1.Endoscopic diagnosis of gastrointestinal graft-versus-host disease

Objective To evaluate the significace of endoscopic diagnosis of gastrointestinal graft-versus-host disease ( GI GVHD) resulted from allogeneic bone marrow transplantation. Methods Five patients with suspected GI GVHD received endoscopic examination and biopsies were taken from antrum, sig-moid colon or the focal lesions. Results The symptoms of GI GVHD included anorexia, nausea, vomiting, watery diarrhea, abdominal pain, GI bleeding etc. The endoscopic appearance in stomach varied from subtle mucosal edema, hyperemia, and erythema necrotic erosion. Besides the above stated, the endoscopic appearance in colon also presented with diffuse mucosal erosion, hemorrhege and ulcer. Histological findings included characteristically crypt epithelial cell apoptosis and sloughing, and lymphocyte infiltration in epithelium and lamina propria. The involvement may be varied from diffuse to focal in stomach or colon. Conclusion Endoscopic and histological evaluation of the stomach and colon and biopsy specimen can be used as the evidences in diagnosing GI GVHD.
2.Study on the endoscopic, pathologic and immunohistochemical characteristics of gastric stromal tumor

Objective To study on the endoscopic, pathologic and immunohistochemical characteristics of gastric stromal tumor. Methods The gastric tumor samples were studied with light microscopy, and in those suspected of mesenchymal tumor the expression of CD11734, CD34 , vimentin, desmin, S-100 etc. were further determined with SP method. The clinical, pathological and endoscopic characteristics were reviewed. Results Twenty-eight cases of mesenchymal cell tumor were diagnosed as stromal tumor and enrolled in this study, with 9 benign, 11 borderline, and 8 malignant cases. Endoscopically, the gastric stromal tumors showed submucosal protuberant lesions, and were situated in fundus (15 cases), body (10 cases) and an-trum (3 cases). Deep concave ulcer presented on tumor surface in 10 patients. Histology revealed the typical stromal tumor features with cell in spindle and/or epithelia shape and arranged in fence-like or spiral forms. All cases were positive for CD117 CD34 , vimentin when assessed, but 28 cases were negative for desmin and 22 cases (18. 2% ) for S-100. Conclusion The gastric stromal tumors were the most common mesenchymal tumors. Microscopically, the tumors were composed of spindle or epithelioid cells. CD117 and CD34 can be used as immune markers to diagnose gastric stromal tumor.
3.Effects of melatonin on proliferation and apoptosis in human pancreatic cancer cell line SW1990

Objective To investigate the effect of melatonin (MT) on the proliferation and apoptosis in human pancreatic cancer cell SW1990 in vitro.Methods SW1990 cell line were treated with MT at different concentrations (0.1,0.5,1.0,2.5 and 5.0 mmol/L) and at different time points (24 h,48 h and 72 h).Cell proliferation was evaluated by MTT and apoptosis was determined by AnnexinV/Pl,cell cycle was determined by flow cytometry,and Western Blot was used to detect the expression of Bcl-2 and Bax.Results MT could inhibit the proliferation of SW1990 cell in a time and dose dependent manner.48 h after 0.1~5.0 mmol/L MT treatment,the proliferation inhibitory rate was 7.4%~85.8%,the proportion of SW1990 cell in phase G0/G1 was 72.6%~85.33%.48 h after 1~5.0 mmol/L MT treatment,the apoptosis rate was 21.5% ~41.7%,and the expression of Bcl-2 was down-regulated and the ratio of Bcl-2/Bax decreased.Conclusions MT could inhibit the proliferation of SW1990 pancreatic cancer cells by up-regulating the expression of Bax and down-regulating the expression of Bcl-2,as well as enhancing apoptosis and blocking SW1990 cell in phase G0/G1.
4.Visual quality evaluation of short-term in pseudophakic eye with 1CU accommodating intraocular lenses

Objective To observe the short-term visual effects of cataract surgery patients with 1CU accommodating intraocular lens (IOL) compared with aspheric monofocal IOL. Design Non-randomed comparative trial. Participants Consecutively 118 senile cataract patients (118 eyes). Methods 59 cataract patients (59 eyes) were implanted 1CU accommodating IOL (1CU group), and 59 cataract patients (59 eyes) were implanted aspheric monofocal IOL (Tecnis ZA9003 group). All patients were assessed at 3 months post-operation in the aberrations of crystalline, the modulation transfer function contrast sensitivity of 10/100 percent in three sorts distance(40, 63, 100 cm), uncorrected distanced visual acuity(UCDVA), uncorrected near visual acuity(UCNVA), best-corrected distance visual acuity(BCDVA), best-corrected near visual acuity(BCNVA), distance corrected near visual acuity(DCNVA), pseudo-accommodation, and the rate of off-near glasses. Main Outcome Measures Visual acuity, aberrations, modulation transfer function, contrast sensitivity, pseudo-accommodation, the rate of off-near glasses. Results At 3 months postoperatively, both UCNVA and DCNVA were significantly better in 1CU group than in the aspheric monofocal IOL group (t=8.85, P=0.00; t=13.05, P=0.00). The accommodative range was (2.58?0.37) D in 1CU group and (1.46?0.25) D in monofocal IOL group (t=14.33, P=0.00). In 3 mm and 5 mm pupil diameter the difference of the total aberration and spherical aberration in Tecnis ZA9003 group or 1CU group was significant (3 mm pupil t=18.48, P=0.00; t=5.70, P=0.00. 5 mm pupil t=19.98, P=0.00; t=6.70, P=0.00). At 40 cm and 63 cm, VA scores were higher in 1CU group (85%)than in Tecnis ZA9003 group (25%) (t=3.64, P=0.00; t=2.74, P=0.00; t=2.65, P=0.00; t=2.44, P=0.01). The rate of off-near glasses in 1CU group was better than in the aspheric monofocal IOL group(?2=41.95, P=0.00). Conclusions The 1CU accommodating IOL can provide better useful near visual acuity than aspheric monofocal IOL, especially for the patients who want to get rid of spectacles.
5.Biological Safety Test Methods for Reduced Glutathione for Injection

Objective:To establish the bacterial endotoxin test method and the abnormal toxicity test method for reduced glutathi -one for injection.Methods:According to the requirements and methods in Chinese Pharmacopoeia (2015 edition, part IV), the bacte-rial endotoxin test and the abnormal toxicity test for reduced glutathione were studied .Results:The limit of bacterial endotoxin for re-duced glutathione was 0.125 EU· mg-1 , and the limit of abnormal toxicity was 1.0 g· kg-1 .Conclusion: The bacterial endotoxin test method and the abnormal toxicity test method are feasible .The abnormal toxicity should be supplemented in the quality standard for reduced glutathione , and the bacterial endotoxin test can replace the pyrogen test .

To explore the relationship between the drug-seeking behavior, motivation of conditioned place preference (CPP) rats and the frontal association cortex (FrA) electroencephalogram (EEG) sample entropy, we in this paper present our studies on the FrA EEG sample entropy of control group rats and CPP group rats, respectively. We invested different behavior in four situations of the rat activities, i. e. rats were staying in black chamber of videoed boxes, those staying in white chamber of videoed boxes, those shuttling between black-white chambers and those shuttling between white-black chambers. The experimental results showed that, compared with the control group rats, the FrA EEG sample entropy of CPP rats staying in black chamber of video box and shuttling between white-black chambers had no significant difference. However, sample entropy is significantly smaller (P < 0.01) when heroin-induced group rats stayed in white chamber of video box and shuttled between black-white chambers. Consequently, the drug-seeking behavior and motivation of CPP rats correlated closely with the EEG sample entropy changes.

8.Efficiency analysis of transcatheter arterial chemoembolization combined with CT-guided percutaneous acetic acid injection for huge hepatocellular carcinoma

Objective To explore the clinical application value of transcatheter arterial chemoembolization (TACE) combined with CT-guided percutaneous acetic acid injection (PAI) in treatment of huge hepatocellular carcinoma (HCC). Methods Forty-three patients with huge HCC were randomly divided into two group. Twenty-one patients in group A underwent routine one course for TACE (three times), and the interval of TACEs was one month. Twenty-two patients in group B underwent TACE combined with PAI, and CT-guided PAI was performed once a week since 2-3 weeks after first TACE, and one course included 6-9 times of PAI. Postoperative follow-up was conducted (including AFP, the size of tumor, etc.). One course of treatment was repeated in case of tumor recurred. Results At the 1st month after treatment, no statistical difference was found of AFP positive rate between two groups. Statistical difference of total effective rate was found between two groups (38.10% vs 77.27%). The 1-, 2- and 3-year survival rate In group A was 47.62%, 23.81% and 9.52%, respectively, while in group B was 81.82%, 54.55% and 36.36%, respectively, and significant differences were found between the two groups for the same period. Conclusion TACE combined with PAI is safe and more effective than TACE alone in the treatment of huge HCC.
9.Clinical Observation of Mailuoning Injection Combined with Benazepril in the Treatment of Chronic Glo-merulonephritis

OBJECTIVE:To observe clinical efficacy and safety of Mailuoning injection combined with benazepril in the treat-ment of chronic glomerulonephritis. METHODS:130 patients with chronic glomerulonephritis were randomly divided into observa-tion group and control group,with 65 cases in each group. Both groups received rountine treatment as low salt and fat diet. Control group was additionally given Benazepril tablet 10 mg,qd;observation group was additionally given Mailuoning injection 20 ml added into 5% Sodium chloride injection 250 ml,ivgtt,qd,on the basis of control group,20 days of drug withdrawal every 10 days. Both group received 4 months of treatment. Clinical efficacy of 2 groups were observed as well as the levels of BUN,UCr, SCr and 24 h urine protein quantitative (UPE) before and after the treatment. The incidence of ADR was compared between 2 groups. RESULTS:Total effective rate of observation group was 83.1%,which was significantly higher than 67.7% of control group,with statistical significance (P<0.05). There was no statistical significance in BUN,UCr,SCr and 24 h UPE between 2 groups before treatment (P>0.05);those indexes of 2 groups decreased significantly after treatment,and the observation group were lower than the control group,with statistical significance(P<0.05). There was no statistical significance in the incidence of ADR between 2 groups (P>0.05). CONCLUSIONS:Mailuoning injection combined with benazepril is effective for chronic glo-merulonephritis,and can effectively improve renal function with good safety.
10.Effect of early continuous enteral nutrition combined with interstinal mucosal protective agents on gut barrier in patients with severe acute pancreatitis:A multicenter prospective randomized controlled trial

Objective To assess the effect of continuous early enteral nutrition(EEN)combined with intestinal mucosal protective agents on gut barrier function in patients with severe acute pancreatitis.Methods A total of 79 patients with severe acute pancreatitis selected from four centers between May 2004 to June 2006 were enrolled and divided into EEN combined with intestinal mucosal protective agents group(combined group,n=39)and total parenteral nutrition(TPN)group(n=40).The patients were received either EEN or TPN when homeostasis were achieved within 72 hours after onset.The patients in combined group were administered pepti-2000 variant combined with glutamine,arginine and intestinal mucosal protective agents.The patients in TPN group were administered through a central vein.APACHE-Ⅱ score was recorded every week;The concentration of serum amylase,plasmic diamine oxi dase(DAO)and endotoxin were mesured on day 1,7,14 and 21 as well as urinary excretion of lactulose (L)and mannitol(M).Complications,lenth and charges of hospital stay were recorded.Results There was no death in both groups.The APACHE-Ⅱ score decreased on day 7,but lower in combined group (6.00±1.60)than that in TPN group(7.08±2.34)(P<0.05).On day 7,14 and 21,the concentrations of endotoxin in combined group was(39.30±15.82),(22.64±14.31),(14.81±10.93)Eu/L,respectively,urinary L/M ratio was 0.28±0.25,0.21±0.18 and 0.08±0.04,respectively,IFABP-c was 15.62±5.26),(5.46±1.18)and(3.26±0.94)pg/ml,respectively.All of these parameters were significantly lower than those in TPN group(P<0.05).The infectious rates including pancreatic,peritoneal and respiratory infection in TPN group were much higher than that in combined group(26.47% vs 3.44%,P<0.01).The composition of flora fecal remained unchange in combined group rather than TPN group.The mean hospital stay was shorter in combined group[(20.0±5.7)days]compared to TPN groups[(34.5±12.9)days].The charges were also significantly lower in combined group,with average cost of RMB 25,900±14,200,while it was 46,800±4,030 in TPN group.Conclusions EEN combined with intestinal mucosal protective agents can improve gut barrier function via reducing the gut permeability,improving the hypoperfusion,maintaining the integrity and gut fecal flora.It might reduce the course and charges of hospital stay.
