1.External-route microsurgery for retinal detachment

Objective To observe the clinical efficacy of external -rou te microsurgery for retinal detachment (RD). Methods In 36 patie nts (36 eyes) with single rhegmatogenous RD, the silica gel piece and/or bucklin g bands were preplaced, and drainage of subretinal fluid, retinal cryotherapy, e xamination of locating the holes, and intraocular injection of gas were performe d under surgical microscope. The surgical effects were compared with those of ot her simultaneous 37 patients with rhegmatogenous RD who underwent surgery under binocular indirect ophthalmscope. Results The simultaneous int raoperative observation of the fundus details and the sclera through the microsc ope was excellent in all cases. Under the surgical microscope, the reaction of r etinal cryotherapy was clearly visible without any serious surgical sequela. The observation of reaction of retinal cryotherapy and the orientation of the holes were not affected by mild opacity of the refractive media. Retinal reattachment was achieved in 31 eyes after the primary surgery and in 3 eyes after the secon dary surgery, with the final rate of reattachment of 94%. The best-corrected vi sual acuity was
2.Antibiotic-resistance of Acinetobacter spp Nosocomial Infection:Analysis from 2003 to 2005

OBJECTIVE To investigate the drug-resistance rate of strains isolated from nosocomial infection patients with Acinetobacter spp.METHODS A total of 198 patients with Acinetobacter spp nosocomial infection were studied retrospectively from 2003 to 2005.The antibiotic resistance profiles were specifically analyzed.RESULTS The strains of Acinetobacter spp were mainly isolated from sputum(74.75%) and wound secretion(11.61%) specimens.There were no significant differences in the resistance of Acinetobacter spp against antibiotics in 3 years except piperacillin/tazobactam and meropenem.The resistance rate of all 198 strains to 13 kinds of antibiotics were as following:imipenem/cilastatin 2.53%,meropenem 2.02%,cefoperazone/sulbactam 30.81%,piperacillin/tazobactam 38.38%,amikacin 53.54%,cefepime 61.11%,levofloxacin 66.67%,ceftazidime 66.16%,ciprofloxacin 68.69%,gentamicin 67.68%,sulfamethoxazole/trimethoprim 63.64%,piperacillin 70.72%,and aztreonam 85.35%.CONCLUSIONS It is indicated that Acinetobacter spp are highly resistant to the common antibiotics,but still sensitive to imipenem/cilastatin and meropenem.
3.Construction of a cDNA Library and Cloning of an Arabinosidase cDNA from Armillariella tabescens

The expression cDNA library of A. tabescens was constructed by SMART technique, which useλTriplEx2 as a vector. The titer and the percentage of the constructed library were about 1.0 × 106pfu/mland 98.3% respectively, and the titer and the capacity of the amplified library were about 3.1 × 108pfu/mland 4.2 × 1010. The library was used to provide expressed sequence tags (ESTs). 147 Expressed SequenceTaqs (ESTs) were gained from 176 clones, which were selected randomly and sequenced at the 5'end. Thesequences were submitted to the EMBL database. Blasting the sequences in the GenBank, 43 of them werefound that they have significant similarity with data in GenBank. EST AJ620046 was has significantsimilarity with the arabinosidase of Bacteroides thetaiotaomicron. Using SMART-RACE a full-length cDNA ofAJ620046 was successfully obtained. In order to initially characterize the biochemical properties ofAJ620046, the ORF of AJ620046 named AF was cloned and expressed in Pichia Pastoris yeast.Recombinant pHIL-S1-AF constructed by inserting AF into pHIL-S1 was transformed into Pichia PastorisGS115. Preliminary experiments indicated that AJ620046 was expressed as a 32 kDa protein in recombinantyeast.
4.Effect of metformin on proliferation and apoptosis in human pancreatic cancer cell line CFPAC-1

Objective To investigate the effect of metformin on the proliferation and apoptosis in human pancreatic cancer cell line CFPAC-1,and to explore the potential mechanism.Methods Human pancreatic cancer CFPAC-1 cells were cultured in vitro,and were treated with metformin at different concentrations (1,2.5,5,10,20,40,60 mmol/L) for different durations (24 h,48 h and 72 h),and cells without treatment were considered as control group.Cell proliferation was evaluated by CCK-8,cell cycle was determined by flow cytometry,apoptosis was determined by Annexin V/PI double staining method,and Western blot was used to detect the protein expression of p-AMPK,FAS,cyclin D1,Bcl-xl,Bax,caspase-3 and survivin.Results Metformin could inhibit the proliferation of CFPAC-1 cells in a time and dose dependent manner.Forty-eight hours after 40 mmol/L metformin treatment,the proportion of CFPAC-1 cells in phase G0/G1 was significantly increased [(65.93 ± 0.27)% vs (42.89± 1.02)%],and the proportion of CFPAC-1 cells in phase G2/M,S was significantly decreased [(22.01 ± 2.95) % vs (38.28 ± 4.93) %,(13.58±0.43)% vs (20.12 ± 3.38)%],and the difference between the two groups was statistically significant (P <0.05).The apoptosis rate was increased from (3.01 ± 0.49) % to (32.97 ± 3.19) %,(P < 0.05) ; and the expression of p-AMPK,Bax,and caspase-3 was increased,while the expression of FAS,cyclin D1,Bcl-xl,survivin were decreased.Conclusions Metformin can inhibit proliferation and promote apoptosis of CFPAC-1 cells mainly by activation of AMPK pathway,and down-regulation of FAS,cyclin D1,survivin and Bcl xl/Bax ratio,as well as up-regulation of caspase-3.
5.Expression of AEG-1 gene in non-small cell lung cancer and its significance

Objective To investigate the expression of astrocyte elevated gene 1(AEG-1) in non-small cell lung cancer(NSCLC) and its clinicalpathological significance .Methods The expression of AEG-1 in NSCLC tissues and adjacent normal lung tissues was detected by RT-PCR and Western blot .The expression of AEG-1 in 87 NSCLC samples and 54 non-cancerous lung tissues was ex-amined with immunohistochemistry .Results RT-PCR and Western blot indicated that AEG-1 was more expressed in NSCLC tis-sues compared with adjacent normal lung tissues .The positive rate of AEG-1 in NSCLC tissues was 52 .9% ,which was significantly higher than that in non-cancerous lung tissues .The difference between the two groups was significant (P=0 .007) .Relevance was analyzed between the expression of AEG-1 and clinicalpathological characteristic .The expression of AEG-1 was positively correlated with T stage and N stage(P<0 .05) .Conclusion AEG-1 was specifically up-regulated in NSCLC tissues compared with non-can-cerous lung tissues ,suggesting that AEG-1 may play an important role in tumor development and progression ,and could be identi-fied as a biomarker for diagnosis of NSCLC .
6.The effects of enteral nutrition versus total parenteral nutrition on gut barrier function in severe acute pancreatitis

Objective To evaluate the effect of enteral nutrition (EN) versus total parenteral nutrition(TPN) on gut barrier function in patients with severe acute pancreatitis (SAP). Methods Sixtythree patients with SAP enrolled from 4 hospitals were randomly assigned into EN group(29 cases) and TPN group(34 cases). EN group patients were fed via a spiral nasojejunal feeding tube placed routinely by endoscopy or fluoroscopy, and TPN group patients were nourished intravenously with TPN during the same period. The changes of serum endotoxin, diamine oxidase, and urinary excretion of lactulose and mannitol ratio (L/M) were observed. Results Plasma concentration of endotoxin were markedly decreased in EN group as compared with that in TPN group at the 7th,14th ,21th day of entry trial [(39. 30 ± 15. 82) EU/L vs (73.05 ±21.16) EU/L,(22.64 ±14.31) EU/L vs (49.34 ±24.54) EU/L,(14.81 ± 10.93)EU/L vs ( 30. 08 ± 14. 10 ) EU/L, P < 0. 05]. Plasma concentration of diamine oxidase were markedly decreased in EN group as compared with that in TPN group at the 7th, 14th day of entry trial [(9. 97 ± 3. 84)U/Lvs (19.89±9.89)U/L,(5.42±1. 84) U/Lvs (8.79 ±4.08) U/L, both P < 0. 05]. The urinary L/M decreased significantly in EN group than those in TPN group at the 7th, 14th,21th day of entry trial (0.28 ±0.25 vs 0. 65 ±0.45,0.21 ±0. 18 vs 0.54 ±0.41,0.08 ±0.04 vs 0.29 ±0.06, all P<0.05).Conclusion EN has better effect on improving intestinal barrier function than TPN in treatment of patients with SAP.
7.Analysis of Electroencephalogram Sample Entropy Measurement in Frontal Association Cortex Based on Heroin-induced Conditioned Place Preference in Rats.

To explore the relationship between the drug-seeking behavior, motivation of conditioned place preference (CPP) rats and the frontal association cortex (FrA) electroencephalogram (EEG) sample entropy, we in this paper present our studies on the FrA EEG sample entropy of control group rats and CPP group rats, respectively. We invested different behavior in four situations of the rat activities, i. e. rats were staying in black chamber of videoed boxes, those staying in white chamber of videoed boxes, those shuttling between black-white chambers and those shuttling between white-black chambers. The experimental results showed that, compared with the control group rats, the FrA EEG sample entropy of CPP rats staying in black chamber of video box and shuttling between white-black chambers had no significant difference. However, sample entropy is significantly smaller (P < 0.01) when heroin-induced group rats stayed in white chamber of video box and shuttled between black-white chambers. Consequently, the drug-seeking behavior and motivation of CPP rats correlated closely with the EEG sample entropy changes.

8.Comparison of EEG complexity between rats under awaking and anesthesia

Objective To compare the EEG complexity between rats under awaking and differ-ent depth of anesthesia via analyzing sample entropy and fractal dimension.Methods Sixteen SD rats were intraperitoneally injected with urethane twice,first with 500 mg/kg and second with 800 mg/kg one hour later.The scalp EEG was collected in stage of awaking (W),light anesthesia (LA)and heavy anesthesia (HA).The sample entropy (SampEn)and fractal dimension (FD)were computed by MATLAB.The characteristic values were denoised by linear dynamic system method during the whole process.Results The value of SampEn and FD gradually dropped from awaking to heavy anes-thesia.The SampEn and FD in W was significantly higher than the value in LA or in HA (P <0.05). The SampEn and FD in HA was significantly lower than that in LA (P < 0.05 ).Conclusion The SampEn and FD of EEG could be used to monitor the depth of anesthesia.
9.Efficiency analysis of transcatheter arterial chemoembolization combined with CT-guided percutaneous acetic acid injection for huge hepatocellular carcinoma

Objective To explore the clinical application value of transcatheter arterial chemoembolization (TACE) combined with CT-guided percutaneous acetic acid injection (PAI) in treatment of huge hepatocellular carcinoma (HCC). Methods Forty-three patients with huge HCC were randomly divided into two group. Twenty-one patients in group A underwent routine one course for TACE (three times), and the interval of TACEs was one month. Twenty-two patients in group B underwent TACE combined with PAI, and CT-guided PAI was performed once a week since 2-3 weeks after first TACE, and one course included 6-9 times of PAI. Postoperative follow-up was conducted (including AFP, the size of tumor, etc.). One course of treatment was repeated in case of tumor recurred. Results At the 1st month after treatment, no statistical difference was found of AFP positive rate between two groups. Statistical difference of total effective rate was found between two groups (38.10% vs 77.27%). The 1-, 2- and 3-year survival rate In group A was 47.62%, 23.81% and 9.52%, respectively, while in group B was 81.82%, 54.55% and 36.36%, respectively, and significant differences were found between the two groups for the same period. Conclusion TACE combined with PAI is safe and more effective than TACE alone in the treatment of huge HCC.
10.A Contrastive Analysis on the Value of Tympanograms and of Temporal Bone CT Scans in Diagnosis of Secretory Otitis Media

Objective To investigate and contrastive analyse the respective value of tympanograms and high resolution CT scan of temporal bone in the diagnosis of secretory otitis media .Methods The clinical data of 150 hos-pitalized patients with secretory otitis media in Department of ENTHNS ,Tongji Hospital ,Wuhan from 2009 to 2012 were retrospectively analyzed .Based on the surgical finding of tympanic cavity fluid ,the diagnostic accordance rate of tympanograms and temporal bone CT for middle ear effusion were calculated respectively ,and their diagnos-tic value were evaluated .Results The diagnostic accordance rate of tympanogram B for the middle ear effusion in secretory otitis media was 94 .5% (138/146 ears) ,the rate of abnormal tympanograms was 91 .6% (206/225 ears) , while temporal bone CT with a rate of 99 .2% (117/118 ears) .The difference between the latter two data was statis-tically significant(P<0 .01) .Conclusion The temporal bone CT scan ,with a higher diagnostic value for middle ear effusion in secretory otitis media ,could be used as a supplement to the acoustic immittance measurement ,especially to non-B tympanograms for the diagnosis of secretory otitis midia .
