Objective To pay more attention to the adverse drug reactions caused by tigecycline. Methods Four cases in our hospital which happened acute or subacute liver failure after the uses of tigecycline were analyzed, including age, basic disease, drug combination,the time when liver function failure happen ,dosage of tigecycline and the course of treatment. Results The aged of four patients was 49-77 years old, the average age (67.5±12.79)years; the time when liver failure happened from 6 d to 20 d, the average(11.75±6.24) d; the course of treatment was 9 d to 23 d, the average(9.75±6.63) d; the total dose of tigecycline was 900 mg to 3650 mg, the average (1812.5±1243.23) mg.The four patients have many basic diseases, combined with many drugs, all of them have mechanical ventilation. Conclusion More attention should be paid to adverse events caused by tigecycline.

Background and purpose:hMLH1 promoter methylation is one of the factors in the development of lung cancer.However,little is known about hMLH1 promoter methylation in association with chemotherapy resistance in lung cancer cells.This study aimed to investigate whether resistance to DDP can be overcame by restoring the expression of hMLH1 in A549/DDP cells after treatment with a demethylating agent,5-Aza-2'-deoxycytidine (5-Aza-CdR).Methods:hMLH1 mRNA was determined by RT-PCR in A549,A549/DDP and A549/DDP treated with 5-Aza-CdR;MSP was carried out to determine the methylation status of hMLH1 in A549 and A549/DDP,A549/DDP treated with 5-Aza-CdR.MTT test,Hoechst 33258 staining and flow cytometry were used to determine the change of cell proliferation,apoptotic morphology and the cell apoptosis index,respectively.Results:hMLH1 mRNA was expressed higher in A549 than in A549/DDP.The unmethylation status of hMLH1 was observed in A549 cells whereas partial methylation status of hMLH1 in A549/DDP cells.After treatment with 5-Aza-CdR,the A549/DDP cells displayed an unmethylation of the hMLH1 gene.After the treatment with cisplatin,the IC50 of A549,A549/DDP and A549/DDP treated by 5-Aza-CdR were (4.7?0.7),(30.1?1.8) and (6.9?0.6)?mol/L,respectively.Apoptotic bodies were less in A549/DDP cells than in A549/DDP cells treated with the 5-Aza-CdR.The rate of apoptosis of A549/DDP treated by 5-Aza-CdR was higher than that of A549/DDP.Conclusion:The methylation of hMLH1 may be relevant to the resistance to platinum-based chemotherapy in the A549/DDP cell line.5-Aza-CdR can inhibit hMLH1 methylation,restore the expression of hMLH1,and enhance cell apoptosis as well as the inhibition of cell proliferation of A549/DDP cells after the treatment of cisplatin.

Objective To explore the influential factors of dangerous placenta previa pathogenesis,and to analyze their peri natal period outcomes.Methods The clinical data in 143 pregnant women and parturients of dangerous placenta previa(research group) and 286 pregnant women and parturients of non-dangerous placenta previa(control group) in our hospital from January 2011 to December 2015 were retrospectively analyzed.Results The average age of the research group was higher than that of the control group (t =2.92,P =0.004);the previous pregnancy frequency,previous parity,previous abortion frequency,previous cesarean delivery frequency and incidence of placenta previa in the research group were higher than those in the control group(P＜0.01);the incidence of placenta previa in the research group was higher than that in the control group(x2 =7.60,P=0.006);the prenatal gestational weeks of the research group were lower than those of the control group(t=-12.33,P＜0.01).The multi-factor analysis showed that the age,previous abortion frequency and previous cesarean delivery frequency were the influence factors of placenta previa occurrence.Among them,the previous cesarean delivery frequency was the main risk factor.The occurrence rates of antepartum hemorrhage,postpartum hemorrhage,uterine removal and premature infants in the research group were higher than those in the control group(P＜0.01);The intraoperative blood loss of the research group was higher than that of the control group(P＜0.05);the neonatal birth weight and the 1 min Apgar score of the research group were lower than those in the control group(P＜0.05).Conclusion The age,previous abortion frequency and previous cesarean section frequency are related to the occurrence of dangerous placenta previa.

Objective To explore the relationship between type 2 diabetes mellitus(T2DM)and thyroid diseases by analyzing the abnormal status of thyroid hormone and antibodies in T2DM patients and the healthy control group.Methods The serum thyroid stimulating hormone (TSH),free triiodothyronine (FT3),free thyroxine (FT4),anti-thyroglobulin antibody (TGAb)and anti -thyroid peroxidase antibody (TPOAb)were measured in 396 patients with T2DM and 411 healthy persons.Results In the healthy group,the incidence rate of abnormal thyroid hormone was 7.5%,in which the male was 5.5% and female was 9.4%,the incidence rate of subclinical hypothyroidism in females was 4.7%,which was higher than 1.5% in males,there was statistically significant difference between them(P <0.05);the positive rate of TPOAb in the healthy control group was 10.2% and the positive rate of TGAb was 6.6%;in T2DM patients,the incidence rate of thyroid hormone abnormalities was 16.2%,which in males and females were 12.3% and 20.5% respectively,among them the incidence rate of subclinical hypothyroidism in females was 9.2%,which was significantly higher than 4.3% in males,the difference had statistical significance (P <0.01),T2DM patients with subclinical hypo-thyroidism accounted for 40.6% of thyroid dysfunction incidence rate;the positive rate of TPOAb was 15.2% and the positive rate of TGAb was 7.1 %.Conclusion The incidence rate of thyroid diseases in T2DM patients is significantly increased compared with the healthy control group,subclinical hypothyroidism in females is predominant;screening thyroid autoantibodies and monitoring FT3,FT4 and TSH at regular intervals in T2DM have the important clinical significance for evaluating the disease condition,jud-ging prognosis and guiding treatment.

Objective To investigate the change of apoptosis and drug resistance after multiple thermotherapy On the HepG2 Cell. Methods After HepG2 cell were treaded by ten repeated cycles of exposure at 43 degree C for 80 minutes twice a day，the sensibility of HepG2 cell to Adfiamyein was analyzed by MTr assay.The apoptosis Was detected by kit，and the expression of Bcl-2 or Bax protein and mRNA were detected by immunohistochemical method and RT-PCR respective.Results The drug resistance to Adfiamycin Was gained by HepG2 cell after multiple thermotherapy.The rate of apoptosis was decreased，the expression of Bcl一2 protein and mRNA Was strengthened，and the ratio of Bcl-2/Bax increased.Conclusions The resistance of heated HepG2 cell to chemotherapeutics Was related with the decreased rate of apoptosis，overexpression of Bcl-2 protein and mRNA，and increased ratio of Bcl-2/Bax.

Objective　To study the experience in prevention and treatment of iatrogenic bile duct trauma(IBDT). Methods　A retrospective study was made on the clinical data of 118 patients with iatrogenic bile　 duct trauma admitted to the Hunan Provincial People's Hospital from March 1990 to September 2000. Results　50.8%　(60/118) of patients with IBDT resulted from the wrong identification of the anatomy of the Calot' Triangle during cholecystectomy. The clinical diagnosis of IBDT depended on the clinical findings, diagnostic abdominocentesis and image examination. The diagnostic rate of ultrasonography for IBDT was 93.2%(110/118). According to the injury site of bile duct, IBDT could be divided into 6 types, the most common type of　 IBDT was resection of partical hepatic duct and part common bile duct(type Ⅲ) which occurred in 83.9%　(99/118) of the patients. The cure rate of IBCT was 100%(118/118) in this series due to the choice of operation according to the trauma type. Conclusions　The key of prevention to IBDT lies in abiding by the princible of “identifying-cut-recognazing” during cholecystectomy. The choice for surgical operative procedure should agree with the trauma type.

Objective To evaluate the value of three dimensional reconstruction images of rotational DSA on measuring aneurysmal necks and make a comparison with traditional DSA so as to provide more abundant and accurate informations for the enbolization of aneurysm. Methods A comparison was made between the measurement of aneurismal necks from 14 cases with traditional DSA examination and a measurement made on three dimensional reconstruction images of the same patient. Results There was a difference shown in the measurment of the aneurysmal necks between three dimensional reconstruction images of rotational DSA and those of traditional DSA, outcoming with more angles and data on three dimensional reconstruction images. Conclusions There are more angles of aneurysmal neck can be shown on rotational 3D DSA especially for the demonstration of the largest aneurysmal neck with a directional value for the intervention. Objective To screen a kind of microorganism which is low poisonous for living creature but has high molluscicidal effect. Methods The Xanthobacter autotrophicus from the soil where Oncomelania hupensis lived were detached, then its molluscicidal effect against the snails were studied by the immersing and contacting methods, respectively. Results The death rates were 1.3%-95.0% immersed for 24 to 72 hours in the different density liquids (1?106,2?106,3?106,4?106 cfu/ml). The LD_~50 immersed for 48 hours was 2.6?106 cfu/ml. The death rates had obviously positive relationship with the immersing time and the density of the liquid. Conclusion The liquid which contains more than 106 cfu/ml Xanthobacter autotrophicus has a stable molluscicidal capacity, and the immersing method is better than the contacting method.[

OBJECTIVE To investigate the drug resistance of Gram-positive organisms infection in our hospital and offer scientific basis for reasonable usage of antibiotics.METHODS The 318 strain organisms were isolated from the specimens of Aikang Hospital since Jan 2001 to Dec 2005.Gram-positive organisms were isolated from the specimens by the routine methods.The drug susceptivity was tested by K-B method.We got the results by 1999 standard of NCCLS.RESULTS Among the 318 strains,Staphylococcus aureus were 128 strains,40.3% of the total organisms;coagulase-negative S.aureus were 69 strains,21.7% of the total organisms;Enterococcus faecalis were 67 strains,21.1% of the total organisms;meticillin-resistant S.aureus(MRSA) were 32.8%;Meticillin-resistant coagulase-negative S.aureus(MRCNS) were 29.0%.The results of drug sensitivity test showed the drug resistant rate of Gram-positive strains for commonly used antibacterials was lower than in Shenzhen,Beijing,Shanghai,Wuhan,Guangzhou and other cities.CONCLUSIONS We should attach in our city importance to supervise the drug-resistance of Gram-positive organism,and preclude dissemination and epidemic of drug resistant strains.

OBJECTIVE To study the distribution and characteristic about drug resistance of extended-spectrum ?-lactamases(ESBLs) producing Klebsiella pneumoniae and Escherichia coli for guiding the clinical drug using. METHODS Using K-B agar diffusion method and confirmed test to detect the drug resistance and distributions of ESBLs-producing K.pneumoniae and E.coli. RESULTS Among 582 K.pneumoniae and E.coli isolates,128(22%) were ESBLs-producing strains.For E.coli,the ratio was 17.5%,while K.pneumoniae was 28.3%.The sensitive rate of K.pneumoniae to imipenem was 100%. CONCLUSIONS ESBLs-producing K.pneumoniae and E.coli are the most common species causing infections,and imipenem is the best choice.
