value of MSCT in cystic nephroma

Objective To discuss the diagnostic performance of MSCT in cystic nephroma(CN).Methods The MSCT findings of CN in 7 patients proved by operation and pathology were retrospectively reviewed.Results All of 7 cases were isolated cystic lesion, including 5 males and 2 females.MSCT scanning showed cystic tumor in the renal parenchyma with well-defined boundary.All cases showed regular cystic wall and septa with no attached cystic wall nodule.The cystic wall and septa appeared homogeneous enhancement on contrast-enhanced CT,while the cystic space showed inhomogeneous density and lack of enhancement.Conclusion The MSCT findings of CN may benefit the definite diagnosis.
2.STUDY ON DIETARY LIPID INTAKES IN CHINESE RESIDENTS

Objective: To study the amount of fat, cholesterol and fatty acid intake in China and provide the basic material for dietary guidance. Methods: Two areas were selected both in North and South China and each area included 3 provinces, or municipality or autonomous region. Three representative survey sites were selected in each province, or municipality or autonomous region. Dietary survey was conducted by the method of weighing and recording and cooking method was also recorded. The amount of food consumption was calculated as standard person (adult male, light physical activity). All foods were gathered as 12 kinds of foods, and each kind of food was cooked and then mixed. The content of fat and fatty acid was analyzed for 8 kinds of foods and the content of cholesterol was analyzed for 4 kinds of foods. The intake of fat, fatty acid and cholesterol per capita was calculated. Results: The amount of fat intake among North I, North II, South I, South II was 70.5 g, 46.5 g, 58.7 g, 71.0 g respectively and the amount of cholesterol intake was 329.6 mg, 128.5 mg, 400.9 mg, 306.0 mg respectively. The main source of dietary fat was from meat and vegetables. Egg was the main source of dietary cholesterol and meat and egg were both the main source of dietary cholesterol in south II area. About 90% of saturated fatty acid was palmitic acid and stearic acid and 90% of monounsaturated fatty acid was oleic acid. Linoleic acid was the principal n-6 polyunsaturated fatty acid and linolenic acid was the principal n-3 polyunsaturated fatty acid. The ratio of S∶M∶P in North I was 1∶1.1∶1, in North II was 1∶1.6∶1.3, in South I was 1∶1.6∶1.3 and in South II was 1∶1.5∶1. Conclusion: The amount of fat intake and fatty acid profile was quite different among different areas and the dietary guidance should be more pertinent The current cholesterol intake was more than dietary guidance in most areas. In addition to egg, meat was also an important source of cholesterol.
3.Clinical significance of simultaneous determination of serum creatine kinase isoenzyme and creatine kinase ratio and cardiac troponin Ⅰ in disease condition judgment of patients with acute organophosphorus pesticide poisoning

ObjectiveTo investigate the changes of serum creatine kinase isoenzyme (CK-MB) and creatine kinase (CK) ratio(CK-MB/CK) and cardiac troponin I(cTnI) simultaneously in patients with acute organophosphorus pesticide poisoning (AOPP) and the clinical value in disease condition judgment.Methods Eight hundred and sixty-eight AOPP patients without disease history of heart,lung,brain,kidney,muscle and other connective tissue were extracted 4 ml of fasting venous blood to determine CK,CK-MB and cTnI simultaneously at 2,12,24,48,96 and 120 hours after poisoning,and the CK-MB/CK was calculated.There were 279 mild poisoning cases (mild poisoning group),289 moderate poisoning cases (moderate poisoning group) and 300 severe poisoning group (severe poisoning group) ; and 208 cases with intermediate myasthenia syndrome (IMS)(IMS group) and 660 cases without IMS (non-IMS group).The result was compared with 288 healthy people (control group) in the same period.Results ( 1 ) The CK,CK-MB and cTnI of mild,moderate and severe poisoning groups between 2 to 12 h,13 to 24 h,25 to 120 h after poisoning were significandy higher than those in control group,CK-MB/CK was obviously lower (P < 0.01 ).The CK,CK-MB,cTnI and CK-MB/CK had significant differences between each two of mild,moderate and severe poisoning groups (P< 0.01 or < 0.05).The CK,CK-MB and cTnI of IMS group in 2 to120 h after poisoning were significantly higher than those in non-IMS group,and CK-MB/CK was lower (P < 0.05 or < 0.01 ).In IMS group,the rates of sudden cardiac death (SCD) and multiple organ dysfunction syndrome (MODS) were 23.08%(48/208),37.50%(78/208) respectively,while 4.24%(28/660),9.85%(65/660) in non-IMS group.The incidence of SCD and MODS of two groups had statistical significance (P < 0.01 ).(2) According to IMS and non-IMS group,discriminant equation with independent variables of cTnI (X1), CK (X2) and CK-MB/CK (X3) was established:Y =-0.0014X1 + 0.0225X2 + 65.2376X3,F=21.4911,P < 0.01.The discriminant critical value(Y0) was 2.8124.If Y < 2.8124 belonged to IMS group and Y ≥2.8124 was in non-IMS group,the contribution rates of X1,X2 and X3 were 34.5%,25.4% and 40.1% respectively.The coincident rate of return was 91.26%.ConclusionsCK-MB/CK change has negative correlation with poisoning degree of AOPP;cTnI level is positively correlated with AOPP poisoning degree.they may be used to assist the clinical classification,disease judgement and to guide the emergency,treatment and prognosis assessment of AOPP.The function equation with cTnI,CK and CK-MB/CK can be used as effective prediction indexes of IMS,MODS and SCD in early period.
4.Study on the changes of gonadotropin releasing hormone agonist in pinopodes

Objective To study the changes of gonadotropin releasing hormone agonist (GnRH-a) in pinopodes during luteal phase and to explore the possible mechanism of GnRH-a in luteal phase support of assisted reproductive technology (ART).Methods Totally 40 primary infertility women who were treated with ART due to male factors were enrolled,according to the order of the group they were randomly divided into experimental group and control group.On the 7th day after ovulation,the experimental group received a subcutaneous injection of 0.1 mg of GnRH-a,while the control group received a subcutaneous injection of placebo only (0.9% salinc 2 ml),3 days later they came to the clinic again.Serum estradiol and progesterone levels were measured before and after treatment in each group.Pinopodes were collected for electron microscopic examination.Levels of ER and PR were detected by western blot.Results (1) There was no significant difference between the experimental group and the control group in the estrogen level before and after the treatment (all P>0.05).The level of progesterone in the experimental group after treatment [(66.8± 14.9) nmol/L] was significantly higher than that before treatment(P<0.05);also significantly higher than the same period of the control group (P<0.05).(2) There was no significant difference in the expression of ER protein in the experimental group before and after treatment (P>0.05).The expression of PR in the experimental group after treatment was significantly lower than that before treatment (P<0.05);also lower than the same period of the control group (P<0.05).(3) Expression amount of pinopodes in the experimental group after treatment was significantly higher than that before treatment [65% (13/20) versus 25% (5/20),P< 0.05],and the development trend was more mature [the percentage of maturation:75% (15/20) versus 35% (7/ 20),P<0.05].Expression amount of pinopodes after treatment and the percentage of maturation in the experimental group were significantly higher than those in the same period of control group (P<0.05).Conclusion GnRH-a in luteal phase support may play a role through the corpus luteum,which may promote the secretion of progesterone,downregulation of PR expression,promote the growth of pinopodes,and improve the endometrial receptivity.

6.Investigation and Analysis on Status of Rehabilitation Services in Comprehensive Hospitals of Capital Cities in China Mainland

Objective To investigate the status of the establishment and service of the rehabilitation departments in comprehensive hospitals in China mainland.Methods A questionnaire survey was conducted in 240 comprehensive hospitals with different grades distributed in the capital city of 31 provinces, autonomous regions, and municipality cities in China mainland in a stratified sampling way. The contents investigated are the settings of rehabilitation departments, the performances of the rehabilitation services, the staffing of the rehabilitation workers and the problems faced in rehabilitation. Then, statistical analysis were done between the developed eastern regions and economically less developed middle-western regions, as well as different grades of comprehensive hospitals. Results In total, 216 valid questionnaires were collected (72 tertiary hospitals, 72 secondary hospitals and 72 primary hospitals), and there are no statistically difference between eastern regions and middle-western regions(P=0.61). About 69% of the hospitals surveyed have built up the rehabilitation departments, and there are no statistically difference between different regions (P=0.30). As far as the performance of the rehabilitation services are concerned, 97.2% of them have carried out rehabilitation service, 60% of them can satisfy the demands of the patients, and there are no difference between the different grades and between the eastern and middle-western regions(P>0.05). In addition, 76.4% of them have rehabilitation doctors, 68.4% have physiotherapists(PT), 52.3% have traditional Chinese medicine doctors, 50.5% have occupational therapists(OT), 36.9% have speech therapists(ST), 17.8% have psychologists, and only 2.3% have social workers. The staff status on PT, OT and ST of the hospitals in eastern regions are significantly better than that in middle-western regions(P<0.01), and the staff status on rehabilitation doctors, PT, OT, ST and psychologists in the tertiary hospitals are significantly better than that in the secondary hospitals and primary hospitals(P<0.01). Finally, the first five problems the rehabilitation department faced are following: short of professional workers(63.9%), lack of training areas(51%), lack of modern rehabilitation service management system (50.5%), out-of-date equipments(49.5%) and low fees of rehabilitation treatment(49.1%). Conclusion Many problems exists in the rehabilitation departments of comprehensive hospitals, such as the development of rehabilitation departments, the professional workers, the quality and width of the service, the efficient use of rehabilitation resources, rehabilitation management and profession permission, the relative policies and regulations, personnel training and so on. Therefore, the government should enhance to put the supported policies into practice, and reinforce investment to the comprehensive hospitals, in particular, enhance the support of the comprehensive hospitals in middle-western regions, as well as the primary hospitals and the secondary hospitals, all of which will play a much more efficient role in rehabilitation for the disabled in the comprehensive hospitals.

StrongpositiveSTAT6stainingwasobservedforthenuclearoftumorcells.Conclusion ItisdifficulttodifferentiateSFT/HPCfrom meningeoma.The"yin-yang"patternonT2WIisthecharacteristicofSFT/HPC.Inaddition,nuclearpositivestainingofSTAT6isalsospecific featureofSFT/HPCcell.
8.A bibliometric analysis of researches on intensive care unit physical environment from 2013 to 2017

Objective? To retrieve and analyze the related researches on intensive care unit(ICU) physical environment from 2013 to 2017, and to provide reference for further improvement of the physical environment and quality of medical and nursing care in ICU. Methods? Relevant literature on ICU physical environment included from 2013 to 2017 by the China National Knowledge Infrastructure(CNKI), Wanfang Data Knowledge Service Platform, VIP Database for Chinese Technical Periodicals, Sinomed, Pubmed, Web of Science,and Scopus database were retrieved and analyzed bibliometrically on the internal and external features of the articles by Excel and CiteSpace software. Results? A total of 287 domestic and oversea papers on ICU physical environment were retrieved, the number of published articles showed an overall stable trend. The biggest number of published articles went to Guangdong Province, accounting for 14.1% of the articles in Chinese language. The United States had the biggest number of published papers in English language (43.8%). Literature journals mainly focused on infection control, critical medicine and critical care nursing journals, with the largest number of articles published by a single author(23.0%). The main types of research were clinical research (79.1%), fund-supported literature accounted for 26.5% of all the articles, and research directions were mainly about infection (40.0%), "nosocomial infection" and "pathogenic bacteria", "environment" and"object surface", "premature infant" and "growth and development", "sleep" and "noise" were closely related. Conclusions? The researches on ICU physical environment involves aspects including object surface, noise, light, air quality, temperature and ward design. Infection control in ICU environment and the influences of ICU physical environment on patient's biological rhythm and the growth and development of preterm infants are hot research issues in recent years. However, the geographical distribution of research is not balanced,and the overall quality of research evidence needs to be improved.
9.Cluster analysis of vital signs of critical patients in emergency department during intra-hospital transport

Objective:To explore a classification method for critically ill emergency patients undergoing intra-hospital transport based on cluster analysis, and analyze the changes in vital signs between different categories, so as to provide references for targeted nursing interventions.Methods:Using the convenient sampling method, a total of 416 critically ill patients who were transferred from emergency department to ICU in 2 hospital areas of Beijing Tongren Hospital, Capital Medical University from January to December 2021 were selected as the research objects. General data and vital signs before and after transport were collected. Five vital signs, including body temperature, heart rate, respiratory rate, systolic blood pressure and peripheral capillary blood oxygen saturation measured at the last time before transport, were taken as cluster variables. Patients were classified based on second-order cluster analysis and compared among different categories.Results:The 416 patients included in this study could be divided into 3 categories, such as stable group, low oxygen saturation group and high heart rate group, and the clustering contour coefficient was 0.30. The systolic blood pressure before and after transport of the three groups were compared, and the differences were statistically significant (all P<0.05). Moreover, there was a statistically significant difference in respiratory frequency between the low oxygen saturation group and the high heart rate group (both P<0.05) . Conclusions:It was of great significance to monitor the vital signs of the critically ill patients transferred in the hospital, and the classified management of patients can better guide the clinical practice of transportation.
10.Isolation and cultivation of goat embryo stem cells.

Morulaes and blastocysts obtained from Guanzhong dairy goats 6-7 days after mating were treated with whole embryo cultivaton, enzymatic digestion and immunosurgery separately. The goat embryonic stem cells (ESC) were isolated and cultured on a feeder layer of mitomycin-inactivated mouse embryo fibroblasts (MEF). The characteristics of goat ESCs were analyzed by immunohistochemisty, RT-PCR and inducing differentiation in vitro. The results indicated that the embryos were easier to attach the culture dish and form primary colonies with whole embryo method. There were colonies that maintained undifferentiated for 18 passages. The ESCs expressed the protein of Nanog, Oct4 and SSEA-3, whereas the protein of SSEA-4 was absent and the protein of SSEA-1 was weakly expressed. In addition, the genes of Nanog, Oct4, TERT and CD117 were expressed in goat ESCs. The cells also could differentiate to myocardial cells when induced in vitro by DMSO. These results suggest that the goat ESCs have characteristics of ESCs.
