1.Study on relationship between IL-6, IL-8 and Helicobacter pylori in patients with gastric cancer

Objective To investigate relationship between IL-6, IL-8 and Helicobacter pylori in patients with gastric cancer.Methods 264 cases patients with gastric cancer and gastritis were selected, and divided into control group, precancerous lesion group and gastric cancer group according to the tissue biopsy, then each group were divided into positive group and negative group according to 12 C test.IL-6 and IL-8 in serum and gastric juice were detected by ELISA, while in gastric mucosa by double antibody sandwich method.Results Compared with control group, IL-6 and IL-8 in serum, gastric juiceand gastric mucosa were higher ( P<0.05 ) .Compared with precancerous lesion group, IL-6 and IL-8 in serum, gastric juice and gastric mucosa were higher(P<0.05).Compared with Hp negative group, IL-6 and IL-8 in serum, gastric juice and gastric mucosa were higher(P<0.05). Compared with gastric cancer group, Hp eradication rate of precancerous lesion group and control group was higher, respectively ( P<0.05 ) . Conclusion IL-6 and IL-8 in gastric cancer patients’ serum, gastric juice and gastric mucosa increased, which is higher in Hp positive group.The levels of IL-6, IL-8 maybe the key mechanism of the formation of gastric cancer, which has important sense of assessment and diagnosis of gastric cancer.
2.Clinical analysis of 11 patients with relapsing polychondritis

Objective: To improve the understanding of the clinical feature and early diagnosis of relapsing polychondritis(RP). Method: Eleven patients with RP were analyzed retrospectively and the reported literatures were reviewed. Result: Nine cases had initial involvement of auricular cartilage while two polyarthritis. The treatment of 2RP patients with respiralory tract involvement was invalid and 1 patient died. Nine cases were initially misdiagnosed, with a misdiagnosis rate of 81.82%. Conclusion: RP involves cartilage and connective tissue. The prognosis with respiralory tract involvement is poor. RP is a sort of paroxysmal and progressive inflammation involving cartilages all over the body with a variety of clinical manifestations. Early diagnosis of RP is based on full understanding of its clinical features.
3.Endoscopic sphincterotomy vs open surgery in the treatment of common bile duct stones

Objective To compare the value of ERCP plus EST and laparotomy in the treatment of common bile duct stones.Methods 56 cases of common bile duct stones were treated with ERCP and EST in our hospital from June 2012 to June 2013 (endoscopy group),78 cases were treated with laparotomy and common bile duct exploration (laparotomy group).The two groups were compared on success rate of stone removal,operation time,intraoperative bleeding volume,incidence of infection and pancreatitis and hyperamylasemia,common bile duct stone recurrence rate,hospital stays,hospital costs.Results Operation time,intraoperative bleeding volume and infection rate in endoscopic group was lower than the laparotomy group,the incidence of hyperamylasemia,hospital costs in the endoscopic group was higher than laparotomy group.The success rate of stone removal and the recurrence rate of common bile duct stone in the endoscopic group was lower than the laparotomy group,incidence of pancreatitis in the endoscopic group was higher than the laparotomy group,though the differences were not statistically significant.Conclusions ERCP and EST is a time saving procedure fast relieving the bile duct obstruction,less traumative,spelling lower infection rate,less hospital stays,though causing higher incidence of hyperamylasemia and hospital costs.
4.The Influence of Furosemide on the Currents of the Delayed Rectification Potassium Channels and Sodium Channels of Mice's Spiral Ganglion Neurons

Objective To study the influence of furosemide on the currents of the delayed rectification potassium channel and sodium channel of mice's spiral ganglion neurons.Methods Postnatal mice(P1~P6) spiral ganglion neurons were obtained by mechanical dissociation and enzymolysis.Delayed rectification potassium channels' currents and sodium channels' currents were recorded with whole-cell patch clamp techniques.Observed was the influence of furosemide on potassium channels and sodium channels.Results When furosemide was added around the spiral ganglion neurons,the delayed rectification potassium currents were inhibited to around +200~+300 pA,and sodium currents were inhibited to about 30% of peak current.Furosemide was washed away after working steadily for 1 minute,5 and 10 minutes.The delayed rectification potassium currents could recover to 98%,64%,and 25% of the peak currents before and sodium currents could recover to 96%,76% and 54% of the peak currents accordingly.Conclusion The currents of delayed rectification potassium channels and sodium channels could be inhibited by furosemide to different degrees.The longer furosemide was used,the greater damage could occur in the ion channels.
5.Analysis of risk factors related to the pathogenesis of urolithiasis of migrant workers in Zhongshan City

Objective To explore the risk factors of urolithiasis incidence of migrant workers in Zhongshan,in order to provide the scientific evidence for preventing urolithiasis.Methods 1 630 workshop migrant workers and 1630 office white collar workers were selected as the subjects of this study.The questionnaire was conducted according to the age,gender,genetic factors,occupation,past illness factors,living environment,work environment factors,diet habit,the habit of drinking water survey,B ultrasound examination,the detection of blood uric acid.Results The incidence rate of urolithiasis was 6.53%,the incidence rate of workshop migrant workers was 8.28%,which was significantly higher than that of white collar 4.72% (x2 =16.972,P <0.01).Conclusion Work environment,eating habits,drinking habits,smoking and sweating are related risk factors of urolithiasis.
6.Gradual and persistent balloon dilatation for traumatic biliary strictures by percutaneous transhepatic cholangiography

The clinical data of 7 patients who underwent the treatment of gradual and persistent balloon dilatation (GPBD) by percutaneous transhepatic cholangiography (PTC)for traumatic biliary stricture in Zhongshan Hospital Affiliated to Dalian University were analyzed retrospectively.Balloon catheters were successfully implanted in 5 cases by PTC,and with the help of ERCP in 2 PTC failed cases.There was no bleeding,acute pancreatitis and other complications.Two balloon catheters were damaged and displaced,respectively.All the biliary strictures were relieved.No biliary sludge was attached on the surface of the balloon and in the bile duct.Bile duct mucosa had congestion edema and cellulose attachment.There was no biliary stricture recurrence in the follow-up of 5 to 27 months.This study showed GPBD by PTC was a simple,safe and effective method for treating traumatic biliary strictures.
7.The ocular optic fiber used in the endoscopic sinus surgery of dacryocystorhinostomy in the treatment of chronic dacryocystitis and recurrent dacryocystitis.

OBJECTIVE:
To explore a method for locating the area of lacrimal sac in dacryocystorhinostomy under endoscopy.
METHOD:
Sixty-eight patients were performed dacryocystorhinostomy under endoscopy. Take light spot of ocular optic fiber as the lacrimal sac projection to the lateral wall of the nasal cavity position.
RESULT:
With the guiding of ocular optic fiber, lacrimal sac can be located accurately. The operating time of dacryocystorhinostomy under endoscopy was shortened significantly, and the operation procedure was simplified. All patients were followed up for 2 years, only 2 recurrent cases were found. The success rate reach to 97.06% (66/68).
CONCLUSION
Ocular optical fiber used in locating the lacrimal sac in dacryocystorhinostomy under endoscopy is simple and feasible, and can be widely used.

8.The clinical analysis of endoscopic pancreaticobiliary separation in the treatment of occult pancreaticobiliary reflux

Objective To evaluate endoscopic pancreaticobiliary separation (EPBS) in patients with gallstone and occult pancreaticobiliary reflux (OPBR).Methods The clinical data of 47 cases with gallstone and OPBR from Oct 2013 to Oct 2016 was analyzed retrospectively.Results The mean gallbladder bile amylase (GBA) was (864 ± 575) U/L.40 cases have undergone the treatment of endoscopic retrograde cholangiopancreatography.26 cases were diagnosed as duodenal papillitis,16 cases as periampullary diverticula,14 cases as long nipple,5 cases as atrophic papilla,3 cases as ampulla stone and 2 cases as papillary tumor.9 were diagnosed as pancreaticobiliary maljunction in the 14 patients with long nipple.33 cases were performed with EPBS.GBA was tested in 16 cases,and the GBA of 15 cases returned to normal level.The difference of GBA was statistically significant [(1 161 ±764) U/L vs.(47 ± 17) U/L,(t =5.641,P < 0.05)].Patients were followed up for 1 to 4 years,among 36 cases without cholecystectomy,there was no recurrence of cholecystolithiasis after EPBS in 27 cases,but 2 cases had recurrent gallstones in 9 cases without EPBS,and the difference of gallstone recurrence rate was statistically significant (x2 =21.340,P < 0.05).Conclusions Pancreaticobiliary junction diseases is an important cause for gallstone formation and OPBR.EPBS can avoid pancreaticobiliary reflux and reduce the recurrence rate of gallstone after choledochoscopic lithotomy.

10.A clinical study of dacryocystorhinostomy per nasal endoscope guided by endo-ophthalmic optical fiber.

OBJECTIVE:
To get a method of dacryocyst allocation for dacryocystorhinostomy per nasal endoscope.
METHOD:
The dacryocyst of 7 chronic dacryocystitis cases(8 eyes) were located through nasal cavity guided by endo-ophthalmic optical fiber.
RESULT:
All the 7 patients got successful operations, the bleeding was 10-20 ml during the operation , postoperative recovery was good, no infection occurred. Five patients received operations twice, all of them were detained anaesthetic tubes for 3 to 6 months, dacryocyst rinsing were conducted once per month after extubation. No obstructions and complications occurred after one year follow up.
CONCLUSION
Endo ophthalmic optical fiber can be used to locate dacryocyst in dacryocystorhinostomy per nasal endoscope.
