Objective To remedy the general defects of external fixator such as poor stability, complexed operation, strong stress shielding and bad extension function. Methods A total of 102 patients with limb shaft fracture were treated with the self-made external fixation and the curative effect observed. There were 84 males and 18 females aged at 5-78 years. Of all, 76 cases had tibial and fibular fractures, 10 femoral shaft fractures (children), 4 intertrochanteric or subtrochanteric fractures, 7 ulnar or radial fractures and 5 humeral shaft fractures. Results All patients got good results through a all-course observation, among which 76 cases with tibial and fibular fractures were followed up for 3 months to one year. Furthermore, the mean substantial bone healing time was only 3.5 months. The joint functions of the knee and the ankle recovered synchronously, with an overall excellence rate of 93.4%. No displacement of the fractured part or further fracture occurred. Conclusions The structure and function of the self-made external fixation accord with the demanding of current external fixator in orthopaedics.

Objective To report the clinical features a nd pathologic findings of 1 case of mixed epithelial and stromal tumor of the ki dney. Methods The patient who had a 20-year history of taking oral contraceptive was a 50-year-old woman with pain in left flank asso ciated with hematuria for half a year.CT and MRI revealed an 8 cm?6 cm cystic a nd solid mass located in the upper pole of the left kidney.A diagnosis of cystic renal carcinoma was made before operation.Radical left nephrectomy was performe d in July 2004. Results The upper pole of the left kidne y was found severely adhesive with peripheral tissues du- ring operation.Gro ssly, the tumor exhibited a 7.9 cm?8.9 cm?9.0 cm multilocular cyst full of coff ee- colored mucoid fluid with varying size ranging from 0.5 to 2.0 cm.Focall y thick-walled cysts with solid areas were seen.Microscopic findings showed the tumor was composed of a mixture of stromal elements formed by spindle cells and epithelial elements that were variable in cell types including cuboidal, columna r and hobnail cells.Differentiation of gastrointestinal tract was found in epith elial elements.Thick-walled blood vessels and distinctive bands of smooth muscl e cells were present in the stromal elements.Immunohistochemical staining reveal ed that the epithelial component was positive for AE1/AE3,and stromal component was positive for ER,PR,and SMA.No specific treatment was given and the patient w as free of signs of recurrence 8 months after operation. Conclusions Mixed epithelial and stromal tumor of the kidney is recognized to b e a kind of benign neoplasm.Differentiation of gastrointestinal tract in epithel ial elements is rare.It should be distinguished from any other benign and malign ant tumors in the kidney.

Objective To explore the association between complement C3 and the prevalence of simple fatty liver disease and nonalcoholic steatohepatitis. Methods A total of 2 799 health check-up participants were recruited from Tianjin Medical University General Hospital-Health Management Centre. Serum C3 levels were measured using immunoturbidimetry method and simple fatty liver disease and nonalcoholic steatohepatitis were diagnosed by liver ultrasonography and serum alanine aminotransferase concentration. Multiple logistic regression analysis was used to examine the association between quartiles of serum C3 levels and the prevalence of simple fatty liver disease and nonalcoholic steatohepatitis. Results After adjusting for covariates, the odds ratios (95% confidence interval) of simple fatty liver disease and nonalcoholic steatohepatitis for increasing quartiles of C3 were: 1.00 (reference), 2.15 (1.55, 3.01), 3.28 (2.35, 4.6) and 4.31 (3.11, 6.02); and 1.00 (reference), 4.54 (1.68, 15.91), 7.81 (3.02, 26.67) and 14.91 (5.94, 50.14) (both P for trend<0.000 1), respectively. Conclusions The study found that the elevated C3 levels were significantly associated with the prevalence of simple fatty liver disease and nonalcoholic steatohepatitis. Further study is needed to clarify whether C3 has a predictive value for the occurrence of nonalcoholic fatty liver disease.

Speckle tracking imaging (STI) was employed to investigate the effect of right ventricular (RV) volume and pressure overload on left ventricular (LV) rotation and twist in 35 patients with atrial septal defect (ASD), 18 of which with pulmonary hypertension, and 21 healthy subjects serving as controls. The peak rotations of 6 segments at the basal and apical short-axises and the average peak rotation and interval time of the 6 segments in the opposite direction during early systolic phase were measured respectively. LV twist versus time profile was drawn and the peak twist and time to peak twist were calculated. LV ejection fraction (EF) was measured by Biplane Simpson. Compared to ASD patients without pulmonary hypertension and healthy subjects, the peak rotations of posterior, inferior and postsept walls at the basal level were lower (P<0.05), and the average counterclockwise peak rotation of 6 segments at the basal level during early systolic phase was higher (P<0.05), and the average interval time was delayed (P<0.05). LV peak twist was also lower (P<0.05), and had a significant negative correlation with pulmonary arterial systolic pressure (r=-0.57, P=0.001). No significant differences were found in LVEF among the three groups. It was suggested that although RV volume overload due to ASD has no significant effects on LV rotation and twist, LV peak twist is lower in ASD patients with pulmonary hypertension. Thus LV twist may serve as a new indicator of the presence of pulmonary hypertension in ASD patients.
