1.Influence of food intake before oral intestinal cleaning drugs on cleansing effect, blood glucose and tolerance during colonoscopy examination

Objective To investigate the influence of oral intake of goop before oral intestinal cleaning drugs on blood glucose,cleansing effect and tolerance of patients.Methods 88 patients who were prepared for intestinal examination or bowel treatment with enteroscope were randomly divided into the food-intake group (group A) and the fasting group (group B) with 44 cases in each group.Group A was given goop 2.5 hours before oral intake of intestinal cleansing drugs,group B fasted before oral intake of intestinal cleansing drugs.Blood glucose was monitored before as well as 6 hours after oral intake of intestinal cleansing drugs.Intestinal cleansing effect was evaluated during intestinal examination or bowel treatment with enteroscope.Cases of patients who ceased intestinal examination or bowel treatment with enteroscope due to intolerance were summarized.The blood glucose,cleansing effect and tolerance of patients were compared between two groups.Results There was significant difference in blood glucose between the two groups before and after oral intake of intestinal cleansing drugs.The cleansing effect showed no significant difference,but the difference of tolerance between two groups was significant.Conclusions Intake of goop 2.5 hours before oral intake of intestinal cleansing drugs does not affects intestinal cleansing effect,and it can obviously increase the blood sugar,improve tolerance of patients,thus ensure the smooth completion of intestinal examination or bowel treatment with enteroscope.
2.Effect and nursing of Mayinglong musk hemorrhoids ointment on anus periderm skin damage of fecal incontinence patients

Objective To study the effect and nursing of Mayinglong musk hemorrhoids ointment on anus periderm skin damage of fecal incontinence patients. Methods Thirty-one patients with fecal incontinence-induced anus periderm skin damage from March 2012 to February 2013 in the hospital were set as the control group and another 32 patients with the same disease from March 2013 to January 2014 as the observation group. The anus periderm skin damage of the observation group was treated with Mayinglong musk hemorrhoids ointment 3 to 4 times a day. The anus periderm skin damage of the control group were treated with zinc oxide ointment, 3 to 4 times a day. Both groups used ostomy. The two groups were compared in terms of total effective rate in a month. Result The total effective rate of the observation group was 93.8% and the control group was 71.0%, with significant difference between the groups (P<0.05). Conclusions Mayinglong musk hemorrhoids ointment can be more effective in the treatment of fecal incontinence caused by anus periderm skin damage than zinc oxide ointment. It can improve the effect of treatment and reduce local wound pain. It is convenient to use, cheap and thus worthy of clinical popularization and application.

4.Impact of fixed specialist nurse triage on the quality of triage

Objective To investigate the influence of fixed nurse specialists triage to improve the quality of patient triage.Methods Nurses were divided into the fixed nurse specialist triage group (group A) and the out-patient department rotation nursing group (group B),respectively,each group was responsible for triage of patients in clinic of medical department for five days.Then the number of two subdiagnosis error and the average daily working overtime for doctors were compared.Results A total of 3806 patients were in clinic triage in group A,165 patients had triage error.A total of 3812 patients were in clinic triage in group B,308 patients had triage error.The average daily working overtime of doctors in group A was(5.24 ± 0.37)hours,and(8.16 ± 0.58) hours in group B.The errors in triage and working overtime of doctors had significant differences between two groups.Conclusions Fixed specialist nurse triage can significantly improve the quality of patient triage in clinic.
5.Effect of JUC on the cuts for episiotomy

Objective To investigate the effect of JUC on the incisions for episiotomy.Methods Three hundred primiparas to undergo episiotomy in our hospital were divided into two groups in equal number.The experiment group was given JUC Spray before suturing and the control group did not use any solution.In the two groups,antibiotics were not used after the operation,and the incisions were only cleaned with 0.5%povidone-iodine 2 times a day.Result There were significant differences between the two groups in terms of postoperative pains,inflammation and healing in the wounds,and hospital stay(P<0.05).Conclusions Application of JUC after episiotomy could be long-acting in antibacteria.It can reduce wound pain,improve wound healing rate, decrease the medical cost and shorten the hospital stay.
6.Diagnosis and management of adverse reaction due to botulinum toxin type A

Objective To analyze the clinical manifestation caused by local injections of botulinum toxin type A for cosmetic reason and the effect of antitoxin for adverse reaction.Methods Diagnosis and treatment process of 40 patients were retrospectively analyzed for adverse reactions due to botulinum toxin injections for beauty.Severe adverse reaction was assessed according to the patients clinical manifestation and the nervous system of physical assessment.The patients were given symptomatic treatment,observation or antitoxin therapy.Results Adverse reactions developed in 40 patients were mainly the nervous system abnormalities.Dyspnea developed in four cases.34 cases (85 %) were given symptomatic treatment.4 cases (10 %) were given botox treatment,and the patients were followed up for 1-3 months.The patients fully recovered to normal and did not leave any neurological sequelae.Conclusions The management of type A botulinum toxin injections should be strengthened.In case of poisoning,antitoxin therapy should be given as early as possible.
7.Awareness of early childhood sex education and influencing factors among rural kindergarten teachers in Sichuan

Objective:
To investigate the awareness of sexuality education knowledge in kindergarten teachers in rural areas of Sichuan and its influencing factors, and to provide a scientific basis for sex education capacity improvement among rural teachers.
Methods:
With multi stage stratified cluster sampling method, 162 teachers selected from 16 rural kindergartens in rural areas of Sichuan Province to conduct questionnaires related to early childhood sex education.
Results:
The awereness of early childhood sex education foreskin care in rural areas in Sichuan was 73.8%. Knowledge regarding foreskin care(58.8%)", "the function of the family includes rest and entertainment functions(65.6%)", "girls need to clean the reproductive truets from the front to back (68.8%)". Multivariate non conditional Logistic regression analysis results showed that teaching experience>4 years(OR=4.16, 95%CI=1.74-9.98), notice of young children with sexual related behaviors(OR=2.97, 95%CI=1.19-7.40), the number of sources of sexual knowledge sources ≥5(OR=3.39, 95%CI=1.42-8.13) were associated with high awareness of preschool sex education knowledge.
Conclusion
The preschool sex education knowledge awareness among Sichuan rural kindergarten teachers needs to be improved. A variety of sex education channels should be constructed to encourage teachers, especially those who are newly employed, to acquire more relevant knowledge, pay attention to children’s sex related behaviors, and improve their ability in sex education.
8.The role of antibiotics in the treatment of Crohn′s disease

Crohn′s disease (CD) is a chronic, progressive and destructive inflammatory disease affecting the entire digestive tract. Changes in the intestinal microbiota, particularly a decrease in gut microbiome diversity, are thought to be associated with chronic intestinal inflammation of CD. As for the mechanism of antibiotics in CD treatment, some scholars believe that antibiotics can affect the course of disease by reducing the concentration of intestinal bacteria and changing the composition of intestinal microbiota. Different antibiotics, including ciprofloxacin, metronidazole and rifaximin, have been proved to have certain therapeutic effect on some patients with CD in clinical practice, but there are still limitations in the use of antibiotics.In this review, the relationship between intestinal flora and the incidence of CD and the application of antibiotics in CD were reviewed, providing reference and help for the standardized application of antibiotics in CD.
9.Efficacy analysis of fecal microbiota transplantation in the treatment of 2010 patients with intestinal disorders

Objective To evaluate the efficacy and safety of fecal microbiota transplantation (FMT) for intestinal disorders. Methods A retrospectively descriptive cohort study was carried out. Clinical data of 2010 patients who underwent FMT and received follow?up for more than 3 months from May 2014 to November 2018 were collected, including 1,206 cases from Tongji University Shanghai Tenth People′s Hospital and 804 cases from Nanjing Eastern Military General Hospital. Of the 2,010 patients, 797 were male and 1,213 were female, with a mean age of (49.4±16.5) years old. Inclusion criteria were those with indications for FMT and voluntary treatment of FMT. Pregnant or lactating women, patients with end?stage disease, cases who were participating or participated in other clinical trials within 3 months, and patients with previous bowel history of pathogen infection, oral antibiotics or proton pump inhibitors (PPI) for the recent2 weeks, and those at immunosuppressive state were excluded. Informed consent was obtained from the enrolled patients and their families. There were 1, 356 cases of constipation, 175 cases of inflammatory bowel disease, 148 cases of chronic diarrhea, 127 cases of radiation enteritis, 119 cases of irritable bowel syndrome, and 85 cases of autism (complicating with intestinal disorders). FMT donor requirements: (1) 18 to 30 years old non?relatives, non?pregnant healthy adults with healthy lifestyle and good eating habits as volunteers to participate in fecal donation; (2) no administration of antibiotics within 3 months; (3) no chronic diseases such as constipation, irritable bowel syndrome, inflammatory bowel disease, etc., no autoimmune disease, not in immunosuppressive state, no history of malignant disease; (4) negative pathogen examination of infectious diseases (hepatitis B virus, hepatitis C virus, syphilis, HIV, etc.); (5) negative fecal examination (C. difficile, dysentery bacillus, Shigella, Campylobacter, parasites, etc.). The donor requirements after enrollment: (1) physical examination was reviewed once every two months, and the result still met the above requirements; (2) 16S rRNA sequencing was performed for every fecal donation in order to ensure that the composition and diversity of the fecal flora was stable and reliable. The preparation of the stool suspension referred to the Amsterdam criteria and the preparation process was less than 1 hour. The preparation of the FMT capsule was processed by pre?freezing the stool suspension after the preparation of the above suspension, and the frozen sample was transferred into a freeze dryer for freezing. The dried and lyophilized powder was encapsulated in capsules, and the capsule shell was made of acid?resistant hypromellose capsule (No.0) and pediatric?specific capsule (No.3), sealed and packaged in a-20℃refrigerator. Three ways of accepting FMT treatment pathways included 6?day transplantation after the placement of the nasointestinal tube, 6?day oral FMT capsule transplantation and one?time transplantation through colonoscopy. Intestinal preparation (nasointestinal tube feeding of polyethylene glycol until watery stool) was carried out before transplantation. Other treatments were stopped during treatment and follow?up, and any medication was not recommended when necessary. Results Of the 2010 patients, 1, 497 cases received nasointestinal tube transplantation (nasointestinal tube group), 452 cases oral capsule transplantation (oral capsule group) and 61 cases colonoscopy (colonoscopy group). At 3 time points of 3, 12, and 36 months after FMT, the clinical cure rates and the clinical improvement rates were 41.3% (560/1 356), 35.2% (320/909), 31.4% (69/220), and 29.0% (393/1 356), 27.8% (253/909), 29.1% (64/220), respectively in constipation patients; 33.1% (58/175), 29.9% (35/117), 24.5% (12/49), and 31.4% (55/175), 27.4% (32/117), 57.1% (28/49), respectively in inflammatory bowel disease patients;87.8% (130/148), 81.8% (81/99), 78.3% (36/46), and 8.1% (12/148), 7.1% (7/99), 4.3% (2/46), respectively in chronic diarrhea patients; 61.4% (78/127), 56.5% (48/85), 47.6% (20/42), and 21.2% (27/127), 15.3% (13/85), 14.3% (6/42), respectively in radiation enteritis patients; 53.8% (64/119), 45.0% (36/80), 6/15, and 21.0% (25/119), 26.2% (21/80), 4/15, respectively in irritable bowel syndrome patients;23.5% (20/85), 22.8% (13/57), 20.0%(5/25), and 55.3% (47/85), 49.1% (28/57), 40.0% (10/25), respectively in autism patients. Meanwhile the clinical cure rates and the clinical improvement rates at 3, 12, and 36 months were 47.7% (714/1 497), 42.8% (425/994), 39.1% (128/327), and 29.1% (436/1 497), 27.0% (268/994), 28.1% (92/327), respectively in the nasointestinal tube group; 38.7% (175/452), 30.2% (91/301), 33.3% (16/48), and 24.3% (110/452), 26.2% (79/301), 25.0% (12/48), respectively in the oral capsule group; 34.4% (21/61), 32.7% (17/52), 18.2% (4/22), and 21.3% (13/61), 13.5% (7/52), 45.5% (10/22), respectively in colonoscopy group. No serious adverse events occurred during treatment and follow?up period. The adverse event of nasointestinal tube group presented higher ratio of discomfort in respiratorytract accounting for 13.1% (196/1497); the oral capsule group had a higher proportion of nausea and vomiting when swallowing capsules accounting for 7.1% (32/452); the colonoscopy group was mainly diarrhea, accounting for 37.7% (23/61). The above symptoms disappeared after the nasointestinal tube was removed, or after treatment ended, or within 1 to 3 days after hospitalization. Conclusion FMT is a safe and effective method for the treatment of intestinal dysfunction.

【Objective】 To investigate the influence of different packaging methods on the volume of low-dose(0.5 U) suspended leucocyte depleted red blood cells(SLD RBC) and provide reference for accurate labeling. 【Methods】 Bags of SLD RBC in 1.5 U and 2 U were randomly sampled to measure the weight and specific gravity of each bag, so as to estimate the blood volume.The relationship between the weight and volume of 0.5 U blood, split from different parent bags, was analyzed and the linear regression equation was put forward.The regression equation was used to calculate and analyze the difference in the volume of 0.5 U SLD RBC prepared by three different packaging methods (A: manual multi-bag average packing; B: instrument multi-bag average packing; C: manual single-bag packing) in actual work. 【Results】 The specific gravity of 1.5 U (38 bags) and 2 U SLD RBC (39 bags) were (1.090±0.011) g/mL and (1.097±0.013) g/mL, respectively, and the difference was statistically significant (P<0.05).
