Objective To evaluate the significance of cerebrospinal fluid(CSF) culture in diagnosis and treatment of childhood bacterial meningitis(BM).Methods Clinical data were collected and the etiology,laboratory results,treatment and disease prognosis were analyzed in children with positive CSF culture during Jan 2011 to Dec 2012 in our hospital.Results A total of 43 organisms were detected from CSF,13 cases of which were clearly diagnosed as BM.The coagulase negative staphylococcus was isolated most frequently (51.2％,22/43),meanwhile,CSF routine examination and biochemical test were abnormal in only 1 case.All results of blood cultures were negative.All cases were cured without changing antibiotics according to the drug sensitive test.Four strains of Streptococcus agalactiae,2 strains of Streptococcus pneumohiae and 4 strains of gram-negative bacilli were separated.Routine examinations of all CSF were abnormal in most cases,the CSF biochemical tests were abnormal and blood cultures were positive.Conclusion Coagulase negative staphylococcus separated in most CSF is contaminated bacteria.Streptococcus agalactiae,Streptococcus pneumoniae and gram-negative bacilli separated in CSF have important diagnostic value in BM.Diagnosis of BM should be combined with CSF culture,routine examination,biochemical test,blood culture results and clinical manifestations,then we can avoid unnecessary usage of antibiotics.

Objective To study the relationship between mental effort and personal time perspective during the process of individual changing bad habits.Methods 230 graduate students were collected to attend this longitudinal study of 21 days by experience sampling method.The data on mental effort and performance were obtained from the process of daily change of the bad behavior.Zimbardo Time Perspective Inventory (ZTPI) was applied to graduate students.The survey results were analyzed by hierarchical linear modeling (HLM).Results In 21 days,the mental effort of subjects showed very clear downward trend,the trend of the change was statistical significant (total score(5.63±2.62),t=-5.590,P＜0.01).Mental effort didn't have significant difference in five kinds of behavioral objectives (keeping early hours,exercising daily,dieting,reading daily,limiting screen time) (P＞0.05).The male obviously put more effort than the female during the study(t=2.743,P＜0.01).Both Present Hedonism and Futurism were inversely related to mental effort,which was statistical significant (t=-2.504,P＜0.05;t=2.39,P＜0.05).Both Present Fatalism and Futurism had the effects on the changing trend of the mental effort regulatory,which was statistical significant (t=-3.099,P＜0.01;t=-2.400,P＜0.05).Conclusion The mental effort of changing the behavior will gradually decline during the process of habit establishment.The mental effort of subjects who had present fatalism or futurism show a faster decline.

Objective To study the effect of parenting styles on the children' s self-control,and distinguish the influence between father and mother.Methods The present study selected 1 531 children (766 girls) between 3-10 years old.Data on parenting style were collected by parenting style questionnaire,while data on children' s self-control were measured by dual-system of self-control scale.Principal component analysis was conducted to examine common method bias.Results (1) No common method bias was found in this study.(2) Boys scored (31.66±5.95) on control system lower than girls (32.91±5.49),t=4.071,P＜ 0.01,but they had higher scores (43.19±8.32) on impulsive system than girls (41.96±8.32),t=2.226,P=0.027.(3)The associations of children's self-control scores with parents' spoiling,neglectful,authoritarian,inconsistent style were significant (r=-0.054～-0.174,P＜0.05),while it correlated positively with parents' democratic style (r=0.231,r=0.243,P＜0.05).The correlation between children's impulsive system scores and parents' spoiling,neglectful,authoritarian,inconsistent style were significantly positive (r=0.116-0.195,P＜0.05),while its associations with parents' democratic style were significant (r=-0.107,r=-0.108,P＜0.05).(4) As controlling children's age and sex,father democracy (β=0.120),inconsistency (β=-0.159),and mother democracy (β=0.188) were significantly correlated with children' s self-control (R 2adj =0.107,P＜0.05).Father despotism (β=0.070),inconsistency (β=0.136),mother's democracy (β=-0.063),despotism (β=0.099) and inconsistency (β=0.061) significantly predicted children's impulsive system (R 2adj =0.082,P＜0.05).Conclusion Each dimension of self-control and parenting style is high correlation.Father and mother's different rearing traits and the combination of different rearing traits contributed variously to children' s self-control system.

OBJECTIVE:To establish a method for the determination of dasatinib concentration in human plasma and study the bioequivalence of 2 kinds of tablets. METHODS:In a randomized two-way crossover study,24 healthy male volunteers were divid-ed into two groups,and were administered respectively with test and reference preparations 100 mg under fasting and fed condi-tions. The plasma concentration of dasatinib was determined by LC-MS/MS. Using imatinib mesylate as internal standard,the deter-mination was performed on Welchrom C18 column with mobile phase consisted of acetonitrile-0.1% formic acid(70∶30,V/V). Posi-tive ion scanning was conducted under MRM mode,ESI,and ion pair for quantitative analysis were m/z 488.5→401.2 (dasatinib) and m/z 494.6→394.2(internal standard). DAS 3.2.8 software was used for data processing and variance analysis was adopted to in-vestigate the bioequivalence of 2 kinds of tablets. RESULTS:The linear rang of dasatinib was 1-300 ng/ml. The pharmacokinetic pa-rameters of test and reference preparations under fasting conditions were as follows:cmax were (165.599 ± 67.592) and (164.533 ± 77.960)ng/ml;tmax were(1.145±0.504)and(1.080±0.467)h;t1/2 were(5.080±2.262)and(3.771±1.596)h;AUC0-36 h were (550.487 ± 256.494) and (585.986 ± 324.885) ng·h/ml. The pharmacokinetic parameters of test and reference preparations under fed conditions were as follows:cmax were(163.058±47.533)and(165.440±53.012)ng/ml;tmax were(1.630±1.066)and(1.576± 0.530)h;t1/2 were(4.720±2.677)and(4.311±2.610)h;AUC0-36 h were(568.036±192.521)and(601.100±216.855)ng·h/ml. Relative bioavailability of AUC0-36 h under fasting and fed conditions were(100.2±7.5)% and(99.2±3.8)%. Analysis of variance showed there were no significant difference in the pharmacokinetic parameters of between 2 preparations and cyde(P>0.05),there was statistical significance among subjects(P<0.05). CONCLUSIONS:LC-MS/MS method can determine the concentration of da-satinib in human plasma rapidly;and the two preparations are bioequivalent.

BACKGROUND: Hyperbaric oxygen can reduce brain ischemic-reperfusion injury, and this effect is closely related to the modulation of hyperbaric oxygen on microglias.OBJECTIVE: To explore the influence of hyperbaric oxygen on the activity of in vitro cultured brain microglias and secretion of interleukin-1, tumor necrosis factor α and nitric oxide (NO).DESIGN: Completely randomized grouping design, control experiment.SETTING: Teaching and Research Section of Diving Medicine, Teaching and Research Section of Immunity, Teaching and Research Section of Pathology, and the Experimental Animal Center, the Second Military Medical University of Chinese PLA.MATERIALS: This experiment was carried out at the laboratory of Diving Medicine as well as Teaching and Research Section of Immunity, the Second Military Medical University of Chinese PLA, between May 1999 and January 2000. Thirty neonatal SD rats of 1-day birth age were selected for the experiment.METHODS: [1] Brain microglias from newborn SD rats were cultured with digestion method, and microglias were identified with non-specific phosphodiesterase staining and cellular immunochemical staining. [2] Primary microglias were inoculated on 48-well culture board by 2×105/well and randomized into 5 groups: control group without hyperbaric oxygen pretreatment, and hyperbaric oxygen (0.2 MPa 1 hour) pretreatment 3, 7, 10,14 days groups. Cells in groups with hyperbaric oxygen pretreatment at the above various time points were then further divided into 2 subgroups, with one added with culture medium containing bacterium lipopolysaccharide of 1 mg/L (for microglia activation), but not in the other group. [3] Interleukin-1 activity was determined using thymocyte proliferation method. The activity of tumor necrosis factor-α was assessed with L929 cell toxicity test.Nitrous acid content detected by Griess method represented NO content.[4] t-test was used to compare the differences in non-paired quantitative data between the two groups.MAIN OUTCOME MEASURES: The activity of interleukin-1 and tumor necrosis factor a and NO content in resting and evoked brain microglias in rats at various time points of hyperbaric oxygen pretreatment.RESULTS: Thirty SD rats entered the result analysis. [1] The activity of interleukin-1 and tumor necrosis factor α and NO content in resting brain microglias: The two groups did not differ obviously. [2] Interleukin-1activity and NO content in lipopolysaccharide-evoked brain microglias: They were significantly lower in 10-day and 14-day hyperbaric oxygen pretreatment groups than those in control group [10-day group: 0.409±0.014,(5.21±0.77) μnol/L; 14day group: 0.381±0.004, (4.93±1.02) μmol/L, P ＜ 0.05].[3] The activity of tumor necrosis factor α in evoked brain microglias: It was obviously lower in 14day hyperbaric oxygen pretreatment group than in control group [(51.20±1.13) %, (70.10±2.26) %, P ＜ 0.05].CONCLUSION: Pretreatment with 0.2 MPa hyperbaric oxygen can suppress the secretion of interleukin-1, tumor necrosis factor α and NO by evoked microglias, but has not obvious effects on resting microglias.

Objective To evaluate the efficacy of PET-CT brain imaging and resting-state fMRI in preoperative localization of temporal lobe epileptic (TLE)focus.Methods PET-CT and resting-state fMRI were performed in 17 patients with refractory TLE,who then underwent surgical treatment.Seventeen healthy volunteers matched with gender and age were recruited as the control group.The resting-state fMRI images were post processed by SPM5 software.Regional homogeneity(ReHo) values of the whole brain and bilateral hippocampus were obtained and analyzed.PET-CT images were analyzed by visual analysis method and asymmetry index method and the standardized uptake value (SUV) of bilateral hippocampus were obtained.The ReHo values and SUV of the bilateral hippocampus were compared by two independent samples t-test,and analyzed by receiver operating characteristic curve (ROC) for optimized diagnostic threshold.Pearson correlation analysis was employed for evaluating the correlation between the SUV and ReHo values of bilateral hippocampus.The consistency between the diagnostic accuracy of PET-CT and resting-state fMRI was assessed by Kappa consistency test.The outcome of the patient group was compared with that of the control group,and with the pathological results,to evaluate the diagnostic value of the two modalities for preoperative localization of temporal lobe epileptic focus.Results Regional or comprehensive low metabolism of 18F-FDG in temporal lobes was presented in all 17 patients,and 11 patients out of 17 showed lateral decreased ReHo value.The diagnostic accuracy of the two examinations was 70.6％ (12/17) and 64.7％ (11/17) for PET-CT and resting-state fMRI respectively compared with pathological results,and could be increased to 76.5％ (13/17) when the two methods were combined for diagnosis.The ReHo values of the TLE group (0.34 ± 0.12)were significantly lower than those of the control group (0.46 ± 0.07) (t =3.230,P =0.003).The sensitivity and specificity of resting-state fMRI were 88.2％ and 94.1％ respectively when the ReHo value was 0.36.There was significant difference between the SUV of the affected (4.17 ±0.63) and healthy side(4.77 ±0.56) of hippocampus in TLE group(t =2.930,P =0.006).The sensitivity and specificity of PET-CT were 88.2％ and 64.7％ respectively when SUV was 4.23.The two values could be used as a threshold in the localization of temporal lobe epileptic focus.Consistency of lesion detection was revealed between PET-CT and resting-state fMRI though it was not high,and the Kappa value was 0.49.However,no correlation was detected between the SUV and ReHo value using Pearson correlation test(r =0.280,P =0.314).Conclusion Combined PET-CT brain imaging and resting-state fMRI as a multi-modality imaging method might improve the diagnostic accuracy of the TLE focus's localization.

Objective To investigate the incidence of attention deficit hyperactivity disorder (ADHD)symptoms among 3-10-year-old children and its relationship with sleep.Methods Taking gender,grade (junior in kindergarten to third grade),and region(urban and rural areas) as stratification variable,1 535 children aged 3 to 10 years old were selected by random in six areas nationwide.ADHD Rating Scale-Ⅳ-Parent Version and Pittsburgh Sleep Quality Index were adopted to investigate ADHD symptoms and sleep of these children.Results 24.95％ of the investigated children had ADHD symptoms.Boys scored higher than girls in all indicators(boys:Inattention (IA):(9.21±3.75),Hyperactivity-Impulsivity (HI):(8.62± 3.87),Total:(17.82±7.01),girls:IA:(8.16±3.77),HI:(7.32±3.76),Total:(15.48±6.98)) while girls had higher incidence of ADHD symptoms than boys (girls:IA:16.19％,HI:27.02％,Total:32.51％,boys:IA:11.70％,HI:8.19％,Total:17.43％).Peak incidence occured in children aged 7-9 (7 years old group:IA:18.22％,HI:21.93 ％,Total:38.66％,8 years old group:IA:20.66％,HI:23.25 ％,Total:38.01％,9 years old group:IA:18.22％,HI:33.78％,Total:32.00％).Rural children had higher incidence of ADHD symptoms than urban children(rural children:IA:18.37％,HI:18.37％,Total:29.48％,urban children:IA:10.47％,HI:15.12％,Total:21.40％).Logistic regression results showed that sleep quality,sleep disorder and daily dysfunction can predict the incidence of ADHD symptoms to a certain degree.Conclusion The incidence of ADHD symptoms appear to be high in children evaluating by ADHD Rating Scale-Ⅳ-Parent Version.More attention should be paid to girls and rural children' s ADHD symptoms.7-9 years old children have the highest incidence of ADHD symptoms.The more problems in daytime dysfunction,sleep disorders and bedtime,the more ADHD symptoms will be observed.

Objective To investigate the perioperative hemodynamic features of mitral replacement of patients with small left ventricle.Methods Patients with left ventricle end-diastolic dimension (LVEDD) less than 40mm received mitral replacement sur- gery were divided into big size M-2 group and small size M-1 group.The perioperative echo results were analyzed with SPSS software. Results The big size M-2 group has lower trans mitral gradient [ (5.9?1.6) mm Hg vs.(10.7?3.2) mm Hg],larger in vivo va- lular acre[ (2.9?0.2) cm~2 vs.(2.6?0.2) cm~2],and high mitral match index [(1.92?0.23) cm~2/m~2 vs.(1.73?0.18) cm~2/m~2 ].Conclusion With the meticulous perioperative treatment and myocardial protection,the patients with small left ventricle should also receive a mitral replacement as big as possible to achieve the ideal hemodynamics results.

Objective To determine the expression level of microRNA-25 in animal models of diabetic nephropathy and human renal tubular epithelial cells (HK-2) cultured in different conditions, and to explore its regulating effect on the fibrosis in diabetic nephropathy. Methods The expression of microRNA-25 was detected by real-time PCR. The downstream target protein of microRNA-25 was verified by bioinformatic prediction, transient transfection of cells and Western blotting. Results MicroRNA-25 was down-regulated in animal models of diabetic nephropathy and HK-2 cells which were cultured in high-glucose medium (P<0. 01). MAP2K4 might be the downstream target protein of microRNA-25. Overexpression of microRNA-25 reduced the protein expression of MAP2K4 and α-SMA (P<0. 01). Conclusion MicroRNA-25 inhibits the fibrosis in diabetic nephropathy by regulating the expression of MAP2K4.

Objective:To evaluate the reliability and validity of the Chinese version of the Self-Regulatory Fa-tigue Scale (SRF-S)in Chinese young people.Methods:A sample of 553 young people from a university and a state-owned enterprise was administrated with the SRF-S,the Simplified Coping Style Questionnaire (SCSQ)and the Dual-Mode Self-Control Scale (DMSC-S).Item analysis was conducted to identify valid items and revise the scale.Then the construct validity of the revised scale was tested through confirmatory factor analysis.Finally,a sam-ple of 238 graduate students was retested two weeks later to examine the reliability of the scale.Results:Item analy-sis showed that the SRF-S had satisfactory item discrimination (r >0.38).Confirmatory factor analysis showed that the whole scale had a three-order factor and good construct validity (χ2 /df=5.08,RMSEA =0.09,NFI =0.90, NNFI =0.90,CFI =0.92,IFI =0.92,GFI =0.90)after removing 2 items with factor loadings lower than 0.32,and the range of factor loading was between 0.37 and 0.71.The SRF-S total and subscale scores were positively corre-lated with the scores of negative coping and poor control (r =0.25 -0.58,Ps <0.001),and were negatively associ-ated with the scores of positive coping and good self-Control (r =-0.22 --0.47,Ps <0.001).The Cronbach's αcoefficients were 0.84 for the total scale,and 0.64 to 0.69 for the three subscales,respectively;the test-retest relia-bility coefficients were 0.73 for the total scale,and 0.62 to 0.67 for the three subscales,respectively.Conclusion:It suggests that the Chinese version of SFR-S could be used as an effective and reliable tool for assessing self-regu-latory fatigue state among Chinese young people.
