1.Practice in accelerating hospital construction in a distressed county

An issue that has to be settled urgently is how to accelerate hospital construction in a distressed county. Specific measures may include: ①clarifying the orientation of development on the basis of an analysis of the status quo; ②speeding up infrastructure construction while doing a good job of planning for the hospital area; ③raising funds from a variety of sources so as to purchase facilities urgently needed in medical treatment; ④taking vigorous steps for intensional building and personnel cultivation, such as formulating criteria for post assumption, with the backing of economic policies, selecting 12 to 15 people each year for higher training in advanced areas, and striving for support from big hospitals in the provincial capital; ⑤adopting integrated management and amplifying rules and regulations; ⑥striving for preferential policies and support from higher authorities: for instance, conformity between hospital construction and municipal planning alone saved over 3 million yuan of funds and the county government raised over 6 million yuan of funds for the hospital.
2.Establishing the SREBP Pathway in the Oocytes Xenopus Laevis and Stuyding the Effects of the Cucurmin

[Aim] The expressed system of sterol regulation by SCAP/SREBP Pathway was established in the oocyte of the Xenopus laevis and was applied to investigate the effect of cucurmin on the SREBP pathway.[Methods] The plasmid of PLXRN-4SRE-fpa was restructed and injected into the nucleus of Xenopus Oocytes in phase V or VI. The oocyte was incubated with different concentrations of the curcumin. After three days the fluorescence intensity on the membrane of the Oocytes was measured by Fluoroanalyzer.[Results] The fluoresecence intensity on the membrane of the oocyte increased with increasing of the concentration of the curcumin. The trail group which have all regulation elements of SCAP/SREBP pathway can express more fluorscence protein than the control group. [Conclusion] The experiment provides that the curcumin can upgrade the expression of low density lipoprotein receptor by SCAP/SREBP regulation pathway. The model can be applied for quick screening of the scap ligand drugs and inverstigating the mechanism of the drugs.
3.Risk factors and pathogenesis analysis in pulmonary infection complicated to acute stroke patients.

Objective To study the risk factors and pathogenesis of pulmonary infection in acute stroke patients. Methods A retrospective analysis was taken in 257 acute stroke cases to investigate the incidence of pulmonary infection and its relationship with the underlying disease, disturbance of consciousness, invasive operation,the type of pathogenic bacteria and drug susceptibility. Results The positive rate of pulmonary infection in acute stroke patients was high and related to many factors including severity of stroke, disturbance of consciousness,primary diseases, and invasive examination etc. The most popular infective bacteria were Gram-negative bacteria. The imipenem was most sensitive to Gram-negative and Gram-positive bacteria, followed by Amikacin and piperacillin too. Conclusions The pulmonary infection in cerebral stroke patients is caused by many factors, of which Gramnegative bacteria play the most important role. Identification of the risk factor and pathogenic bacteria is helpful in treating cerebral stroke infectious complications.
4.Effect of relaxation therapy applied on patients with sleep disorders because of post craniocerebral traumatic syndrome

Objective To study the effect of relaxation therapy on sleeping quality of patients with post cran- iocerebral traumatic syndrome. Methods 90 cases with sleep disorders were randomly divided into the treatment group and the control group. The control group was given routine care and the treatment group was given relaxation therapy combined with routine care. Take pittsburgh sleep quality index (PSQI) as evaluation criteria. Results There was significant difference between the two groups( P < 0.01 ). Sleep quality was more improved by relaxation therapy in the experimental group than that of the control group. Conclusion Relaxation therapy can effectively im- prove sleep quality in patients with post craniocerebral traumatic syndrome.
5.Correlation Factors of Nosocomial Infection in Patients after Receiving Craniocerebral Operation

OBJECTIVE To analyze the correlation factors of nosocomial infection in patients after receiving craniocerebral operation.METHODS Totally 388 hospitalized patients after receiving craniocerebral operation from Jan 2006 to Dec 2006 were studied retrospectively.RESULTS From them 45 pateints were suffering from nosocomial infection and total incidence of nosocomial infection was 11.59%.Among them,60.0% was lower respiratory tract infection,11.11% was surgical wound infection and 8.89% was upper respiratory infection.Aging,length of stay and invasive operation(deep vein needle detaining,trachea incision,tracheal intubation,urinary canal and drainage tube dwelling) were the main correlation tactors.CONCLUSIONS Minimizing invasive operation and early removing various kinds of dwelling tubes,using antibiotics correctly,improving hospitalizd environment and strengthening wards management are the measures to reduce nosocomial infection according its correlation factors.
6.The influence of early rehabilitation intervention on the recovery of patients who have undergone the low rectal cancer resection

Objective To investigate the influence of early rehabilitation on the recovery of patients who have undergone the low rectal cancer resection. Methods 130 patients with low rectal cancer resection were divided into the rehabilitation group and the control group randomly, there were 65 cases in each group. The control group were accepted the routine nursing care, and the rehabilitation group were accepted the early recovery intervention. Compared the time of anus exhaust, the time of stomach extubation, the incidence rate of postoperative complication, the time of bed rest and the quality of postoperative life in these two groups. 7.The clinical efficacy of vitrectomy with intravitreal ranibizumab at different injection time for proliferative diabetic retinopathy

Objective To assess the clinical efficacy of vitrectomy with intravitreal ranibizumab (IVR) at different injection time for proliferative diabeticretinopathy (PDR).Methods This was a prospective,comparative,and randomized study.Ninety-seven eyes of 97 patients were enrolled and randomly assigned to three different treatment groups:30 eyes (30 patients) in the preoperative IVR group,32 eyes (32 patients) in the intraoperative IVR group and 35 eyes (35 patients) in the no IVR injection group.The best corrected visual acuity (BCVA) (F=0.18) and the grading of vitreous hemorrhage (x2 =1.39) before surgery did not differ significantly among the 3 groups,respectively (P > 0.05).All eyes enrolled underwent conventional 23-gauge pars plana vitrectomy (PPV).The preoperative IVR group received intravitreal 0.5 mg/0.05 ml ranibizumab injection 3 to 7 days before PPV,intraoperative IVR group received intravitreal 0.5 mg/0.05 ml ranibizumab injection at the end of PPV and non-drug injection group received PPV only.Postoperative BCVA,fundus color photography,optical coherence tomography examination was performed in all eyes at 1 week and 1,3,6,9,12 months after surgery.Early RVH was defined as RVH occurred within 1 week to 1 month postoperatively;while late RVH was defined as RVH occurred 1 month later after the operation.Results The mean BCVA were all improved among the 3 groups compared with the preoperative vision at 1 month after operation.At the beginning of 3 months after surgery,the average BCVA of the preoperative injection group and the intraoperative injection group tended to stable;while 3 eyes in the non-drug injection group began to decreased.There was no significant difference in average BCVA at 1,3 and 12 months of follow-up periods among the 3 groups (F=1.42,1.17,0.26;P>0.05).The incidences of early RVH were 16.7%,9.4%,28.6% in the preoperative injection group,intraoperative injection group,and non-drug injection group,respectively (x2 =5.12,P<0.05).The incidence of early RVH in the intraoperative injection group reduced compared to preoperative injection group and non-drug injection group (x2 =4.04,4.93;P<0.05).The incidences of late RVH were 13.3%,9.4%,14.3% in preoperative injection group,intraoperative injection group,and non-drug injection group,respectively (x2 =0.47,P>0.05).The average centeral foveal thickness (CFT) decreased among the 3 groups in different degrees at 1 month when compared with that of 1 week after operation and the decreasing was statistically significant (F=59.50,P<0.05).A subgroup pairwise analysis showed no significant difference of decreasing CFT in preoperative injection group compared with that of intraoperative injection group (t=0.23,P>0.05).The average CFT of the 3 groups had different degrees of thickening at 3,6,9,12 months after surgery,and the increasingof CFT among the 3 groups were not differ significantly (F=2.92,2.86,3.07,3.12;P>0.05).Conclusions The adjunctive use of IVR can reduce the incidence of early postoperative RVH in vitrectomy for PDR,decrease in macular thickness and obtain favorable visual recovery.The effect of preoperative IVR injection was slightly better than that of the intraoperative IVR injection.

9.The effectiveness of oral placement therapy in management of sialorrhea of stroke patients

Objective To observe the therapeutic effects of oral placement therapy (OPT) on managing sialorrhea after stroke.Methods A total of 37 stroke inpatients with sialorrhea were enrolled from January 2011 to September 2013 in the authors' department for the study and divided into 2 group on the basis of the time of enrollment.The control group (n =18) received 30min of routine treatment (including such neuromuscular facilitation techniques as Bobath and Rood techniques,motor relearning program,neuromuscular electrical stimulation and ice stimulation),twice daily,and the treatment group (n =19) received 15 min of routine treatment plus 15 min of OPT,twice daily.Frenchay Dysarthria Assessment was used to evaluate the sialorrhea severity and clinical efficacy before and 1,2 and 4 weeks after initiation of treatment.Results Sialorrhea symptom was significantly improved in treatment group after 1 week (P < 0.05),while no significant improvement was observed in control group (P > 0.05).After 2 and 4 weeks of treatments,significant improvements of sialorrhea were noted in both groups.After 1,2 and 4weeks of treatment,the total effective rate were 63.16%,94.74% and 94.74%,respectively,in the treatment group,versus 5.88%,61.11% and 61.11%,respectively,in the control group.The total effective rate of the treatment group were significantly higher in the above three time-points than the control group (P < 0.05).Conclusions Oral placement therapy can improve sialorrhea symptom after stroke more effectively than routine treatment.
10.Strain rate imaging assessment of left atrial early function in pilots with essential hypertension

Objective To assess the left atrial early function in pilots with essential hypertension by strian rate imaging (SRI). Methods Twenty-five hypertensive pilots without left ventricular hypertrophy and expansion (LVN) were selected. Twenty-ifve healthy pilots were included as control group. At the left ventricular apical four-chamber view, three-chamber view, two-chamber view, the systolic, early and late diastolic peak strain rate (SRs, SRe, SRa) curves were acquired by SRI in the basal section, the middle section and the upper section of the wall of left atrium. The average strain rate (mSRs, mSRe, mSRa) was calculated. The LA maximal, minimal and pre-systolic volume (LAVmax , LAVmin and LAVp), LA passive ejection fraction (LAPEF), LA active ejection fraction (LAAEF) , left atrial stroke volume (LASV), E/A and E/E′ were calculated. Results The LAPEF, LAAEF, LASV, E/A, E/E′were (38.96±6.43)%, (61.11±5.86)%, (14.74±5.33)mm, 1.23±0.08, 8.63±1.77 in the control group, and were (34.18±5.72)%, (63.23±6.89)%, (10.12±3.23)mm, 1.04±0.26, 7.16±0.84 in the study group. Compared with the control group, signiifcant difference was detect in LAPEF, LAAEF, LASV, E/E′(t=4.840, 2.690, 4.801, 2.950, all P<0.05). No signiifcant difference was found in E/A between the study and control group (t=-1.424, P=0.178). There was signiifcant positive correlation between SRa and E/E′(r=0.632, P<0.05). Conclusion SRI could be applied in the evaluation of early left atrial function. In pilots with essential hypertension, the changes of strain rate in left atrial wall, which is prior to the modeling geometry of left ventricular, could be detected by SRI.
