AIM: To instruct the self-care ability of patients underwent the kidney transplant with Orem self-nursing theory. METHODS: A total of 118 patients received kidney transplant were recruited from the Ruikang Hospital Affiliated to Guangxi College of Traditional Chinese Medicine and the Yiyang Municipal People's Hospital from January 2003 to June 2006, with all the patients' informed consents. The subjects were divided into two groups:①Orem self-nursing theory group (n =68): Taking the Orem self-nursing theory as the instruction, the self-care ability and self-care defect of patients were appraised. They were treated with different nursing systems and healthy educations, participated in designing the nursing plan and activity.②Control group (n =50): The patient of this group received usual care. The self-care ability of patients in the two groups were evaluated 1, 3, 6 months after the kidney transplant, grading Ⅰ as completely recovery to routine work; Ⅱ as semi-recovery to routine work and daily life was revived. Both Ⅰ and Ⅱ grades indicated the good quality of life (QOL). Even though a single prescription may be applied in different treatments, it may not be effective in treating patients with complex conditions. In cases like this, the problem can only be solved through the expanding of medicines in a prescription. Because a new prescription is not easy to create, the usage of adding another or more appropriate prescriptions into the prescription can be considered to expand the curative effects. The existing prescriptions were left by the confirmation of their curative effects from clinical tests which made by doctors of several generations. Therefore, the combination of prescriptions is a method which is convenient and easy to control the curative effects. By analogical reasoning, prescription user often uses a single prescription as a basic unit to generate the combined prescriptions from the existing corresponding prescription. It follows the rule of treatment based on syndrome differentiation to take a dynamic adjustment for complex symptoms. Disease with multiple major symptoms is the main basis for a doctor to use combined prescriptions. The combination of different prescriptions may produce the curative effects of addition, reinforcement, antagonism, or even new effects. Since traditional prescriptions have experienced many years of clinical tests, doctors who utilize traditional prescriptions actively will have more methods and ways to treat diseases.

Objective To identify the effect of oral cod liver oil on serum 25-Hydroxyvitamin D [25 (OH)D] level in breast-fed infants during winter.Methods Healthy newborn infants were recruited at Children's Hospital Affiliated to Capital Institute of Pediatrics between December 2009 and February 2010 for the self-controlled study.Serum samples were obtained with 2 ml umbilical cord blood for each neonate.The neonates received vitamin D supplementation in the form of oral cod liver oil from 15 days to 4 months after birth.For each exclusively breastfed infant,2 ml fasting venous blood was collected at the end of postnatal 4 months to separate serum.Serum 25 (OH) D concentrations of the infants were determined with umbilical cord blood serum and venous blood serum.Results Altogether 43 exclusively breastfed infants were enrolled,including 26 boys and 17 girls.The mean serum 25 (OH)D concentration in umbilical cord blood was (20.9 ± 5.1) nmol/L (12.5-32.1 nmol/L).All the neonates were vitamin D deficient.The mean serum 25 (OH) D concentration in infants at 4 months was (106.3 ± 42.4) nmol/L (30.2-208.5 nmol/L).Among the infants,the serum 25 (OH) D level was ＜ 50 nmol/L in 4 infants (the rate of vitamin D deficiency being 9.3％),50-75 nmol/L in 7,75-150 nmol/L in 25,and ≥150 nmol/L in 7.A positive correlation was detected between serum 25(OH)D level and cod liver oil dose (r =0.530,P =0.001).The average percentage of the missed doses (missed doses/prescribed doses) was 36.1％,negatively correlated with serum 25 (OH)D level (r =-0.634,P =0.001).Conclusions Vitamin D deficiency is common among infants born during winter in northern China.Daily oral intake of cod liver oil in infants can significantly increase serum 25 (OH) D level; However,the prevention of vitamin D deficiency cannot be ensured if doses are missed.

There are more prognostic factors affecting the patients with cerebral hemorrhage, including bleeding volume, bleeding site, cause of bleeding, and blood pressure regulation during acute phase. Among them, the regulation of blood pressure during acute phase is a man-controlled important factor. However, the ideal blood pressure level has not yet to he determined at present. Therefore, more clinical trials are needed to determine the exact time window of blood pressure management in patients with intracerebral hemorrhage during acute phase and the range of blood pressure control.

For this reason, it is necessary to take the following steps: changing concepts, identifying goals, implementing personnel training strategies and taking the initiative in selecting talented personnel; making exertions in personnel training through expanding channels and conducting classifications; creating the right circumstances, optimizing the environment and making a rational use of personnel; adopting innovative mechanisms, perfecting systems and scientifically managing personnel. It is important to take specialty advancement as the guide, personnel training as the prop, and scientific research and teaching as the driving force in achieving the sustainable development of special hospitals.

Objective To summarize the experience of ventriculo-peritoneal (V-P) shunt under micro-laparoscope for the treatment of hydrocephalus. Methods Twenty-eight patients with hydrocephalus were treated by V-P shunt under micro-laparoscope from November 2000 to May 2004. An infraumbilical trocar was placed through a 5 mm incision to introduce the operating laparoscope. Under laparoscopic visualization, a 3 mm trocar was introduced into the abdominal cavity along the right side of the ligamentum teres hepatis through an incision just to the right of the xiphoid process. A 1.5 mm subcutaneous incision at the corresponding site of the diaphragmatic surface of the liver on the right anterior axillary line was made and through it a sewing needle with 4-gauge unabsorbable silk suture was delivered into. A grasping forceps was inserted through the xiphoid port into the abdominal cavity to carry the two sides of the suture out. The V-P tubing was fixed in the interspace between the liver and the diaphragm by the ligation from the outside, and then was tunneled to the head. Results All the operations were successfully accomplished. The operation time for iutraabdominal tube placement was 5~10 min.All the patients experienced complete alleviation of symptoms of intracranial hypertension postoperatively. No surgical complications occurred.Follow-up check-ups for 6~36 months(mean,12 months) in 25 patients found no tube obstruction. Conclusions Ventriculo-peritoneal shunt under micro-laparoscope has the advantages of minimal invasion, precise positioning, excellent visualization and rapid recovery, being a safe and feasible method which is worthy of recommendation.

Objective To study the synthesizing site,transporting mode,and storing position of essential oil in Pogostemon cablin stems and leaves.Methods Ultrathin slicing and histochemical methods were used.Results In parenchyma cells in the stem of P.cablin,dark osmiophilic material was observed in plastids and mitochondria,otherwise,some small gray vesicles were also found in cytoplasm.Some dark osmiophilic droplets were found to adhere to outer membrane of small gray vesicles,and many small gray vesicles with osmiophilic droplets gathered in outer membrane of large vacuole.In leaves,it was different case.Because palisade tissue cells were arranged closely,which had smaller volume and rich chloroplasts,so there was not large vacuole in palisade tissue cells.Plastids,mitochondria,and small vacuole were distributed in high density,but the first two organelles contained more dark osmilphilic substance than latter.Conclusion The plastid is the main synthesis site of essential oil,and the essential oil was mainly transferred by small gray vesicles into large vacuole to store in the stem.In leaves,chloroplast not only has photosynthesis,but also is the main synthesis site of essential oil,and essential oil is permeated into small vacuole to store.
