1.Separation and Identification of Various Effective Compositions in Two kinds of Traditional Chinese Patent Medicines by Microemulsion TLC

Objective: To investigate the application of microemulsion TLC in separation and identification of various effective compositions in traditional Chinese patent medicines. Methods: The various compositions in Kangganmao Granules and Fangfeng Tongsheng Pills were separated and identified simultaneously on polyamide film with six kinds of SDS/n C 4H 9OH/n C 7H 16 /H 2O microemulsions as the mobile phases. Affections of kinds and acidity on separationg results of microemulsions were investigated. Results: The baicalin, chlorogenic acid, Saposhnikovia and glycyrrhiza in Kangganmao Gronules and the emodin, chrysophanol, baicalin, Saposhnikovia and Glycyrrhizae in Fangfeng Tongsheng Pills may be separated and identified simultaneously with microemulsion (containing 75% water) formic acid (9∶1) as mobile phase. Conclusion: Microemulsion TLC is an efficient, simple new method for the separation and identification of various compositions in traditional Chinese patent medicines.
2.The Effect of Ultraviolet Irradiation on the Morphological Changes in Epidermal Keratinocytes.

A total of 40 adult black-mice was used and divided into two groups for expeiment. Group A was irradiated by UVB only and Group B had SPF 15 sunscreen a.pplied to the back followed by irradiation by UVB. Each group was divided again into 5 subgroups according to the days of UVB irradiation frcm 2 to 10 days. A Waldmann combination UVA+UVB Radiation Treatment Cabin 8001 was used as the light source and the UVB dosage was 50 mJ/cm2 daily. Skin specimens were taken 24 hours after the last irradiation. Histologic changes in epidermis were reviewed by the light microscope. In group A, the characteristic sunburn cells(SBC) were observed with 100 mJ/ cm2. SBC number was maximum with 400 mJ/cm2. The other epidermal changes were parakerat.osis, crusts, atypical cells, and mitoses of basal cells, which showed graded responses to the UVB doses. Pretreatment with the sunscreen completely prevented these changes.

5.Solitary Keratoacanthoma Developing on the Abnormally Sun - exposed Labial Mucosa.

The case of a 56-year-old woman with a keratoacarithoma(KA) of the labial mucosa is herein reported, The tumor was excised and the pathological features were characteristic of a KA, This case is reported because the oral mucosa is such an unexpected location for solitary KA and the labial mucosa of this patient received chronic actinic damage, which suggested an etiological relationship. Follow-up examination of the patient after four months showed no recurrence.

6.The Pulmonary Hemodynamics in Essential Hypertension.

The pulmonary arterial pressure(PAP) was reported to be higher in essential hypertensives than in normotensives, but the underlying mechanisms for the higher PAP were not known exactly. In order to observe the changes of pulmonary hemodynamics and to get an insight into the mechanism of the pulmonary hypertension, if any, in essential hypertensive patients, the autors performed the cardiac catheterization in 13 normal controls(group A), 14 coronary patients with normal systolic left ventricular function and systemic blood pressure(group B), and 15 essential hypertensives with normal coronary artery and systolic left ventricular function(group C). 1)Pulmonary arterial pressure(PAP) was 19.2+/-3.2/8.1+/-1.9/12.0+/-1.9(s/d/m)mmHg in group A, 25.0+/-6.2/12.8+/-4.0/16.3+/-4.5mmHg in group B, 29.3+/-6.1/12.8+/-4.0/18.2+/-3.6mmHg in group C. The PAP was higher in group B and C than that of group A(p<0.005). 2) Pulmonary vascular resistance(PVR) was 88.2+/-34.9 dyne.sec.cm(-5) in group A. 137.8+/-74.5 dyne.sec.cm(-5) in group B and 173.9+/-77.5 dyne.sec.cm(-5) in group C. In group B and C, PVR was increased compared to that of group A(p<0.05, p<0.005, respectively). 3) Pulmonary capillary wedge pressure(PCWP) was 6.6+/-2.8 mmHg in groupa A. 9.9+/-2.9mmHg, and 9.6+/-3.6 mmHg in group C. PCWP in group B and C were higher than that of group A(p<0.005). 4) Systemic vascular resistance(SVR) was 1298+/-340 dyne.sec.cm(-5) in group A, 1466+/-362 dyne .sec.cm(-5) i group B and 2255+/-439 dyne.sec.cm(-5) in group C. In group C, SVR was increased compared to that of group A and B(p<0.002). 5) Significant correlation was demonstrated between PVR and PCWP in group A(r=-0.74, p<0.05). 6) In group B, significant correlations were shown between PAP and PCWp(r=0.55, p<0.05), between PAP and PVR(r=0.69, p<0.05). 7) In group C, significant correlations were demonstrated between PAP and PCWP(r=0.55, p<0.05), between PVR and SVR(r=<0.51, p<0.01). Above results revealed that PAP and PVR were increased in essential hypertensives without left ventricular failure and this elevated PAPseemed to be affected in part by increased PCWP, and increased PVR in accordance with increased SVR.

7.Various Epidermal Changes Overlying A Dermatofibroma.

Various epidermal changes overlying a dermatofibroma can be induced through the proliferation of pluripotential epidermal cells by the stimulation of mesenchymal factors. The authors report a case of dermatofibroma in a 55-year-old male who had a reddish brown, asymptomatic nodule of 5 years duration on his left forearm. The biopsy specimen revealed a dermatofibroma of cellular type associated with epidermal changes: (1) acanthosis; (2) elongation of rete ridges; (3) thin interlacing epidermal strands with increased pigmentation resembling seborrheic keratosis or fibroepithelioma; (4) immature hair follicles and sebaceous lobules; (5) hair germ like buddings; (6) superficial basal cell epithelioma-like proliferations.

8.A Case report of Generalized Pustular Psoriasis Associated with Spondylitis, ARthritis and Glomerulonephritis.

Generalized pustular psoriasis is a severe form of paoriasis and is frequently associated with arthritis and nail deformity. We present a case of generalized pustular psoriasis in a 40 year-old man, with nail change, arthritis of both knee and ankle joints, spondylitis, and glomerulonephritis. We found a positive results of HLA A-9, B-27 in this patient. Skin lesions, arthralgia and hematuria improved marked1y by plasmapheresis and application of topical corticostertoid.

9.Immunologic changes after treatment with cyclosporin a in atopic dermatitis.

BACKGROUND: Atopic dermatitis(AD) exibit multiple immune akno malities including elevated serum IgE levels and impaired cell mediated immunity, but basic immunology defect is largely unknown. OBJECTIVE: The purpose of this study was to evaluate the immnologic changes after treatment with Cyclosporin A(CsA) in AD patients. MEHTODS: Eight patients of severe AD were treated with CsA(4mg/kg body weight per day) for 5 weeks, and evaluated clinical and immunological parameters before aid after treatment. RESULTS: Peripheral blood eosinophil counts were significantly decreased after treatment(P<0.05), and serum IgE level was decressed but not significantly. In immediate hypersensitivity reaction by pinprick test, antigen score was.significantly decreased, and in the evaiuation of cell mediated immunity to recall antigens, total induration score was significantly increased(p<0.05). The number of helper T cell and helper/suppressor 7 cell ratio in peripheral blood, and the niimber of helper T and suppressor T cell in skin lesion were both significantly decreased after treatment(P<0.05). Clinical state after treatrnent was good in all of patients. CONCLUSION: The results indicate that CsA act on peripheral blood osinophil and immediate and delayed hypersensitivity reaction as well as helper T-cells in peripheral blood and skin lesion, and it may by useful in the treatrnent of severe AD.

10.Linear Sebaceous Hyperplasia.

We report an unusual case of sebaceous hyperplasia in an 18-year-old male manifestated clinically as yellowish, grouped papules with a linear distribution, present on the right side of forehead since birth. Histopathologically, a large sebaceous gland composed of numerous lobules grouped around a centrally dilated duct was seen. The sebaceous lobules distributed in the upper dermis showed direct connection to the skin surface, which suggested a transepidermal elimination of sebaceous lobules.
