1.An analysis of postoperative complications in aged patients receiving pancreaticoduodenectomy

Objective To analyze the postoperative complications in aged patients receiving pancreaticoduodenectomy. Methods Pancreaticoduodenectomy was employed for the treatment of 52 aged patients. Of the 52 patients with a mean age of 70.5(65-76), 32 were male and 20 female. Postoperative pathological examination confirmed that 24 patients suffered from Vater,s ampullary and duodenal cancers, 16 from pancreatic carcinoma and 12 from cholangiocarcinoma. Results The mortality was 7.7%. Severe complications included bleeding (upper gastrointestinal tract or abdominal cavity),MOF, ARDS, postsurgical gastroparasis syndrome(PGS), mental disorder, pancreaticogastrostomy leak and biliary-enteric anastomotic leak et al. Conclusion MOF is the main lethal complication. Incidence of PGS and mental disorder is high.

The INK4a-ARF locus is located on CDKN2A point in human chromosome 9p21 and is known to encode two functionally distinct tumor-suppressor genes p16INK4a (MTS1/CDKN2/INK4a/p16) and p14ARF, have been shown to play significant roles in cell-cycle regulation in cancer. p16 is encoded by the exon 1a-exon 2-exon 3 transcript and functions as a negative regulator of the cell cycle through its inhibition of cyclin-dependent kinases (CDKs) 4 and 6 and subsequent blockage of the cyclindependent phosphorylation of pRB . Thus, loss of p16 function leads to deregulation of pRB's suppressive block of the G1-to-S transition and cell proliferation. In contrast, p14ARF is encoded by the exon 1?-exon 2 transcript and acts upstream of p53. Biochemical evidence has demonstrated that p14ARF physically interacts with human double minute 2 (HDM2) gene product and consequently stabilizes p53, leading to G1 cell-cycle arrest. Overall, somatic mutation of the INK4a/ARF tumor suppressor locus, resulting in functionally deficient p16 and possibly p14ARF proteins, seems to be a prevalent event in the development of oesophagus carcinoma.

4.Expression and clinical significance of leptin in esophageal squamous cell carcinoma

Objective To explore the relationship of serum leptin concentration,body mass index (BMI) and esophageal squamous cell carcinoma.Methods Blood samples were collected from 47 patients with esophageal squamous cell carcinoma and 20 healthy controls in Shanxi Cancer Hospital.Serum leptin was measured by ELISA.Results Serum leptin concentration in patients group was significantly higher than control group[(13.09±5.94) ng/ml vs (7.58±4.16) ng/ml,(t =3.76,P =0.001)].In 47 patients,BMI,albumin and lymphocytes were (25.20±3.14) kg/m2,(43.58±2.15) g/L and (2.02±0.55)×109/L,respectively.The serum leptin concentration of all patients (47 cases) were positive correlation with BMI (r =0.561,P =0.006),not with blood albumin and lymphocytes (r =0.206,P =0.242; r =0.122,P =0.412).Multiple logistic regressionanalysis reveal statistically significant association between serum leptin and esophageal squamous cell carcinoma incidence (OR =1.442,95 % CI 1.094-1.848,P=0.09).Conclusion Higher serum leptin levels seem to represent an additional and independent risk factor for esophageal squamous cell carcinoma.
5.Application of ATP-TCA in chemotherapy of human cardia cancer

Objective To investigate the drug sensitivity of cardia tumors in vitro by ATP-TCA, and to provide basis of clinic chemotherapy. Methods 42 cardia tumors were tested by ATP-TCA to determine and assess the drug sensitivity of 8 common chemotherapeutics. Correlation analysis was done between the clinical classification and the drug sensitivity. Results The drug sensitivity had significant difference among cardia tumors. From high to low, it was ADM>MMC>PTX> 5-Fu>HCT =VP16>CDDP>IFO, Sensitive degree were 50.00 %, 35.45 %, 31.25 %, 23.53 %, 10.53 %, 10.53 %, 7.69 %, 5 %. Sensitivity of the same drug in different pathologic stages had no significant difference. Conclusion ATP-TCA can play a guiding role in individual chemotherapy for human cardia cancer.

Objective To explore the early diagnostic value of quantitative sensory testing (QST) in diabetes with peripheral neuropathy (PNP).Methods The nerve conduction velocity (NCV) and QST were examined in 46 diabetic patients, and their results were compared. Results The abnormal ratio of NCV in 46 patients was 72.8%. 32 cases (69.6%) were diagnosed as diabetes with PNP. 7.Influence of Butylphthalide Soft Capsules on hemodynamics in acute cerebral infarction

Objective To evaluate the effect of Butylphthalide Soft Capsules on cerebral hemodynamics in patients with acute cerebral infarction by 64 slice spiral CT perfusion(CTP) imaging .Methods The patients with acute cerebral hemisphere infarction within 6 h after the onset were selected and randomly divided into the experimental group and the control group .Conventional plain CT and CTP were performed in both groups .In addition to the routine treatment the experimental group was orally administrated Butylphthalide Soft Capsules 0 .2 g ,once per 6 h .The CTP examination was performed again after 12 h .Results The CTP exami-nation showed that the cerebral perfusion in experimental group was improved in the ischemic marginal region ,the cerebral blood flow(CBF) and cerebral blood volume(CBV) were increased compared with before treatment ,the mean transit time(MTT) and the time to peak( TTP) were shortened compared with the before treatment .Conclusion Butylphthalide Soft Capsules can significantly improve the CBV of ischemic penumbra region and brain ischemic hypoperfusion .
8.Effect of nicotine on the degeneration of dopaminergic neurons in the substantia nigra of PD rats

Objective To explore the effect of nicotine on the degeneration of dopaminergic neurons in PD rats. Methods Forty-five SD rats were randomly divided into three groups: PBS group (CON), normal saline + LPS ( NS ) group and nicotine +LPS ( NIC ) group. On 24 hours after LPS or PBS injection, inducible nitric oxide synthase ( iNOS ) protein expression was examined by immunoblotting. ON 14d after LPS or PBS injection, the numbers of tyrosine hydroxylase ( TH ) positive neurons and morphological changes of OX-42 positive cells in the substantia nigra(SN) were observed by immunohistochemistry. TH mRNA and TH protein expressions were examined by RT-PCR or immunoblotting. Results Compared with CON group, iNOS protein increased markedly 24 hours after LPS injection in NS group. TH positive neurons, TH mRNA and TH protein in the SN decreased remarkably 14 days after nigrainjiection. Most of microlglial showed big cell body with thicker and shorter processes. However, nicotine reversed the above changes, Compared with NS group, TH positive neurons, TH mRNA and TH protein in the SN increased remarkably in NIC. Besides, most microglia showed small cell body with slim and long processes. Conclusion Nicotine could prevent LPS-induced degeneration of DA neurons, probably because of that the pretreatment with nicotine blocks the activation of microglia and the expression iNOS protein.
9.Effect of extrinsic vascular endothelial growth factor on viability of skin flap treated with radiotherapy in rat at different time after operation

Objective To study the effect of vascular endothelial growth factor(VEGF) on the viability of skin flap treated with radiotherapy at different time after operation and clarify its enhancing effect on angiogenesis.Methods Fifty Wistar rat were randomly divided into 3,5,7,9 d groups and control group.A 3 cm?4 cm full thickness dorsal flap with the pedicle remaining attached at the posterior end in rats and were treated with 60Co radiotherapy at the 2nd,4th,and 6th day postoperatively.The total quantity was 12 Gy.In the experiment groups every rat was given VEGF 120 ng on the 3rd,5th,7th and 9th day.The rats in control group were given the same quality normal saline and radiotherapy.Pathological observation,fluorescence staining and sesuccinic dehyrogenase(SDH) assay were used to measure the blood vessel density,blood vessel diameter,microcirculation and cell vitality of skin flap.Results In 5 d and 7 d VEGF group,the number of average blood vessels and cells of the skin falp was increased compared with control group(P
10.Application of short peptide enteral nutrition in preoperative bowel preparation for patients with gynecological laparoscopy

Objective:To explore the feasibility and efficacy of short peptide enteral nutrition in preoperative bowel preparation for patients with gynecological laparoscopy.Methods:A total of 100 patients with gynecological laparoscopy were randomized into the trial group (n =50) and the control group (n =50) by using the random number table method.Patients in the study group received short peptide enteral nutrition for three days before operation without mechanical bowel preparation.Patients in the control group received traditional liguid diet and mechanical bowel preparation.The satisfaction of bowel preparation,postoperative anal exhaust time,hospitalization time and nutritionrelated parameters were compared between the two groups.Results:Doctor's satisfaction of bowel preparation in the study group was significantly higher than the control group (90% vs 64%) (P < 0.01).Postoperative anal exhaust time and hospitalization time were shorter in the control group (both P < 0.05).The levels of postoperative albumin and hemoglobin in study group were significantly higher than the control group (bothP < 0.05).Conclusion:Short peptide EN in preoperative bowel preparation of gynecological laparoscopy reveals a better efficacy and safety,a better recovery in postoperative intestinal function and nutritional status,and a shorter postoperative hospitalization time.It is well worth clinical promotion.
