China Pharmacy 2015;(29):4104-4106,4107
OBJECTIVE:To observe the effect of trantuzumab combined with preoperative adjuvant chemotherapy in the treat-ment of human epidermal growth factor receptors 2 (Her-2) positive breast cancer, and to investigate its clinical value. METH-ODS:80 patients with Her-2 positive breast cancer were selected and randomly divided into observation group and control group, with 40 cases in each group. The control group received routine preoperative adjuvant chemotherapy+modified radical mastectomy for Her-2 positive breast cancer;observation group was additionally treated with trastuzumab intravenously before operation, initial dose of 4 mg/kg,followed by 2 mg/kg,once a week, for consecutive 3 weeks,on the basis of control group. The effects of two regimen on the expression of HER-2,CD-34,MGA,E-cadherin and GCDFE-15 in Her 2 positive breast cancer focus were com-pared between 2 groups. ADR and follow-up survey were recorded. RESULTS:Compared with before chemotherapy, the expres-sion of HER-2(+)in observation group increased significantly, while that of HER-2(+ + +)decreased significantly;MGA posi-tive rate and GCDFP-15 positive rate decreased significantly while E-cadherin positive rate increased significantly;MDV of CD34 expression decreased significantly, with statistical significance(P<0.05 or P<0.01). Relapse rate and distance transport rate of ob-servation group were significantly lower than those of control group, with statistical significance(P<0.05). There was no statisti-cal significance in the incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:Trastuzumab combined with preoperative adjuvant chemotherapy have good clinical efficacy and safety, and it can effectively reduce related indicator of Her-2 positive breast cancer focus. So it has positive significance in reducing recurrence and metastasis of breast cancer.

Objective To verify and adjust the leaf position accuracy of a multi-leaf collimator (MLC) in a linear accelerator. Methods Ionization chamber arrays ( StarTrack, IBA) were used to measure the leaf position accuracy of a MLC in a linear accelerator (Precise, ELEKTA) and adjust the leaves out of tolerance. Results After the adjustment of leaf position of the MLC, the analysis of the verification films taken at the offset position showed that the leaf position of the MLC was accurate. Conclusions The method developed in this study is convenient and simple for measurement and calibration of leaf position of the MLC in the linear accelerator, which meets the MLC position accuracy requirement for the linear accelerator.

Objective To discuss the application of digital simulation positioning machine in radiation oncology. Methods We used digital simulation positioning machine to shoot isocenter reset digital simulator images of 0 degrees direction and 90 degrees direction, 10 × 10 cm2 rectangular field and actual time radiation field with any field number. Compared the images taken with the TPS corresponding DRR image through bone marker matching to measure the error of isocenter and actual time radiation field , the linear accelerator Iview-GT system with the same method was shot in rectangular field simulator the same field number EPID image , and the TPS corresponding DRR image. Finally, we compared the measured error of the digital simulator positioning machine with the error of Iview-GT system image. Results The result of measured isocenter error of the digital simulator positioning machine was better than EPID image error (P=0.000). Conclusion The application of digital simulation positioning machine in radiation oncology can be more accurate to measure the error and correct the position deviation, however, it can also make the radiotherapy and the treatment more accurate.

Objective To investigate the effect of trastuzumab for injection on apoptosis and the expression of related genes in locally advanced breast cancer and related indexes. Methods 166 patients with locally advanced breast cancer were selected and divided into two groups, 83 cases in the control group treated with neoadjuvant chemotherapy before operation, 83 cases in the experimental group received trastuzumab for injection on the basis of the control group, serum tumor marker levels, tumor cell apoptosis index and proliferation activity, Bcl-2, Bax gene expression levels, the clinical effect and incidence of adverse reactions were compared after the treatment. Results The effective rate in the control group(78.32%)was lower than the experimental group(90.36%), with significant difference (P<0.05).Compared with the control group, serum levels of CA153,CA125,CEA,TSGF were lower in the experimental group after chemotherapy, levels of apoptosis index(AI) in tumor tissue was higher, levels of S-phase fraction (SPF), proliferation index ( PI) were lower after treatment, Bcl-2 gene in tumor tissue expression level was lower, Bax gene expression level was higher after treatment, all with significant difference (P<0.05).There was no significant difference in the incidence of adverse reactions between two groups. Conclusion The trastuzumab for injection treatment can significantly improve the total remission rate of patients with locally advanced breast cancer , reduce the levels of serum tumor markers, can down-regulate the expression of apoptosis related gene Bcl-2, up-regulate the expression of Bax gene, promote the apoptosis of tumor cells.

OBJECTIVE:To observe the efficacy and safety of Shenqi jiangtang granule in the adjuvant treatment of type 2 dia-betes knee arthritis. METHODS:62 patients with type 2 diabetes knee arthritis were randomly divided into control group(31 cas-es) and observation group (31 cases). Control group received hypoglycemic and basic treatment for arthritis, including diet con-trol,exercise therapy and health education,as well as 0.25 g Metformin hydrochloride tablet with a meal,3 times a day + 50 mg Acarbose tablet with a meal,3 times a day, chewing;patients with arthritis pain 100 mg Aspirin enteric-coated tablet after a meal, once a day (chewing or breaking apart was prohibited). Observation group additionally received 3 g Shenqi jiangtang granule half an hour before a meal with 50 ml warm water,3 times a day. The treatment course for both groups was 6 months. Clinical effica-cy,and fasting plasma glucose(FPG),2 h postprandial blood glucose(2 h PG),glycated hemoglobin(HbA1c),interleukin-1β(IL-1β),IL-6 before and after treatment, and the incidence of adverse reactions in 2 groups were observed. RESULTS:The total effective rate in observation group was significantly higher than control group, the difference was statistically significant(P<0.05). Before treatment, there were no significant differences in the FPG,2 h PG,HbA1c,IL-1β and IL-6 between 2 groups(P>0.05). After treatment,FPG,2 h PG,HbA1c,IL-1β and IL-6 in 2 groups were significantly lower than before, and observation group was lower than control group, the differences were statistically significant(P<0.05). And there was no significant in the incidence of adverse reactions between 2 groups (P>0.05). CONCLUSIONS:Based on conventional treatment,Shenqi jiangtang granule shows obvious efficacy in the adjuvant treatment of type 2 diabetes knee arthritis.,it can reduce blood glucose and inflammation cy-tokine levels, mild symptoms of adverse reactions.

Objective To explore the correlation between thyroid nodules and uric acid levels and to find their gender differences. Methods A total of 68 056 subjects in a regional medical physical examination center of Shanxi Province from January 2013 to June 2015 were enrolled in this study. All the participants′ general information and parameters were recorded.Thyroid nodules were detected by color Doppler ultrasonography. Results The total prevalence of thyroid nodule was 35.5%, 30.7% in males and 40.0% in females. The prevalence of single nodule was 50.1%, and multiple 49.9%.Compared with no nodule group, thyroid nodule group tended to be older, with higher BMI, and with a worse metabolic status(all P<0.01).The uric acid levels were lower[(352.37±78.14 vs 357.70±77.51) μmol/L, P<0.01] in thyroid nodule group in male and higher[(260.22±61.91 vs 253.91±59.18) μmol/L, P<0.01] in female. Conclusion Thyroid nodules may be associated with metabolism and inflammation. In males, hyperuricemia group had lower, while in females, hyperuricemia ones were with a higher prevalence of thyroid nodules.
