1.Viral inactivation in von Willebrand factor preparations via UVC irradiation: an experimental approach

Objective: To evaluate the efficacy of ultraviolet-C (UVC) irradiation in inactivating porcine parvovirus (PPV), encephalomyocarditis virus (EMCV), pseudorabies virus (PRV) and vesicular stomatitis virus (VSV) within von Willebrand factor (vWF) preparations. Methods: An ultraviolet inactivator (254 nm primary wavelength) was employed to optimize UV exposure dosages for the samples, and the initial test groups were set at five irradiation gradients: 100, 200, 400, 1 000, and 1 500 J/m

Virucidal efficacy against various pathogenic strains was evaluated via cytopathic effect (CPE) observation methodology, while simultaneously quantifying von Willebrand factor antigen levels (vWF∶Ag) and collagen-binding activity (vWF∶CBA) using enzyme-linked immunosorbent assays (ELISA) and automated coagulation analysis systems before and after UVC treatment. Results: The results showed that > 100 J/m
UVC irradiation achieved a≥4.0 log reduction factor in vWF. For vWF antigen, retention rates were observed to be 93.67%, 91.72%, 93.54%, 79.05%, and 85.50% at UVC doses of 100, 125, 150, 175, and 200 J/m
, respectively. Similarly, the retention rates of vWF collagen-binding activity were 81.11%, 87.81%, 86.15%, 73.73%, and 73.72% under the same dose conditions. Conclusion: Considering both inactivation efficacy and functional preservation while ensuring safety and effectiveness, 100-150 J/m
represents the optimal inactivation dose.
2.Identification of tortoiseshell glue and antler glue in Qixiong zuogui granules and content determination of 12 chemical components

OBJECTIVE To identify tortoiseshell glue and antler glue in Qichong zuogui granules, and determine the contents of 12 chemical components. METHODS Identification and content determination were performed by using liquid chromatography- tandem mass spectrometry (LC-MS/MS) method. The identification was performed on Hypersil GOLD column with a mobile phase consisted of acetonitrile-0.1% formic acid solution (gradient elution); the electrospray ion source was used to scan in the positive ion multi-reaction detection mode. The mass charge ratio (m/z) 631.3→546.4, 631.3→921.4 was the detection ion pair for tortoiseshell glue, and the m/z 765.4→554.0, 765.4→733.0 was the detection ion pair for antler glue. The determination method for 12 chemical components was as follows: Accucore C18 column, methanol-0.1% formic acid as mobile phase (gradient elution); scanning range of positive and negative ions was m/z 100→1 000 with the electric spray ion source and single ion detection scanning mode. RESULTS Average retention times of the molecular ion peaks for characteristic peptide segments of tortoiseshell glue and antler glue were 6.28 and 6.77 min, respectively; the linear relationship of 12 chemical components was good within their respective concentration ranges, such as astragaloside Ⅳ, calycosin-7-O-β-D-glucoside, calycosin, chlorogenic acid, ferulic acid, betaine, amygdalin, rutin, hydroxysafflor yellow A, hyperoside, loganin, cyasterone (r>0.999); RSDs for precision, stability (24 h) and reproducibility tests were all less than 5%. The average sample recovery rates ranged from 98.04% to 101.08%. The average contents of 12 components were 1.83, 25.73, 13.76,56.71, 23.80, 49.82, 807.49, 15.01, 317.02, 60.21, 202.71 and 17.70 μg/g, respectively. CONCLUSIONS In this study, tortoiseshell glue and antler glue in Qixiong zuogui granules are identified, and the contents of 12 chemical components therein are determined. This provides a reference for the quality control of this granule.
3.Exosome derived from human adipose-derived mesenchymal stem cells prevented bone loss induced by estrogen deficiency.

OBJECTIVE:
To investigate the effect of human adipose-derived mesenchymal stem cells (hASCs) exosomes on osteogenic differentiation of bone marrow mesenchymal stem cells (BMSCs) extracted from osteoporotic mice, and to evaluate the effect of hASCs exosomes on preventing bone loss induced by estrogen deficiency.
METHODS:
hASCs exosomes were extracted by ultracentrifugation. The osteoporotic mice were established by bilateral ovariectomy (OVX). BMSCs were isolated from osteo-porotic mice and cultured for further analysis. In the experimental group, these BMSCs were exposed to an osteogenic induction medium supplemented with hASCs exosomes to evaluate their potential effects on osteogenesis. In contrast, the control group was treated with the same osteogenic induction medium, but without the addition of hASCs exosomes, to serve as a baseline comparison for the study. To comprehensively assess the osteogenic differentiation of BMSCs influenced by hASCs exosomes, alkaline phosphatase (ALP) staining, ALP activity quantitative analysis and quantitative reverse transcription polymerase chain reaction (qPCR) were performed. These evaluations provided critical insights into the role of hASCs exosomes in promoting osteoblast differentiation and bone formation in osteoporotic conditions. The fluorescence labeled hASCs exosomes were injected via the tail vein to observe the biodistribution of exosomes. Two weeks after OVX, the mice were divided into three groups: The experimental group consisted of estrogen-deficient mice receiving hASCs exosome injections; the negative control group consisted of estrogen-deficient mice receiving phosphate-buffered saline (PBS) injections; and the positive control group consisted of mice that underwent Sham surgery and received PBS injections.The injections were administered once every 3 days, for a total of 8 injections. Afterward, the femurs were collected from the mice, and micro-computed tomography (micro-CT) was performed to measure bone mineral density and conduct bone morphometric analysis.
RESULTS:
hASCs exosomes were successfully extracted using ultracentrifugation. After the induction by hASCs exosomes, ALP staining and ALP activity in the BMSCs extracted from osteoporotic mice were significantly enhanced, the expression of osteogenesis related genes in BMSCs were significantly up-regulated. More trabecular bone and higher bone mineral density were observed in estrogen-deficient mice injected with hASCs exosomes compared with estrogen-deficient mice injected with PBS, and there was no significant decrease in bone mineral density compared with the Sham operation group.
CONCLUSION
hASCs exosomes promoted the osteogenic differentiation of BMSCs extracted from osteoporotic mice. hASCs exosomes prevented bone loss induced by estrogen deficiency.

4.Effect of heterologous expression of Scenedesmus quadricauda malic enzyme gene SqME on photosynthetic carbon fixation and lipid accumulation in tobacco leaves.

Microalgae possess high photosynthetic efficiency, robust adaptability, and substantial biomass, serving as excellent biological resources for large-scale cultivation. Malic enzyme (ME), a ubiquitous metabolic enzyme in living organisms, catalyzes the decarboxylation of malate to produce pyruvate, CO2, and NAD(P)H, playing a role in multiple metabolic pathways including energy metabolism, photosynthesis, respiration, and biosynthesis. In this study, we identified the Scenedesmus quadricauda malic enzyme gene (SqME) and its biological functions, aiming to provide excellent target genes for the genetic improvement of higher plants. Based on the RNA-seq data from S. quadricauda under the biofilm cultivation mode with high CO2 and light energy transfer efficiency and small water use, a highly expressed gene (SqME) functionally annotated as ME was cloned. The physicochemical properties of the SqME-encoded protein were systematically analyzed by bioinformatics tools. The subcellular localization of SqME was determined via transient transformation in Nicotiana benthamiana leaves. The biological functions of SqME were identified via genetic transformation in Nicotiana tabacum, and the potential of SqME in the genetic improvement of higher plants was evaluated. The ORF of SqME was 1 770 bp, encoding 590 amino acid residues, and the encoded protein was located in chloroplasts. SqME was a NADP-ME, with the typical structural characteristics of ME. The ME activity in the transgenic N. tabacum plant was 1.8 folds of that in the wild-type control. Heterologous expression of SqME increased the content of chlorophyll a, chlorophyll b, and total chlorophyll by 20.9%, 26.9%, and 25.2%, respectively, compared with the control. The transgenic tobacco leaves showed an increase of 54.0% in the fluorescence parameter NPQ and a decrease of 30.1% in Fo compared with the control. Moreover, the biomass, total lipids, and soluble sugars in the transgenic tobacco leaves enhanced by 20.5%, 25.7%, and 9.5%, respectively. On the contrary, the starch and protein content in the transgenic tobacco leaves decreased by 22.4% and 12.2%, respectively. Collectively, the SqME-encoded protein exhibited a strong enzymatic activity. Heterologous expressing of SqME could significantly enhance photosynthetic protection, photosynthesis, and biomass accumulation in the host. Additionally, SqME can facilitate carbon metabolism remodeling in the host, driving more carbon flux towards lipid synthesis. Therefore, SqME can be applied in the genetic improvement of higher plants for enhancing photosynthetic carbon fixation and lipid accumulation. These findings provide scientific references for mining of functional genes from S. quadricauda and application of these genes in the genetic engineering of higher plants.

5.Correlation analysis of functional constipation and its related factors with acute appendicitis in children

Objective:To explore the correlation between functional constipation (FC)and its related factors with acute appendicitis(AA)in children,so as to provide a reference for the prevention and clinical diagnosis and treatment of children with AA. Methods:A case-control study was conducted on 170 children diagnosed with AA in the Department of Pediatrics,the Second Affiliated Hospital of Air Force Military Medical University,from August 2022 to March 2023,and 170 non-AA children during the same period were selected as control objects.The clinical data,incidence of FC,symptoms related to FC,Bristol stool classification,past constipation history and other information were compared between two groups.Results:The incidence of FC in 170 children with AA was 22.9%(39/170),which was significantly higher than 10.6%(18/170)in the non-AA group( P<0.01);For children under four years old,the proportion of faecal retention in AA group was higher than that in non-AA group(25.6% vs. 9.3%, P<0.05);For children ≥ 4 years old,the proportion of faecal retention and dyschezia in AA group were higher than those in non-AA group(28.2% vs. 6.9%,29.0% vs. 16.4%,respectively,all P<0.05).The proportion of past constipation history in AA group was higher than that in non-AA group(29.4% vs.14.1%).The duration of constipation in AA group was longer than that in non-AA group [0.00(0.00,1.25)month vs. 0.00(0.00,0.00)month,all P<0.01].The proportion of children with low Bristol stool classification in AA group was higher than that in non-AA group( P<0.01).Multivariate Logistic regression analysis showed that faecal retention[ OR=6.186(95% CI 2.336~16.380)] and long constipation time [ OR=1.310(95% CI 1.095~1.567)]were independent risk factors for AA in children(all P<0.05). Conclusion:The incidence of faecal retention in children with AA is higher than that in children without AA,and the median duration of constipation is longer than that in children without AA.Fecal retention and long-term constipation are independent risk factors for AA in children.

Traumatic supraorbital fissure syndrome (TSOFS) is a symptom complex caused by nerve entrapment in the supraorbital fissure after skull base trauma. If the compressed cranial nerve in the supraorbital fissure is not decompressed surgically, ptosis, diplopia and eye movement disorder may exist for a long time and seriously affect the patients′ quality of life. Since its overall incidence is not high, it is not familiarized with the majority of neurosurgeons and some TSOFS may be complicated with skull base vascular injury. If the supraorbital fissure surgery is performed without treatment of vascular injury, it may cause massive hemorrhage, and disability and even life-threatening in severe cases. At present, there is no consensus or guideline on the diagnosis and treatment of TSOFS that can be referred to both domestically and internationally. To improve the understanding of TSOFS among clinical physicians and establish standardized diagnosis and treatment plans, the Skull Base Trauma Group of the Neurorepair Professional Committee of the Chinese Medical Doctor Association, Neurotrauma Group of the Neurosurgery Branch of the Chinese Medical Association, Neurotrauma Group of the Traumatology Branch of the Chinese Medical Association, and Editorial Committee of Chinese Journal of Trauma organized relevant experts to formulate Chinese expert consensus on the diagnosis and treatment of traumatic supraorbital fissure syndrome ( version 2024) based on evidence of evidence-based medicine and clinical experience of diagnosis and treatment. This consensus puts forward 12 recommendations on the diagnosis, classification, treatment, efficacy evaluation and follow-up of TSOFS, aiming to provide references for neurosurgeons from hospitals of all levels to standardize the diagnosis and treatment of TSOFS.
7.Effectiveness Analysis of Laparoscopic and Open Surgeries in the Treatment of Intrahepatic Cholangiocarcinoma:a Propensity Score Matching Study

Objective To preliminarily analyze the efficacy of laparoscopic surgery versus open surgery in patients with intrahepatic cholangiocarcinoma(ICC).Methods A retrospective analysis was conducted on 84 patients with ICC who underwent surgical resection from September 2010 to March 2023,including 45 cases in the laparoscopic surgery group and 39 in the open surgery group.By using propensity score matching,34 matched pairs were successfully identified.The study compared intraoperative blood loss,proportion of achieving≥6 lymph nodes dissection,R0 resection rate,complication,postoperative hospital stay,survival,and recurrence between the two groups.Results In the laparoscopic group,intraoperative blood loss was significantly less than that in the open surgery group(median:250 ml vs.750 ml,Z=-3.406,P=0.001).The proportion of achieving≥6 lymph node dissection in the former was lower than that in the latter,but the difference was not statistically significant(5.9%vs.17.6%,χ2=1.275,P=0.259).There was no statistical difference in R0 resection rate between the laparoscopic group and the open surgery group[88.2%(30/34)vs.82.4%(28/34),χ2=0.469,P=0.493].The complication rate in the laparoscopic group was 17.6%(6/34),significantly lower than 47.1%(16/34)in the open surgery group(χ2=6.719,P=0.010);however,the rate of severe complication was the same in both groups at 2.9%(χ2=0.000,P=1.000).Laparoscopic surgery significantly shortened the postoperative hospital stay(median:7 d vs.10 d,Z=-3.021,P=0.003).The median overall survival in the laparoscopic group was 60.6 months,significantly longer than 15.9 months in the open surgery group(log-rank χ2=5.788,P=0.016).The median recurrence-free survival was 26.4 months in the laparoscopic group,significantly longer than 7.6 months in the open surgery group(log-rank χ2=4.532,P=0.033).Conclusions Compared to open surgery,laparoscopic surgery for ICC offers advantages such as less blood loss,lower complication rate,shorter postoperative hospital stay and longer survival.It achieves comparable R0 resection rate and adequate lymph node dissection.
8.Expression of PD-L1/B7-H3 in intrahepatic cholangiocarcinoma and its correlations with clinicopathological features and prognosis

Objective:To explore the expression of PD-L1 and B7-H3 in intrahepatic cholangiocarcinoma (ICC) and their relationship with the prognosis after surgery.Methods:Clinicopathological data of 63 ICC patients undergoing surgery at the Department of General Surgery, Peking University Third Hospital, from Sep 2016 to Mar 2023 were retrospectively analyzed.Results:PD-L1 and B7-H3 are highly expressed in 41.3% and 60.3% of ICC patients, respectively, and their expression levels were positively correlated with each other( P=0.005). High PD-L1 expression is significantly associated with poor tumor differentiation ( P=0.014), while high B7-H3 expression is significantly related to lymph node metastasis ( P=0.035). In patients with high co-expression of PD-L1 and B7-H3, the overall survival ( P=0.031) and recurrence-free survival ( P=0.031) were significantly decreased. High co-expression of PD-L1 and B7-H3 is an independent risk factor for recurrence in ICC ( HR=3.998, 95% CI: 1.211-13.198; P=0.023). Conclusions:ICC patients with high expression of both PD-L1 and B7-H3 have a poor prognosis. Using both as the combined biomarker significantly improves the prediction of long-term survival after surgery, compared to using either one alone.
9.Bibliometric and visual analysis of neurological damage caused by electrical welding operations

Objective:To analyze and summarize the trends and hot spots in the field of neurological damage caused by electric welding operations, and to provide ideas for new researches by searching the domestic and international literature.Methods:In December 2022, using Web of Science Citation Index (Web of Science), China Journal Full-Text Database (CNKI) and Wanfang Database as search databases, literature search was conducted on the Chinese and English search terms related to eletrical welding operations and neurological damage. The bibliometric analysis software VOSviewer 1.6.18 and CiteSpace 6.1.6 were used to visualize the publication year, publication quantity, country, research institution and key words of the literature.Results:A total of 309 articles (112 in Chinese and 197 in English) were included in this study. The first domestic and international papers were published in 1976 and 1994 respectively, and the number of papers reached the peak in 2006 and 2018, and then showed a downward trend to varying degrees. In China, Shandong First Medical University (including Shandong Institute of Occupational Health and Occupational Disease Prevention and Shandong Academy of Medical Sciences) and Wuhan University of Science and Technology had the largest number of publications. The 309 articles were from 52 Chinese journals and 86 English journals. The co-occurrence analysis of key words showed that the domestic research mainly focused on eletrical welding operation, welding workers, neurobehavioral function and manganese, and the nervous system damage caused by manganese in welding smoke was the field of international attention. Long term exposure, risk, and performance were key buzzwords in the field.Conclusion:The research focus in the field of nervous system damage caused by electric welding operation has an obvious trend of time evolution, gradually transiting from clinical manifestations to its toxic mechanism and early biomarkers.
10.Predictive model of general anesthesia post-induction hypotension based on machine learning

Objective To construct a clinical prediction model of general anesthesia post-induction hypotension(PIH)in patients undergoing laparoscopic cholecystectomy(LC)based on machine learning.Methods The data of patients who underwent LC surgery from May 2019 to September 2023 were retro-spectively selected.The training set and the validation set were allocated at a ratio of 7:3.Logistic regres-sion,random forest(RF),and support vector machine(SVM)were used to construct the prediction model of PIH.The discrimination accuracy of the model was evaluated by the area under the receiver operating characteristic(ROC)curve,and the calibration of the model was evaluated by calibration curve and Hos-mer-Lemeshow(H-L)test.The nomogram was used for visual interpretation of the model.Results A total of 260 patients were included,and 82 patients(31.5%)developed PIH.There were 182 patients in the training set and 78 patients in the validation set.Fifty-eight patients(31.9%)in the training set developed PIH,and 24 patients(30.8%)in the validation set developed PIH.Age,BMI,use of angiotensin conver-ting enzyme inhibitors(ACEIs)/angiotensin receptor blockers(ARBs),basal HR and MAP,HR and MAP on entering the operating room,HR change,and MAP change were important factors for the model to predict PIH.The logistic regression model had the best prediction performance,with AUC,accuracy and re-call rate of 0.93(95%CI 0.90-0.99),0.81,and 0.81 in validation set,respectively.Conclusion The logistic regression model based on machine learnin has good performance in predicting PIH.The selected predictive variables are age,BMI,use of ACEIs/ARBs,basal HR and MAP,HR and MAP on entering the operating room,HR and MAP change,which can quickly and accurately assess the risk of PIH.
