1.Epidermal growth factor receptor mutations in non-small cell lung cancer of tissue and peripheral blood by fluorescence quantitive polymerase chain reaction

Objective To investigate gene mutations of the epidermal growth factor receptor of tumor tissue and peripheral blood in non-small-cell lung cancer, and to compare the consistency of EGFR mutations in tumor tissue and peripheral blood, fluorescence quantitive polymerase chain reaction (FQ-PCR) was performed according to standard procedure. Methods 112 peripheral blood and 87 tumor tissue of NSCLC were detected by FQ-PCR, among which 45 cases were analyzed using peripheral blood-tumor tissue matched samples. The relationship between clinical efficacy and EGFR mutations Was analyzed. Results Our results showed that there is 32 mutations in EGFR in 112(28.6%)of the patients'peripheral blood samples and 27 mutations in EGFR in 87(31.O%)tumor tissue. EGFR mutations were mainly found in adenocarcinoma and non-smokers in NSCLC (P<0.05). The consistency of EGFR mutation between tumor tissue and peripheal blood reached 71.4%. Objective To investigate the relationship between serum adipocyte fatty acid binding protein (FABP4),insulin resistance and maternal complications in patients with gestational diabetes mellitus (GDM) with satisfactory glucose control.Methods 60 cases of pregnancy between 24 and 28 weeks of OGTT diagnosed GDM pregnant women were selected.30 cases with satisfactory blood sugar control were selected as satisfactory blood sugar control group,30 cases with unsatisfied blood sugar control were selected as unsatisfied blood glucose control group.Healthy control group was selected from 24 to 28 weeks of healthy pregnancy in 30 patients.Fasting serum FABP4,fasting blood glucose(FPG) and fasting insulin(FINS) were measured in three groups of pregnant women after 24 to 28 weeks and pregnant term.To calculate the insulin resistance index (HOMA-IR) of the steady state model,and record and observe the complications of the three groups.Results In the satisfactory blood glucose control group,the incidence rates of pregnant hypertensive disorders,excessive amniotic fluid,premature rupture of membranes,postpartum hemorrhage,cesarean section,macrosomia,neonatal asphyxia,neonatal hypoglycemia,hyperbilirubinemia of newborn were 10.0%,6.7%,13.3%,6.7%,23.3%,16.7%,10.0%,16.7%,10.0%,which were significantly lower than those in the unsatisfied blood glucose control group (23.3%,20.0%,23.3%,16.7%,33.3%,33.3%,20.0%,30.0%,36.7%),the two groups had significant differences(x2 =4.33,6.12,6.01,3.97,7.41,5.46,10.02,4.79,9.22,all P < 0.05).The incidence rates of pregnant hypertensive disorders,excessive amniotic fluid,macrosomia between the satisfactory blood glucose control group (10.0%,6.7%,16.7%) and healthy control group(3.3%,0.0%,6.7%) had statistically significant differences (x2 =7.45,8.46,4.69,all P < 0.05).The incidence rates of postpartum hemorrhage,premature rupture of membranes,cesarean section,neonatal asphyxia,neonatal hypoglycemia,neonatal hyperbilirubinemia between the satisfactory blood glucose control group (6.7%,13.3%,23.3%,10.0%,16.7%,10.0%) and healthy control group(10.0%,10.0%,20.0%,6.7%,13.3%,3.3%) had no statistically significant differences (all P > 0.05).Pregnant 24 ~ 28 weeks,FABP4,FINS,FPG and HOMA-IR of the satisfactory blood glucose control group were (1.78 ± 0.33) ng/mL,(12.35 ± 0.48) mIU/L,(5.51 ± 0.96) rmmol/L,(3.88 ± 0.55),which of the unsatisfied blood glucose control group were (2.36 ± 0.08) ng/mL,(13.92 ± 1.17) mIU/L,(5.46 ±0.74)mmol/L,(3.95 ± 1.17),the differences between the two groups were not statistically significant (all P > 0.05),and compared with the healthy control group,the differences were statistically significant(t =15.32,10.36,11.54,7.34,all P < 0.05).After term pregnancy,the FINS,HOMA-IR of the satisfactory blood glucose control group were still higher than those in the healthy control group,the differences were statistically significant(all P < 0.05).However,FABP4,FPG between the satisfactory blood glucose control group and healthy control group had no statistically significant differences (all P > 0.05).Conclusion With the control of blood glucose levels,decreased in patients with GDM FABP4,fasting blood glucose,glycosylated hemoglobin level,but high insulin resistance still persists and glycemic control satisfaction does not completely reduced the occurrence of the complications in both mothers and neonates.
3.Effects intensive health management on 585 diabetic patients from Desheng Community of Beijing

Objective To evaluate the effects of known-to-have health management on diabetes patients.Methods A total of 585 diabetic patients from Desheng Community of Beijing received an intensive health management for 3 months,including diet intervention,physical exercises,medication,health education and individual health guidance.Body weight (BW),body mass index (BMI),waist circumference (WC),blood pressure (BP),fasting blood glucose (FBG),2-h postprandial blood glucose (PBG),glycated hemoglobin A1C (HbA1 c),total cholesterol (TC),triglycerides (TG),high-density lipoprotein cholesterol (HDL-C),low-density lipoprotein cholesterol (LDL-C),total physical exercises,effective physical exercises,effective physical exercise ratio to BW and dietary intake were compared before and after the intervention.Results Of 585 patients with type 2 diabetes mellitus,240 were male and 345 were female (average age (64.4 ± 9.1) years old).BW,BMI,WC,systolic blood pressure,diastolic blood pressure,FBG,2-h PBG,HbA1c,TC,TG,HDL-C,LDL-C,amount and time or frequency of effective physical exercises,effective physical exercise ratio to BW and total dietary intake were significantly improved after intensive health management (t values were 20.35,20.34,23.74,14.06,12.35,13.35,16.50,9.90,7.53,6.37,-3.74,4.91,-7.44,-7.91,-5.60,-8.41 and 5.21,respectively ; all P < 0.05).More healthy eating habits were found in 321 subjects (54.8%).Those having normal FBG were increased from 54.2% to 80.1% following known-to-have health management,with 2-h PBG from 60.4% to 87.8% and HbA1c from 58.9% to 77.9% (x2 values were 88.21,109.31 and 39.97,respectively; all P < 0.05).Conclusion Known-to-have management may provide an effective tool for community diabetics.
4.Needs of Community Nursing-based Continuing Home Care in Old Patients with Chronic Diseases:A Qualitative Study

Objective To investigate the needs of nursing-based continuing home care in old patients with chronic diseases in communi-ty. Methods From June to August, 2016, 14 old patients with chronic diseases were purposively sampled, and interviewed with semi-struc-ture. The data were collected and refined with phenomenological analysis. Results The patients were very positive in nursing-based continu-ing home care. The main requirements included the knowledge about chronic diseases, psychological comforts, rehabilitation nursing, daily security help and medical insurance support. Conclusion It is necessary to support the continuing home care for old patients with chronic diseases, and strengthen the profesional nursing team building in community.

AIM: To investigate the effects of nitric oxide (NO) on hepatic encephalopathy in cirrhotic rats induced by LPS. METHODS: The cirrhotic model of rats was established by complex pathogeny. Since the end of the 8 th week, the rats were intragastrically-infused with 0.9% salt, L-arginine(L-arg) and LNNA respectively for 2 weeks.The hepatic encephalopathy in cirrhotic rats were induced by 3 mg/kg LPS (ip) 4 hours before the rats were sacrificed. RESULTS: The normal behaviors and electroencephalograph were appeared in L-arg group. LNNA group showed hepatic encephalopathy. OBJECTIVETo investigate the efficacy and safety of nebulizing glucocorticoid combined with olfactory training in the treatment of postviral olfactory dysfunction.METHODSPatients with postviral olfactory dysfunction were recruited in this study. All patients underwent T&T olfactory testing, sinonasal computer tomgraphy scanning, as well as magnetic resonance scanning of the olfactory pathway. Nebulizing glucocorticoid (pulmicort repulse) was inhaled once daily at the starting dose of 2 mg tapered to 1 mg after two weeks combined with olfactory training for 4 weeks. T&T olfactory testing were repeated after 4-week treatment.RESULTS Twenty four patients received teatment, with a mean age of 54 years old(range 37 to 81 years old), a mean olfactory dysfunction course of 2.20 months(range, 0.25-9 months). Of whom, 21 were anosmia, 3 were hyposmia. After teatment, complete recovery were achieved in 4 patients(16.7%), obvious improvement in 9 (37.5%), improvement in 5 (20.8%), no improvement in 6 (25.0%). No side effect and untoward effect were found.CONCLUSIONThe primmary outcomes suggest the efficacy and safety of nebulizing glucocorticoid combined with olfactory training in the treatment of postviral olfactory dysfunction.
7.Effect of TNF-related apoptosis inducing ligand on the biological activity of hepatocarcinoma cell line

AIM: To explore the effect of TNF-related apoptosis inducing ligand (TRAIL), a new apoptotic inducing molecule on the biological activity of hepatocarcinoma cell line. METHODS: The expression of membrane binding TRAIL on HepG2 cells was detected by immuno-cytochemistry. Quantity of secretory TRAIL was assayed by ELISA method. The cytotoxicity and apoptosis induced by TRAIL was detected by MTT and TUNEL method, respectively. The telomerase activity of HepG2 cells was detected by TRAP-PCR assay kit. The expression of hTERT, the catalytic subunit of telomerase, was detected by FCM. RESULTS: TRAIL was constitutively expressed on the membrane of HepG2 cell line. Soluble TRAIL was also expressed to a certain degree. Cytotoxicity assay showed that TRAIL significantly inhibited the growth of hepatocarcinoma cells. TUNEL assay indicated that TRAIL induced apoptosis in hepatocarcinoma cells. Detection of telomerase activity showed that TRAIL inhibited telomerase activity and the expression of telomerase catalytic subunit. CONCLUSION: TRAIL is an effective molecule to inhibit the growth of hepatocarcinoma through multiple pathways, such as inducing apoptosis and inhibiting the activity of telomerase.
8.Study on gene expression changes of arsenic sulfide treated NB4 cells.

OBJECTIVETo compare the gene expression status of NB4 cells before and after arsenic sulfide treatment by cDNA microarray.
METHODSTwo cDNA probes were made from mRNA of untreated or arsenic sulfide treated NB4 cells. The cells were labelled with Cy3 or Cy5 fluorescence dyes individually, hybridized with cDNA microarray, and scanned for fluorescent intensity. The altered gene expression was screened through the analysis of difference in gene expression profile.
RESULTSThirty four genes related to apoptosis, cell cycle and others expressed different after the treatment of arsenic sulfide, 28/34 were up-regulated, 6/34 down-regulated.
CONCLUSIONABC50, PNAS-2 and cyclin G(2) might take part in the process of NB4 cell apoptosis induced by arsenic sulfide.

9.The apolipoprotein E-CI-CII gene cluster polymorphisms and coronary artery disease.

OBJECTIVETo investigate associations between the apolipoprotein E-CI-CII gene cluster polymorphisms and coronary artery disease (CAD).
METHODSapoE genotypes were identified by multiplex amplification refractory mutation system (multi-ARMS) and the polymorphisms of both apoCI and apoCII genes were detected by using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) in 203 cases of CAD and 365 controls. Pairwise linkage disequilibrium coefficients (D, D') were estimated by the LINKAGE program.
RESULTSThe frequencies of apoE E3/4 genotype (0.259) and epsilon4 (0.139) in CAD group were significantly higher than that in control group (0.125, 0.069), (P<0.05). The significant difference was also found for the apoCI locus, the frequencies of H2 allele were 0. 205 in the CAD and 0.113 in the control. Linkage disequilibrium coefficient D' was 0.672 (P<0.01) between apoE and apoCI genes. Significant differences for a deficit of epsilon3-H1-T1 and excess of epsilon4-H2-T1 were found in the CAD by estimation of the haplotype frequencies. After adjustment for possible confounding factors, the multivariate Logistic analysis showed a significant interaction among epsilon4, H2 and smoking, OR value was 18.3 (95%CI:2.35-150.81, P<0.05), attributable proportions of interaction (API) was 57.3%, it was a multiplicative model. An additive model was shown among epsilon4, H2 and bibulosity; the odds ratio (OR) (95%CI) and API of their interaction were 12.7(2.8-58.6, P<0.05) and 43.5%, respectively.
CONCLUSIONThe results suggested that both apoE and apoCI on chromosome 19 were the susceptibility loci for CAD, their linkage disequilibrium should be responsible for the development of CAD. Smoking and bibulosity can significantly increase the risk of CAD.

10.Investigation and improvement of the problems which are caused by pretreating reused medical instruments after usage

Objective To discuss the reason of the problem, which are caused by pretreating reused medical instruments after usage in each hospital clinical departments;ensure the washing quality for the reused medical instruments and prolonging the working life of them. Methods The execution rate, washing quality, damage of which pretreating reused medical instruments in clinical departments were counted The Clinic of Infection, Nursing Department and Central Sterile Supply Department observed and adopted questionnaire for information, and then formulated targeted guidance and improvement according to the information. Results The execution rate of pretreating reused medical instrument after usage reached from 64.6% to 99.4%, the qualified rate of instrument washing had improved from 93.0% to 99.7%,the damage rate of instruments had decreased from 1.84% to 0.74%,and there were significant differences(χ2=9 413.8,38 160.6,2 820.2,all P<0.05). Conclusions Pretreating reused medical instrument after usage correctly and timely,can not only ensure the washing quality for the medical instruments, but also prolong the working life of them. It ensures the application value of the medical instruments.
