Objective:To study the effect of repeated transient ischemic attacks(TIAs)on cumulative injury of cognitive function.Methods:The cognitive condition of 42 patients with repeated TIAs were assessed after each TIA using the event related potential,modified version of Wisconsin card sorting test,trail making test,digit span task,and verbal fluency test.The cognitive condition was compared and analyzed after different attack times.Results:There were significant differences among the cognitive condition after different attack times,latency and amplitude of P300,and detection rate of mild cognitive impairment.With the increase of the attack times,cognitive impairment aggravated gradually,and the detection rate of mild cognitive impairment increased gradually.Conclusions:Repeated TIAs have cumulative injury on cognitive function.The prevention and treatment of cerebral ischemia may become an important approach in decreasing cognitive impairment.

Objective To investigate the effect of lliac fascia compartment block on early analgesia of patients during performed the whole knee replacement.Methods Forty patients undergoing single whole knee replacement were randomly divided into group A and B,20 cases in each group.Before anesthesia,patients in group A underwentthree in one femoral nerve block guided by nerve stimulator with 20 ml of 0.5％ ropivacaine.Patients in group B underwent lliac fascia compartment block with 25 ml of 0.5％ ropivacaine.Visual analogue score (VAS) during rest and movement were recorded at 0 (T0),0.5 (T1),1 (T2),2 (T3),4 (T4),6 (T5),and 8 h (T6) after a patient fully awoke from surgery.Results In group B,Resting VAS scoresofpatientswere(4.5±0.7),(4.5 ±0.6),(4.2 ±0.8),(4.4 ±0.5),(4.3 ±0.7),(4.4 ±0.6),(5.2 ±0.6) at T0-T6 time point,and moving VAS were(4.7 ±0.6),(4.4 ±0.7),(4.4 ±0.5),(4.3 ±0.6),(4.4±0.6),(5.0 ±0.7),(5.1 ±0.6),all higher than that of group A((1.5 ±0.6),(1.7 ±0.6),(1.7 ± 0.6),(1.7 ±0.6),(1.9 ±0.6),(2.0 ±0.6),(2.2 ± 0.5) and(1.6 ±0.6),(2.0 ±0.6),(1.8 ±0.6),(1.7 ± 0.6),(2.0 ± 0.6),(2.3 ± 0.6),(2.5 ± 0.7),and there was significant difference (resting VAS∶ F (between group) =17.672,P ＜ 0.05,F(within group) =5.062,P ＜ 0.001,F(interaction) =1.443,P =0.143) ; Moving VAS:F (between group) =8.183,P ＜ 0.001,F (within group) =7.742,P ＜ 0.001,F (interaction) =0.85,P =0.424).Conclusion Iliac fascia compartment block had proved with early analgesic effect in patients with total knee replacement.Iliac fascia block is simple,safe,and can be performed without nerve stimulator.Therefore iliac fascia block has a certain clinical application.

Objective To find out the defects existing in the rinsing and sterilizing process of the automatic bottle washer and to put forward an improvement measure and efficiency assessment.Methods To make the washer more effective,an automatic brush pre-washing process and a rotary spraying device were designed and used.The total bacteria amount,the total coliform group and the disinfectant (ClO2) residual level were employed as the assessment parameters.Results An external automatic pre-washing machine was installed on the bottle washer;the bottle washer was innovated so that its spray nozzle could be in directions and the rinsing and sterilizing process could be enhanced.The results showed that the improved machine ran well in treating the polluted and recycled bottles and all the assessment indexes were better than that of before improvement.Conclusion The improved automatic bottle washer is considered effective in cleanout and sterilization of bottles with a satisfactory result.

Objective: To predict and identify HLA-A * 0201 restricted CD8+ CTL epitopes in Mycobacterium tuberculosis (Mtb) antigen Ag85C, so as to provide evidence for epitope-based study for tuberculosis (TB) vaccine. Methods: The online database SYFPEITHI was applied to predict the potential HLA-A * 0201 restricted epitopes from Ag85C, an antigen of Mycobacterium tuberculosis. T2 cell line was used to assay the affinity between the predicted peptides and HLA-A * 0201 molecules. The specific CTL lines were induced from peripheral blood mononuclear cells (PBMCs) of HLA-A * 0201 positive TB patients and PPD+ healthy donors by peptides with high binding affinity to HLA-A * 0201 molecules. IFN-γ production, in vitro proliferation and cytotoxicity of peptide-induced CTL were determined to screen HLA-A * 0201 restricted CD8+ CTL epitopes from those candidates. Results: Fourteen potential epitopes were identified from the SYFPEITHI database. After binding affinity assay, 3 of the 14 peptides (170-178 aa, 317-325 aa, and 144-153 aa) were found to have high binding affinity to HLA-A * 0201 molecules. However, only one peptide (144-153 aa) stimulated its specific CTL to release IFN-γ, proliferate in vitro and produce specific cytotoxicity. Conclusion: We have successfully identified a HLA-A * 0201 restricted CD8+ CTL epitope of Mtb Ag85C-FLTREMPAWL(144-153 aa), which might be a candidate epitope for TB vaccine designing. Our findings provides a basis for developing novel and effective anti-TB vaccine.

Objective: To construct a recombinant multiepitope fusion polypeptide containing 6 HLA-A * 0201 restricted CTL epitopes derived from Mycobacterium tuberculosis (Mtb.) antigens and express it in E. coli. Methods: The full-length sequence encoding 6 HLA-A * 0201 restricted CTL epitopes were derived from Mtb. antigens Rv03O9, Rv0173, and Ag85A, Ag85C; pan DR Th epitope (PADRE), Trojan peptide, and furin-sensitive linker RVKR were synthesized, amplified by polymerase chain reaction(PCR), inserted into the expression vector pGEX-4T-1, and transformed into E. coli BL21 (DE3). After induced by isopropylthio-β-D-galactoside (IPTG), the recombinant protein expression was examined by SDS-PAGE and Western blot. Results: The recombinant multiepitope fusion polypeptide plasmid was successfully constructed and expressed(about 40 000) in E. coli BL21 (DE3). Conclusion: The recombinant multiepitope fusion polypeptide may provide a basis for developing novel TB vaccine.
