1.Comparison of stress responses to the intubation with lightwand and direct laryngoscope in elderly patients

Objective To compare the effects oflightwand(LW) and direct laryngoscope(DL) in elderly patients undergoing surgery on hemodynamics and stress responses.Methods Forty elderly patients with ASA grade Ⅰ-Ⅱ grade who selective surgery were divided into LW group and DL group by random digits table method,each group with 20 cases.Systolic blood pressure (SBP),diastolic blood pressure (DBP),heart rate(HR) were recorded respectively before intubation,30 seconds and 5 minutes after intubation.Extracted patients with peripheral arterial 3 ml at the same time,measured by radioimmunoassay epinephrine and norepinephrine levels.Results The SBP,DBP,HR,epinephrine,norepinephrine 30 seconds after intubation in LW group were significantly lower than those in DL group [(140.50 ± 21.91) mm Hg(1 mm Hg =0.133 kPa) vs.(167.95 ± 17.85) mm Hg,(67.80 ± 6.76) mm Hg vs.(84.15 ±9.89) mm Hg,(85.10 ± 11.76) times/min vs.(96.30 ± 13.72) times/min,(49.47 ± 8.09) ng/L vs.(61.25 ± 9.43) ng/L,(171.27 ± 17.11) ng/L vs.(187.40 ± 16.13) ng/L,P < 0.01].The SBP,epinephrine,norepinephrine 5 minutes after intubation in LW group were significantly lower than those in DL group [(120.75 ± 17.12) mm Hg vs.(136.65 ± 15.43) mm Hg,(39.80 ±7.38) ng/L vs.(47.63 ±8.48) ng/L,(155.93 ± 17.11) ng/L vs.(172.76 ± 13.45) ng/L,P < 0.01].Conclusion Both LW and DL can be well done in elderly patients intubation,but LW can reduce stress responses during the intubation.
2.Da Vinci robot surgical system versus traditional open surgery for old patients with upper abdominal surgical diseases: a case control study

Objective To compare the clinical application of Da Vinci robot surgical system (RSS) with traditional open surgery (TOS).Methods From Feb 2015 to Jul 2016,48 cases of upper abdominal surgical disease patients were divided into RSS group (23 cases) and TOS group (25 patients) randomly.Results The anesthesia time [(194 ±16)min vs.(181 ±11)min,t=3.262,P=0.002] and operation time [(167 ± 14) min vs.(158 ± 14) min,t =2.292,P =0.027] were much longer in the RSS,while the blood loss during operation significantly less than the TOS [(128 ± 62) ml vs.(190 ± 86) ml,t =-2.886,P =0.006].The RSS has obvious advantages in 24 h-drainage [(69 ± 27) ml vs.(114 ± 54) ml.t =-3.680,P =0.001],time to out-of-bed activity [(27.7 ± 8.0) h vs.(35.7 ± 9.9) h,t =-3.067,P =0.004],BPS,postoperative exhausting time[(27.2 ±5.9)h vs.(32.8 ±8.3)h,t =-2.690,P =0.01] and length of hospital stay [(10.4 ± 1.8) d vs.(11.8 ± 1.9) d,t =-2.600,P =0.013].But the total hospital cost was higher in RSS [(117 000 ± 10 000) yuan vs.(77 000 ± 8 000) yuan,t =15.087,P =0.000)].Conclusions The RSS is a much minimally invasive surgery,reducing blood loss and postoperative pain,promoting rapid recovery,shortening hospital stay.
3.Structure-activity relationship of diosgenin derivatives as Bcl-2 antagonists.

The purpose of this paper is to clarify the structure-activity relationship of anti-tumor activity of diosgenin derivatives in vitro. Study has found that diosgenin can inhibit the reproduction of tumor cells by inducing apoptosis and the main target spot of this effect is Bcl-2. Based on the characteristics of pharmacophoric points' of the three-dimensional pharmacophore for Bcl-2 inhibitors, we have docked lots of diosgenin derivatives with Bcl-2, then synthesized 31 compounds of them, finally assessed the anti-tumor activity of the diosgenin derivatives in vitro against A375, A549, HepG-2 and K562. Preliminary studies of SAR have indicated that the aliphatic esters, and aromatic esters of diosgenin without F ring have no anti-tumor activity in vitro. The triazole bromides of diosgenin all achieve fairly good anti-tumor activity in vitro, and those with larger hydrophobic group have the better activity. The stronger is the hydrogen bonding interaction and dipole-dipole interaction of the heterocyclic of diosgenin and diosgenin without F ring and the acid ester of diosgenin without F ring, the better is the activity of derivatives.
4.Exploration of factors influencing shimming and water suppression on hepatic 1H-MR spectroscopy in vivo on 3.0 T

Objective To characterize the clinical factors which influence water suppression and auto-shimming line width for liver 3.0 T 1H-MRS.Methods Fifty-seven cases with liver 1H-MR spectroscopy(1H-MRS) were retrospectively studied, including chronic type B hepatitis (n=5), fatty liver (n=14), chronic type B hepatitis combining fatty liver (n=3) and normal situation (n=35).Independent t test was used to characterize the difference of general condition (height, weight, body mass index etc.) between different water suppression effect groups and between different shimming effect groups.Using Chi-square test to analyze whether water suppression rate and auto-shimming line width between fatty liver groups and non-fatty liver exist significance difference .Results By comparing WS ≥90% (n=47)group with WS < 90% (n=10) group, the former showed smaller average height[(164.7±8.0) and (170.5±3.7) cm respectively], weight[(61.8±10.4) and (73.4±5.4) kg respectively], BMI [(22.7±3.2) and (25.2±1.3)kg/m2 respectively]and LW[(17.7±3.7)and(24.6±6.3) Hz respectively]than the latter (t=-3.488,-3.415,-4.002 and-3.327, P < 0.05).By comparing LW 20 Hz (n=41) group with LW 20 Hz (n=16) group, the former showed better water suppression rate [(93.0±2.7)% and (86.1±8.5)% respectively]than the latter(t=3.213,P <0.05), whereas larger weight[(62.2±11.6) and (68.1±6.1)kg respectively]and BMI[(22.8±3.4) and (24.2±1.9)kg/m2 respectively](t=-2.516,-2.024,P <0.05).LW≤20 Hz in the fatty liver and non-fatty liver group was 7/17 and 34/40, respectively (X2=11.347, P < 0.05).WS < 90% in the fatty liver and non-fatty liver group was 10/17 and 0/44, respectively (X2=28.536, P < 0.05) .Conclusion Hepatic steatosis exerts an adverse effect in water suppression and shimming.
5.Dynamic changes of PD-1 expression on CD4 + and CD8 + T cells in patients with HBeAg-positive chronic hepatitis B treated with telbivudine

Objective To investigate the expression of PD-1 on CD4 +and CD8 + T cells in patients with HBeAg-positive chronic hepatitis B(CHB)treated with telbivudine.Methods Fifty-six HBeAg-positive CHB patients admitted in the First Affiliated Hospital of Zhengzhou University during January 201 3 and June 201 4 were enrolled in this study.The expression of PD-1 on CD4 +and CD8 + T cells was detected with flow cytometry at baseline,24,48 and 72 wks after telbivudine treatment.The relationship of PD-1 expression with alanine aminotransferase (ALT)level,HBeAg seroconversion and HBV DNA loads was analyzed.t test and completely random variance analysis were used to analyze the data.Results The PD-1 expression on CD4 + and CD8 + T cells at baseline was higher in patients with low ALT levels compared to those with high ALT levels(t =1 2.20 and 9.69,both P <0.01 ),while higher levels of PD-1 expression was also observed in patients with high HBV DNA load (≥5 lgIU /mL)compared to those with low HBV DNA load (t =4.39 and 4.85,both P <0.01 ).PD-1 levels on CD4 + and CD8 + T cells presented a declining trend after telbivudine treatment(F =6.98 and 8.97,both P <0.01 ),PD-1 expression in patients with HBeAg seroconversion showed lower levels compared with baseline values (t =1 8.45 and 1 8.01 ,both P <0.01 ). Conclusion In HBeAg-positive CHB patients,the expression of PD-1 on CD4 + and CD8 + T lymphocytes shows a decreasing trend during the treatment with telbivudine,indicating that antiviral therapy may alleviate the immunosuppression in these patients.
6.Improved effect of curcumin on mitochondrial dysfunction induced by high glucose in L6 cells

To investigate the effects of curcumin on mitochondrial dysfunction induced by high glucose(40 mmol/L glucose, 24 h)in L6 cells, curcumin(10, 20, 40 μmol/L)was administered for 24 h after high glucose culture. The effects of curcumin on the mitochondrial dysfunction were evaluated by mitochondrial membrane potential, reactive oxygen species(ROS), ATP content and mtDNA copy number. The mRNA and protein expression of uncoupling protein 2(UCP2), PPARγ coactivator 1α(PGC-1α)and sirtuin-1(Sirt3)were also determined. As improvement of high glucose damage, curcumin significantly raised mitochondrial membrane potential and ATP content, and decreased ROS level. Curcumin significantly ameliorated the down regulation of UCP2 yet with little effect on mtDNA copy number and PGC-1α and Sirt3 expression. In conclusion, curcumin could significantly ameliorate mitochondrial dysfunction in L6 cells induced by high glucose, which involved the mechanism of multiple antioxidants.
7.Antiviral and anti-inflammatory mechanism of Anti-601 Mixture based on network pharmacology and molecular docking

The potential antiviral and anti-inflammatory mechanism of Anti-601 Mixture, a traditional Chinese medicine compound preparation, was studied by network pharmacology and molecular docking. The chemical constituents and targets of astragali radix, phellodendri chinensis cortex, rhei radix et rhizome, isatidis radix and lonicerae japonicae flos in Anti-601 Mixture were searched by Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP). The genes corresponding to the target were searched through the UniProt database, and the drug-compound-target (gene) network was constructed by Cytoscape 3.7.2. Then the functional enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis were conducted through Webgestalt to predict the mechanism. The network of drug-compound-target (gene) contained 5 drugs, 100 compounds and 207 targets. Functional enrichment analysis resulted in 717 GO items (P≤0.05), among which 240 were biological process (BP) items, 240 were cell composition (CC) items, and 237 were molecular function (MF) items. The 209 signaling pathways were obtained by enrichment screening of KEGG pathway (P≤0.05). Molecular docking showed that the active ingredients of Anti-601 Mixture, such as Stigmasterol, quercetin, luteolin, acacetin, β-sitosterol, kaempferol,had strong affinity with PTGS2 target. Active compounds in Anti-601 Mixture may regulate multiple signaling pathways including advanced glycation end products and its receptor (AGE-RAGE), IL-17, tumor necrosis factor (TNF) through target of prostaglandin-endoperoxidase synthase 2(PTGS2), thus playing an antiviral and anti-inflammatory role.
8.Chrysin promotes osteogenic differentiation via ERK/MAPK activation.

The effect of the anti-inflammatory flavonoid chrysin on osteogenesis was determined in preosteoblast MC3T3-E1 cells. Results demonstrated that chrysin could induce osteogenic differentiation in the absence of other osteogenic agents. Chrysin treatment promoted the expression of transcription factors (Runx2 and Osx) and bone formation marker genes (Col1A1, OCN, and OPN) as well as enhanced the formation of mineralized nodules. During osteogenic differentiation, chrysin preferentially activated ERK1/2, but not JNK nor the p38 MAPKs. Further experiments with inhibitors revealed the co-treatment of U0126, PD98059, or ICI182780 (a general ER antagonist) with chrysin effectively abrogated the chrysin-induced osteogenesis and ERK1/2 activation. Thus, the effect of chrysin on osteogenesis is ERK1/2-dependent and involves ER. Therefore, chrysin has the significant potential to enhance osteogenesis for osteoporosis prevention and treatment.

9.Prenatal ultrasonic manifestations and prognostic analysis of 17q12 microdeletion syndrome

Objective To summarize and analyze the prenatal ultrasonic manifestations of 17q12 microdeletion syndrome ,so as to provide help for prenatal diagnosis . Methods It carried out a retrospective analysis of 21 cases of 17q12 microdeletion syndrome prenatally confirmed by chromosomal microarray analysis(CMA) in the third affiliated hospital of Zhengzhou University from 2015 .01 to 2018 .05 . Prenatal ultrasound findings were collected . A close follow-up was given for the cases . Results Of the 21 fetus ,20 cases were presented with bilateral hyperechogenic kidneys ,and 1 case showed duodenal obstruction . There were 4 cases accompanied with polyhydramnios ,8 cases with mild polyhydramnio ,and 9 cases with normal amniotic fluid index . Parents of 6 fetus underwent chromosomal examination ,of which 2 cases were inherited from the mother and 4 cases had new chromosomal mutations;Seven cases had a family history of diabetes and 3 cases had a family history of kidney disease;Twelve cases performed induced labor ,2 cases lost follow-up and 1 case was unborn . Six cases were born ,of the 6 cases ,1 case associated with mild dysmorphic facial features , and 1 case associated with bilateral renal cysts . Conclusions 17q12 microdeletion syndrome has specific prenatal ultrasound characteristics ,while echogenic kidneys are of great value in the diagnosis of 17q12 microdeletion syndrome prenatally .
10.Clinical value of (18)F-FDGPET/CT in differentiation between benign lesions and lung cancer for large shadows in patients with pneumoconiosis.

OBJECTIVETo evaluate the clinical value of (18)F-FDG PET/CT in the differentiation between benign lesions and lung cancer for large shadows in patients with pneumoconiosis.
METHODSA retrospective study was conducted in 21 patients with a confirmed diagnosis of pneumoconiosis who had a total of 37 large shadows in the lung fields as shown by whole-body (18)F-FDG PET/CT imaging, and (18)F-FDG uptake was evaluated using the maximum standardized uptake value (SUVmax). Twelve lesions were examined by CT-guided aspiration biopsy, and one case underwent fiberoptic bronchoscopy, as well as tests based on bronchoalveolar lavage fluid and sputum; the other cases were followed up for more than 2 years. The differences between pneumoconiosis nodules with high (18)F-FDG uptake and lung cancer were evaluated by t test.
RESULTSSeventeen lung masses or nodules with high (18)F-FDG uptake were detected in 13 patients; among the 13 patients, 5 lesions of 5 cases were confirmed as non?small cell lung cancer (1 case of squamous cell carcinoma, 3 cases of adenocarcinoma, and 1 case of recurrent squamous cell carcinoma after operation), and 7 lesions of 7 cases were progressive massive fibrosis as shown by CT-guided aspiration biopsy; one case had no tumor cells detected by fiberoptic bronchoscopy, and 2.5-year follow-up revealed no changes in the lesions. The other 8 patients showed no increased (18)F-FDG uptake in their 20 lung nodules, which were confirmed as benign lesions by follow-up. The diameter range of lung cancer was 1.6∼6.8 cm, and the SUVmax range was 4.8∼14.0; the diameter range of pneumoconiosis nodules with high (18)F-FDG uptake was 1.5∼4.6 cm, and the SUVmax range was 2.6∼12.4. There were no significant differences in diameter and SUVmax between the lung cancer and pneumoconiosis nodules with high (18)F-FDG uptake (P > 0.05 for both). (18)F-FDG PET/CT had a specificity of 62.5% (20/32), an accuracy of 67.6% (25/37), a false-positive rate of 37.5% (12/32), and a negative predictive value of 100% (20/20) for the diagnosis of lung cancer. The lung cancer detection rate was 23.8% (5/21).
CONCLUSIONIn (18)F-FDG PET/CT imaging for patients with pneumoconiosis, the lung lesions without (18)F-FDG uptake or symmetrical lesions with high (18)F-FDG uptake are considered as benign pneumoconiosis nodules; however, (18)F-FDG PET/CT might have a limited role in evaluating the solitary lung lesions in patients with pneumoconiosis and needs further study.
