1.The clinic significant of determination of T-lymphocyte subsets in patients with pulmonary tuberculosis combined with diabetes

Objective To investigate the clinical diagnostic significance of detecting T in peripheral blood lymphocyte subsets (CD4+,CD8+ and the ratio of pulmonary tuberculosis with diabetes) and explore drug therapy of the five thymus peptide combined anti-tuberculosis on the diabetes complicated with pulmonary tuberculosis.Methods Eighty cases of pulmonary tuberculosis complicated with diabetes mellitus were selected as our treatment subject,who were treated with anti-tuberculosis drug treatment and five thymus peptide regulating immunity treatment.T lymphocyte subsets (CD4 +,CD8 + and their ratio) were detected.Meanwhile acid-fast bacilli and image detection were performed.Another 80 cases were as our control group that treated with anti-tuberculosis drug.Results Following the thymus peptide five treatment for 1,2,and 6 months,T lymphocyte subsets were (CD4+:(34.34 ± 7.32) %,(39.23 ± 6.62) %,(41.06 ± 7.13) %),(CD8+:(31.42 ±8.91)%,(32.52 ±8.58)%,(32.12 ±8.59)%),(CD4+/CD8+:(1.23 ±0.31),(1.34 ±0.30),(1.36 ± 0.32),higher than that of before treatment (30.26 ± 8.25) %,(30.36 ± 9.86) %,(1.13 ± 0.42) CD4 +.Through 2 months treatment,CD4+,CD4+/CD8+ of patient in treatment groups were different with that of control group (P < 0.01 or P < 0.05).80 patients after anti-TB plus application thymopentin regulating immunity for 2 months,normal sputum negative conversion rate of CD4+ was 97.2% and improvement rate was 98.0%.However cases with high CD4+ T lymphocyte subsets showed 78.6% negative rate of acid-fast bacilli and 88.0 % cured rate,and there was significant difference (x2 =4.764,7.680,respectively,P < 0.05).Conclusion Thymic peptide five might have the ability of improving the cellular immune function in patients with pulmonary tuberculosis and diabetes.The prognosis is better when T lymphocyte subsets CD4+ of patient was back to normal through treatment.
2.The Control of Apoptosis and Drug Resistance in Ovarian Cancer Cells Which Overexpressed bcl-2

Objective: To investigate whether bcl-2 directly contributes to the development of drug resistance and apoptosis in o- varian cancer cell lines cells OC3. Methods: Retrovirus expression cector pLXSN-bcl-2 was constructed and was transfected to ovarian cancer cell line cells OC3 using Lipofectin,with empty vector pLXSN as a control. bcl-2 expession of transfected cells was analyzed by FACS and Western blot and Adr-induced cytotoxicity was measured by MTT assay. Apoptosis was also detected by PI DNA staining and DNA Ladder analysis.Results:pLXSN/bcl-2 transfected cells OC3/bcl-2 overexpressed bcl-2 and were resistant to Adr-induced cytotoxicity.The ability against Adr-induced apoptosis was increased. Conclusion: bcl-2 may play an important role in the resistance to Adr-inducing apoptosis, thereby increasing resistance of OC3/bcl-2 cells to chemotherapy.

Objective To explore security and preventive measures of the single medical oxygen cabin.Methods The safety risks of the cabin were analyzed in design,installation,management,usage,maintenance and test with considerations on its characteristics and clinical application,and then some preventive measures were proposed accordingly for safety management.Results Safety management of the cabin had to be improved from the aspects of design,manufacture,installation,patient management and etc.Conclusion Safety of the cabin can be enhanced by management system,operating instruction,personnel awareness and etc.
4.EXTRACTION OF L-ARGININE FROM FERMENTATION BROTH BY ION-EXCHANGE PROCESS

The study showed that the L-argmine-adsorbing capacity of strong-acidic resin 001?7 was 1.135 meq per ml of wet resin by controlling the flow rate of the broth at 1/52 vvm, and the efficiency of the elution was higher by using ammnia water as an eluent at the flow rate of 1/50 vvm. About 160 ml of the above eluant was decolorized with 10 ml of 201?4 resin, the transparance of the decolorized eluant was over 90%, and no adsorption of L-arginine by the 201?4 resin was observed. Under the controlled conditions, the extraction yield of L-arginine reached over 95%.
5.MRI diagnosis of “focal asymmetry of the breast”signs on mammography

Objective To evaluate the diagnostic value of MRI for the mammographic sign of focal asymmetry of the breast (FAB) .Methods The morphological features of MRI and Time‐signal intensity curve (TIC) types of 46 cases of mammographic signs of FAB were analyzed retrospectively .Results In all 46 patients ,MRI detected 46 lesions in the corresponding areas of FAB on mammography ,in which 35 cases of benign lesions and 11 cases of malignant lesions were confirmed by pathology .The accuracy , positive predictive value and negative predictive value of the sub‐leaf shape and edge burr of the breast lesions were 82 .61% ,66 .67% , 86 .49% and 86 .96% ,85 .71% ,87 .18% respectively .TIC showed:in type Ⅰ ,all 17 cases were benign;In type Ⅱ ,4 cases were be‐nign and 2 cases were malignant ;In type Ⅲ ,9 cases were all malignant .The diagnostic accuracy of MRI for the benign and malignant breast lesions was 86 .96% .The positive predictive value and negative predictive values were 81 .82% and 96 .88% respectively .Con‐clusion MRI can definitely estimate the potential character of lesion in FAB ,and more accurately discriminate malignant and benign breast lesions .
6.An investigation on binocular summation response of visual evoked potential in children with hyperopia amblyopia

Objective Conventional examination for vision function adopts subjective psychophysics methods.Pattern visual evoked potential (P-VEP) binocular summation response is a new objective way for the test of vision function.But its clinical value in evaluating vision function of amblyopia children is still in controversy.This study was to explore the binocular vision and the feature of P-VEP binocular summation in children with amblyopia and evaluate the significance of P-VEP binocular summation in binocular vision.MethodsThis is a case-controlled study.P-VEP binocular summation response and single ocular P-VEP response was respectively recorded in 151 hyperopia amblyopia children and 80 age- and gender-matched normal children.P-VEP response from children with hyperopia amblyopia was recorded under the corrected vision.The hyperopia amblyopia was diagnosed based on the standard of National Children Amblyopia and Strabismus Prevention and Treatment Working Group.The feature of P-VEP binocular summation was further analyzed and compared with monocular VEP response.Informed consent was obtained from each subject or custodian prior to the trail.ResultsNo significant difference was found in latency between P-VEP binocular summation response and monocular P-VEP response whether amblyopia children or normal children(P>0.05).Binocular response/monocular response value in amblyopia children was lower than that in normal children(P<0.05).Binocular response/monocular response value in mild or moderate amblyopia children was higher than that in severe amblyopia children(P<0.05).Binocular response/monocular response value was significantly reduced in the children with binocular vision than in the children without binocular vision (P<0.05).ConclusionP-VEP binocular vision,acting as an evaluation factor,could impartially reflect the function of binocular vision of amblyopia children.
7.Serum CEA and CYFRA21-1 correlates with EGFR mutation and EGFR-TKI effectiveness in untreated NSCLC patients

Objective To explore the predictive value of serum CEA and cytokeratin-19 fragments (CYFRA21-1)prior treatment for the epidermal growth factor receptor (EGFR) mutation and efficacy of tyrosine kinase inhibitors ( TKI ) in patients with non-small cell lung cancer.Methods The study was a clinical research.Totally 101 matched tissue and plasma samples were collected from Peking University People′s Hospital from 2012 to 2013.All clinical specimens were analyzed for EGFR mutations in exons of 18, 19, 20 and 21 by ADx-ARMS and direct sequencing, and the serum levels of CEA and CYFRA21-1 were analyzed by ECLI.The correlation between EGFR mutant status and efficacy of EGFR-TKI and clinicopathological parameters were analyzed by χ2 test, Log-rank text and Cox proportional hazards regression model.Results The mutation rate was 60.4%(61/101) by ADx-ARMS and 33.7%(34/101) by direct sequencing.Mutations were more frequently observed in the higher serum CEA level patients(≥5μg/L,78.8%).However, the rates of EGFR mutations of different CEA levels were similar.Among the patients receiving TKI therapy, the efficacy of EGFR-TKI was closely related to serum CYFRA21-1 level prior treatment and EGFR mutation (χ2 =8.903, P =0.003; χ2 =28.590, P <0.001 ).And serum CYFRA21-1 level prior treatment and EGFR mutation were independent factors for EGFR-TKI treatment affecting PFS (RR=0.298, P<0.001;RR=0.086, P<0.001).Conclusion The mutation rate of EGFR was significantly related with the expression level of CEA prior treatment, and serum CEA and CYFRA21-1 levels prior treatment could be potential predictors of EGFR-TKI efficacy.
8.Safety and Efficacy of Adopting Different Vascular Access During Autologous Peripheral Blood Stem Cells Collection

Objective To assess the safety and efficacy of adopting different vascular access during autologous peripheral blood stem cells collection.Methods 87 patients received autologous peripheral blood stem cells collection were divided into two groups:One was peripheral vein harvesting group (43 cases),which used the 16G disposable fistula needle for autolo-gous peripheral blood stem cell collection and the other central venous harvesting group (44 cases),which used double cavity of femoral vein catheter for autologous peripheral blood stem cell collection.The observation indicators included venous ac-cesses,collection efficiency,patient tolerance,the number of mononuclear cell and CD34 positive cells.Results The numbers of mononuclear cells and CD34 positive cells in two groups were all above the standard and there was no significant differ-ence (P >0.05).However,the success of venous accesses,the efficiency smooth of collection and-patient tolerance were bet-ter in double cavity of femoral vein catheter group (P <0.05).Conclusion Harvesting the autologous peripheral blood stem cell through central venous by using double cavity of femoral vein catheter had the advantages as follows:high success rate of puncture,acquisition smoothly and reducing the suffering extent of the patients,and also it did not affect the acquisition effi-ciency and effectiveness.

ABPM can more comprehensively provide 3 types of information that has potential clinical value:the assessment of real or mean blood pressure level, the diurnal rhythm of blood pressure, and the blood pressure variability. This article review s the roles of ABPM in the outcome assessment in patients w ith acute ischemic stroke.
10.Effects of early pain intervention provided by an acute pain service team on efficacy of postoperative patient-controlled analgesia

ObjectiveTo evaluate the clinical effects of early pain intervention provided by an acute pain service team on the efficacy of postoperative patient-controlled analgesia ( PCA).MethodsOne thousand four hundred and sixty-seven patients receiving postoperative PCA in November and December 2011 were enrolled in this study.Patients were excluded from the study if they were < 12 yr,unconscious or uncoorperative.The patients were divided into control group (group C,n =725) and intervention group (group 1,n =742).Group 1 received preoperative systematic pain education on the significance and conect use of PCA including pharmacology of analgesic (sufentanil 100 μg in normal saline 100 ml) by an acute pain service team made up of anesthesiologists and nurses specializing in pain management.The education was repeated immediately before operation and when the patients were discharged from recovery room.While in grup C the correct use of PCA was explained routinely before operation by anesthesiologists and nurses taking care of the patients.The incidence of incomplete analgesia and adverse reactions and patient' s satisfaction were rated.ResultsThe incidence of incomplete analgesia was significantly lower and the patient's satisfaction higher in group 1 than in control group.There was no significant difference in adverse reactions between the two groups.ConclusionEarly pain intervention provided by an acute pain service team is effective in improving the efficacy of postoperative PCA and patient's satisfaction.
