Methods: After the herbs was extracted and refined, micropills was produced with pectin selected as coating material and thallium monochloride was as internal standard substance. Results: The preparation was testified to attain schedule colon target. Conclusions: This research provided a way for traditional Chinese drug preparation of colon targeting.
3.Drug Resistance and Pathogens in General Surgery Department:Analysis of 158 Cases

OBJECTIVE To understand the pathogens and their drugs resistance in general surgery department and provide rational suggestion of antibiotics use for clinic treatment.METHODS A total of 158 cases with nosocomial infection among the general surgery department inpatients from Jun 2006 to Oct 2008 were retrospectively analyzed and summarized.RESULTS The common nosocomial infection sites were the lower respiratory tract,gastrointestinal tract,urinary tract and surgical sites.The G-bacilli of nosocomial infections in turn were Escherichia coli(18.02%),Acinetobacter baumannii(11.26%),Pseudomonas aeruginosa(7.66%),and Klebsiella pneumoniae(4.50%).The main G+cocci were Staphylococcus aureus(21.62%),Enterococcus faecium(5.86%)and E.faecalis(3.15%)in turn.In G-bacilli,the sensitivity to imipenem was the highest from 58.82% to 100.00%.The sensitivity to amikacin were more than 70.00% except A.baumannii,and to sulbactam/cefoperazone were more than 50.00% except Pseudomonas aeruginosa.In G+ cocci,the sensitivity to vancomycin of S.aureus and E.faecium was 100.00% and 84.62%.CONCLUSIONS Investigating the pathogens and their drug resistance in general surgery department is very important to prevent and control nosocomial infections.
4.The effect of norcantharidin on proliferation and apoptosis of transplanted human gallbladder carcinoma in nude mice

Objective To explore the effect of norcantharidin (NCTD) on proliferation and apoptosis of implanted human gallbladder carcinoma in nude mice. MethodsGBC-SD cells of human gallbladder carcinoma were implanted subcutaneously into nude mice. Mice were randomly divided into control, 5-FU, NCTD and NCTD+5-FU -treatment groups. Tumor size, growth curve and inhibitory rate was respectively evaluated. Cell cycle and apoptosis were measured. Morphological changes of tumorous cells were observed. ResultsLD_ 50 of NCTD for nude mice was 139.96mg?kg -1. Tumor volume (5.61?0.39cm3 vs. 9.78?0.61cm3, P=0.000), percentage of the S phase cells (43.47%?2.83% vs. 69.85%?1.96%, P=0.000) in NCTD group was smaller than that in control group, with tumor inhibitory rate (42.63% vs. 0, P=0.012) and cell apoptosis rate (5.49%?0.59% vs. 15.08%?1.49%, P=0.000) being increased. Compared with other groups,the difference on tumor volume (4.51?1.11 cm3), tumor inhibitory rate (53.89%), percentage of the S phase cells (33.76%?2.39%) and cell apoptosis rate (18.68%?2.38%) in NCTD+5-FU group was statistically significant (P=0.000), with increased nuclear shrinkage, karyorrhexis and typical apoptosis. Conclusion NCTD inhibits the growth of implanted tumor of human gallbladder carcinoma in nude mice. The inhibitory effect could be intensified when combined with 5-FU.
5.Clinical characteristics of 7 patients with gestational diabetes insipidus

Objective To investigate the clinical feature,treatment and prognosis of both themother and the fetus with gestational diabetes insipidus.Methods A total of 7 cases of gestational diabetes insipidus collected in the First Affiliated Hospital of Wenzhou Medical College,Wen'zhou Combination ofTraditional Chinese Medicine with Western Medicine Hospital,and Zhejiang Taizhou Hospital from June 1993to June 2006 were analyzed retrospectively.Resuits Seven cases symptoms all characterized by excessive thirst polydipsia and polyuria.The average 24 h urinary output was between 11 L to 13 L and manifested of hypobaricuria.After effective treatment(three cases were treated with 1-deamino-8-D-arginine vasopressin,another three patients were managed with hydrochlorothiazide,and the last one was cured with antisterone),seven patients with gestational diabetes insipidus did not have any severe consequences.Their symptoms of excessive thirst,polyuria,and polydypsia disappeared from 7 days to 3 months after parturition.Urinary volume returned to normal standard of 1000-2000 ml during 24 hours.Specific gravity of urine recovered normally between a range 1.015-1.025 and serum sodium recovered between 135-147 mmol/L Theaverage duration of illness was 52 days.Eight newborn infants survived.Two of them were sent to neonatal intensive care unit for treatment.One was because of premature delivery caused by antepartum eclampsia,and the other case was one of the twins who had hydronephrosis.The baby of the first case left hospital after 3 weeks'treatment.The latter one's symptom disappeared 2 weeks after delivery.No obvious symptom was discovered among all the babies through follow-up telephone calls 42 days after childbirth.Conclusion Gestational diabetes insipidus is a rare endocrinopathy complicating pregnancy.This disorder is characterized by excessive thirst,polydypsia,polyuria,hypobaric urine and electrolyte disturbances usually manifesting in the third trimester of pregnancy or puerperium.This is a transient syndrome.The first treatment of choice in patients with gestational diabetes insipidus is 1-deamino-8-D-arginine vasopressin and the second-choice is hydrochlorothiazide.Early diagnosis and appropriate management of the disease may reduce the hazard forboth the mother and the fetus during perinatal period.
6.Effect of lipoxins on proliferation and secretion of peritoneal macrophages from patients with precclampsia in vitro

Objegtlve To study the effect of lipexins on the proliferation and secretion of peritoneal macrophages from patients with preeclampsia in vitro.Methods Peritoneal macrophages were obtained from 24 patients with preeclampsia(preeclampsia group)and 24 normal pregnant women(normal pregnant group)who were treated in the First Affiliated Hospital of Wenzhou Medical Coilege from March to July 2007.Enzyme linked immunosorbent assay (ELISA) was used to detect the concentration of tumor necrosis factor-α(TNF-α)in the supernatant of macrophages which were pulsed with lipoxins at different concentrations(0,10,100 nmol/L)in both groups after 48 hours.Methyl thiazolyl tetrazolium (MTT)assay was used to detect the inhibition rate of cell proliferation of macrophages which were pulsed with lipoxins at different concentrations(0,10,100 nmol/L)in both groups after 24 hours.Results (1)The concentration of TNF-α:the levels of TNF-α were(1867.5±47.3),(1836.9±4.5) and (1800.5±2.7)ng/L after treatment with differed concentrations of lipoxins(0,10,100 nmol/L)in preeclampsia group vs normal pregnant group[(791.3±62.2),(789.4±2.3),(781.5±1.9)ng/L].The levels of TNF-α in preeclampsia group were significantly higher than that in normal pregnant group(P<0.05).Lipoxins significantly inhibited the concentration of TNF-α in a dose-dependent manner in preeclampsia group (P<0.05),while it had no significant effect in normal pregnant group(P>0.05).(2)Cell proliferation inhibition:Incubation with lipoxins produced a dose-dependent(0,10,100 nmol/L)inhibitory effect on proliferation in preeclampsia group,[(14.8±6.3)%,(32.9±3.6)%,(36.7±3.8)%],vs normal pregnant group[(16.8±6.9)%,(16.7±5.4)%,(15.9±2.1)%].The rate of cell proliferation in preeclampsia group was significantly hisher than that in normal pregnant group.Lipoxins significandy inhibited this growth(P<0.05),while it had no significant effect in normal pregnant group(P>0.05).Conclusion Lipoxins can inhibit the proliferation of macrophage and secretion of TNF-α in preeclampsia in a dose-dependent manner.Lipoxins may be potentially useful in prevention and treatment of preeclampsia.
7.Clinical efficacy of rectal lapse valve repair in the treatment of patients with middle and low rectovaginal fistula

Objective To explore the clinical efficacy of transrectal lapse valve repair in the treatment of patients with middle and low rectovaginal fistula.Methods Totally 24 patients with middle and low rectovaginal fistula were divided into transanal group (n =15) and transvaginal group (n =9).The transanal group was treated by rectal lapse valve repair,and the transvaginal group was treated with the transvaginal repair.The indexes of oxidative stress such as serum IL-6,ROS,CRP were compared between the two groups,and the success,recurrence and satisfaction rate were compared as well.Results After treatment,the levels of IL-6,ROS and CRP in both groups were significantly higher than those before treatment,but there were no significant differences in the stress response indexes between the two groups (P > 0.05).The success rate and satisfaction rate in the transanal group were significantly higher than those in the transvaginal group (P < 0.05),and the recurrence rate was significantly lower than the transvaginal group (P < 0.05).Conclusion Transrectal lapse valve repair has higher success rate and low recurrence rate in the treatment of patients with middle and low rectovaginal fistula.

9.Mast Cells Tryptase Promotes Intestinal Fibrosis in Natural Decellularized Intestinal Scaffolds

BACKGROUND:
Standard two-dimensional (2D) culture has confirmed the mechanism of mast cells (MCs) in the pathogenesis of inflammatory bowel disease (IBD), but the regulation of signaling responses of MCs may well differ in three-dimensional (3D) microenvironments. The aim of the study was to develop a 3D culture model based on decellularized intestinal scaffolds (DIS) and verify how MCs influenced fibroblasts phenotype in the 3D model.
METHODS:
DIS were achieved using the detergent technique and extracellular matrix (ECM) components were verified by histologic analysis, quantification and scanning electron microscope. After human colon fibroblasts recellularized into the scaffolds and activated by MCs tryptase and TGFb1, the changes in genes and signaling pathways during fibroblasts activation in 3D were studied and compared with the changes in 2D cell culture on plastic plates.
RESULTS:
Decellularization process effectively removed native cell debris while retaining natural ECM components and structure. The engrafted fibroblasts could penetrate into the scaffolds and maintain its phenotype. No matter whether fibroblasts were cultured in 2D or 3D, MCs tryptase and transforming growth factor b1 (TGF-b1) could promote the differentiation of fibroblasts into fibrotic-phenotype myofibroblasts through Akt and Smad2/3 signaling pathways. Furthermore, the pro-collagen1a1 and fibronectin synthesis of myofibroblasts in 3D was higher than in 2D culture.
CONCLUSION
Our results demonstrated that the DIS can be used as a bioactive microenvironment for the study of intestinal fibrosis, providing an innovative platform for future intestinal disease modeling and screening of genes and signaling pathways.
10.Research progress of metabonomics and nutritional intervention in Crohn's disease

Crohn's disease (CD) is a chronic nonspecific intestinal inflammatory disease with unclear etiology and pathogenesis. At present, it is believed that CD can cause different degrees of metabolic abnormalities, and that the related metabolites also play an important role in the development and progression of the disease. In recent years, metabonomics is becoming more and more well-developed and has provided guidance for clinicians on rational nutritional support. Here is a review on the metabolic changes of multiple systems in CD patients, the application of metabonomics in CD diagnosis and treatment, and the research progress of clinical nutrition intervention.
