4.A Study on the Physical Growth and Health Status in University Students.

For the purpose of ascertaining the status of physical growth and health status of the University students, a intensive survey was conducted by the authors towards a total of 2,638(male; 2,228, female;410) who passed the written entrance examination at the Seoul National University. The items included the measurements on physical growth, various physical indexes, visual acuity, tuberculosis and dental status. The findings and results can be summarized as follows : 1. Physical Growth. No signigicant difference was found in the physical growth between classes(literature and science) and by ages. In males, the means of body height, body weight, chest-girth and sitting height were 168.0+/-5.07cm, 55.7+/-5.71kg, 85.8+/-4.4cm and 91.5+/-2.21cm, respectively. In females, these were 157.4+/-4.57cm, 00.4+/-5.75kg, 80.3+/-4.51cm and 86.1+/-3.30cm, respectively. Basing on the above measurements several physical indexes were calculated as follows ; Relative body weigh 33.15 in males and 32.02 in females. Relative chest-girth 51.07 and 51.01, relative sitting-height 54.46 and 54.70 Rohrer index 1.00 and 1.02, Kaup index 1.97 and 2.04, Vervaeck index 83.63 and 83.03, and Pelidisi index 89.94 and 92.45. 2. Visual Acuity. Among the students of science classes those of 0.8 and below in vision on left and right occupied 46.4% and 45.0%, those of 0.6 and below 39.1% and 41.2%, respectively. Among the students of literature classes those of 0.8 and below occupied 46.3% and 42.9%, and those of 0.6 and below occupied 41.4% and 39.0, respectively. In males those of 0.8 and below occupied 46.8% and 44.5% and 0.6 and below occupied 41.9% and 39.7%, while in females 0.8 and below occupied 43.9%, and 42.4%, and 0.6 and below occupied 38.3%% and 37.3%, respectively. In males those of 0.8 and below in corrected vision occupied 12.1% and 10.3%, while in females 12.9% and 12.2%, respectively. More students of abnormal vision were found among science classes and the proportions of corrected vision were 38.9% and 37.4% in males, and 33.9% and 33.2% in females, respectively. 3. Tuberculosis. Among the total, 55 students were diagnosed as tuberculosis, of whom 50 were minimal cases, 2 were moderately advanced cases and 3 were far advanced cases. 4. Dental Status. Among the total, 81.2% were of decayed teeth, 20.8% filled, 9.5% missed, 5.0% extraction needed, 9.1% sunplatinum bridges, 3.3% golden bridges and 5.5% golden inaly. Regarding tartar on teeth moderate, heavy and light grades occupied 49.5%, 29.7% and 20.3%, respectively and 0.8% had none. 5. Comprehensive evaluation. According to the criteria 4 grades were applied to comprehensive evaluation as follows : A---excellent in physical status B---with some physical defects but no difficulties in study C---need treatment D---need treatment and rest. Out of the total, A grade occupied 21.6%, B 76.2% and C. and D 2. 2.

5.Detection of bcl-2/IgH Gene Rearrangement and Expression of c-myc and p53 Oncoprotein in B-cell Lymphoma.

Many kinds of genetic changes have been known to be associated with malignant lymphoma and bcl-2, p53 and c-myc are some examples. We investigated the expression of p53 and c-myc protein in follicular and diffuse B cell lymphoma by immunohistochemistry to study the possible role of these proteins in the lymphomagenesis and transformation of the tumor. The rearrangement of bcl-2 oncogene and the immunoglobulin heavy chain gene was searched for in those cases by polymerase chain reaction(PCR). Paraffin-embedded tissues of fifteen follicular lymphomas and 14 diffuse lymphoma cases were used. The results of immunohistochemical staining are summarized as follows: 1) p53 positivity is significantly higher in diffuse lymphoma than in follicular lymphoma(P=0.001); 2) c-myc expression is not increased in diffuse lymphoma compared with follicular lymphoma; 3) PCNA index is significantly higher in diffuse lymphoma than in follicular lymphoma(P=0.03) but there was no statistically significant correlation between PCNA index and p53 positivity(P=0.44); 4) Eight out of 14 cases of follicular lymphoma and 12 of 14 cases of diffuse lymphoma showed rearrangement of the immunogloblulin heavy chain gene; 5) bcl-2 oncogene rearrangement was identified in only one case of follicular lymphoma and all the diffuse type lymphomas were negative in bcl-2/IgH rearrangement. In conclusion, assuming that the follicular pattern of B-cell lymphoma often transforms to diffuse type in later stages, c-myc over-expression might be an earlier event than p53 mutation in the process of tumor progression in B-cell lymphoma. bcl-2/IgH gene rearrangement in follicular lymphoma is a rare finding in Korea compared to that of Western countries.
6.Classification and Management of Epiphyseal Injury

Injuries involving epiphyseal plate is important because of special problems in diagnosis, management and tl later complications of serious disturbance of growth which may be predictable and preventable. We revieviewed 40 cases of epiphyseal injuried in 35 patients, managed at Department of Orthopaedic Surgery, Nation Medical Center from Jan. 1975 to Dec. 1981. The longest follow up was 4 ywars and 6 months and the shortest, year, the average being 2 years. The results were as follows: 1. The incidence of epiphyseal injuries was 35 patients in 211 patients of padiat.c fractures. 2. There were 29 males and 6 females. 3. The causes were falling down accident, slipping down accident, impinging injury, direct blow and traff accident in order 4. Roentgenographically, 23 cases were Type II of Salter & Harris classification, 8 cases were Type IV, cases were Type I, 2 cases were Type III, and 1 case was Type V. 5. Epiphyseal Injuries were found with order of decreasing frequency of distal radius, distal humerus, dist femur, phalanges, distal tibia, proximal humerus, distal fibula and proximal humerus. 6. The methods of treatment were C/R & 'cast immobilization, C/R & K-wire fixation, 0/R & K-wire fixation and C/R & screw fixation. 7. Complications were found in 2 cases of angular deformity in Type IV injury, 1 case of shortening in Type injury, and 1 case of angular deformity in Type V injury.

8.Symptoms and Cognitive Function in Chronic Schizophrenia: 6 Months Follow-up Study.

OBJECTIVES: The purpose of this study was to investigate whether longitudinal changes in positive and negative symptoms affect cognitive functioning in chronic schizophrenia. METHODS: Sixty-eight patients diagnosed with DSM-IV schizophrenia were examined on two occasions over 6 months for symptoms and cognitive changes. Symptoms were measured by PANSS. Cognitive functions were examined for sustained attention, executive function, concentration and attention, and verbal memory and learning using Degraded Stimulus Continuous Performance Test, Wisconsin Card Sorting Test, Digit Span, and Rey Auditory Verbal Learning Test, respectively. Twenty control subjects were assessed to compare the cognitive scores of remitted schizophrenic patients. RESULTS: Patients showed significant improvement in symptoms and all cognitive tests after 6 months treatments. Significant improvements in positive and negative symptoms did not predict improvements in any aspect of cognitive functioning measured. Normal controls performed significantly better than remitted schizophrenic patients on all cognitive tests. The results show no relationship between change in symptoms and change in cognition in chronic schizophrenia. CONCLUSION: We suggest that symptomatic and cognitive impairment may be a distinct construct. These findings highlight the importance of treating cognitive impairment in addition to the clinical symptoms of schizophrenia.

9.A Comparative Study of Positive Culture Rates Between Urethral Discharge and Urinay Sediment for the Diagnosing of Neisseria gonorrhoeae.

BACKGROUND: Although the culture of urethral discharge using a cutton swab is regarded as one of the most sensitive and reliable diagnostic methods of gonorrhea, it may give pain or diagnostic methods of gonorrhea, it may give pain or discomfort to patients, and its detection rate may be influenced by the examiner's technical ability. OBJECTIVE: The purpose of this study was to compare the positive culture rates between ure4thral discharge and urinary sediment for the diagnosing of Neisseria gonorrheae. METHODS: We compared this technique with the culture of urethral discharge using the specimens from 357 male patients with gonococcal infection at VD clinic of Jungku Pvblic Health center in Seoul from Jan 1991 to Jun 1993. RESULTS: The culture of urinary sediment was positive in 339 of 357(94.95), whereas the culture of urethral discharge was positive in 346 of 357(96.95). However, these data have no statistical significance(P<0.05). The obe observed sensitivity and specificity of culture of urinary sediment comjpared to culture of urethral discharge were 97.75 and 90.9%, respectively. The predictive value of positive and negative test compared to the culture of urethral discharge were 99.7% and 55.65, respectively. As our data showed low negative predictive value, a negative culture result of urinary sediment does not always imply a negative culture result of urethral discharge. CONCLUSION: Therefore, at present, the previous method of the culture of urethral discharge is more highly recommended and further studies for this diagnostic and/or new method of gonorrhea based on larger group of patients is encouraged.

10.A Case of Apocrine Hidrocystoma.

Apocrine hidrocystoma is an uncommon benign tumor derived from the secretory segment of apocrine sweat glands and another name for it is apocrine cystadenoma. A 48-year-old female patient had a solitary, translucent red-brownish cystic nodule, 10 × 6 × 7mm in size, on the periorbital area of her left eye. The lesion had enlarged very slowly for 25 years. We excised the lesion and identified it as apocrine hidrocystoma. This is a case of apocrine hidrocystoma with typical clinical and histopathological findings.
