Objective·To investigate the effects of a novel chitosan-silver nitrate gel (CSNG) dressing on anti-septic in wound healing. Methods·By using the ion membrane, the release rate of the new composite materials of silver ion was tested in vitro.Meanwhile, the anti-septic effects of CSNG on methicillin resistant Staphylococcus aureus (MRSA),Escherichia coli and Candida albicans were tested by colony counting. The rat wound healing model was used to detect the ability of new material to kill MRSA in vivo. Results·Compared with control group, the release of silver ions of CSNG was much slower.Sterilization experiment showed that CSNG killed the MRSA,Escherichia coli and Candida albicans much more efficiently, compared to that in the other treatments.Animal experiments also showed that CSNG promoted the rats of wound healing. Conclusion·Combining chitosan with silver nitrate and supplementary material could develop a novel chitosan-silver nitrate gel material, which not only has the obvious effects on antibacterial, but also on promoting wound healing.

Objective·To investigate effect and the possible molecular mechanism of JQ1,a specific inhibitor of bromodomain containing protein 4,on human hypertropic scar. Methods·Primary fibroblasts were isolated from human hypertrophic scars and treated with JQ-1 of different concentrations (0.1,0.5,1.0,2.0,2.5,and 12.5μmol/L) for 48 h.Then CCK-8 kit and wound healing assay were used to measure proliferation and migration of the fibroblasts.ELISA was adopted to detect the levels of collagen type Ⅰ (COL Ⅰ) and TGF-β1 after JQ-1 treatment for 24 h.Thirty-six nude mice were used for hypertrophic scar models.Human hypertrophic scars (1.0 cm× 1.0 cm×0.5 cm) were grafted subcutaneously at the backs of nude mice to establish scar animal models. After 4 weeks, the nude mice were averagely divided into two groups,i.e.JQ-1 group and DMSO group, which were respectively injected with 0.5 μmol/L JQ-1 and 0.1% DMSO each mouse every day.COL Ⅰ / Ⅲ and α-smooth muscle actin (α-SMA) were examined by immunohistochemical method and sirius red staining. Results·Cell experiments showed that JQ-1 with the concentration of 0.5 μmol/L and above significantly inhibited proliferation of fibroblasts (P<0.01).JQ-1 inhibited migration of fibroblast (P<0.01).JQ-1 inhibited secretion of COL Ⅰ and TGF-β1 of fibroblasts (P<0.01).Animal experiments showed that concentration and proportion of COL Ⅰ / Ⅲ in JQ-1 group decreased compared to DMSO group (P<0.05).α-SMA protein expression in JQ-1 group also decreased (P<0.05).Conclusion·JQ-1 can inhibit proliferation, migration, secretion of COL Ⅰ,and production of TGF-β1 of human sear fibroblasts in vitro;it can also inhibit secretion of COL Ⅰ /Ⅲ and fibroblast-myofibroblast differentiation in the human hypertrophic scars in nude mice.

Objective:To explore the effect of teachers-standardized patients combined with case based learning (CBL) in clinical probation of gastrointestinal surgery. Methods:A total of 140 clinical medicine undergraduates in gastrointestinal surgery were selected as the research subjects, and were randomized into the intervention group and the control group, with 70 cases in each group. Traditional teaching method is performed in the control group, and teachers-standardized patients combined with CBL is conducted in the intervention group. Student knowledge assessment and satisfaction survey were made for teaching effect evaluation. The t test and chi-square test were performed by SPSS 24.0 for comparison between the two groups. Results:The medical records writing of the students in the intervention group and the results of the theoretical assessment [(83.20±7.94) and (82.74±7.19) points] were significantly higher than those of the control group [(79.57±9.26) and (79.49±7.86) points] ( P<0.05). Students in the intervention group have a higher level of satisfaction of this teaching mode. Conclusion:Teachers-standardized patients combined with CBL can improve teaching quality and learning enthusiasm in gastrointestinal surgery, and it is worthy of promotion and application in teaching.

Objective:
To investigate clinical application of self-designed organ traction bag in male genital immobilization during radiotherapy.
Methods:
The male patients in the Department of Radiation Oncology, Suzhou Municipal Hospital from August 2018 to June 2019 were immobilized by using organ traction bag. Clinical effect was evaluated by subjective and objective ways which were the location relationship between contents and reference field under visual observation and fluoroscopy, and setup errors by CBCT images.
Results:
A total of 11 patients were selected, including 3 seminoma, 2 low rectal carcinoma, 2 prostatic carcinoma, 4 pelvic lymph nodes and bone metastases, with mean age 57.18 years old. Patients received 273 times of treatment and 55 times of CBCT verifications in all. Analysis results that the setup errors of the whole group in upper and lower (Vrt), head and foot (Lng), left and right (Lat) directions are less than 0.5 cm, within acceptable clinical threshold. The contents under visual observation and fluoroscopy were not in reference field and were fixed well.
Conclusions
Organ traction bag provides stable effect on male genital immobilization while taking patients’ privacy and clinical operability into account. It has high clinical application value in radiotherapy involving male genital protection.

Neurogenesis decline in hippocampal dentate gyrus (DG) participates in stress-induced depressive-like behaviors, but the underlying mechanism remains poorly understood. Here, we observed low-expression of NOD-like receptor family pyrin domain containing 6 (NLRP6) in hippocampus of stress-stimulated mice, being consistent with high corticosterone level. NLRP6 was found to be abundantly expressed in neural stem cells (NSCs) of DG. Both Nlrp6 knockout (Nlrp6-/-) and NSC-conditional Nlrp6 knockout (Nlrp6CKO) mice were susceptible to stress, being more likely to develop depressive-like behaviors. Interestingly, NLRP6 was required for NSC proliferation in sustaining hippocampal neurogenesis and reinforcing stress resilience during growing up. Nlrp6 deficiency promoted esophageal cancer-related gene 4 (ECRG4) expression and caused mitochondrial dysfunction. Corticosterone as a stress factor significantly down-regulated NLRP6 expression, damaged mitochondrial function and suppressed cell proliferation in NSCs, which were blocked by Nlrp6 overexpression. ECRG4 knockdown reversed corticosterone-induced NSC mitochondrial function and cell proliferation disorders. Pioglitazone, a well-known clinical drug, up-regulated NLRP6 expression to inhibit ECRG4 expression in its protection against corticosterone-induced NSC mitochondrial dysfunction and proliferation restriction. In conclusion, this study demonstrates that NLRP6 is essential to maintain mitochondrial homeostasis and proliferation in NSCs, and identifies NLRP6 as a promising therapeutic target for hippocampal neurogenesis decline linked to depression.
