1.Current situation of mental health service in domestic general hospitals and measures for improvement

By analyzing the scope and demands of mental health service in general hospitals, the authors point out that at present domestic general hospitals are confronted with such problems as insufficiency of resources for mental health service, low recognition rate of dysphrenia, and lack of systematic consultations and liaison in psychiatric work. Then they put forward a number of measures for improving mental health service in general hospitals: ①establishing a department of psychiatry in each general hospital; ②widely conducting consultations so as to maintain a strong liaisom in psychiatric work; ③genuinely achieving the transformation of the medical mode; ④strengthening education in psychological medicine and the spread of popular science so as to broaden the channel and coverage of mental health service and meet the needs of reform and development of the health cause in the contemporary era.
2.Clinical study of postoperative individualized chemotherapy based on genetic testing results for non-small cell lung cancer

Objective To explore the efficiency of postoperative individualized chemotherapy based on genetic testing results for non-small cell lung cancer (NSCLC). Methods Fifty-six NSCLC patients at stageⅡorⅢA who accepted video-assisted thoracic operation were divided into two groups:the individualized chemotherapy group (n=26) and non individual?ized chemotherapy group (n=30). The fresh lung tumor tissue of individualized chemotherapy group was tested target gene,in?cluding excision repair cross complementing 1 (ERCC1),ribonucleotide reductase subunit M1 (RRM1),β-tubulinⅢ,thymi?dylate synthase(TS),epidermal growth factor receptor (EGFR) and breast cancer gene 1 (BRCA1). The theraputic plan was based on genetic testing results in individualized chemotherapy group, and the non individualized chemotherapy group re?ceiving gemcitabine plus cisplatin. The 1-year disease free survival (DFS), 2-year disease free survival (DFS), the progres?sion-free survival (PFS) and the overall survival (OS) were compared between two groups. Results The 2-year DFS (57.69%), PFS (22.1 ± 5.0 months) and OS (24.1 ± 3.2 months) were significantly higher in the individualized chemotherapy group than those of non individualized chemotherapy group (respectively 30.00%, 18.9 ± 6.2 months, 21.9 ± 4.3 months, P<0.05). There was no significant difference in 1-year DFS between two groups (88.46%vs 83.33%, P<0.05). Conclusion The individualized chemotherapy based on genetic testing results can enhance the 2-year DFS, PFS, OS and the efficiency of postoperative adjuvant chemotherapy for NSCLC.
3.Clinical efficacy and safety study on full-term pregnancy termination with misoprostol

Objective To investigate clinical efficacy and safety of misoprostol for termination with full-term pregnancy.Methods The number who had terminations with full-term pregnancy in our hospital obstetrics from July 2013 to March 2014 were 78, divided them into the experimental and control groups.Experimental group used misoprostol for induction of labor, the control group used oxytocin for induction of labor.Indicators were observed and compared between two groups of pregnant women, induction of labor effect indicators: the success rate of induction of labor, postpartum hemorrhage in 2h, the time of labor and the time of fetal delivery, cesarean section rate; induction of labor safety indicators: maternal complications and side effects, the physiological condition of the newborns and Apgar score.Results Basic information was no significant different between the two groups of pregnant women.induction of labor of experimental group success rate was 97.44%, which was 82.05% in the control group, success rate in experimental group was higher than the control group, with statistically significant results (P<0.05), postpartum hemorrhage in 2h, labor time, fetal delivery time, cesarean section in experimental groups were less than which in the control group, the difference was statistically significant (P<0.05).Experimental group of pregnant women postpartum complications and side effects were mainly in the gastrointestinal adverse reactions, uterine contractions too strong but compared with the control group showed no significant difference.Two newborn babies 1 minute (9.4 ±1.3 vs.9.2 ±1.5) and 5 minute (9.9 ±1.2 vs.9.5 ±1.3) Apgar score were not significantly different (P>0.05), the experimental group had one case of mild neonatal asphyxia, there are two cases in the control group and one case of mild asphyxia neonatal asphyxia.Conclusion Effect of misoprostol which is used to terminate the pregnancy term is significant, and has high security, has high clinical value.
4.Development of a Rapid Screening System of Environmental Estrogen Based on Recombinant Yeast Cell

Objective To develop a rapid screening method of environmental estrogen in vitro. Methods Estrogen responsive element (ERE) was inserted into the upstream of Lac Z (?-galactosidase ) reporter gene and the reporter gene was regulated under estrogen receptor(ER) in the recombinant yeast cells by the molecular biological technique. When the yeast cells was affected by environmental estrogen, conformational change of ER lead to expression through binding to ERE. There exist the functional relationships between the expression and the pollution degree of environmental estrogen. Therefore, the pollution degree can be reflected by assaying the activity of ?-galactosidase. Results It was identified by PCR that recombinant yeast cell, whose Lac Z reporter gene regulated under estrogen receptor, was successfully reconstructed. The experimental results showed that the time-dose-effect response had no significant changes when the recombinant yeast cell bad been exposed to ?-estradiol for 4 and 8 hours respectively; among the three methods of yeast disintegration, the ultrasonic vibration method had more advantage compared with that of the other methods and this dose-effect curve exhibited "S" shape; among 3 test chemical products, ?-estradiol was the most estrogenic activity, biosphenol A take second place, estradiol benzoate was the lowest. Conclusion The assay system of yeast cell for environmental estrogen hormone mediated by estrogen receptor is stable and applicable.

Aim To investigate the impact of UVA on expression of tumour necrosis factor related apoptosis inducing ligand (TRAIL) and study the role of TRAIL in UVA-induced apoptosis of HaCaT cells as well as the influence of Polypeptide from Chlamys farreri(PCF)on TRAIL apoptotic pathway induced by UVA.Methods Cells were divided into five groups: control group, UVA model group,UVA+5.69 mmol?L-1 PCF group, UVA+2.84 mmol?L-1 PCF group, UVA+1.42 mmol?L-1 PCF group.Expression level of TRAIL mRNA was assayed by Real-Time PCR.Western blot analysis was used to determine the protein level of TRAIL and caspase-8 activation. The effect of TRAIL neutralization antibody on UVA-induced apoptosis was also investigated.Results TRAIL mRNA and protein levels increased after 8 J?cm-2 UVA radiation and the discrepancy was significant compared with control group(P

Aim To investigate whether the polypeptide from Chlamys farreri(PCF)protected HaCaT cells from UVB-induced apoptosis through Fas-caspase-3 and ROS-cytochrome C.Methods Experiment designs were divided into five groups:control group,UVB model group,UVB+5.69 mmol?L-1PCF group,UVB+2.84 mmol?L-1 PCF group,UVB+1.42 mmol?L-1 PCF group.SiRNA for Fas inhibited Fas expression of UVB-induced HaCaT cells.Using agarose gel electrophoresis,the effects of Fas siRNA and ROS scavenger NAC on UVB-induced apoptosis of HaCaT cells were investigated.Expression levels of cytochrome C andcaspase-3 after inhibitory Fas were determined by Western blot analysis.Intracellular ROS was detected by means of an oxidation-sensitive fluorescent probe(DCFH-DA).Results SiRNA for Fas had inhibitory effects on UVB-induced apoptosis of HaCaT cells and caspase-3 expression.NAC had inhibitory effects on UVB-induced apoptosis of HaCaT cells.PCF inhibited UVB-induced generation of ROS and cytochrome C release dose-dependently.Conclusions PCF protected HaCaT cells from UVB-induced apoptosis.Its inhibitory effect on apoptosis could be attributed to inhibition of Fas-caspase-3 and ROS-cytochrome C pathways.
7.Monitoring of Electrophile Chemical Pollutants Using Transgenic Firefly Luciferase Gene Cells

Objective To assess quickly the potential environmental pollution by using the luciferase transgenic cells. Methods Many electrophile compounds in the environment can generate oxidative stress,so that the transcription of certain protective genes is induced via specific DNA motifs called electrophile response elements (EPREs). We have made a vector containing a single EPRE fused to the TK minimal promoter and the gene encoding firefly luciferase (EPRE-LUC) by adopting bio-molecular techniques. From this vector the stable LUC expression vector regulated by EPRE had been successfully reconstructed. This reporter construct was transfected into HeLa cells,and the clones resistant to G418 were selected. The resistant cells were treated by the different concentrations of sodium arsenate(NaAsO2),cadmium chloride (CdCl2),mercury chloride(HgCl2) and diethyl mateate (DEM). After that,the expression of luciferase was determined by luciferase assay kit. Results The correct construct frame of LUC reporter vector regulated by EPRE was identified by DNA sequencing; the dose-dependent relationships between the LUC expression and the test substance concentration were found. Among them,the relationship produced by DEM was the most significant. Conclusion The LUC transgenic cells regulated by EPRE have been successfully constructed.
8.Exploration of the significance of using Kappa index in clinical teaching of acute appendicitis abdominal signs

Objectives To study the significance of using kappa value in surgical clinical teaching.Methods The interns in Department of General Surgery were included,and were divided into routine teaching group and targeted teaching group randomly. Kappa statistic was used to compare the consistency of physical examination.Results The results showed that Morris's point tenderness was grasped more efficiently in targeted teaching than routine teaching group.Conclusion With the help of kappa value teachers can find the teaching items more correlated with training and give students targeted teaching so as to improve the teaching effect.
9.Ulinastatin protects intestinal mucosal barrier by inhibiting the activation of intestinal NLRP3 inflammasomes in septic rats

Objective:To explore the damage of the intestinal mucosal barrier of septic rats by the activation of NOD-like receptor family, pyrin domain-containing 3 (NLRP3) inflammasomes and the role of Ulinastatin (UTI) on the expression of intestinal nuclear factor-κB (NF-κB)/NLRP3 inflammasome signaling pathway in septic rats.Methods:According to the random number table method, 64 male Wistar rats were divided into sham operation group (Sham group), cecal ligation and puncture (CLP) group, UTI treatment group (100 kU/kg UTI was intraperitoneally injected 1, 6, 12 and 18 hours after CLP), and UTI pretreatment group (100 kU/kg UTI was given 1 hour before CLP), with 16 rats in each group. The survival of rats was observed after 24 hours, and the blood was collected from abdominal aorta at 24 hours after modeling, then rats were killed and their ileum tissues were taken. Hematoxylin-eosin (HE) staining was used to observe histopathological changes and Chiu score. The levels of tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β) and intestinal fatty acid binding protein (I-FABP) in serum were detected by enzyme linked immunosorbent assay (ELISA). The protein expression of NF-κB p65 in intestinal tissue was detected by Western blotting. The expression of intestinal tight junction proteins Claudin-1, Occludin and the inflammasome NLRP3, apoptosis-associated speck-like protein containing CARD (ASC) and caspase-1 were detected by immunohistochemistry.Results:Compared with Sham group, the 24-hour survival rate of CLP group was significantly reduced. Histopathological results showed that the CLP group had severe edema of mucosa and submucosal stroma with obvious infiltration of inflammatory cells and disordered villi arrangement. Some glands were incomplete, and the villus structure was severely damaged. The Chiu score was significantly increased. The levels of TNF-α, IL-1β, I-FABP in serum and the protein expression of NF-κB p65 in intestinal tissue were significantly increased. The positive expressions of NLRP3, caspase-1 and ASC were also significantly increased. However, the positive expression of tight junction protein in small intestine tissue such as Occludin and Claudin-1 were significantly reduced. It suggested that when sepsis occurs, small intestinal mucosal barrier dysfunction happens, and mucosal permeability increases, while tight junction protein expression decreases, NLRP3 inflammasome and its upstream molecule NF-κB p65 were activated. After UTI treatment and UTI pretreatment intervention, although there was no significant difference in 24-hour survival compared with CLP group (62.5%, 68.8% vs. 43.8%, both P > 0.05), the intestinal tissue damage of septic rats was significantly improved. Specifically: Chiu score and the levels of TNF-α, IL-1β, I-FABP in serum were significantly decreased [Chiu score: 3.37±0.25, 3.23±0.16 vs. 4.08±0.13, TNF-α (ng/L): 147.62±20.74, 140.71±24.81 vs. 222.82±16.84, IL-1β (ng/L): 80.64±5.68, 78.11±4.75 vs. 133.73±3.92, I-FABP (μg/L): 38.29±3.60, 35.88±4.52 vs. 59.81±4.66, all P < 0.05]; the protein expression of NF-κB p65 was significantly decreased (NF-κB p65/β-actin: 0.65±0.10, 0.69±0.11 vs. 0.99±0.10, both P < 0.05), the positive expressions of Claudin-1 and Occludin in the small intestine tissue were increased [Claudin-1 positive expression area: (19.43±3.08)%, (23.99±6.27)% vs. (7.77±2.03)%; Occludin positive expression area: (19.58±4.75)%, (23.28±3.68)% vs. (11.69±4.30)%, all P < 0.05], while the positive expressions of NLRP3, caspase-1, ASC were decreased [NLRP3 positive expression area: (7.80±3.14)%, (6.86±2.63)% vs. (14.44±3.68)%; caspase-1 positive expression area: (10.62±3.52)%, (9.49±3.09)% vs. (26.69±8.05)%; ASC positive expression area: (9.95±2.81)%, (10.53±3.61)% vs. (24.16±5.48)%, all P < 0.05]. However, there was no significant difference in the improvement effect between UTI treatment group and UTI pretreatment group.Conclusions:Intestinal barrier dysfunction in sepsis may be related to the activation of NLRP3 inflammasomes in the intestinal mucosa. The protective effect of UTI in the intestinal mucosa may be related to inhibiting the activation of NLRP3 inflammasomes in the intestinal mucosa, but UTI pretreatment has no obvious advantage compared with UTI treatment.
10.Research status of literatures on Alzheimer disease published at home and abroad from 2008 to 2012

Objective To explore the distribution and research status of literatures on Alzheimer disease (AD) published recently,to compare the differences in the research papers on AD between China and foreign countries,and to provide bibliometrics data for Alzheimer disease research.Methods Using bibliometrics methods,the characteristics and contents of Alzheimer disease research documents were analyzed in Pubmed database and Wanfang web by analysis tools.Research status of Alzheimer disease including the type of literatures and journal distribution from 2008 to 2012 was analyzed.Currently hot topics or top high frequency words were emphatically analyzed.Results There were 646 matched literatures for Alzheimer disease retrieved in Chinese core periodicals,and 19 423 literatures in Pubmed database.The articles for clinical study of Alzheimer disease were more than experimental and epidemiologic study among the 646 literatures.The disciplines distribution of AD in Chinese core journals was similar to international journals,mainly including nerves,brain,and geriatrics journals.The domestic research papers on Alzheimer disease were less than 5% in Pubmed database,and the articles from Chinese journals in Chinese or English edition accounted for only a small proportion of Pubmed database.The recent research direction of Alzheimer disease in China was similar to foreign countries such as amyloid proteins,diagnosis and gene,but foreign research paid more attention to the metabolism,image and the aging mechanism in AD,while in China there were quite a lot of reports in the literatures on cholinergic drugs,tau protein,hippocampus and Chinese herbal treatments.Conclusions The AD-related disciplines distribution in China and foreign country is relatively fixed in recent 5 years.The AD researches carried out in China are close to those in foreign country,but researches on some areas still lag behind.The experimental and epidemiologic study on AD should be strengthen in China.
