1.Effects of Microiontophoretically Applied Some Different Transmitter Receptor Antagonists on the Evoked Response of “Pain” Units of Hypothalamic Paraventricular Nucleus

Effects of microiontophoretically applied transmitter receptor antagonists on the evoked response of “pain” units of PVN in rats by sciatic stimulation were observed. The results showed: (l)The evoked response of 7 out of 38 PVN “pain” units could be blocked by atropine (7/38); 5 out of 27 by hexamethonium; 6 out of 31 by phentolamine (6/31); and 4 out of 25 by propranolol (4/25). Seventeen out of 52 were blocked by cyproheptadine (17/52), but 3 out of 52 were augmented by cyproheptadine (3/52). (2) The evoked response of the same “pain” unit could be blocked by two antagonists: the evoked response of 2 out of 27 PVN “pain” units could be blocked by atropine and cyprohepadine; 3 out of 20 by hexamethonium and cyproheptadine; 2 out of 25 by prepamolol and cyproheptadine. These results suggest that the noxious somatic input to PVN involves 5-HT, cholinergic and adrenergic transmitter receptor mechanisms and that the convergence of various transmitter systems on PVN “pain” unit is indicated.
2.Integrated traditional Chinese medicine and Western medicine treatment of gastrointestinal dysmotility of severe acute pancreatitis patients

Objective To investigate the effects of rhubarb and mirabilite and compound danshen injection on gastrointestinal dysmotility of severe acute pancreatitis (SAP). Methods Forty-one SAP patients were randomly divided into two groups: Integration group (n=21) were treated with rhubarb and mirabilite and compound danshen injection. Control group (n=20) were treated with conventional therapy. Recovery of gastrointestinal motility of two groups was observed, and the complication rate and hospital stay was recorded.Results The recovery of gastrointestinal motility, first defecation and hospital stay in integration group was (3.1±0.8)d, (3.1±0.8)d, (21.5±2.8 )d, which were significantly shorter than those in control group [(5.2±1.4) d, (4.7±1.3 ) d, (32.1±3.60) d, P < 0. 05 or P < 0.01. Five patients in integration group developed complication, which was significantly lower than 8 patients in control group ( P < 0.01 )]. One patient died and one patient received operation in integration group, and one patient died in control group,without significant difference between the two groups (P > 0.05 ). Conclusions The combination of rhubarb and mirabilite and compound danshen injection in severe acute pancreatitis can promote the recovery of gastrointestinal motility.
3.Cause and management of difficult laparoscopic appendectomy

Objective To study the cause and management of difficult laparoscopic appendectomy.Method A retrospective analysis of 102 difficult laparoscopic appendectomies was performed. Results The associating factors leading to operative difficulty were as follows:adhesion of peri-appendix in 44 cases,gangrene at root of appendix in 21 cases,special location of appendix in 17 cases,abdominal adhesion in 12 cases,obesity and inflatable intestine in 8 cases. Laparoscopic appendectomy was successfully performed in 96 cases(94.1%,96/102) ,and no intestinal leakage or massive hemorrahge occurred. Conclusions Adhesion of peri-appendix is the most common cause in difficult laparoscopic appendectomy,and gangrene at root of appendix is the most difficult one. Reasonable management for appendix stump contributes to successful operation.

Without the constriction of majors, students participate in group with 3 people, design their own experiments around the hot spots and practical problems in public health area and are awarded according to the grades of the experiment design and thesis defence. The most topics in the first and the second competitions are about environmental hygiene and occupational hygiene, and the infectious diseases are attracting increasing attention. More and more topics combine the method of experiment with investigation. Some tactics are also suggested that innovation, view of general population, autonomy and investigation should be included in topic selection for this contest.

6.The effect of palliative resection on incurable colorectal cancer survival

Objective To quantify the benefit of primary Pdmor removal in patients with differently presenting incurable coloreetal cancer,while no other therapy combined.Methods One hundred and forty-three consecutive patients were operated for incurable colorectal cancer(91 undergoing resective and 52 non-resective procedures),with the purpose of comparing homogenous populations and of identifying whether the patients got benefit from primary tumor resection.Results In patients with resectable primary tumors,resective procedures were associated with longer median survival than non-resective procedures(10 months vs 3 months),patients with distant spread without neoplastic ascites/implanting metastasis got benefit from primary tumor removal(P<0.01).The complication of resective procedures was not significantly differ-ent from that of non-resective procedares(P>0.05).Conclusion Palliative resection of primary colorectal cancer should be pursued in patients with unresectable distant metastasis whenever the primary tumor is technically resectable.
7.The effect of p53mut,P53wt and p16 on the expression of Smad4 in lung cancer H1299 cells

Objective To investigate the influence of exogenous p53mut, p53wt and p16 on the expression of Smad4 in lung cancer H1299 cells. Methods Target genes (p53mut, p53wtand p16) were amplified by PCR and inserted into effective eukaryotic expression vector pIRES2-EGFP, respectively. These recombinant plasmids were transfected into H1299 cells by lipofectamine. The fluorescence microscope was employed to observe the transfected cells and the expression of EGFP. RT-PCR was used to validate the transfection efficiency. Western blot assay was used to detect the change of the Smad4 expression in H1299, Results Green fluorescence was observed under fluorescence microscope in the transfected H1299 cells at 72 hour post transfection. RT-PCR indicated that p53mut, p53wt and p16 genes were highly expressed in H1299 cell. There was no significant difference in Samd4 expression between the empty plasmid group and control group(P>0.05). But the expression of Samd4 in p53mut transfected group was decreased(P<0.05). On the contrary, the expression of Smad4 was increased in the p53wt transfected group and P53wt and p16 co-transfected group. Moreover, the increase was more obvious in the P53wt and p16 cotransfected group(P< 0.05). Conclusion P53mut gene transfection reduces the expression of Smad4 and P53wt. The co-infection of p53mut and p16 increases the expression of Smad4 in the H1299 cells. The tumor promoting effect of p53mut and the antitumor effect of p53^ and p16 may be related to the signal pathway of TGF-β/Smad4.
8. Comparison of the effect of endoscopic multi-band mucosectomy and endoscopic mucosal resection for colorectal submucosal tumors

Objective:
To compare the therapeutic effect and safety of endoscopic multi-band mucosetomy (EMBM) and endoscopic mucosal resection (ESMR) in the treatment of colorectal submucosal tumors.
Methods:
From January 2012 to June 2014, the clinical data of 40 patients with colorectal submucosal tumors in Tengzhou Central People′s Hospital were divided into EMBM group (22 cases) and ESMR group(18 cases) according to different treatment method.The complete resection rate, operation time and complications were compared between the two groups.
Results:
The complete resection rate of the EMBM group was 95.45%, which of the ESMR group was 94.44%, the difference between the two groups was not statistically significant (χ2=0.273,

A cornual pregnancy occurs when the conceptus implants at the cornus of the uterus which is the junction between the fallopian tube and uterine cavity. Cornual pregnancy is a rare form of ectopic pregnancy and its diagnosis is difficult. It accounts for 2 to 4% of tubal pregnancy and rupture usually occurs between 8 weeks and 16 weeks of gestation. The rich vascularity in this location makes the rupture particularly dangerous, resulting in higher maternal mortality. We have experienced a case of unruptured cornual pregnancy at 9 weeks of atypical ultrasonographic findings, which was hourglass appearance. We present this case with a brief review of the literature.

10.Acute renal failure associated with ureteral stone of the unilateral kidney and uterus didelphys with hemivaginal obstruction.

The unique clinical syndrome of the uterus didelphys, a unilateral partially or completely obstructed vagina, and ipsilateral renal agenesis is quite rare, and observed on the right side in greater frequency than on the left. The case of fifteen year old girl with uterus didelphys, unilateral partially obstructed vagina and acute renal failure associated with ureteral stone in unilateral kidney is described. Extracorporealshock wave lithotripsy (ESWL) and evacuation of the hematocolpos and excision of the vaginal septum were done. Aware of this relatively rare condition is mainstay of prompt diagnosis which will prevent permanent renal failure and unnecessary and destructive surgery.
