Objective To evaluate the effect and safety of point injection and em bedding therapy on postoperative pain of hemorrhoid patients. Me thods Totally 105 hemorrhoid patients were rand omized into a treatment group (54 cases) and a control group (51 cases). The tre atment group was treated by point injec tion and embedding therapy at Changqiang point (GV 1) under the local anesthesia . The control group was treated by Indometacin Suppository after operation. On the fir st, second, four th and seventh day of the treatment, the pain occurrence, pain severity, and com prehensive effect scores were recorded for the evaluation of therapeutic effect and safety. Results On all interview periods, the compr ehensive effect score, pain occurrence, pain severity, patient satisfaction, occurrence of untoward eff ect, complications, and analgesics combination in the treatment group were all superior to those in the control group (P

Objective To evaluate the treatment of interapleural talc on the patients with malignant pleural effusion.Method There were 44 patients with malignant pleural effusion in the study.Every cases was inserted a chest drain to release the effusion with the rate of 200 ml per 2 hours.When there was no evidence of fluid in the pleural space as assessed by plain chest roentgenography,the talc slurry containing 4 g talc、40 ml of saline solution (0.9%) and 5 ml of 2% lidocaine was injected via the intercostal drain into the pleural space.An additional 20 ml of saline solution was used to flush the drain.Then,the drain was clamped,and the patient was asked to change position to allow adequate distribution of talc.After 2 hours,the drain was opened again.When the drainage decreased to less than 150 ml per 24 hours,the chest drain was removed.Result complete success was observed in 36 cases (81.8%),partial success in 6 cases (13.7%),and ineffective success in 2 cases (4.5%).There were 21 (47.7%) and 24 (54.4%) cases experienced pleuritic pain and fever after talc pleurodesis respectively,1 cases suffered from respiratory insufficiency which controled by using glucocorticoid later.Conclusion The talc pleurodesis is an effective treatment for the patients with malignant pleural effusion.It is safe and easily performed,and should be used extensively in clinic.

Objective To discuss the surgical treatment of differentiated thyroid carcinoma ( DTC) viola-ting larynx and trachea .Methods 29 patients with DTC violating larynx and trachea received primary tumor re-section.Among them, 6 patients were with larynx violation , 3 patients with larynx and trachea violation , 16 pa-tients with trachea violation , and 4 patients with anterior strap muscles and skin violation .In addition, 5 patients were type I , 4 patients type II , 9 patients type III , and 11 patients type IV .All patients accepted the total thy-roidectomy.For patients with larynx and trachea violations , 9 received the slashing tracheal surgery ( type I and II) , 15 patients received the window resection and sternocleidomastoid muscle periosteal flap reconstruction , sleeve resection and anastomosis reconstruction , and window resection and gastrostomy .3 patients received total laryngectomy , 2 patients received partial laryngectomy and 4 patients with skin invasion received the reconstruc-tion with pectoralis major muscle flap .Results For these patients , 25 patients were with papillary adenocarcino-ma, and 4 patients with follicular carcinoma .All patients were followed up for 1 to 8 years.3 cases suffered from recurrence, 2 cases with tracheal recurrence received reoperation .3 cases with lung metastasis received the I 131 therapy, among whom 2 cases achieved the local control of lung tumor and the other one survived with tumor .1 patient died of the neck lymph nodes recurrence .25 patients survived over 3 years.13 patients survived over 5 years.Conclusions For patients with DTC with larynx and trachea violation , we should try our best to eliminate the tumor tissues.For the organs invaded by tumors , if possibly, elimination is also needed .This will eliminate or release the suffocation resulted from bleeding or obstruction .In addition , the function of larynx and trachea can be reconstructed and the life quality of these patients can be improved through the flap reconstruction and trachea anastomosis.Standard endocrine therapy and nuclear medicine therapy contribute to the prognosis improvement .

Objective To investigate the expressions of Th17, CD4 +CD25 +Treg, HLA-DR mRNA in peripheral blood of children with EV71-induced hand, foot and mouth diseases ( HFMD ) and their clinical significance.Methods Stratified random sampling was used to select 60 children with HFMDs from Liaocheng People’s Hospital from February to October, 2014, including 20 mild, 20 severe and 20 critically ill cases.Twenty healthy children were also enrolled as the control group.All the children with HFMDs were given ribavirin (10 mg/kg) for the treatment.The percentages of Th17 and CD4 +CD25 +Treg cells in CD4 +T cells of peripheral blood were determined by flow cytometry, and the expression of HLA-DR mRNA in peripheral blood mononuclear cells was detected by reverse transcription polymerase chain reaction ( RT-PCR).Analysis of variance and SNK-q test were used to compare the expressions of Th17, CD4 +CD25 +Treg and HLA-DR mRNA among groups, and Pearson correlation analysis was performed to reveal the correlations between HLA-DR mRNA and Th17, CD4 +CD25 +Treg.Results Compared with the control group, the expression of Th17 was increased, while CD4 +CD25+Treg and HLA-DR mRNA expressions were decreased in children with HFMDs on d1 of treatment (F=310.4, 81.5 and 545.4, P<0.01).After treatment, Th17 levels in mild group, severe group and surviving children of critically ill group were decreased, CD4 +CD25 +Treg and HLA-DR mRNA expressions were increased, while in fatal cases, Th17 level was still on the rise, and CD4 +CD25+Treg and HLA-DR mRNA expressions were still decreasing. After 10 d of treatment, the difference in Th17 and CD4 +CD25 +Treg levels among mild group, severe group, surviving children of critically ill group and control group was of no statistical significance ( P >0.05), but Th17 level in fatal was still higher and CD4 +CD25 +Treg level was still lower than those in control group (t=16.4 and 12.0, P<0.05).After 10 d of treatment, HLA-DR mRNA expressions in mild group and severe group were increased to the normal level.HLA-DR mRNA expression in surviving patients of critical ill group was still lower than that in mild group and severe group (P<0.05), but was higher than that in fatal patients (t=7.8, P<0.05).Pearson correlation analysis showed that, HLA-DR mRNA was negatively correlated with Th17 level (r=-0.770, P<0.01), and positively correlated with CD4 +CD25 +Treg level (r=0.883, P<0.01).Conclusion The expressions of Th17, CD4 +CD25 +Treg cells, and HLA-DR mRNA are correlated with the severity of HFMD, and may be used for evaluation of disease severity and prediction of disease outcomes.

Objective To study the therapeutic effect and immunologic regulation of human amniotic mesenchymal stem cells (hAMSCs) in rats with experimental type 1 diabetes mellitus (T1DM).Methods The hAMSCs from human amnion were isolated and cultured in vitro,then phenotype was analyzed by flow cytometry.T1DM were produced by administering streptozocin to rats.The rats were divided into normal control group (n =6),T1 DM model group (n =6),medium treated group (n =6),hAMSCs transplanted group(n =6),and insulin treated group(n =6).5 × 106of hAMSCs or vehicle were administered to rats via sublingual vein.Blood glucose levels of rats were recorded weekly in the groups for six weeks by Blood Sugar Meter.At the end of 6 weeks after hAMSCs transplantation,concentrations of plasma insulin were detected by ELISA; histopathological changes of pancreas,surviwl and differentiation of transplanted hAMSCs in pancreatic tissue were studied with HE staining,and immunofluorescence staining; percentages of lymphocyte subsets in peripheral blood mononuclear cells were determined by flow cytometry ; concentrations of plasma cytokines were determined by cytometric bead array.Results After hAMSCs transplantation,blood glucose levels in rats with T1DM were decreased (P ＜ 0.01),while concentrations of plasma insulin were increased significantly (P＜0.01).At 6 weeks,cell-treated animals showed an improvement in pancreas damage ; the percentages of CD4 + IFN-γ+ (Th 1) and C D4 + interleukin (IL)-17 + (Th 17) cells were reduced (all P＜0.05),while the percentages of FoxP3-positive regulatory T cells (FoxP3 +Treg) and CD4+ IL-4+(Th2) cells were increased (all P＜0.01) ; plasma concentrations of interferon-γ,IL-2,and tumor necrosis factor-αwere decreased (all P＜0.01),but IL-4 level was increased (P＜0.05).No histological evidence of insulin producing cells from hAMSCs was seen within pancreas.Conclusions hAMSCs may reduce blood glucose and alleviate the islet damage in rats with T1 DM,which is related to their potential to up-regulate FoxP3 +Treg cells.

Objective To explore the relationship between histological chorioamnionitis(HC),fetal vasculitis(FV)and the morbidity of neonatal respiratory distress syndrome(RDS).Methods Three hundred and forty-seven cases of infants at the gestational ages of 28 +0 to 31 +6 weeks who were admitted to the Neo-natology Department of our hospital from October 2009 to June 2013 were analyzed retrospectively.They were divided into four groups according to the occurrence of HC and FV,namely,HC positive group and HC negative group,FV positive group and FV negative group.The patients in the HC positive group were further divided into FV positive group and FV negative group according to the occurrence of FV.The morbidity of RDS among above-mentioned groups was compared.Results The clinical characteristics including propor-tion of male,gestational age,birth weight,cesarean delivery,antenatal corticosteroid use,gestational hyperten-sion,gestational diabetes and cholestasis of pregnancy showed no statistically significant difference among all the groups(P 〉0.05).The incidence of RDS in the HC positive group(145 cases)was 49.6%(72 cases), which was significantly lower than that in the HC negative group(67.3%,126 /202,P 〈0.05).The incidence of RDS in FV positive group(64 cases)was 42.2%(27 cases),which was significantly lower than that in FV negative group(63.3%,179 /283,P 〈0.05)In the HC positive group(145 cases),the incidence of RDS in FV positive group (64 cases)was 42.2% (27 cases),and 55.6% (45 cases)in FV negative group (81 cases),which showed no significant difference(P ﹦0.01 ).Conclusion (1 )HC or FV can reduce the incidence rate of RDS in premature infants.(2)HC combined FV cannot furtherly reduce the incidence of RDS.

Objective:Detecting serum IL-37 concentrations and HBeAg seroconversion in chronic hepatitis B virus(HBV) patients during the treatment of telbivudine( LDT).Methods:Fifty patients with chronic hepatitis B were included in the study;and 20 healthy people were selected as the control group.The expression levels of interleukin ( IL)-37, IL-6 and CRP were measured by enzyme-linked immunosorbent assay.As the same time,the relationship between serum IL-37 concentrations and HBeAg seroconversion was dynamically observed at 0 w,12 w,24 w,36 w,48 w after treatment.Results:The serum levels of IL-37 in chronic HBV patients were higher than the control group at baseline(q=22.716,P<0.01).The serum levels of IL-37 were decreased in HBeAg seroconverted CHB patients(t=-19.480,P<0.001).The levels of IL-37 were positively correlated with ALT,IL-6 and CRP(r were 0.697,0.725, 0.895,respectively;all P<0.001).The levels of IL-37 and HBV DNA was no correlation(r=0.136,P>0.05).Conclusion:IL-37 may be involved in the process of HBV infection in CHB patients,and can reflect the degree of liver’ s inflammatory damage.IL-37 may play a significant role in the immune response of CHB patients with HBeAg seroconversion.In future,IL-37 may be an effective therapeutic measure to HBV infection.

Objective To explore the peripheral blood T lymphocytes cell level in different grades of cerebral gliomas pa-tients at the time of before and after operation and its clinical significance .Methods A total of 80 cases of brain tumor patients from February 2010 to February 2012 in this hospital were chosed as study objects ,included 57 cases of glioma ,23 cases of other brain tumors ;57 cases of glioma were divided into 19 cases of low grade group and 38 cases of high grade group accorded to WHO grading standards ,and 50 cases of healthy people in same period were selected as control group .Venous blood in three groups were extracted at 1st day and 1st week after operation ,to detected the level of T lymphocyte subsets in peripheral blood and analyze the relationship between the different grade gliomas and prognosis .Results The peripheral blood CD3 + ,CD4 + ,CD4 + /CD8 + in control group were significantly higher than that of other brain tumor group and the glioma group ,CD8 + was significantly lower than that of the two groups(P< 0 .05) .Compared with other tumor group ,peripheral blood CD3 + ,CD4 + ,CD4 + /CD8 + in glioma group de-creased significantly ,CD8 + increased significantly(P< 0 .05) ;before and after the operation ,the CD3 + and CD4 + /CD8 + levels were significantly higher in low grade group than in the high grade group ,CD8 + was significantly lower in low grade group than in the high grade group(P< 0 .05) ,at the time of 1st week after operation ,CD3 + and CD4 + /CD8 + increased ,CD8 + decreased significant-ly of two groups when intra-group comparison (P < 0 .05) .The median survival time were 31 months in low grade group and 13 months in high grade group ,and the singnificance was found in two groups(P< 0 .05) .The median survival time were 34 months in CD4 + /CD8 + > 1 group and 17 months in CD4 + /CD8 + < 1 group ,the singnificance was found in two groups(P< 0 .05) .Conclusion The immune function of patients with brain glioma is inhibited ,the higher the malignancy of the tumor ,the more obvious of im-mune inhibition ;T lymphocyte subsets level could be used as evaluating index of malignant degree and prognosis of brain glioma .

Objective To test the validity and reliability of global physical activity questionnaire (GPAQ) by using two methods for collecting data,and to evaluate the equality of two measure methods.Methods The physical activity (PA) in two hypertension groups was examined twice with GPAQ by using self-administered and interview methods respectively.The test-retest reliability coefficient and consistency coefficient were calculated based on the results of two groups.The criterion validity of GpAQ was assessed by adopting pedometer and international physical activity questionnaire (IPAQ).Results The intra-group relative coefficient detected by two methods had statistical difference in the aspects of,the vigorous PA (work),total moderate PA and total moderate-vigorous PA(P＜0.05).In the aspect of different grades of PA level measurement,theconsistency coefficients detected by two methods were 0.37 and 0.31 respectively;in the aspect of whether reaching recommended PA level in hypertensive patients,the consistency coefficients detected by two methods were 0.33 and 0.34 respectively.The relative coefficient detected by pedometer,IPAQ and GPAQ was 0.19-0.46.Conclusion GPAQ has good reliability and validity,is suitable to be popularized and applied in the group of chronic disease hypertension.The validity and reliability of GPAQ measured by the self-administered method are consistent to those measured by using the interview method,promptingthat the self-administered method may be used to measure PA status in population by GPAQ.
