1.Fungus Isolation from Clinically Used Abacterial Body Fluid:Flora Distribution and Drug Resistance

OBJECTIVE To study fungal isolation and its drug resistance from abacterial body fluid.METHODS The abacterial body fluid for patients in our hospital was cultured by BacT Alert 120 and routine method.The strains which had been isolated were identified by development culture method and VITEK-YBC card or API-20C AUS reagent stripes.Furthermore,drug sensitive test was done by ATB FUNGUS2 drug sensitive reagent kit.RESULTS From the 898 samples with positive cultures of abacterial body fluid.8 species and 79 strains were isolated.Among the 79 fungal strains(8.8%),Candida albicans was 40 strains(50.6%),C.tropicalis was 12 strains(15.2%),C.glabrata was 11 strains(13.9%),C.parapsilosis was 9 strains(11.3%),C.guilliermondii was 4 strains(5.1%),C.krusei was 1 strain(1.3%),Cryptococcus were 1 strain(1.3%),and Mucor were 1 strain(1.3%).From them,13 strains(16.5%) were isolated in ICU,10 strains(12.7%) were in neurosurgical department,9 strains(11.4%) in oncological department,8 strains(10.1%) in urological surgery department,7 strains(8.9%) in thoracic surgery department,7 strains(8.9%) in pneumology department,6 strains(7.6%) in hematology department,5 strains(6.3%) in gerontology department,4 strains(5.1%) in orthopedic department,4 strains(5.1%) in endocrinology department,2 strains(2.5%) in nephrology department,and 4 strains(5.1%) in other units.The antifungal activity of amphotericin B,nystatin and flucytosine for fungus was quite perfect with sensitive ratio of 90%.But the drug resistance ratio to miconazole,ketoconazole and econazole was upward on 20-30%.CONCLUSIONS It′s important and necessary to monitor the circumstance of fungal infection and resistance of the pathogenic fungi due to its increased morbidity.

Objective To investigate the feature and general trend of the contingent negative variation (CNV) in the aged with memory impairment (AMI) and dementia tendency in the course towards advanced dementia. Methods CNV in 69 AMI subjects (serving as the study group) and 22 healthy elderly subjects selected from well age matched (serving as the control group) were examined. 3.Determination of Hydrogen Chloride in Air by Ion-Chromatography

Objective To make a method for determining the hydrogen chloride in the air by ICS 1 000 ion chromatographic system. Methods Hydrogen chloride in the air of workplace was determinated by using ICS-1 000 ion chromatographic with Ion Pac AS14 chromatographic column, a mixed solution of 3.5 mmol/L Na2CO3 and 1.0 mmol/L NaHCO3 were used as eluent and absorbing solution for the air sample. The sampling was performed at a flow rate of 1.2 ml/min. Results The detection limit was 10 ?g/L, RSDs were 1.65%-3.68% and the rates of recovery were 91.8%-99.7%. Conclusion This method was proved to be accurate, simple, rapid and the disturbance of the other anion and chlorine ion can be avoided. It is also considered as an ideal method to determine the hydrogen chloride in the air.
4.Unsuspected gallbladder cancer: clinical and progrostic factors on 77 patients

Objective To analyse the clinical data of 77 patients with unsuspected gallbladder carcinoma,and to determine the impact of TNM stage and the timing of the second operation on postoperative survival.Methods A retrospectively analysis of 77 patients operated between January 2008 and January 2013 in our hospital for unsuspected gallbladder carcinoma was carried out.The case inclusion criteria was in strict accordance with the medical records which described without preoperative diagnosis,or cholecystectomy for benign gallbladder diseases,or intraoperative or postoperative pathological diagnosis of gallbladder cancer.References to previous published medical literature and the intervals from initial cholecystectomy to further treatment were analyzed to see whether the prognosis and survival varied depending on the TNM staging and whether secondary radical surgery was carried out.Results For the 77 patients in this study,they all had high risk factors associated with gallbladder cancer which included:women,aged > 50 and gallbladder stones.The initial surgery included laparoscopic cholecystectomy (n =53),and open cholecystectomy (n =24).In 26 patients,intraoperative frozen section confirmed the diagnosis and they were treated according to the TNM staging (radical operation n =7,palliative surgery n =17).The postoperative 1-,2-,3-year cumulative survival rates were 65%,45% and 20%,respectively.For the 54 patients who underwent radical or extended radical cholecystectomy within a short interval from the first operation,the 1-,2-,3-year cumulative survival rates were 82.5%,62.5% and 45.7%,respectively.Conclusions For resectable gallbladder cancer,the prognosis of unsuspected gallbladder carcinoma was related not only to the clinical staging,but also to the timing of the radical operation.The shorter time interval,the longer the survival,and the better the prognosis.There was no significant difference in prognosis for the group of patients with time intervals between the two operations of less than two weeks when compared with the group with immediate radical surgery.For advanced stages of gallbladder cancer,palliative surgery should be given according to the patient's general condition,aiming to improve quality of life.
5.Efficacy comparison of two conservative strategies in the treatment of malignant pleural mesothelioma

Objective To compare the survival rates from two conservative strategies in the treatment of malignant pleural mesothelioma (pemetrexed plus cisplatin or pemetrexed plus carboplatin ).Methods Seventeen cases diagnosed of malignant pleural mesothelioma at clinical stages of or over Butchart Ⅲ in our hospital during 2005 -2009 were treated with pemetrexed plus cisplatin (10 cases,Group A)or pemetrexed plus carboplatin (7 cases,Group B ).The difference in the survival rates between these two groups was compared.Results The early survival rate (0 -3 months)in pemetrexed plus cisplatin (group A)-treated group was more than that of pemetrexed plus carboplatin group (group B),but the difference was not statistically significant (x2 =0,1.52,1.52,respectively,P > 0.05 ).For the medium-term survival rate (4 - 9 months),group A was better than group B,and the difference was statistically significant( x2 =5.21,4.41,4.41,4.50,4.50,4.55,respectively,P < 0.05 ).The two groups produced comparable long-term survival rate s (10 -12months) ( x2 =1.31,0.09,0.09,respectively,P > 0.05 ).Conclusion To increase the medium-term survival rate (4 -9 months)of patients with MPM,we prefer to using pemetrexed plus cisplatin regime.For the patients with more severe side effects which can be relieved by folic acid and VitB12,this regime is also recommended.Otherwise,for the patients with unrelievable side effects,especially for those with poor physical condition or a short survival expectation,pemetrexed plus carboplatin is suggested.
6.The prognosis value of galectin-3 in patients with acute ischemic stroke

Objective To evaluate the prognosis value of the expression of galectin 3 in patients with acute ischemic stroke. Methods 165 Patients with acute ischemic stroke had been observated for 12 months and their occurrence of recurrent stroke e-vents were recorded.The concentration of galectin-3 was detected.Results 65 patients had recurrence stroke events with a rate of 39.39% within 12 months.It was also found that the serum concentration of galectin-3 in acute ischemic stroke recurrence patients were significantly higher than those of patients without recurrence,the difference was statistically significant(P <0.05).The rate of acute ischemic stroke recurrence in patients with high concentrations of galectin-3 was higher than that in patients with low concen-trations of galectin-3,the difference was statistically significant(P <0.05).Conclusion Galectin-3 is a good indicator to predict the prognosis of patients with acute ischemic stroke,and it could be used to evaluate the risk of recurrence in patients.

8.Effect of protection motivation theory on the self-nursing ability of high-risk diabetic foot patients

Objective To explore the influence of the protection motivation theory (PMT) on the self-nursing ability of high-risk diabetic foot (DF) patients. Methods The outpatients in our hospital were selected with high-risk DF between January 2013 and May 2014, randomly divided into the control group and PMT group, 52 cases in each group. Guided under the protection motivation theory, PMT group received a six-month health education and management; and the control group accepted conventional health education of diabetes. Before and after the intervention, some observation indexes of the two groups respectively were evaluated including the ability of diabetic foot self-nursing, foot condition, fasting blood sugar, 2 h postprandial blood glucose. Result After six months, the scores of the self-care ability of diabetic foot and foot condition from the patients of PMT group were higher than that of PMT group before the intervention and that of control group after intervention (P<0.05). Conclusion PMT can help patients with high-risk DF enhance their foot self-care ability, improve their foot condition, control their blood sugar, and prevent the DF onset.
9.Progress in the experimental studies of traditional Chinese medicine for benign prostatic hyperplasia.

The present article reviews the progress in the experimental studies of traditional Chinese medicine for benign prostatic hyperplasia (BPH) in the recent years. Traditional Chinese medicine can inhibit prostatic hyperplasia, after the growth factor of prostate cell proliferation, regulate nerves and endocrine, and act on the kinetic factor (alpha-receptor) and static factor (augmented prostate) of BPH formation. We also investigate the selection of the animal model of prostatic hyperplasia. It has certain directive effect to further discover the action mechanism of traditional Chinese medicine in the treatment of BPH.

10.Analysis and Countermeasure for Complex Cases in Adoption Paternity Testing

[Objective] To explore how to deal with the paternity test of complex adoption cases. [Method] Samples from 13 families, in which adoptive parents were suspected related to biological parents, were genotyped using "Identifder + Sinofder + Powerplex 16" combined system (D8S1179, D21S11, D7S820, CSFIPO, D3S1358, TH01, D13S317, D16S539, D2S1338, D19S433, VWA, TPOX, D18S51, D5S818, FGA, D6S1043, D12S391, PentaD, PentaE) followed by further statistical analysis. [Result] Among all 13 cases, 2 were completely accordance with the Mendel law, PI > 10 000. There found more than 3 inconsistent loci in 8 cases. And found 1~2 inconsistent loci in 3 cases, needed to test more STR loci until PI≥10 000. The half sibling index (HSI) was also calculated with ITO method. The adoptive parents of 2 cases were not excluded from a full sibling with biological parents. In addition, Y-STR loci were tested for 4 cases (father/son). Two adoptive fathers of them were not excluded from the paternal relationship with biological fathers. [Conclusion] The most (76.9%) of all (13) complex adoptive cases of paternity test could be drawn a definite conclusion with combined system of "Identifder + Sinefiler + Powerplexl6". Minority (23.1%) of them was not definite yet and needed testing more STIR loci. Meanwhile, we suggested adding Y-STR tests and providing HSI for reference.
