1.Influence of NiTi metal-stent on radiation dosimetry in esophageal carcinoma

Objective To study the dose distribution of intra-cavitary NiTi metal-stent for correction in radiotherapy of esophageal carcinoma.Methods Thermoluminescence Dosimeter(TLD) with LiF dose units was used to simulate the dose variation in wax phantom with or without Chinese and Japanese metal stents in esophageal lumen on 60Co, 4 MV X-ray and 6 MV X-ray machines. The dose distribution on the stent fringe (i.e.,on the esophageal mucosa) was compared with that at the center of esophagus without metal stent.Results When a single anterior field was used to irradiate the esophageal carcinoma, the dose enhancement on the anterior and posterior point of the Chinese metal stent was 16.2% and 7.8% by 60Co,15.4% and 6.8% by 4 MV X-ray and 12.8% and 5.8% by 6 MV X-ray. With the same set-up, the dose enhancement of Japanese stent was 13.0% by 6 MV X-ray. When anterioposterior and posterioanterior (AP/PA) opposing fields were used ,the dose enhancement ratio was 11.7%~24.0%. When three fields from three different directions were used ,the dose were increased by 3.2~16.2%.Conclusions When irradiating esophageal carcinoma with intra-cavitary metal stent,we suggest the fraction dose be below 1.7 Gy when one field is used and not exceed 1.5 Gy when AP/PA fields are used. It is advisable to use the three field technique with the diameter of the stent limited to 1.5 cm.
2.Comparison of the short-term and long-term curative effect between two implantation methods for treating refractory glaucoma

AIM:To compare the short-term and long-term curative effect between two implantation methods for treating refractory glaucoma.METHODS:A total of 150 patients with refractory glaucoma (150 eyes) were divided into Group A (treated by implantation with Ex-press glaucoma drainage device) and Group B (treated by implantation with Ahmed glaucoma valve) according to different implantation methods.Changes of intraocular pressure and anti-glaucoma drugs at different time points, the success rate of surgery, improvement of visual acuity and complications were compared between the two groups.RESULTS:The intraocular pressure of two groups at 1d, 1wk, 3, 6 and 12mo after surgery was significantly lower than that before surgery (P<0.05).The intraocular pressure of Group A at 3, 6 and 12mo after surgery was significantly lower than that of Group B, while anti-glaucoma drugs at 6 and 12mo after surgery were significantly fewer than those in Group B at the same time point (P<0.05).There were no significant differences between the two groups in 12-month success rate of operation, improvement rate of visual acuity and incidence of postoperative complications (P>0.05).CONCLUSION:The effects of implantation with Ex-press glaucoma drainage device are similar to those of implantation with Ahmed glaucoma valve in the treatment of refractory glaucoma.However, the intraocular pressure control effect of the former is better than the latter, and used anti-glaucoma drugs are fewer.
3.Comparison of the Effects of Qili Qiangxin Capsules and Compound Danshen Drop Pills on Related Index-es of AMI Patients without Reflow after PCI

OBJECTIVE:To compare the effects of Qili qiangxin capsules and Compound danshen drop pills on related indexes of acute myocardial infarction(AMI)patients without reflow after percutaneous coronary intervention(PCI). METHODS:A total of 90 AMI patients without reflow after PCI were randomly divided into Compound danshen drop pills group (45 cases) and Qili qiangxin capsules group(45 case). Both groups were given Aspirin chewable tablets 300 mg and Clopidogrel sulfate tablets 300 mg before PCI. After surgery,both groups were given Aspirin chewable tablets 100 mg,once a day+simvastatin chewable tablets 10 mg,once a day. Based on it,Compound danshen drop pills group was given Compound danshen drop pills,10 pills,3 times a day. Qili qiangxin capsules group was given Qili qiangxin capsules,4 capsules,3 times a day. Both groups were treated for consec-utive 6 months. The blood flow classification of TIMI,LEVF,β1-AR,β2-AR,α1-AR,bleeding and the occurrence of major cardio-vascular events were observed in 2 groups before and after treatment. The occurrence of ADR was recorded. RESULTS:Bafore treatment,there was no statistical significance in the blood flow classification of TIMI,the levels of β1-AR,β2-AR and α1-AR be-tween 2 groups(P>0.05). After treatment,the case number of 0-2 degree blood flow classification of TIMI in 2 groups were signif-icantly lower than before treatment while the case number of 3 degree were significantly higer than before treatment;the levels ofβ1-AR,β2-AR and α1-AR were significantly lower than before treatment,while LEVF was significantly higher than before treatment;LEVF of Qili qiangxin capsules group was significantly higher than that of Compound danshen drop pills group,with statistical signifi-cance (P<0.05). There was statistical significance in the incidence of bleeding and major cardiovascular events between 2 groups (P>0.05). No obvious ADR was found in 2 groups during treatment. CONCLUSIONS:Qili qiangxin capsules is similar to Com-pound danshen drop pills in improving no-reflow efficacy and safety of AMI patients after PCI,but it is better than Compound dan-shen drop pill in improving cardiac function.
4.Clinical analysis of post-stroke epilepsy in children

Objective To analyze the clinical features,treatment and prognosis of children with post-stroke epilepsy.Methods Sixty-nine children with epilepsy after cerebral stroke who were admitted to Guangdong General Hospital from Jan.2009 to Mar.2013 were retrospectively reviewed,including etiology,clinical features,laboratory examinations,treatment and prognosis.Results Among 260 stroke patients,69 of them (26.5 %) developed epilepsy,but there was no significant difference between boys and girls(x2 =0.725,P =0.394).The incidence of post-stroke epilepsy differed significantly among different types (x2 =12.000,P =0.02),highest in the children with subarachnoid hemorrhage(47.8%,11/23 cases),followed by those with cerebral hemorrhage (34.6%,27/78 cases),and lastly,those with cerebral ischemia (19.5%,31/159 cases).Among them,53 patients developed seizures in the early stage (76.8%,53/69 cases),others in the late stage(23.2%,16/69 cases),early-onset epilepsy was more common in hemorrhagic stroke and late-onset epilepsy was more common in ischemic stroke,which differed significantly(x2 = 4.778,P =0.029).The electroencephalogram background generally showed slow-wave in the early-onset epilepsy.The composition of seizure types differed significantly among different types of stroke (x2 =8.461,P =0.015).Forty-five of 58 patients (77.6%) suffered from cortical lesions and 13 of 58 patients (22.4%) suffered from subcortical lesions.Those children with post-stroke epilepsy who regularly use of antiepileptic drugs were followed up for 6 months to 4 years,81.2% (56/69 cases)of clinical seizures were controlled.Conclusions The incidence of epilepsy after stroke is higher in children than in adults,cerebral stroke seizure occurs more commonly in cases with hemorrhagic stroke,especially with subarachnoid hemorrhage.Conventional antiepileptic treatment is effective.
5.Acupuncture combined with medication for postherpetic neuralgia affecting the head and face: a randomized controlled trial

Objective: To observe the efficacy of needling the Xi-Cleft points of yang meridians plus topical surrounding needling and pregabalin in treating postherpetic neuralgia (PHN) affecting the head and face and its influence on quality of life (QOL). Methods: Eighty patients with PHN affecting the head and face were randomized into an acupuncture-medication group and a Western medication group by their visiting sequence, with 40 cases in each group. The Western medication group took pregabalin capsules orally, 75 mg each time, twice a day. The dosage could increase to 150 mg each time and twice a day within 1 week based on the efficacy and tolerance. The treatment lasted for 5 weeks. The acupuncture-medication group was given additional surrounding needling at the herpes zoster on the head and face, and Xi-Cleft points of yang meridians were chosen according to the affected area for acupuncture, once every other day, for 5 weeks in total. Results: Prior to treatment, there were no significant differences in the visual analog scale (VAS) and QOL scores between the two groups (both P>0.05). After treatment, VAS and QOL scores dropped significantly in both groups (all P<0.01), and were significantly lower in the acupuncture-medication group than in the Western medication group (both P<0.01). The total effective rate was 67.5% in the Western medication group, versus 92.5% in the acupuncture-medication group, and the between-group difference was statistically significant (P<0.05). Conclusion: Based on oral administration of pregabalin, needling the Xi-Cleft points of yang meridians and topical surrounding needling can reduce PHN, notably improve patients' QOL, and produce more significant efficacy than oral administration of pregabalin alone.
6.Study on CD19 and CD27 of B lymphocyte subsets in peripheral blood of patients with rheumatoid arthritis and its correlation

Objective To study B lymphocyte subsets(na(?)ve B cells,memory B cells and plas- mablasts)of peripheral blood in patients with rheumatoid arthritis(RA)and its relationship with autoantibod- ies and clinical manifestation.Methods Blood samples and clinical data of 60 patients with RA were enrolled into this study.They were divided into three groups:active,inactive and refractory RA based on clinical mani- festations and 24 healthy controls were included.CD19 and CD27 of B cells in peripheral blood of RA patients and healthy controls were detected using flow cytometry at single-cell level.Frequence of na(?)ve B cells (CD19~+CD27~-),memory B cells(CD19~+CD27~(dim)),plasmablasts(CD19~+CD27~(high))and average fluorescence in- tensity of CD19 were analyzed,and their relationship with clinical manifestations and rheumatoid factor(RF), anti-typeⅡcollagen(anti-CⅡ),anti-cyclic citrullianted peptide(CCP)antibodies were investigatied.Results Frequence of na(?)ve B cells and plasmablasts in peripheral blood of patients with RA was increased compared with normal control.In contrast,memory B cells in patients with RA were decreased.The na(?)ve B cells subset in inactive and refractory RA was higher than that of healthy controls(P<0.05),and the memory B cells subset in those groups was lower than that of healthy controls(P<0.05).The plasmablasts in active and refractory groups of RA were higher than those of healthy controls(P<0.05).The average fluorescence intensity of CD19 in peripheral blood in patients with RA was positively correlated with ESR,C-reactive protein(CRP),healthy assessment questionaire(HAQ),and plasmablasts was positively correlated with arthrocele index.Na(?)ve B cells,memory B cells and plasmablasts subsets had no relation with RF,anti-CⅡand anti-CCP antibodies. Conclusion B cell subsets in peripheral blood of patients with RA are significantly abnormal,characterized by expanded naive B cells and plasmablasts but diminished memory B cells.Plasmablasts are increasesd in active and refractory groups of RA,and have positive correlation with swollen joint index.B cells may play an important rote in the pathogenesis of RA.
7.Applied exploration of formative evaluation in teaching of pediatric surgery

To explore the promotion of formative assessment for teaching, the surgery teaching and research section of Institute of Pediatric has carried out formative assessment in theory teaching of pediatric medicine pediatric surgery for seven-year program for two consecutive years , and the teaching effects and assessment results have been made detalled analysis. The results shows that for-mative assessment combining with summative assessment can guide the students to adjust learning method in time and improve teaching methods effectively. Compared with the pure summative evalua-tion, formative assessment has enhanced the students' learning initiative, and improved their abilityof learning and solving problems. Formative assessment can comprehensively measure the overall quality of students.

ObjectiveTo explore the feasibility of bone marrow stromal cells (BMSC) of rats differentiating into endothelial cells.MethodsBone marrow cells were obtained by an aseptic technique. Afterwards, the obtained cells were divided into two groups: cells in test group were propagated in 1640 medium with recombinant human GM-CSF (rhGM-CSF) (400ng/ml), and that in control group were propagated in medium with 1640 medium only. The differentiated cells were detected by specific immunology marker by immunocytochemistry and immunofluorescence at day 8.ResultsThe differentiated cells demonstrated the characters of endothelial cells under phase contrast microscopy. Cells of the test groups demonstrated specific characters by immunocytochemistry stain.ConclusionBone marrow stromal cells can multiply vigorously and differentiate into cells with endothelial cells characters in the medium with high concentration rhGM-CSF.
