1.Laparoscopic-assisted small incision operation for the treatment of common bile duct stones

Objective To investigate the feasibility of laparoscopic-assisted small incision exploration and stone extraction for the treatment of common bile duct stones. Methods Following the laparoscopic cholecystectomy, the location of the common bile duct was determined under laparoscope. A small abdominal incision was made at the corresponding site and the exploration of common bile duct and extraction of stones were conducted via the incision. Results Among a total of 17 cases, the operation was successfully accomplished in 16 cases and a conversion to open surgery was required in 1 case. The operative time was 90~160 min (mean, 120 min). The number of extracted stones per case was 1~4, with the maximum size of 2.5 cm. All the stones were removed on one session. Postoperative cholangiography 3~4 weeks after the surgery indicated no residual stones. Follow-up for 8~36 months (mean, 14 months) showed no residual stones or occurrence of cholangitis. Conclusions Laparoscopic-assisted small incision exploration and stone extraction for common bile duct stones is feasible, being readily to popularize in basic hospitals.
2.Application and Research of Membrane Separation Technology in Banlangen Keli Production

Objective To investigate the refining and concentration of the Banlangen aqueous extractives using membrane separation technology. Methods The membrane separation process for refining and concentration Banlangen aqueous extractives was optimized by the quality of products and membrane operation parameters, and was tested through medium-scale trials. Results The selected G2+GM+RO2 membrane process was suitable. The inherent quality of the products obtained by membrane separation process was better than that obtained by traditional process. Conclusion G2+GM+RO2 membrane process is suitable to refine and concentrate Banlangen aqueous extractives, and it has technical conditions for industrial scale trials.

OBJECTIVE:To review the occurrence of ophthalmic injuries and analyse the relationship between the injuries and the kinds of drugs used and the route of administration METHODS:The cases of drug-induced ophthalmic injuries which were reported in medical journals published from 1960 to 2000 year were collected and analysed RESULTS:There were 967 cases of drug-induced ophthalmic injuries caused by 154 kinds of drug The clinical manifestations of ophthalmic injuries included ocular pigmentation,visual disturbance,loss of sight and injuries to cornea,conjunctiva,optic nerve,retina or optic papilla,and the inducing drugs were mostly antimicrobials,central nervous system and circulatory system drugs,such as chlorpromazine,perphenazine,amiodaron,ethambutol etc CONCLUSION:Drug-induced ophthalmic injuries caused by systemic administering drugs in commonly-used dosage and route pertain to ADRs The ophthalmic injuries caused by local application of drugs to the eyes result from improper drugs used,faulty manipulation and unsuitable applications of drugs during or after operations
4.POST-EXERCISE CHANGE OF THE QTC INTERVAL IN CORONARY HEART DISEASE

The present study summarized the results of QTc. in 54 subjects, 8 of which were patients with coronary heart disease and 46 normal.The exercise tests were carried out on an ergometric bicycle with a submaximal exercise andQT interval was corrected for heart rate according to Bazett's formula (QTc=QT/R-R-). All patients were subjects to resting ECG before the test, during excerxise, and immediately fol owing the end of the exercise. Electrode jelly contact disk electrodes were used for the CMS. Results showed that coronary heart disease patients all had a prolongation of the QTc following exercise with variations of 0.06+0.02s The QTc of all normal subjects, except one, Showed a shortening or no ohange.Our study thus reveaded that the finding of QTc variations with exercise all of value for the diagnosis of coronary heart disease.

BACKGROUND: Colorectal cancer is a common malignant tumor in digestive tract. Theory of tumor stem cells (TSCs) offers a new approach to understand the development mechanism of colorectal cancer. OBJECTIVE: To review the progress in the existence, origin and identification of colorectal cancer stem cells (Co-CSCs). RETRIEVAL STRATEGY: Pubmed was undertaken to identify relevant articles published from January 1990 to July 2007 with the of "colorectal cancer,stem cell,tumor stem cells" in English.At the same time, Vip database was undertaken to identify the relevant articles published between January 1990 and July 2007 with the key words of "colorectal cancer, stem cell, tumor stem cells" in Chinese. The data were selected primarily, and then quotations of each article were checked. Inclusive criterion: the articles related to the advances of the existence, origin and identification of Co-CSCs were selected. Exclusive criterion: the articles with repetitive research were excluded. Totally 63 relevant articles were selected and 33 of them met the inclusive criterion. The 30 excluded articles were of old or repetitive content. LITERATURE EVALUATION: Of the 33 inclusive articles, 28 were on the existence and origin of Co-CSCs, 4 were about the isolation and identification of Co-CSCs, and 1 was about the problems and prospect. DATA SYNTHESIS: TSCs is a rare population of cells in tumor tissues with infinitely proliferative and self-renewal potential that drive the formation and growth of tumors. It may become a new target to eradicate malignant tumor. TSCs have been discovered in leukemia and some solid tumors. Intestine stem cells are poorly differentiated. Its phenotype is difficult to identify. There are many reports on the quantity, location, origin, function and relation with intestinal disease. Co-CSCs have been isolated and identified as EpCAMhigh/CD44+ cells recently, which accelerate to understand the mechanism of colorectal cancer. CONCLUSION: The discovery of Co-CSCs is one of the advances in researching of malignant tumor. TSCs may become a new target to eradicate malignant tumor, and brings hope to cure it.
6.Clinical comparative study of breast-conserving surgery and improved surgical in breast cancer patients

Objective To comparative analysze of the applications value of breast-conserving surgery and improved surgical in breast cancer patients.Methods One hundred and sixty-eight female breast cancer patients from February 2005 to October 2010 in the People's Hospital of Rizhaoour hospital were selected.The patients were divided into breast-conserving surgery group (n =80) and improved surgical group (n =88).The patients in two groups were followed-up and investigated for three years.Operative time,blood loss,postoperative drainage,postoperative drainage time and hospital stay,incidence of complications,local recurrence,distant metastasis and survival after 3 year,satisfaction ratio of breast cosmetology,psychological anxiety scores and quality of life scores after 3 years of the patients in two groups were compared and analyzed.Results The complication rate of breast-conserving surgery group was 12.50% (10/80),lower than the improved surgical group,and the difference was significant statistically (29.55% (26/88),x2 =7.23,P<0.05).The operative time ((72.68±5.28) min),blood loss ((39.21 ± 8.59) ml),postoperative drainage ((162.25 ± 28.56) ml),postoperative drainage time((8.16±2.58) d) and the length of hospital stay((11.43±1.98) d) of the patients in breast-conserving surgery group were lower than those of the improved surgical group,and the differences of two groups were significant statistically ((92.48 ± 6.01) min,(68.26 ± 10.23) ml,(343.29 ± 62.18) ml,(16.59±4.96) d,(18.47±2.94) d;t =22.72,19.99,24.61,13.99,18.35;P<0.05).The satisfaction rate after 3 years of breast-conserving surgery group was 92.50%(74/80),higher than that of the improved surgical group,and the difference was significant statistically (81.82% (72/88),x2 =4.20,P< 0.05).The psychological anxiety scores after 2 and 3 years of breast-conserving surgery group were (40.02 ± 2.63) points and (34.26 ± 3.59) points,lower than those of improved surgical group((42.58±2.37) points and (39.85±2.10) points),the quality of life scores after 3 years of breast-conserving surgery group was (67.58± 11.26)points,higher than that of improved surgical group ((50.85 ± 10.42) points),and the differences were significant statistically (t =6.60,12.16,9.96;P < 0.05).Conclusion Breast-conserving surgery could reduce the incidence of complications and psychological anxiety situations,improve clinically relevant indicators and satisfaction ratio of breast cosmetology and quality of life for the breast cancer patients.

AIM To observe the effect of intravenous injection of erythromycin (EM) on interdigestive migrating motor complex (MMC) and postprandial gastrointestinal contraction in conscious dogs , and to study its possible mechanism. METHODS Gastrointestinal contractile activity was recorded using low compliance capillary water perfusion manometric system. EM was administered intravenously during phaseⅠ and after meal, and blood samples were drawn for measuring plasma motilin concentrations. RESULTS ①Plasma motilin levels showed cyclical fluctuations in different phases of MMC, and plasma motilin reached peak during phaseⅢ and lowest during phaseⅠ.②EM induced phaseⅢ-like contractions in the antrum and duodenum, which was not accompanied by a peak in plasma motilin level. The optimum dose of EM for resulting in a premature phaseⅢ with the same characteristics as the spontaneously occurring phaseⅢ was established to be 0.5 mg*kg-1. The dose of 10 mg*kg-1 EM induced gastrointestinal continuous contractions and duodeno-gastric retrograde peristalsis which was associated with retching and vomiting. ③Atropine obviously inhibited EM-induced phaseⅢ activity in the antrum and duodenum. ④EM powerfully enhanced postprandial gastrointestinal contractile activity. CONCLUSIONS The results suggests that EM is a potent prokinetic agent. The mechanism is not related to the release of motilin, but may be mediated partially by cholinergic pathway.

10.Morphology and distribution of EpCAMhigh/CD44+ colorectal cancer stem cells

BACKGROUND: Cancer stem cells have bean a hot topic in life science research. EpCAMhigh/CD44+ colorectal cancer stem cells are beneficial to observe morphological characteristics and distribution of cancer stem cells.OBJECTIVE: To investigate the quantity, location, distribution and hematoxylin-easin staining morphologic features of colorectal cancer stem cells (Co-CSCs).DESIGN, "rIME AND SETrlNG: The observational study was performed at the Department of Pathology, Lanzhou General Hospital of Lanzhou Military Area Command of Chinese PLA from March to August 2008.MATERIALS: A total of 67 colorectal cancer paraffin embedding samples were collected from 200512007 archive at the Department of Pathology, Lanzhou General Hospital of Lanzhou Military Area Command of Chinese PLA, comprising 27 males and 40 females, aged 29 90 years.METHODS: EpCAM and CD44 was used to label cancer stem cells in paraffin embedding colorectal cancer samples. The locations of EpCAM and CD44 double positive cells were detected by SP(streptavidin HRP) immunohistochemical staining and double immunohistochemical staining. The morphologic features of them were investigated on hematoxylin-easin staining at the same position.MAIN OUTCOME MEASURES: Number, location, distribution of EpCAM and CD44-posltive cells, and morphology following hematoxylin-eosin staining.RESULTS: The number of double-positive cells accounted for 0.1% to 30.0% of all tumor cells, and the cells were scattered or distributed focally along the basement of glandular-like structure. The cells with scarcely cytoplasm were cube or oval, and its nucleus was oval or high cylindrical, deep stained and homogeneous; The quantity of double-positive cells were negatively correlated with the differentiation of colorectal cancer.CONCLUSION: Cancer stem cells are the causes of tumor development, metastasis and recurrence and drug resistance. To observe the number, location, distribution and morphology of Co-CSCs and to analyze the relationship between Co-CSCs and pathological parameters will provide guidance for the diagnosis, staging, prognostic evaluation and clinical prognosis of cotorectal carcinoma.
