1.Effect of calcitriol pulse therapy on maintenance hemodialysis patients 'microinflammation and oxidative stress

Objective To investigate the effect of calcitriol pulse therapy on maintenance hemodialysis patients'microinflammation and oxidative stress.Methods The clinical data of 106 patients with maintenance hemo-dialysis were retrospectively analyzed.106 patients were randomly divided into two groups,each group had 53 cases. The observation group was treated with high dose pulse oral calcitriol therapy(2.0μg,twice a week),while the control group received conventional calcitriol therapy (0 .5 μg ,qd ).Both two groups were treated for 12 weeks.Then,the levels of iPTH,serum calcium and phosphorus,microinflammation and oxidative stress before and after treatment were compared.Results There was no difference between the two groups in iPTH indicators(t =0.354,P >0.05).After treatment,the levels of iPTH,hs -CRP,IL -6,TGF -β1,MDA,MPO in both two groups all decreased(t =7.678, 7.417,13.663,25.709,16.114,9.940,all P <0.001;t =2.130,3.006,5.945,12.961,7.367,4.567,all P <0.01),the levels of GSH -PX all increased(t =13.173,P <0.001;t =4.815,P <0.001),and there were signifi-cant differences between the two groups (t =10.326,P <0.001;t =3.601,P <0.001;t =9.263,P <0.001;t =11.094,P <0.001;t =8.262,P <0.001;t =3.693,P <0.001;t =9.289,P <0.001).The levels of serum calcium and Ca ×P remained unchanged in 4 and 8 weeks(t =1.354,0.128,0.992,0.879,all P >0.05;t =0.777,1.310, 1.210,1.957,all P >0.05 ),which of the observation group were significantly higher than the control group in 12 weeks(t =2.805,P =0.006;t =3.070,P =0.002).Conclusion For treatment of patients with maintenance hemodialysis,calcitriol pulse therapy can reduce systemic microinflammatory response and enhance antioxidant capacity.
2.How does the traction force affect the intervertebral disc:a three dimensional finite element analysis

Objective To identify the effective traction conditions by comparing the effects of traction forces on the intervertebral disc under various mechanical conditions. Methods A three dimensional finite element model was used to carry out mechanical analysis of the traction under conditions with different forces and angles, then to carry out the comparative analysis of the stress distribution and the change of the volume of the intervertebral disc. Results In general, the stress distribution on the intervertebral disc was not even under different traction forces, The maximum stresses appeared at the back rim of the intervertebral disc. With the increase of the traction force and angle, the enlargement of the intervertebral disc volume increased, and the enlargement rate was more significant. Conclusion The volume of the intervertebral disc can be most effectively enlarged at the condition of 30?forward bending of the neck when a force of 90N was applied. This is the most effective traction condition.
3.Clinical profile and characteristics of serum GP73 in cirrhotic patients with chronic hepatitis B infections

Objective To explore clinical profile and significance of serum golgi protein 73 (GP73) in cirrhotic patients with chronic hepatitis B virus (HBV) infection .Methods A total of 109 cirrhotic patients with HBV infection were included ,in which 60 patients were compensated cirrhosis ,and 49 patients were decompensated cirrhosis .The receiver operating characteristic curve (ROC) analysis were used to evaluate the clinical significance of GP73 for diagnosing decompensated cirrhosis in cirrhotic popula-tion .Results Serum GP73 level in cirrhotic patients was correlated with alanine aminotransferase (ALT ) and HBV DNA ,but neg-atively correlated with albumin levels .Serum GP73 levels in patients with decompensated cirrhosis was (215 .9 ± 96 .56)ng/mL , which was significant higher than (113 .7 ± 68 .95)ng/mL in patients with compensated cirrhosis .The area under ROC was 0 .82 (95% CI :0 .74 ~ 0 .90 ,P< 0 .05) .The sensitivity and specificity of GP73 diagnosing decompensated cirrhosis in cirrhotic population with chronic HBV infection were 77 .55% and 75 .00% respectively ,when the cut-off was set at 140 .5 ng/mL .Conclusion Serum GP73 level is related with liver injury and HBV replication .GP73 might be an useful marker for diagnosing decompensated cirrhosis in cirrhotic population with chronic HBV infections .
4.Adhesion, proliferation and differentiation of human adipose mesenchymal stem cells on PLGA

Objective To study the ability of adhesion, proliferation and differentiation of human adipose mesenchymal stern cells,which were cultured in PLGA,and thus to provide a basis for their in vivo implantation.Methods PLA was mixed with PGA in a ratio of 1∶ 1,and PLGA was obtained by insert moldling with organic solvents.In vitro, human adipose mesenchymal stem cells were adhered to PLGA.The attachment and the proliferation of human adipose mesenchymal stem cells in PLGA were analysed by scanning electron microscope,and the morphological change of the cells was investigated before and after induction.Results Human adipose mesenchymal stem cells were able to attach, proliferate and secrete extracellular matrix in porous PLGA (pore size:100~400 pm) with porosity of 85 %, and to differentiate into round lipoblast-like cells under the induction of adipose medium.After 14 days, the particulate of PLGA was overlaid by extracellular matrix .Conclusion Human adipose mesenchymal stem cells have the capacity to adhere to PLGA and proliferate, indicating that PLGA can be used as a carrier for human adipose mesenchymal stem cells in adipose tissue engineering.
5.THE PHYSIQUE STATUS AND ITS CHRONOLOGICAL CHANGES IN RECRUITED YOUTHS IN CHINA

This study was aimed to obtain a general knowledge of physical status and its chronological changes in young Chinese recruits. 73 000 physical examination data of young male recruits were collected in 100 sample units selected by the method of cluster sampling from 31 provinces in China. Height, weight, chest circumference, and body mass index(BMI) were analyzed. The results showed that the recruits from Northeast China and North China were the tallest (170 3cm and 170 6cm on average), the heaviest (60 4kg and 61 6kg), their chest circumference largest(83 4cm and 84 6cm), and their BMI were the largest(20 8 and 21 5kg/m 2). Comparing with historical data, the average height increased 6 6cm since 1955 and 4 1cm since 1974. The body weight increased 5 6kg since 1955 and 3 5kg since 1974. Chest circumference increased slightly, the mean values of which in the three periods were 20 3, 20 1 and 19 8kg/m 2, respectively. The recruits from southwertern and south China showed least growth, and their BMI was virtually lower than before. The results of this survey indicated that a significant difference in the physical status among youth recruits from different areas in China. Lack of growth of chest circumference and BMI in youths from some areas should arouse our attention
6.Repair of mandibular defect with bone marrow stroma cell transplantation combined with multi-pore mineralization ossein

Objective:To study the effect of bone marrow stroma cell( BM SC) transplantation combined with multi-pore mineralization ossein(Bio-Oss) in the plerosis of mandibular defect.Methods:A round whole-layer defect in the diameter of 1.5 cm was created on each side of mandible in 16 whit e New Zealand rabbits.The rabbits were randomly divided into two groups.In group 1 BMSCs were collected and cultured with mineralization induce medium for 2 wee ks. Then cell suspension at the density of 5?10 7 /ml and gelatum were mixed a nd dripped into Bio-Oss bones, incubated in DMEM in 50 ml/L CO 2 incuba tor at 37 ℃ for 4-6 h.The BMSC-Bio-Oss was applied to make plerosis of the m andibular defects.In group 2 the defects were repaired with the Bio-Oss ossein only.Samples were collected after 8 and 12 weeks after opreation respectively an d observed with radiology and histology.Results:12 weeks after o peration, (1)in group 1 previous defects were basically consisted of new bone tissue;(2)in group 2 the previous defects were mainly consisted of fibrous tis sue. Conculsions:BMSC-Bio-Oss transplantation may effectively repair bone defect.
7.Ketamine and propofol in combination for sedation during cystoscope in elderly male patients

Objective To study the safety and efficacy of target-controlled infusion (TCI) sedation with low dose ketamine and propofol during rigid cystoscopy in elderly male patients. Methods Forty-five elderly male patients with I - III grade of the American Society of Anesthesiologists (ASA)rigid cystoscopy examination were divided into 3 groups randomly: group A, 2% lydocaine gel was filled in urethra, n=15; group B, 2% lydocaine gel filled with propofol TCI sedation, n=15; group C, 2% lydocaine gel filled with combined ketamine and propofol TCI sedation, n= 15. The mean artery pressure (MAP), heart rate(HR), pulse oxygen saturation (SPO2)were measured at 5 time points; preoperation, during local anesthesia, inserting the scope, during testing and the end of testing. The concentration of effect room, wake time when alertness and calm grading (OAA/S)was 3 scores in B and C groups and visual analogue scale( VAS) score of pain after operation were detected. Results The MAP and HR at the time point of inserting the scope in group A had significant difference,compared with preoperation(P0. 05) . In group A, 10 cases pain VAS score were light pain, 4 moderate and 1 severe. In group B and C,no pain and no memory of the operation were reported. Conclusions TCI sedation with low dose of ketamine and propofol in elderly male patients under rigid cystoscope has good effects on sedation, analgesia and anterograde amnesia. The hemodynamics is stable and wake time is short.
8.Effect of parecoxib on hippocampal inflammation in partial hepatectomy rats and its possible mechanism

Objective To investigate the effect of parecoxib on hippocampal inflammation following partial hepatectomy in rats.Methods 72 SPF male healthy SD rats were selected and randomly divided into four groups:control group(group C),anesthesia group(group A),operation group(group S),parecoxib group (group P ), each had 18 rats.Using partial hepatectomy operation trauma to establish animal model of hippocampal inflammation.Rats were sacrificed 1,3,7 days after anesthesia,the expression of inflammatory mediators in hippocampal tissue were detected.The expression of cyclooxygenase -2(COX-2)mRNA were detected by real-time quantitative PCR,and COX-2 and cysteine proteinase -3(Caspase-3)protein expression were detected by Western blot,the prostaglandin E2(PGE2)content were detected by enzyme linked immunosorbent assay(ELISA).Results Compared with control group,COX-2 expression(protein and mRNA),PGE2 production and Caspase-3 protein expression in group A had no significant difference.In group S and P,COX-2 expression,PGE2 and Caspase-3 protein expression increased significantly on 1st day postoperatively to 3rd day (P<0.05 or P<0.01),and they were obvious especially in the postoperative 1st day(P<0.01).The above inflammation index expression returned to normal in 7th days after operation.Compared with group S,COX-2 expression,PGE2 and Caspase-3 protein expression increased significantly on 1st day postoperatively to 3rd day(P <0.05 or P <0.01 ).Conclusion Partial hepatectomy could induce hippocampal inflammatory response in the rats.Parecoxib could suppress the hippocampal inflammation via the down-regulation expression of COX-2 mRNA and protein,PGE2 and Caspase-3 protein in rats.

10.Application of serum and CSF procalcitonin detection in adult patients with CNS bacterial infection

Objective To analyze the clinical value of serum and cerebrospinal fluid(CSF) procalcitonin in the diagnosis of adult central nervous system(CNS) infection to provide reference for its clinical diagnosis and treatment .Methods Sixty adult patients with CNS diseases admitted to our hospital from February to December 2015 were selected as the research subjects .All patients were suspected of CNS infection at initial visiting due to suspected CNS symptoms and signs ,and the CSF pathogenic examination and separation were performed to determine the types of infected pathogenic microorganisms .Meanwhile serum and CSF procalcito-nin was detected .The detection results of CSF pathogen and procalcitonin were summarized .The detection results of CSF pathogen served as the gold standard ,serum and CSF procalcitonin> 0 .5 ng/mL as infection positive .The diagnostic test fourfold table was adopted .Then the procalcitonin detection results and CSF pathogenic microorganism detection data were inputted .The consistency of serum and CSF procalcitonin for judging CNS infection was analyzed and calculated .The sensitivity ,specificity and accuracy of serum and CSF procalcitonin detection method for detecting pathogenic microorganisms were analyzed .Results Among 60 cases ,31 cases were bacterial infection ,22 cases were viral infection and no pathogenic bacterium was detected out in 7 cases ,the positive rate was 88 .34% .In the serum procalcitonin detection :49 cases were positive and 11 cases were negative ;in CFS procalcitonin detec-tion:37 cases were positive and 23 cases were negative .The Kappa coefficients of serum procalcitonin and CFS pathogenic detection results was 0 .769 ,indicating that serum procalcitonin and CFS pathogenic detection results had higher consistency ;the Kappa coef-ficients of CSF procalcitonin and CFS pathogenic detection results was 0 .315 ,indicating that CFS procalcitonin and CFS pathogenic detection results had poor consistency .The sensitivity ,accuracy and specificity of serum procalcitonin for diagnosing CNS infection were 96 .08% ,93 .33% and 77 .78% respectively ,which of CSF procalcitonin were 73 .33% ,45 .00% and 38 .46% respectively ;se-rum procalcitonin detection had a higher clinical value in the diagnosis of adult CNS infection ,while CSF procalcitonin detection had little value .Conclusion The serum procalcitonin detection method has higher sensitivity ,accuracy and specificity for the diagnosis of adult CNS infection ,possesses a better guidance role in selecting the treatment scheme and evaluating the curative effect ,and has higher clinical value in adult patients with CNS bacterial infection .
