1.Monitoring of Medical Staff's Hand Contamination after Change for Infected Wound and Managing Hand Hygiene

OBJECTIVE To monitor the hand contamination after dressing change for the infected wound and to explore an effective way for hand hygiene. METHODS Caregivers were divided into three groups after applying dressing on infected wound:group 1(no hand-washing),group 2(regular hand-washing),and group 3(standard hand-washing),the pathogens in hands were monitored and the effects of two ways of hand-washing were compared. RESULTS The rate of hand-carrying pathogens after dressing change for the infected wound was 100.0%,the qualified rate of regular hand-washing was only 47.9% and that of standard hand-washing (six-steps method)was 97.9%. CONCLUSIONS The standard hand-washing is more effective than regular hand-washing. Correct hand hygiene should be emphasized to prevent and control nosocomial infection.

BACKGROUND: The glycoprotein which ends of a-galactosyi residues (a-Gal) is the major heterogenic antigen for hyperacute rejection. OBJECTIVE: To observe the distribution characteristic of the small tissue engineered vessel scaffold, and a-Gal in endothelial cells and smooth muscle cells of Wistar rats and Japanese white rabbits, in addition, to discuss the feasibility of applying acellular tissue vessel scaffold to heterogeneous blood vessel transplantation. DESIGN, TIME AND SETTING: The contrast observation was conducted at the Department of Human Anatomy of China Medical University between March 2003 and December 2004. MATERIALS: Totally 30 caudal arteries of Wistar rats were collected. Fifteen of which were prepared for small vessel scaffold (small vessel scaffold group), the remained 15 served as caudal artery group. Additionally, 15 central arteries were obtained from each ear of Japanese white rabbits (central artery group). METHODS: Totally 16 mg/L Bandeiraea Simplicifolia I Isolectin B4 (BSI-B4) was added for DAB staining with Affinity histochemistry method. Then MetaMorth/C5050/BX41 microscopic image analysis system was used to detect the positive reaction product of a-Gal. MAIN OUTCOME MEASURES: Color changes of vascular wall were observed under light microscope; the optical density of the positive reaction product of a-Gal was measured. RESULTS: The expression of a-Gal was mainly located in the cell membrane, as well as cell nucleus of endothelial cells in the central artery group. The expression of a-Gal of endothelial cell was strong positive in the caudal artery group, which was weak or negative expressed in the small vessel scaffold group. The optical density of a-Gal expression was lowest in the tunica intima of small vessel scaffold group, which was less in the central artery group than the caudal artery group (P < 0.001). The a-Gal expression in the tunica media of small vessel scaffold group was less than the central artery and the caudal artery groups (P < 0.001). CONCLUSION: In the caudal artery of Wistar rat, the expression of a-Gal is higher than that in the central artery of Japanese white rabbit. Therefore, the heterogeneous tissue engineered vessel material from acellular caudal artery of Wistar rats can be used in blood vessel transplantation.
4.Determination of Five Anions in Drinking Water by Ion Chromatography

Objective To develop a conductance-ion chromatography for simultaneous determination of 5 anions in drinking water. Methods The orthogonal experiment method L9(34) was used to select the optimum working conditions of IC-2000 ion chromatography. Under the founded chromatography condition, 5 anions in water samples were determined simultaneously after filtered with 0.45 ?m filter membrane. Results Both of the precision and accuracy of this method were desirable, all the relative standard deviation was less than 5.0%, the rates of recovery were 90%-106.7%, the correlation coefficient was more than 0.999 except NO2-(0.990 5). Conclusion This method is simple, accurate, quick and suitable for simultaneous determination of 5 anions in drinking water with no need of sample pretreatment.
5.Diagnosis and treatment of primary duodenal papilla neoplasm

ObjectiveTo study the diagnosis and treatment of primary duodenal papilla neoplasm(PDPN). MethodsThe clinical data of 64 patients with PDPN were analysed retrospectively. Results Among 38 cases received deodunscopy, PDPN were found in 32 cases (84.2%). Tumors were found in 49 of 51 cases received ERCP. There were 2 cases of benign tumor and 62 of maligancies in this series. 4 of 46 cases survived 4 years after Whipple operation,and 4 of 11 cases underwent lacal resection of PDPN suvived 4~6 years. ConclusionsTEndoscopy and ERCP are main methods for the diagnosis of PDPN; It is essential to use the rationale operation to improve the results of surgical treatment.
6.Impact of Disinfection Quality in Surgical Dressing Room on Wound Healing

OBJECTIVE To investigate the impact of disinfection quality in surgical dressing room on the healing of wound with dressing exchanges in order to implement procedures to prevent nosocomial infection. METHODS A retrospective investigation was performed and cases of poor surgical wound healing from Jan 2006 to Dec 2007 were collected and analised. Effective measures were taken to enhance the management and surveillance in the dressing room. RESULTS After taking the prevention procedure,the passing rate of disinfection was increased. Before implementing the prevention procedure the passing rates of air,surface and finger cultured were 83.3%,83.3% and 75.0%,respectively,After taking the prevention procedure the passing rates of above were all 100.0%.The rate of poor wound healing decreased from 4.16% to 1.59%. CONCLUSIONS The disinfection quality of dressing room has direct effects on the wound healing. The management of dressing room disinfection should be emphasized to cut down the nosocomial infections of wound.
7.Explortation of the clinical application of total thyroidectomy

Objective:To discuss the method of surgery treatment for nodular goiter,Graves disease,Hashimoto thyroiditis and differentiated thyroid carcinoma. Methods:A retrospective analysis of the clinical records of 76 patients undergoing total thyroidectomy was analyzed for the method of surgery treatment for thyroid diseases from resection extent,operative techniques,intraoperative and postoperative complications. Results:Five patients(78.4%)of all the patients accepted total thyroidectomy in first operation had transient or short-term completely recovered hypocalcemia postoperatively,and there were no cases with injury of recurrent laryngeal nerve. 4 patients(33.3%) had one-sided recurrent laryngeal nerve injury and temporary hypoparathyroidism in recurrent patient,and one of the cases had permanent hypoparathyroidism. Conclusion:Total thyroidectomy could be served as the routine operative method for multinodular goiter,Graves disease,Hashimoto thyroiditis and differentiated thyroid carcinoma.
8.The clinical value of ANA,anti-dsDNA antibody and anti-ENA antibody repertoire in systemic lupus erythematosus diagnosis

Objective To investigate clinical value of the anti-nuclear antibody (ANA ) ,anti-double strand DNA (dsDNA ) anti-body and anti-extractable nuclear antigen(ENA) antibody repertoire in systemic lupus erythematosus (SLE) diagnosis .Methods 158 SLE patients were served as SLE group and another 50 healthy people as the control group .Indirect immunofluorescence(IIF) and immunoblotting test(IBT ) were employed to detect ANA ,anti-dsDNA antibody and anti-ENA antibody repertoire .Results Positive rates of ANA ,anti-dsDNA antibody and anti-ENA antibody repertoire in SLE diagnosis were 81 .65% ,68 .35% and 87 .34% ,respectively ,which were all lower than that of their combined detection (93 .04% ) ,with statistically significant difference (P<0 .05 ) .Among anti-ENA antibody repertoire ,the positive rate of anti-U1-nuclear ribonucleoprotein (U1-Nrnp ) antibody (52 .53% ) was the highest .Conclusion Combined detection of ANA ,anti-dsDNA antibody and anti-ENA antibodies repertoire has some clinical value of early diagnosis of SLE .
9.Changes of circulating Tfr and Tfh cells in children with myasthenia gravis

Objective To investigate the changes of follicular regulatory T cells ( Tfr cells) and follicular T helper cells ( Tfh cells) in peripheral blood of children with myasthenia gravis ( MG) . Methods We recruited 28 MG patients and 20 healthy subjects in this study. The percentages of Tfh and Tfr cells in peripheral blood samples were measured by flow cytometry. Real-time PCR was performed to detect the ex-pression of transcription factors and regulatory factors of Bcl-6, c-MAF, Blimp-1 and PD-1 at mRNA level. ELISA was used to detect the levels of IL-2, IL-6, IL-10 and IL-21 in plasma samples and the titers of Ach-Rab and PsMab. Results Compared with the healthy subjects, the MG patients showed higher percentages of Tfh cells and lower percentages of Tfr cells before receiving treatment. The expression of Bcl-6 and c-MAF on CD4+T lymphocytes cells at transcriptional level were significantly enhanced, while the expression of Blimp-1 on CD4+T cells and the expression of PD-1 on Treg cells at transcriptional level were inhibited in the MG patients in comparison with those in healthy subjects. Moreover, decreased levels of IL-2 and increased levels of IL-21 were found in plasma samples collected from the MG patients. Conclusion The decreased percentages of Tfr cells and increased percentages of Tfh cells in patients with MG resulted in abnormal ratios of Tfr/Tfh cells, which might be involved in the immunological pathogenesis of MG. Several changes in the patients with MG might be responsible for the imbalanced ratio of Tfr/Tfh cells, which included changes of IL-2 and IL-21 in microenvironment, enhanced expression of Bcl-6 and c-MAF at mRNA level and inhibited expression of Blimp-1 at mRNA level on CD4+T cells as well as over-expression of PD-1 at mRNA level on Treg cells.
10.The effects of cadherin-11 transfection on the migration and differentiation of rat dental pulp cells

Objective:To investigate the migration and odontogenesis of rat dental pulp cells transfected with adenovirus vector enco-ding cadherin-11.Methods:The dental pulp cells were cultured and transfected with pDC316-mCMV-EGFP-Cadherin-11.Cad-11 protein expression of the cells was examined by immunohistochemistry staining.The odontogenic differentiation of the cells was studied by alizarin red staining and ALP staining.The adhesion and migration of the cells were tested.Results:Transfection of pDC316-mC-MV-EGFP-Cadherin11 promoted Cad-11 protein expression and ALP activity,increased calcified nodule formation(P <0.05),adhe-sion and migration of rat dental pulp cells(P <0.05).Conclusion:Cadherin-11 may promote the odontoblast differentiation and mi-gration of rat dental pulp cells.
