1.CT evaluation of benign and malignant solitary fibrous tumor of the pleura

Objective To analyze CT findings of solitary fibrous tumors of the pleura (SFTP) for differentiating benign and malignant lesions.Methods CT findings of 20 benign and 11 malignant SFTPs proved by pathology were retrospectively reviewed. The size,margin,internal structures,blood supplies,invasion to adjacent structures or metastasis and pleural effusion were analyzed and compared between malignant and benign groups.Results The mean diameter of malignant lesions( median diameter:13.5 cm) was larger than that of the benign ( median diameter:7.6 cm,F =6.411,P =0.017 ).Malignant lesions tended to be more heterogeneous (9/11,P =0.002),more invasive to adjacent structures ( 6/11,P =0.001 ) than benign lesions ( 4/20,0/20 ).Serpiginous vessels ( 10/11,P =0.008 ) and pleural effusion (6/11,P =0.038 )were more common in malignant lesions than those in benign lesions (8/20,3/20).Calcification could be found in both benign (2/20) and malignant lesions ( 2/11,P =0.6 ).Furthermore,3D CT angiography could show blood supplies into tumor in 12 lesions (4 benign and 8 malignant SFTPs).Conclusion CT and 3D CTA are helpful in differentiating benign from malignant SFTPs and evaluating the excision of SFTPs.
2.Basal cell adenomas of the parotid gland: CT and MRI features

Objective To investigate the CT and MR] features of basal cell adenoma of the parotid gland. Methods CT and MRI features of 9 basal cell adenoma cases (4 men and 5 women, median age 58) confirmed by operation and pathology were reviewed retrospectively. All CT and MR images were retrospectively evaluated with respect to location, size, morphology, margin, CT density/MR] signal intensity and enhancement behavior. Results Each of the 9 patients had only 1 tumor. Eight lesions were located in the superficial lobe and 1 in the deep lobe. Seven tumors were round without lobular appearance, and 2 tumors were elliptic with small lobular appearance. All the 9 lesions were well circumscribed with smooth contours. A capsule-like rim with low intensity on MR[ was observed in 2 lesions, and a rim with high intensity on T1 WI and T2 WI was detected in another lesion. These 3 lesions showed low intensity on T1 WI and T2 WI with heterogeneously strong enhancement and delayed enhancement. Four lesions showed cystic areas in the center with mural nodules on CT, which were moderately or obviously enhanced (the mean increase of CT values: 65.5 H U ), and 2 lesions showed homogeneous enhancement. Conclusion Basal cell adenoma of parotid gland should be first considered when the old women had single round lesion located in the superficial lobe with smooth contour and low intensity on T2WI, especially when cystic areas were observed in the center with mural nodules on CT/MR.

Objective To investigate the clinical curative effect and safety of Shenmai combined with meglu-mine adenosine cyclophosphate in the treatment of chronic heart failure.Methods 118 patients with chronic heart failure were selected as the research subjects.They were randomly divided into control group and observation group, 59 cases in each group.The patients in the control group received routine comprehensive treatment,the observation group was given intravenous Shenmai injection 50mL,1 /d,meglumine adenosine cyclophosphate injection 120mg, 250mL 5% glucose injection intravenous drip note,the 1 time /d,7d for a course of treatment.Two groups of patients received a total of 2 courses of treatment.Before and after treatment,left ventricular ejection fraction (LVEF)was detected by cardiac ultrasound,and the brain natriuretic peptide (BNP)level in patients was measured.The total effective rate of the two groups was analyzed.Results Before treatment,LVEF of the control group and the observa-tion group were (32.19 ±5.72)%,(32.50 ±6.01 )%.After treatment,LVEF of control group was (36.62 ± 4.13)%,LVEF indexes in the observation group was (42.09 ±5.52)%,LVEF increased significantly in the two groups,but that in the observation group was higher than that of the control group (t =-3.882,P =0.017),the difference was statistically significant.Before treatment,BNP index of the observation group and the control group were (485.16 ±206.15 )pg/mL,(489.11 ±178.96)pg/mL,after treatment,BNP level of the observation group was (159.29 ±93.62)pg/mL,BNP level in the control group was (322.36 ±156.58)pg/mL,BNP in the observation group was significantly lower than that of the control group(t =-7.443,P =0.000).24 cases in the observation group after treatment,effective in 30 cases,the total effective rate was 91.53%,which was significantly higher than 76.27% of the control group (χ2 =9.524,P =0.003).No obvious adverse reactions or allergic reaction etc.were observed in the observation group.Conclusion The therapeutic effect of combination of Shenmai injection and meglumine in the treatment of chronic heart failure is accurate,it can significantly improve cardiac function,and has high safety,and without adverse reaction reports,it is worthy of the clinical application and popularization.
5.Acute megakaryocytic leukemia transformed from idiopathic myelofibrosis after 26 years: a case report and review

Objective To investigate the clinical data and laboratory features of acute megakaryocytic leukemia transformed from idiopathic myelofibrosis after 26 years in one case and the prognostic factors of myelofibrosis.Methods A case of acute megakaryocytic leukemia (M7) transformed from idiopathic myelofibrosis after 26 years was reported,and the clinical data was analyzed.Bone marrow cytology,cytogenetic and mutation detection in JAK2V617F were detected before and after transformation.Standard chemotherapeutic protocols including idarubicin plus cytarabine (IDA),homoharringtonine and cytarabine (HA),mitoxantrone and cytarabine (MA),pirarubicin plus cytarabine (TA) sequential therapies were performed.Results JAK2V617F mutation and normal karyotype were found before and after the transformation.This patient was treated with standard chemotherapeutic protocols of IDA,HA,MA and TA sequential therapies until getting complete remission,and he lived well till now.Conclusions Chromosome karyotype is related to the prognosis of IMF.Acute megakaryocytic leukemia (M7) with the normal karyotype transformed from the IMF can achieve complete remission by rational consecutive chemotherapy.
6.Effect of lidocaine on LPS-induced NF-κB activity in rat peritoneal macrophages

Objective To investigate the effect of lidocaine on the LPS-induced NF-κB activity in rat peritoneal macrophages. Methods The peritoneal macrophages obtained from male Wistar rats were placed in 12-well plates at 2 × 106 cell/ml after being cultured for 3 days. Each well contained 1 ml of cell suspension. The cells were randomized into control group (group C), LPS group and 3 LPS + lidocaine group S (group LL1.2.3)(n = 10 wells each). In group LPS and LL1,2,3, the cells were exposed to LPS 100 ng/ml. In group LL1,2,3 the cells were exposed to lidocaine 2, 20 200 kg/ml respectively in addition to LPS 100 ng/ml. After being incubated for 24 h, the HMGB1 concentration in the supernatant (by ElISA) and HMGB1 mRNA expression (by RT-PCR)and NF-κB activity in the cells were measured. Results LPS signiticantly increased HMGB1 concentration,HMGB1 mRNA expression and NF-κB activity in the supernatant. Lidocaine treatment significantly attenuated the LPS-induced increase in HMGB1 concentration HMGB1 mRNA expression and NF-κB activity in a dose-dependent manner. Conclusion Lidocaine can inhibit NF-κB activity in the rat peritoneal macrophages and in turn inhibit the synthesis and release of HMGB1.
7.Exploration of factors influencing shimming and water suppression on hepatic 1H-MR spectroscopy in vivo on 3.0 T

Objective To characterize the clinical factors which influence water suppression and auto-shimming line width for liver 3.0 T 1H-MRS.Methods Fifty-seven cases with liver 1H-MR spectroscopy(1H-MRS) were retrospectively studied, including chronic type B hepatitis (n=5), fatty liver (n=14), chronic type B hepatitis combining fatty liver (n=3) and normal situation (n=35).Independent t test was used to characterize the difference of general condition (height, weight, body mass index etc.) between different water suppression effect groups and between different shimming effect groups.Using Chi-square test to analyze whether water suppression rate and auto-shimming line width between fatty liver groups and non-fatty liver exist significance difference .Results By comparing WS ≥90% (n=47)group with WS < 90% (n=10) group, the former showed smaller average height[(164.7±8.0) and (170.5±3.7) cm respectively], weight[(61.8±10.4) and (73.4±5.4) kg respectively], BMI [(22.7±3.2) and (25.2±1.3)kg/m2 respectively]and LW[(17.7±3.7)and(24.6±6.3) Hz respectively]than the latter (t=-3.488,-3.415,-4.002 and-3.327, P < 0.05).By comparing LW 20 Hz (n=41) group with LW 20 Hz (n=16) group, the former showed better water suppression rate [(93.0±2.7)% and (86.1±8.5)% respectively]than the latter(t=3.213,P <0.05), whereas larger weight[(62.2±11.6) and (68.1±6.1)kg respectively]and BMI[(22.8±3.4) and (24.2±1.9)kg/m2 respectively](t=-2.516,-2.024,P <0.05).LW≤20 Hz in the fatty liver and non-fatty liver group was 7/17 and 34/40, respectively (X2=11.347, P < 0.05).WS < 90% in the fatty liver and non-fatty liver group was 10/17 and 0/44, respectively (X2=28.536, P < 0.05) .Conclusion Hepatic steatosis exerts an adverse effect in water suppression and shimming.
8.Comparison of Resovist and Gd-DTPA as susceptibility contrast agents for MR perfusion weighted imaging of hyperacute cerebral infarction in rats

Objective To compare the value on assessment of perfusion weighted imaging (PWI) using Gd-DTPA and the superparamagnetic iron oxide (SPIO) particles (Resovist) in the experimental model of hyperacute cerebral infarction in rats. Methods Thirty Wistar rats were randomly divided into Group A and B (each n=15). After occluding the left middle cerebral artery with thread for 1 h in all rats, two dynamic MR perfusion imaging series were acquired after intravenous bolus injections of 8 μmol Fe/kg SPIO (Group A) and 0.2 mmol/kg Gd-DTPA (Group B), respectively. The doses were chosen to obtain similar maximum signal change in normally perfused brain. The perfusion imaging was compared between the region of interest (ROI) in the core area with severe ischemia and in the penumbra area of moderate ischemia. The parameters were compared between two groups with pathologic findings and TTC stain. Results The first-passage profiles were found to be similar for Gd-DTPA and SPIO, both in the core area with severe ischemia and in the penumbra area of moderate ischemia of rats models. No statistical difference of the parameters (CBV, MTT, TTP) of PWI for ischemic penumbra was detected between Group A and B. Conclusion The efficacy of the Resovist using in the MR PWI is similar to Gd-DTPA for diagnosis of the perfusion reduction in the rat stroke models. Strong susceptibility effects may be achieved with small injection volumes. Resovist may play an important role in MR PWI.
10.Experimental Study on Kidney-yang Reinforcing Effect of Bama Nectar

Objective:To observe the effects of bama nectar in kidney-yang deficienct mice and on the mating ability and sperm counts of normal mice. Methods:The weight, body temperature, locomotor activity, cold-resistant,genital organ coefficient and lipid peroxidation of the hydrocortisone-induced kidney-yang deficient mice were detected. The genital organ ccoefficient, immune organ co-efficient and erectile function of the ovariectomized mice were detected. The mating ability and the sperm counts of normal mice were measured. Results:Compared with the model group, bama nectar obviously increased the weight, body temperature and locomotor ac-tivity of the hydrocortisone-induced kidney-yang deficient mice, and improved the locomotor activity, the organ coefficient of seminal vesicle, prostate gland, preputial gland and epididymis. Furthermore, bama nectar significantly increased the SOD activity in serum and reduced the level of MDA in serum when compared with the model group (P<0.05 or P<0.01). Compared with the model group, bama nectar could increase the organ coefficient of foreskin and thymus gland in the ovariectomized mice, and shorten the erec-tile latency (P<0. 05). Compared with the normal group, bama nectar improved the mating ability and the sperm counts of the male mice (P<0. 05 or P<0. 01). Conclusion:Bama nectar has beneficial effects on invigorating kidney and strengthening yang.
