1.Construction and application of self-management program based on WeChat platform among colorectal cancer survivors with ostomies

Objective To construct and evaluate the effect of WeChat platform self-management program among colorectal cancer survivors with ostomies. Methods Totally 80 colorectal cancer survivors with ostomies were divided into two groups according the inclusive time with 40 cases each. The patients in the experimental group were applied WeChat platform self-management program, the control group were applied regular discharge telephone follow-up. The patients′ self-efficacy and quality of life were measured and compared prior to discharge, at 1 and 6 weeks after discharge. And the patients' occurrence of stoma complications were measured at 1 and 6 weeks after discharge. Results Due to quit research, finally complete the study of the experimental group for 36 cases, the control group for 35 cases. The total score of self-efficacy at 1 and 6 weeks after discharge were (72.05± 9.68), (84.91±16.01) points in the experimental group, and (63.00 ± 13.26), (72.20 ± 15.61) points in the control group, and there were statistically significant differences (t=-2.586, 3.388, P<0.01). The total score of quality of life at 1 and 6 weeks after discharge were (43.68±8.32), (49.49±8.61) points in the experimental group, and were (37.09± 5.57), (40.42 ± 8.01) points in the control group, and there were statistically significant differences (t=3.926, 4.596, P<0.05). Repeated measures analysis of variance showed that the intervention effect on the overall self-efficacy, stoma care self-efficacy, social self-efficacy and stoma quality of life was significant (F=4.287-10.744, P<0.05);the time effect on the overall self-efficacy and quality of life was significant (F=146.109, 90.105, P < 0.05); there were interactions between the effect of WeChat platform self-management program and time on the overall self-efficacy, stoma care self-efficacy, social self-efficacy, dietary efficacy, sexual life satisfaction efficacy, keep active efficacy and stoma quality of life ( F=4.452-21.777, P<0.05) . Chi-square analysis showed that the patients′occurrence of stoma complications at 1 and 6 weeks after discharge in the experimental group were lower than those in the control group:8.33%(3/36), 5.56%(2/36) and 31.43%(11/35), 25.71%(9/35), the differences were significant (χ2=5.980, 5.508, P < 0.05). Conclusions WeChat platform self-management program may be helpful for colorectal cancer survivors with ostomies after discharge in improving self-efficacy and quality of life, and in reducing stoma complications incidence.
2.Effects of WeChat self-management program on self-management behavior in discharged patients with ostomies

Objective To explore the effects of WeChat self-management program on self-management behavior in discharged patients with ostomies. Methods Using convenience sampling method, a total of 120 cases of colorectal cancer patients undergoing colostomy from March 2015 to February 2016 in the colorectal cancer treatment center of a ClassⅢ Grade A hospital in Dalian were selected as the research subjects. The patients from March to August 2015 were assigned as the control group, and the patients from September 2015 to February 2016 were assigned as the intervention group, with 60 cases in each group. The control group was guided by routine methods, while the intervention group was managed by self-management project of Wechat platform, including Wechat public platform intervention, Wechat group intervention, enterostomy diary monitoring and case management. The self-management abilities of the two groups were compared. Results Of the 120 patients, 111 were completed, 56 in the intervention group and 55 in the control group. The time difference of self-management ability in each dimension and total score between the two groups were statistically significant (P<0.01), and the improvement of stoma nursing management, daily life management, information management, psychological management and overall self-management ability in the intervention group were higher than those in the control group. There were significant differences between the two groups at 1 and 6 weeks after the intervention (P<0.05). Conclusions The Wechat self-manegement program can improve the ability of self-management among patients with ostomies after discharge.
3.Evidence summary of risk grading management of high-risk diabetic foot patients

Objective:To retrieve and evaluate the evidence on the risk grading management of high-risk diabetic foot patients, so as to provide reference for the management of diabetic foot.Methods:According to the "6S" pyramid model of evidence-based resources, evidence on risk grading management of high-risk diabetic foot patients was retrieved in Web of Science, UpToDate, Cochrane Library, PubMed, China National Knowledge Infrastructure, WanFang Data, VIP and other databases, guide websites, and professional association websites. The search period was from the establishment of the database to January 1, 2024.Results:A total of 11 articles were included, including 8 guidelines, one systematic review, one expert consensus, and one expert opinion. There were 24 pieces of best evidence about the risk grading management of high-risk diabetic foot patients, covering 8 aspects of risk assessment, risk grading, health records establishment, screening frequency, health education, foot management, exercise management and referral.Conclusions:The best evidence of risk grading management of high-risk diabetic foot patients summarized can provide reference for clinical nurses to carry out evidence-based practice.
4.Efficacy comparison between video-assisted thoracoscopic surgery and thoracotomy in the treatment of patients with mediastinal tumors

Objective To compare the clinical efficacy of video-assisted thoracoscopic surgery and thoracotomy in the treatment of mediastinal tumors.Methods A total of 90 mediastinal tumor patients with surgical treatment between January 2015 and January 2015 in our hospital were as the research objects,and were randomly divided into observation group (n=45) and the control group (n=45) according to random number table method,the observation group was treated with video-assisted thoracoscopy surgery method,and the control group with a standard open thoracic surgery for treatment.Intraoperative related indicators,postoperative recovery related indicators,and the incidences of postoperative complications were compared.Results Observation group had lower open thoracic blood loss,shorter open and closed chest time than control group (P<0.05).The difference in total operating time of two groups showed no statistical significant difference (P>0.05).The observation group had less postoperative drainage volume,less postoperative catheter time and postoperative hospital stay and lower postoperative complication rate than that in the control group (P<0.05).Conclusion Effect of video-assisted thoracoscopy surgery in the treatment of mediastinal tumor is remarkable,it has fewer traumas,faster recovery and less complications,so it is worth popularizing in clinic.
