1.Evaluation of different doses dezocine combined with propofol intravenous anesthesia for artificial abortion

Objective To evaluate clinical results of different doses of dezocine combined with propofol in-travenous anesthesia in abortion.Methods 150 patients with routinely abortion were randomly divided into group A, B,C,at induction the patients were given anesthesia dezocine intravenous doses of 0.025mg/kg,0.05mg/kg, 0.1mg/kg,10min after slowly intravenous injection of propofol,the clinical results were compared in three groups of artificial abortion.Results Each time during surgery,mean arterial pressure(MAP),heart rate(HR)and blood pres-sure pulse saturation(SpO2 )indicators in group B were more stable than groups A,C the operations security was higher (tA =6.06,tB =0.88,tC =1.73,PA <0.05,PB >0.05,PC >0.05).Dose of propofol,postoperative recovery time of group B [(113.56 ±18.12)mg,(6.72 ±1.69)min]were significantly better than those in group A [(150.01 ±21.45)mg,(10.43 ±3.38)min],and the differences were statistically significant(t =9.17,6.94,all P <0.05 ).The propofol dose,postoperative recovery time in group B were slightly higher those in group C [(110.21 ±19.32)mg,(7.40 ±1.87)min],and the differences were not statistically significant(t =0.89,1.91,all P >0.05).The incidence rate of adverse reactions of group B (8.00%)was significantly lower than that in A group (30.00%),and the difference was statistically significant(χ2 =2.21,P <0.05).The incidence rate of adverse reac-tions in group B,C had no significant difference(P >0.05).Conclusion Abortion application dose of 0.05mg/kg of dezocine combined with propofol intravenous anesthesia has the best clinical effect,each relatively stable vital signs af-ter surgery,can effectively reduce the dose of propofol,shorten the operation time and postoperative recovery time,and has fewer adverse reactions.
2.Clone and expression of nuclear factor ?B p65 TAD

Objective To acquire NF-?B p65 TAD(transcription activation domain) and construct its eukaryotic expression vector and express it in endothelial cells.Methods Human umbilical vein endothelial cells(HUVECs) were cultured and total RNA was extracted.The p65 TAD gene was amplified by RT-PCR.After sequencing,the p65 TAD gene was inserted into the eukaryotic expression vector pEGFP-N1 with the green fluorescence protein,named pEGFP-N1-p65 TAD.pEGFP-N1-p65 TAD was transfected into HUVECs and its expression was observed under fluorescence microscope and analyzed by Western blotting.Results p65 TAD(288-548) was cloned successfully.The constructed plasmid including p65 TAD gene was identical to the designed.p65 TAD gene was expressed successfully in HUVECs.Conclusion The construction of eukaryotic expression vector including p65 TAD gene and its expression in HUVECs are very successful.

Objertive:To search for the measures of improving the effects on aesthetics of front teethmetal-poircelain crown. Method:The causes and mechanisms of different effects on aesthetics of front teethmetalprocelain crowns in 52 cases were analyzed. Resuts:The main causes that influenced aesthetics offront metalporcelain crown were out of color and bad imitation of teeth. Conclusion: Important factors thatimprove the effects on aesthetics of front teeth metal-porcelain crown include the design and the prepara-tion of cervical layer,accurate choice of the color and correct making process.
4.Improvement and evaluation of modeling method in animal models of osteoporosis

BACKGROUND:Animal models of osteoporosis play an important role in the research of the pathogenesis, occurrence and development of osteoporosis, as wel as in the clinical diagnosis, prevention and treatment of osteoporosis. OBJECTIVE: To summarize and discuss the establishment and research ideas of osteoporosis models, explore the current situation and advance of osteoporosis models, compare the advantages and disadvantages of various methods, and provide evidence for clinical investigation. METHODS: A computer-based online search was conducted in SinoMed, VIP, Wanfang and PubMed databases by using the key words of “animal model, osteoporosis” from January 1969 to October 2015. The language was limited to both Chinese and English. Relevant articles were screened according to inclusion and exclusion criteria. The documents about the methods of osteoporosis model preparation, method improvement as wel as their advantage and disadvantage were summarized. RESULTS AND CONCLUSION:A total of 576 articles were included. Among them, articles published earlier, duplicated, and similarly were excluded, and 53 articles were finaly included. Various animal models of osteoporosis may only focus on the certain causes, certain stage, some of the main symptoms and some pathophysiological changes of disease. Accordingly, appropriate modeling methods and experimental animals should be selected based on research objective. Rat undergoing castration is the most commonly used model in the modeling of osteoporosis. Among drug methods for constructing osteoporosis model, glucocorticoids is the most commonly used one. 5.Efficacies of fluid resuscitation plus continuous veno-venous hemofiltration in the treatment of septic shock

A total of 58 patients with simple septic shock were recruited from intensive care unit and divided into control group (n =28) and treatment group (n =30) according to treatment modalities.The control group was routinely treated.The treatment group received continuous veno-venous hemofiltration (CVVH) for 10-14 days plus routine measures.After 3 days,the score of acute physiology and chronic health evaluation-Ⅱ (APACHE-Ⅱ) of the treatment group was better than that of the control group (P < 0.01).And sequential organ failure assessment (SOFA),left ventricular end diastolic diameter (LVEDD),left ventricular end systolic diameter (LVEDS) and left ventricular ejection fraction (LVEF) improved significantly (P < 0.05).And the relevant blood biochemical parameters improved significantly better than the control group (P < 0.05).In the group CVVH,there were mortality (n =7,23%) and multiple organ failure (MODS) (n =6,20%) ; In the control group,mortality (n =14,50%) and MODS (n =13,46%).The mortality rate had inter-group differences of statistical significance (x2 =4.38,P <0.05).Thus early volume resuscitation plus CVVH had excellent curative efficacies for septic shock.
6.A Meta-analysis of the risk factors of delirium in ICU

Objective To identify the risk factors of delirium in ICU by Meta-analysis. Methods Quality of the studies was assessed in terms of study design, definitions of main variables, statistics, and bias control. Analysis of sensitivity and heterogeneity were performed and cumulative effects were calculated using either fixed or random effects models by RevMan 4.2.Results Ten studies met all inclusive and exclusive criteria. Simple sizes range from 100 to 3308. Twenty-one risk factors of delirium in ICU were involved, but only alcohol abuse, respiratory disease, infection, APACHE Ⅱ,elevated level of serum urea nitrogen, hyponatremia, hyperbilirubinemia and using sedatives were identified as having a cumulative effect on delirium in ICU. Conclusions Infection, abnormality or disturbance of metabolism and intoxication or acute withdrawal from drug or alcohol are independent predictors of delirium in ICU, while advanced age and hypoxemia, which are commonly considered as independent risk factors of delirium in ICU, are still inconclusive.
7.Establishment of a HL-60 cell line with stable RYBP silencing

Objective To establish stable HL-60 cell line with stable RYBP gene silencing using lentivirus-mediated RNA interference. Methods Five special shRNAs for RYBP gene were cloned to lentivirus vector.Recombinate lentivirus vectors were packed into lentivirus,which were used to infect HL-60 cells, and took empty vector and non-specific shRNA as control groups. Stable infected cells were selected with puromycin in 8 μg/ml concentration.The expression levels of RYBP were analyzed by Western blot,and confirmed the most effective RYBP shRNA.Then the level of mRNA was analyzed by real-time PCR.Results Stable infected cells were selected by puromycin successfully.Comparing to control groups,the expression of RYBP were reduced at different degrees (P < 0.01). And RYBP shRNA2 took the most silencing effect, the RYBP mRNA was decreased by more than 95 % (P < 0.05).Conclusion The shRNA2 targeting RYBP gene can effectively inhibit the expression of RYBP. HL-60 cell line with stable RYBP gene silencing were constructed successfully,which had provided experiment fundament for further studying the function of RYBP.
8.Ectopic Splenic Autotransplantation after splenic rupture and splenic resection: our experience in 15 patients

Objective To study the clinical characteristics and to summarize our experience in the diagnosis and treatment of ectopic splenic autotransplantation (ESAP) many years after splenic rupture and splenectomy.Methods The clinical data of 15 patients with ESAP who were treated in the People's Hospital of Zhengzhou from January 1998 to January 2013 were retrospectively analyzed.Results Of the 15 patients,2 patients presented with intestinal obstruction due to the ectopic splenic tissues and required partial resection of small intestine.In 5 patients,the ectopic splenic tissues were in the greater omentum which required resection of part or the whole of the greater omentum.In 4 patients,the ectopic splenic tissues were in the mesentery which required partial or total resection of the tissues.The ectopic tissues were in the pelvis in 3 patients and resection of the left fallopian tube was carried out in 2 patients and resection of the pelvic tissues in 1 patient.In 1 patient the diagnosis was made under laparoscopic biopsy,thus avoiding open surgery.Three patients had multiple implanted sites for these ectopic splenic tissues.Conclusions ESAP is an uncommon disease and it has no clinical characteristics.It is easy to confuse the condition with metastatic deposits.A diagnosis could be made only by combining the history with magnetic resonance imaging,or 99mTc study of red blood cells,or laparoscopic exploration and biopsy.
9.The prevalence of intradialytic hypotension under different diagnostic criteria and its association with mortality

Objective Intradialytic hypotension (IDH) is one of the common complications during hemodialysis,however its diagnostic criteria are highly controversial at present.In order to fully understand the prevalence of IDH in our center and figure out which diagnostic criteria is better for Chinese maintenance hemodialysis (MHD) patients,we choose several IDH definitions by reviewing published literatures and analyze their association with mortality.Methods The patients were recruited from Blood Purification Center of Ruijin Hospital undergoing hemodialysis during July 2012.Pre-,intra-and post-dialysis blood pressure were recorded.Patients' clinical characteristics,laboratory results and cardiac ultrasound results were collected.Based on several IDH definitions,we investigated the prevalence rate of IDH and its frequency among MHD patients.SPSS 23.0 was used to analyze data and conduct survival analysis.Results Totally 219 MHD patients underwent 16084 hemodialysis in 6 months.The prevalence rate,overall and individual frequency of IDH fluctuated between 45.21%-100.00%,4.64%-37.60% and 0.00%-33.00% respectively.For every IDH criteria,the patients were recruited into the group IDH(+) if they ever met the corresponding definition,otherwise the group IDH(-).Survival analysis found that IDH (the criteria of an absolute systolic blood pressure (SBP) < 90 mmHg or with a decrease of SBP≥ 20 mmHg) could decrease the risk of patients' cardiovascular mortality but was not relevant to all-cause mortality.Further analysis showed these patients had better cardiac functions mainly reflecting in lower Pro-BNP (2880 ng/L vs 6909 ng/L),lower prevalence rate of left ventricular hypertrophy (52% vs 83%) and higher left ventricular ejection fraction (65.0% vs 62.5%) than IDH(-) patients.No correlation was found between other IDH criteria and mortality.Conclusions The prevalence rate,overall and individual IDH frequency of IDH are of high variability when diagnosed by different IDH criteria.All IDH episodes defined by our selected definitions are of no association with all-cause mortality.An absolute SBP < 90 mmHg or with a decrease of SBP≥20 mmHg can decrease the risk of cardiovascular mortality due to their better cardiac function.Large scale researches should be conducted to find optimal IDH definition and explore the association of IDH and mortality.
10.Evaluation of the Diagnostic Value of Ulnar Nerve Compression in Cubital Tuunel Syndrome by Ultrasonography

Objective To evaluae the meaning and value of ultrasonography in the diagnosis of cubital tunnel syndrome(CuTS).Methods Forty patients(unilateral) of cubital tunnel syndrome(contralateral control group)were used to measure the ulnar nerve entrapment and the most extensive cross-sectional area and circumference.All cases underwent cubital incision preoperative,intraoperative direct vision under surgical line wrapped around the ulnar nerve compression and the most rough after a lap cut,and then we measured with a ruler,the two measurement methods comparison.The diagnostic threshold and the sensitivity and specificity of the most extensive cross -sectional area of the ulnar nerve and perimeter was analysed by using the receiver operating characteristic (ROC) curve.Results The most extensive cross-sectional area and circumference of the ulnar nerve in the CuTS group were significantly higher than those in the control group,and the difference was statistically significant.Forthe ultrasound measurement and intraoperative measurement of CuTS group at the pressure and the most rough circumference,the difference was not statistically significant,indicating that ultrasound can accurately measure the ulnar nerve circumference.The optimal cut-off point of the most rough cross-sectional area of the ulnar nerve and the circumference of the CuTS group were respectively 14.2mm2 and 14.8mm,and sensitivity were respectively 92.5% and 90%,the specificity were respectively 97.5% and 87.5%.Conclusion Ultrasonography is a new and valuable method for the diagnosis of cubital tuunel syndrome by measuring cross-sectional area and circumference of elbow.The optimal cut-off point of the most rough cross-sectional area of the ulnar nerve and the circumference of the CuTS group were respectively 14.2mm2 and 14.8mm.
