1.Ceramic-on-ceramic total hip arthroplasty in young and middle-aged patients of femoral neck fracture

BACKGROUND:Ceramic-on-ceramic total hip arthroplasty can be applied to young patients requiring frequent activity and high quality of life.
OBJECTIVE:To explore the short-term efficiency of ceramic-on-ceramic total hip arthroplasty in young and middle-aged patients of femoral neck fractures.
METHODS:Fifty-one young and middle-aged patients under 50 years old with ceramic-on-ceramic total hip arthroplasty were selected to analyze the function of hip joints and the complication after operation.
RESULTS AND CONCLUSION:Al the patients were fol owed up for 6 months to 44 months. At the last fol ow-up, the average Harris score was 92.0±3.4 points with an excellent and good rate of 94.1%. No dislocation was observed and three cases exhibited mild limping. Imaging examination showed that, no prosthesis loosing, broken, sinking or radiolucent line around prosthesis was observed. Ceramic-on-ceramic total hip arthroplasty in young patients with femoral neck fractures can effectively decrease the osteolysis, prosthesis loosing and sinking caused by the debris of wearing, prolong the working life of the prosthesis, and improve the patient’s quality of life.
2.A survey of fall-related knowledge and behaviors and ways after injury by chief caregivers of inpatients with neurological disease

Objective To investigate the fall-related knowledge and behaviors and ways after injury by chief caregivers of inpatients with neurological disease.Method We designed a questionnaire of fall-related knowledge,behaviors and ways after injury and used it among 208 chief caregivers of inpatients with neurological disease.Results On fall-related knowledge,the three items with the lowest scores about fall-related knowledge included fall-related medicine,postural hypotension and abnormal excretion.Of fall-prevented behaviour score,the lowest 3 items were three steps for getting up,placing things correctly and using wheelchair protection derive.Of the inquiry and fall measures score,the lowest 3 item were helping the patient up,observation and prevention after fall.There were significant differences in the total scores on education,time for taking care of the patients,history of fall and health education on fall (P<0.05).The scores of those with education of senior middle school,college or above were significantly higher than those with education of junior middle school or lower (P<0.001).The following time for taking care of the patients were compared with statistics significant:<1month and 3~6 month,<1month and >6 months,1~3 months and 3~6 months (P<0.001).Conclusion The caregivers of the patients with neurological disease are short of knowledge in fall-related knowledge,normal behaviors and should carry out individualized health education to them.
3.Therapeutic effect of intracoronary versus intravenous bolus tirofiban in elderly patients with acute coronary syndrome

Objective To analyze the therapeutic effect of intracoronary versus intravenous bolus tirofiban on myocardial perfusion and major cardiovascular events (MACE) in elderly patients with non-ST-segment elevation acute coronary syndrome (NSTE-ACS) and explore the optimal route of tirofiban application. Methods From July 2009 to July 2010, 120 NSTE-ACS patients undergoing percutaneous coronary intervention (PCI ) were consecutively enrolled in this study. They were randomly divided into two groups: intracoronary (60 cases) versus intravenous (60 cases) bolus tirofiban. Thrombolysis in myocardial infarction (TIMI) flow, TIMI myocardial perfusion grade (TMPG) and MACE 30 days after PCI were observed. Results The incidence of TIMI flow and TMPG 3 grade in intracoronary group were higher than in intravenous group [53(88.3%) vs. 38(63.3%); 53(88.3%) vs. 40(66.7%), respectively, both P<0.05]. However, MACE incidence and bleeding complications during hospital 30 days after PCI had no significant difference between the two groups [1 (1.7%) vs. 0; 3(5.0%) vs. 5(8.3%)], which were not statistically significant (P>0.05). Conclusions Intracoronary bolus tirofiban before PCI more effectively increases coronary blood flow and myocardium blush than intravenous route in elderly NSTE-ACS patients.
4.STUDY ON OPTIMIZATION OF ENZYMOLYSIS TECHNOLOGY OF ACAUDINA LEUCOPROCTA BY RESPONSE SURFACE METHOD AND THEIR ANTIOXIDANT ACTIVITIES

Objective To optimize the enzymolysis condition for Acaudina leucoprocta to research lencoprocta the antioxidant activity of its hyoholysate in vivo.Method Response surface methodology (RSM) was employed to optimize the hydrolysis conditions with protamex.The ICR mice were given ig the hydrolysate of Acaudina lencoprocta obtained under optimal conditions.The activity of superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) and the content of MDA in blood serum and liver in mice were analyzed.Results and Conclusion The best hydrolysis conditions were:the amount of enzyme was 2.02%;enzymolysis temperature was 51.18 ℃;enzymolysis time was 127.81min,hydroxyl radical scavenging rate was 73.10%.The activities of SOD,GSH-Px in hydrolysate were significantly enhanced and the content of MDA was significantly lower,which showed that this hydrolysate had remarkable antioxidation activity.So the enzymolysis technology for Acaudina leucoprocta was optimal.
5.Content Determination of Esomeprazole Sodium for Injection by UPLC-MS/MS

OBJECTIVE:To establish a method for the content determination of Esomeprazole sodium for injection. METH-ODS:UPLC-MS/MS was performed. The column of ZORBAX Eclipse Plus C18 with 0.1% formic acid-methanol(73∶27,V/V)at a flow rate of 0.3 ml/min,column temperature was 35℃,analysis time was 5 min,injection volume was 2μl;ionization mode was positive ion mode,ion source temperature was 150 ℃,capillary voltage was 3.0 kV,cone voltage was 50 V,cone flow was 150 L/Hr,desolvation temperature was 450 ℃,desolvation gas flow was 900 L/Hr,nebuliser pressure was 7.0 Bar,parent ion scan range was 100-800 mz/mz,working mode was multiple reaction monitoring mode. RESULTS:The linear range of esomeprazole was 0.2-20.0 ng/ml(r=0.999 7);the limit of quantitation was 50 pg/ml;RSDs of precision,stability and reproducibility tests were lower than 2%;recovery was 99.93%-100.05%(RSD=0.1%,n=9). CONCLUSIONS:The method is simple and accurate,and can be used for content determination of Esomeprazole sodium for injection.
6.Cognitive Analysis on Plant Metaphor in Cancer Growth

Cancer is a serious threat to human health at present,so it is an important task in the medical field to overcome it.The first step is to recognize cancer.Metaphor is a universal cognitive mode.This paper analyzed the metaphorical phenomenon which the tumor growth process was described as plant growth.The results showed that in modern medicine,growth pattern,tumor shape characteristics,growth velocity and diffusion were using plant metaphors to illustrate.Tumor growth is the growth of stems and leaves of plants,plant root growth,plant fruit growth,plant growth in four seasons,special types of plant growth.The tumor metastasis is the seed spreading of the plant.The result implied that tumor in modern medicine was a metaphorical cognition of plants.This metaphor showed the vitality of tumor.
7.Investigation on the cognition of fall prevention in patients with cerebrovascular disease during hospitalization

Objective To perform a questionnaire survey on patients with cerebrovascular disease, so as to find problems in the fall-preventing education and provide scientific evidence for health education improvement. Methods From October 2015 to December 2016, a self-designed "Let's mitigate falling danger"questionnaire was applied to investigate a total of 193 patients with cerebrovascular disease from geriatric department, neurology department and neurosurgical department, and analyzed the resules. Results The average score of the 193 patients was (25.05±4.63). The three items with lowest score were "Using fall-leading drugs" (1.30±0.86), "Getting-up behavior" (1.32±0.88) and "Feeling embarrassed seeking help"(1.35±0.64) in the questionnaire. Besides, the fall-preventing score were different in patients with different gender, education level, diagnosis, history of fall and whether received fall-preventing education or not, and the differences were statistical significant (t=2.013, 11.638, 10.750, 5.238, 2.147;P< 0.05). Generalized linear regression analysis showed that receiving fall health education, gender, education level and disease type were the influencing factors of fall prevention cognition in patients with cerebrovascular disease (P< 0.05). Conclusions Cerebrovascular patients are lack of awareness of fall risks in hospital. The nursing management should take risk control on these patients. The responsible nurse should choose personalized education mode and content according to relevant awareness deficits.
8.Survival properties of ETEC surface-displayed K88ac-LT(B) on Lactobacillus casei.

K88ac-LT(B) gene derived from pQE30-K88ac-LT(B) was cloned into the expression vector pLA and then the recombinant vector was transformed into the competent cells Lactobacillus casei 525. The recombinant bacteria were grown at 37 degrees C, in MRS broth. Western blotting analysis with rabbit-anti-K88ac-LT(B) polyclonal serum indicated that the recombinant protein reacted with the specific antibodies. The results showed that the molecular weight of the recombinant protein was about 71.2 kD. The K88ac-LT(B) fusion protein on the cell surface was confirmed by immunofluorescence mciroscopy and flow cytometric analysis. In addition, the survival of recombinant Lactobacillus casei 525 was studied in imitative gastrointestinal environments such as artificial gastro fluid (pH 1.5-5.5), artificial intestinal fluid, bile(0.3-3.0 g/L). The results indicated that the recombinant strain survived well in artificial gastric fluids at pH 2.5-4.5 in 5 h. The recombinant Lactobacillus casei 525 could slowly grow in the artificial intestinal fluid for different time, and could survive in 0.3% bile.

9.Combined detection and analysis of multiple indicators of second children in ABO-HDN

Objective For the second children diagnosed as ABO hemolytic disease of the newborn(ABO-HDN),we made a comprehensive analysis of the related indicators of prenatal and postpartum,so as to achieve early prevention,early diagnosis and early treatment.Methods Prenatal microcolumn gel assay was used to detect the father and mother's blood type and mother's irregular antibody,mother serum IgG anti A(B)anti-body titer.Microcolumn gel assay was used to detect hemolysis three tests after the production of pregnant women.The results were divided into five groups according to the results of hemolysis three tests:group A[di-rect antiglobulin test(+),free antibodies test(+)and antibody releasing test(+)],group B[direct antiglobu-lin test(-),free antibodies test(+)and antibody releasing test(+)],group C[direct antiglobulin test(+), free antibodies test(-)and antibody releasing test(+)],group D[direct antiglobulin test(-),free antibod-ies test(-)and antibody releasing test(+)]and group E[direct antiglobulin test(+),free antibodies test (-)and antibody releasing test(-)].Total bilirubin,unbound bilirubin,hemoglobin,reticulocyte percentage and lactate dehydrogenase were detected by automatic analyzer.Results ABO-HDN children hemolysis three tests,in the 5 groups,the higher the titer of the mother's IgG anti A(B)antibody,the more serious the child' s condition was,the difference was statistically significant(P<0.05).Reticulocyte percentage and lactate de-hydrogenase in the five groups,the difference was statistically significant(P<0.05).Conclusion A combina-tion of antenatal and postnatal multiple laboratory test parameters is more accurate in predicting the second child ABO-HDN.At the same time,it helps to master the state of the disease and reduce the occurrence of complications and sequelae.
10.A qualitative study on the working status of outpatient nurses in general practice departments in general hospitals in Beijing

Objective:To survey the working status of outpatient nurses in general practice departments of general hospitals.Methods:From March to April 2021, personal in-depth interviews were conducted with outpatient nurses from general practice residency training bases in Beijing. The thematic framework analysis method was used to analyze the interview data and refine the themes.Results:Fourteen nurses working in general practice outpatient clinics were interviewed in this study. Four themes were extracted from the interview data: (1) inadequate staffing of full-time outpatient nurses in general practice departments of comprehensive hospitals; (2) unclear job functions for outpatient nurses in general practice departments; (3) no standardized patient health education in the department; (4) no relevant training received systematically.Conclusion:General practice departments in general hospitals should setup full-time general practice nurse positions, clarify the job responsibilities, strengthen the relevant training and enhance core competency for nurses working in general practice department.
